Prediction of Suitable Site to Measure Abdominal Skin Fold Thickness and Correlation among Obesity Indicators in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자에서 피부두겹두께의 측정부위 예측 및 비만지표들 간의 관련성)

  • Purpose: The purpose of this study was to predict measuring site suited for abdominal skin fold thickness (ASFT) by measuring the distribution of abdominal subcutaneous fat thickness (AScFT) and ascertain the correlations among obesity indicators. Methods: The size of analysis materials was 124 secondary data measured by ultrasonic device, bioelectrical impedance analyzer and caliper. Data were analyzed using t-test, and Pearson's correlation. Results: The average of AScFT was 10.63± 6.79mm with its range 1.39-36.16 mm, and AScFT of female and of central parts were thicker than those of male and outer parts in the abdomen. The average of ASFT was 29.26±12.59 mm. Site 5 on Figure 1 was most similar to the average of AScFT in both sexes. Body mass index (BMI) and waist hip ratio (WHR) were 23.65±3.98 and 0.88±0.05 respectively. The body weight, BMI, WHR, visceral fat, ASFT vs AScFT revealed in significant correlation (r= .29, r= .55, r= .39, r= .33. r= .29). Conclusion: BMI and WHR seem more useful than other obesity indicators, when obesity control is necessary for Type 2 diabetes patients. Site 5 on Figure 1 is most suitable site to measure ASFT.

A Research on Anti-diabetic Functional Food intake of the Subjects with type 2 Diabetes mellitus in Daejeon (대전지역 제 2형 당뇨병 환자의 항당뇨기능성 식품 섭취 실태조사)

  • The present study was investigated on type 2 diabetics' actual status of anti-diabetic functional food intake by patients who came to oriental medicine clinics in Daejeon. The male was 37.3% and female was 62.7% of the subjects. The onset of the disease was most common in the age of 50's. Also 40% of the subjects have been suffering for more than 5 years. 35.7% of men, 53.2% for women had family history. Also 61.5% of the patients chose to carry out both diabetic therapy and exercise at the same time. 69.3% of the subjects have had experiences in anti-diabetic functional foods. Anti-diabetic functional foods used to surveyed people were bean, ginseng, and ginseng steamed red of which efficacies were already well known. Besides these foods, other foods such as loach, crucian carp were also used as anti-diabetic foods, but their benefits as anti-diabetic functional foods are not yet fully investigated. Subjects first knew about the therapy because friends or relatives recommended to them and most of them have used for 6 months to 1 year. However, 78.9% of subjects answered 'not so effective' in a question about satisfaction of anti-diabetic functional food. Although many of the patients have tried taking functional foods, they only use them for short period of time and they don't show prominent effect. Therefore these results suggest that in order to use scientifically studied functional foods, education about anti-diabetic functional foods should taken.

Incretin-based Treatment for Type 2 Diabetes Mellitus (제2형 당뇨병 환자에게 인크레틴 기반 약물치료요법)

  • Incretin hormones such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide delay gastric emptying, increasing satiety, and enhance insulin secretion. Two new classes of treatments related to incretin hormones for the management of type 2 diabetes mellitus have emerged: GLP-1 receptor agonists (e.g., exenatide, liraglutide) and the dipeptidyl peptidase-4 (DPP-4) inhibitors (e.g., sitagliptin, saxagliptin, vildagliptin, alogliptin), which prevent the degradation of GLP-1. A MEDLINE search was conducted in order to evaluate the efficacy and safety of incretin-based therapies and publications were reviewed. Data from clinical trials indicated incretin-based treatment showed clinically significant reductions in hemoglobin A1c with low risk of hypoglycemia. Weight reductions were observed with GLP-1 receptor agonists where as DPP-4 inhibitors are weight neutral.

Analysis of Bone Mineral Density and Incidence of Osteopenea in Type II Diabetic Patient's (제2형 당뇨환자의 골감소증 발생빈도와 골밀도 분석)

  • Purpose: This study aims to suggest clinical basis of physical therapy to skeletal system complication in type II diabetic patients through a variety of analysis methods. Methods: 75 subjects(type II diabetic patients; age from 40 to 80) of this study were participated in BMD(Bone Mineral Density) and Dual energy X-ray absorptiometry, blood test to their skeletal system. Results: Incidence of osteopenea was higher than normal group as women(91.2%) and men(53.5%) in diabetes group. However, women were showed no significantly difference in comparison group. Above age 60(in diabetes group; men and women) showed to highly incidence of osteopenea as well as significantly difference in osteopenea(peripheral neuropathy). As a result of analyzing blood through presence and absence of osteopenea in diabetes group, men were showed significantly difference in DPD/creatinine ratio and women were showed significantly difference in Alkaline phosphatase, Corrected Calcium. Conclusion: This study shows that decreased BMD and increased osteopenea through type II diabetes. Women were particularly lower BMD and higher incidence of osteopenea than men and men influence of type II diabetes on change of BMD was greater than women.

Factors Related to Perceived Health Status in Patients with Type 2 Diabetes (제2형 당뇨병 환자의 기능적 헬스 리터러시가 주관적 건강에 미치는 영향: 일개 대학병원 외래 환자를 대상으로)

  • Objectives: This study was performed to identify factors related to perceived health status among patients with type 2 diabetes. Methods: This is cross-sectional observational study. Respondents were 106 visitors in an outpatient diabetes clinic of a university hospital. Self-report questionnaire which included general information inquiry, diabetes-related, sociopsychological factors, functional health literacy and perceived health status was used for this study. The data was analyzed by using descriptive statistics, independent simple t-test, one-way ANOVA, and hierarchical multiple linear regression. All analysis were conducted using SAS 9.3. Results: Among the respondents, 43.4% engaged in poorly perceived health status. After adjusting for control variables, functional health literacy is significantly related to perceived health status(${\beta}$=0.095, p=0.016). Conclusion: Independent of diabetes-related, sociopsychological factors, higher functional health literacy is associated with better perceived health status of patients with type 2 diabetes. In order to improve perceived health status in the type 2 diabetes patients, it is necessary to develop strategy to enhance the functional health literacy.

Pathogens and Their Antimicrobial Susceptibilities of Endodontic Infection in Type II Diabetes Mellitus Patients (제 2형 당뇨 환자의 근관 감염에서 나타난 병원균과 그 항생제 감수성)

  • Aim: The purpose of this study was to identify dominant pathogens and their antimicrobial susceptibilities of endodontic infection in type II diabetes mellitus (DM) patients to determine effective empirical antibiotics. Methodology: Pathogens from endodontic infection in six patients with DM and in six patients without DM were cultured, identified and their antimicrobial susceptibility was tested using Vitek2 systems ($bioM{\acute{e}}rieux$, Marcy l' Etoile, France). The results were analyzed using Chi-square test and Fisher's exact test at P< 0.05 level. Results: Pathogens of opportunistic infection were dominant in DM patients (P=0.015). However, there was no significant difference of antimicrobial susceptibility between DM and non-DM patients. Relatively high percent (27%) of pathogens showed resistance to penicillin. Conclusions: More cautions should be paid to DM patients because they are prone to opportunistic infection. Penicillin is not effective in the control of endodontic infection.

Antioxidant and anti-diabetic effects of Ixeris strigosa extract (선씀바귀 추출물의 항산화 및 항당뇨 효과)

  • Purpose: Ixeris strigosa (IS) is a perennial plant that commonly grows in meadows. The leaves and roots of IS have been used in medicine as a sedative. This study evaluated the antioxidant and carbohydrate-digestive-enzyme inhibitory effects of IS to determine its potential as an essential antioxidant and glycemic inhibitor for type 2 diabetics. Methods: The antioxidative and α-amylase and α-glucosidase inhibitory activities were examined using the water extracts (ISW), ethanol extracts (ISE), and solvent fractions from IS. The antioxidative activities were measured using in vitro methods by measuring the 1,1-diphenyl-2-picrylhydrazyl and 2,2'-azino-bis-3-ethylbenzothiazoline-6-sulfonic acid radical scavenging activity. Results: Investigations of the total polyphenol, flavonoid content, in vitro antioxidant activity, and α-amylase and α-glucosidase inhibitory activities of the IS extract showed that the ISE had higher total phenolic and flavonoid contents than the ISW, as well as high antioxidant activity. The ethanolic extracts of IS (70%) had an α-amylase inhibitory activity of 78.55%. The ethyl acetate fraction (90.56%) showed higher α-glucosidase inhibitory activity than the positive control, acarbose (83.01%). Conclusion: Among the ISE fractions, the ethyl acetate and butanol fractions showed the best digestive enzyme inhibitory activity. Moreover, the antioxidant activity of the extract and the carbohydrate, α-amylase, and α-glucosidase inhibitory effects showed a stronger correlation with the total phenol and flavonoid contents compared to the ISW. As a result, the antioxidant and digestive enzyme inhibitory activities of high ISE are due to the phenolic compounds, particularly the flavonoid compounds. Therefore, ethyl acetate and butanol fractions of the 70% ethanol extract are excellent anti-diabetic functional materials.

Differences in Prevalence and Risk Factors of the Metabolic Syndrome by Gender in Type 2 Diabetic Patients (제 2형 당뇨병환자 성별 대사증후군 유병률과 위험요인 차이)

  • Purpose: The purpose of this study was to assess the difference between prevalence and risk factors of the metabolic syndrome(MS) by gender in type2 diabetic patients. Method: 108 participants(males 69, females 39) were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. MS was defined by a third report of the national cholesterol education program (NCEP) expert panel on detection, evaluation, and treatment of high blood cholesterol in adults(Adult Treatment Panel III)(ATP III), and abdominal obesity was determined by Asia-Pacific criteria in waist circumference. Results: The prevalence of MS by definition of NCEP-ATP III and the Asia-Pacific criteria in waist circumference was 39.3% in males and 66.7% in females type2 diabetic patients. The abdominal obesity prevalence was seen in 44.9% of males and in 79.5% of females patients. The prevalence of low HDL-cholesterolemia in serum was 26.2% in male, 52.8% in female type2 diabetic patients. Conclusion: These results show that nurses should focus on female diabetic patients for preventing MS.

Effect of Self-Monitoring of Blood Glucose Based Diabetes Self-Management Education on Glycemic Control in Type 2 Diabetes (자가혈당 측정결과기반 당뇨교육프로그램이 제2형 당뇨병환자의 혈당조절에 미치는 효과)

  • Purpose: The aim of the study was to evaluate the effect of self-monitoring of blood glucose (SMBG)-based Diabetes Self-Management Education (DSME) on glycemic control in type 2 diabetes. Methods: This study was designed to compare changes in glycemic control over 12months in SMBG-based DSME group (n=65) versus control group (n=65). Data were obtained from medical records type 2 diabetic patients treated with oral antidiabetic agents and above HbA1c 7.0% from June 2006 to August 2008. All participants completed DSME defined as informational intervention of lifestyle habits and reinforcement of educational Monthly News letter delivered by the diabetes nurse educator. SMBG-based DSME group requested to measure blood glucose 7 times a day for a week and to record their diary and received counseling with a focus on diet and lifestyle during the education. Assessments were conducted baseline, 3, 6 and 12 months. HbA1c was used as an index of glycemic control. Results: 12 months later, the level of HbA1c was reduced by $1.28{\pm}1.68%$ in experimental group and $0.49{\pm}1.05%$ in the control group. We found a significant effect of $Time^*$ Group interaction (p=.013). Conclusion: SMBG-based DSME for patients with type 2 diabetes with oral antidiabetic agents was effective in improving glycemic control and maintaining long-term glycemic control.

Effects of Diabetic Education Fortified with Individual Practice on Plasma Glucose, Self-care, and Self Reported Physical Symptom in Type 2 Diabetic Patients (개별실습을 강화한 당뇨교육이 제2형 당뇨병 환자의 혈당, 자가 관리 및 자각증상에 미치는 효과)

  • Purpose: This study was attempted to estimate the effects of diabetic education fortified with individual practice on plasma glucose, self-care, and self reported physical symptoms in type 2 diabetes patients Method: The subjects consisted of 46 patients with type 2 diabetes patients, who took the hospitalization diabetes education program from July 2003 until February 2004 at Seoul C university hospital. The experimental group was assessed at preand post intervention. The diabetes education was provided for one week. The education consisted of diabetes education videos for the diabetic introduction, group education for medication therapy, dietetic treatment and diabetes complication education. Also individual education for nurses examination of glycemia and insulin injection practice. Results: The HbA1c values significantly decreased from 9.6% on the time of hospitalization to 7.4% 3 months after discharge. In respect to the number of days of self-care, medication, diet, exercise, cleansing feet, and carrying sweets to prepare for hypoglycemia all significantly increased 3 months after discharge compared to the values at the time of hospitalization. Self reported physical symptoms were also significantly increased 3 months after discharge compared to the time of hospitalization. Conclusion: The diabetic education fortified with individual practice can be practically used as a plan for managing glycemia, self-care, and self reported physical symptom of diabetes patients.
