Restless Legs Syndrome in Children and Adolescents with Type 1 Diabetes (제 1형 당뇨병 소아 청소년의 하지불안증후군)

  • Objectives: Restless legs syndrome (RLS) is a common sleep disorder in adults with diabetes. This study investigated the frequency of RLS and clinical correlations in children and adolescents with type 1 diabetes. Methods: This study included 55 consecutive patients (21 males, age $12.6{\pm}3.4years$) with type I diabetes that were regularly treated at the Department of Pediatric Endocrinology. RLS was diagnosed by intensive interviews which also included the Epworth Sleepiness Scale (ESS) and International RLS Rating Scale (IRLSRS). Patients also received neurological examinations and laboratory tests for diabetes, iron metabolism and renal function. Results: Thirteen patients (23.6%, 6 males) were compatible for the diagnostic criteria of RLS. None of the RLS patients showed abnormal findings in neurological evaluations and 7 patients had familial history of RLS. Demographic and laboratory findings were not different between the patients with or without RLS. The RLS group showed significantly increased ESS and IRLSRS scores. Conclusion: RLS was prevalent in children and adolescents with type 1 diabetes. The association between RLS and diabetes-related laboratory findings requires further investigation.

The Effect of Exercise Program in Korea for Controlling Blood Glucose Levels in elderly women patients undergoing type II diabetes mellitus (제 2형 당뇨병 여성 노인 환자의 혈당조절을 위한 국내 운동 프로그램의 효과분석)

  • The purpose of this study was to investigate the effects of exercise program intervention in Korea for the control of blood sugar levels in elderly women patients undergoing diabetes mellitus typeII and to investigate the effects of exercise program intervention. Research methods were searched through RISS, KISS, KoreaMed, and KMbase as domestic databases and limited to domestic papers published from 2010 to May 2017. Diabetes, female diabetes, and diabetes mellitus typeII were used as the search terms related to the patient group. Programs related to intervention, intervention, exercise, blood glucose, glucose insufficiency, glucose control, fasting glucose and HbA1c were used. A total of 12 papers were provided according to the intervention method. The mediation period was 12 weeks(50%), 3 times per week(58.3%), and 50 to 69 minutes(83.4%) was the most. The combined aerobic and anaerobic exercise was found to be an effective exercise method to control not only blood glucose level but also HbA1c. Individualized exercise program intervention should be provided considering the physical fitness level and health status of elderly patients undergoing diabetes mellitus typeII and should be encouraged to maintain effective blood glucose control through continuous exercise rather than one-off exercise.

  • With the recent development of medical technology, the diagnosis rate of type 1 diabetes is increasing and patients are increasing. However, diabetes education content is not aligned with the interest level of children. As a result of the interviews with experts, It was found that the measures of coping with the change of blood sugar and the behavior therapy require steady and repetitive learning. Therefore, this study proposes Edutainment content which can be repeatedly Learned by 10~11year old children. For effective learning, the contents of the laboratory practice were constructed and the hybrid method was used for the repetitive learning. Usability test showed that this configuration is effective. This study is expected to contribute to the study of diabetic education content that is suitable for the children's level of understanding which will be developed easily in the future.

The Effects of Regulatory Exercise on Adipokines and Inflammatory Reaction in Type 1 Diabetic and Obese Children (규칙적인 운동이 제1형 당뇨병 및 비만 환아의 아디포카인과 염증반응에 미치는 영향)

  • The purpose of this study was to investigate the effects of aerobic exercise on adipokines and inflammatory reaction in obesity and type 1 diabetes mellitus (T1DM) children. We studied obese (OG, n=9), type 1 diabetic (DG, n=9), and normal (NG, n=9) children groups. Measurement factors included body weight, % fat, body mass index (BMI), $VO_2max$, lipid profiles and adipokines. The results showed significant differences in body weight, % fat, BMI, and $VO_2max$ (ml/kg/min) among the OG, DG, and NG (p<0.05) groups. There were significant differences in LDL-C and HDL-C between the OG, DG, and NG groups (p<0.05). In addition, adiponectin and retinol binding protein (RBP)-4 were significantly changed in DG and NG after 12 weeks exercise training (p<0.05), and there were also significant differences among the OG, DG, and NG groups (p<0.05). Monocyte chemoattractant protein (MCP)-1 in the OG, DG, and NG groups was significantly increased after 12 weeks exercise training (p<0.05). In conclusion, regulatory aerobic exercise does not change body composition in obese children with T1DM, but exercise and decreased blood inflammatory factors in T1DM may protect obese children from metabolic syndrome.

  • 제 2형 당뇨병 환자의 아연 영양상태를 파악하고 당뇨병성 합병증 보유상태와의 상관관계를 분석하기 위하여, 대구지역 33-69세의 제 2형 당뇨병환자 80명과 정상인 77명을 대상으로 영양소 섭취상태 및 신체계측, 생화학적 분석을 하였다. 당뇨병군의 평균 연령은 57.3세이었으며, 유병기간은 평균 8.7$\pm$6.4년이었다. 유병기간은 합병증 보유상태별로 유의적인 차이(p<0.01)를 나타내었다. 합병증 보유수가 1가지인 경우 평균 유병기간이 가장 길었고(16년), 다음으로 4가지 합병된 경우(약 11년), 2가지 합병된 경우(약 10년)의 순서였다. Effect of a Self Lead Camp for Type 1 Diabetes on Depression, Anxiety, Self-respect and Self-efficacy with Type 1 Diabetes (1형 당뇨인 자기주도 캠프가 1형 당뇨병 대상자의 우울, 불안, 자아존중감 및 당뇨관리 자아효능감에 미치는 영향)

  • Purpose: It is thought to be crucial to guide diabetic children and adolescents through their adolescent period psychologically and emotionally. Therefore, we investigated what positive roles a type 1 diabetic camp program would play on subjects in terms of depression, anxiety, self-esteem and self-efficacy. Methods: This study evaluated 38 diabetic children and adolescents with type 1 diabetes who attended summer camp. Subjects were analyzed using a one-group pretest-posttest quasi-experimental research design. Results: There was a significant decrease in depression (t=3.55, p=.001) and anxiety (t=3.09, p=.004) after attending camp, as well as a significant improvement in self-efficacy (t=-3.64, p=.001). However, there was no significant change in self-esteem (t=0.14, p=.891). Conclusion: The results of this study indicate that type 1 diabetic camps are an effective intervention program to promote psychological and emotional stability among diabetic children; therefore, it is recommended that various diabetes programs related to camps be developed.

The Influence of Self-management Knowledge and Distress on Diabetes Management Self-efficacy in Type 2 Diabetes Patients (제2형 당뇨병 환자의 자기관리지식, 스트레스가 당뇨관리 자기효능감에 미치는 영향)

  • This study was a descriptive research study performed to identify the degree and correlation of variables and also explain the factors that influence self-efficacy of diabetes management. The participants were 150 diabetes patients who visited a primary medical institution in K city in Korea from September 17, 2015, to October 15, 2015. The data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson correlation coefficients, and multiple regression with SPSS 18.0. Significant differences in age and education were detected in self-efficacy of diabetes management according to general characteristics. The levels of self-management knowledge and diabetes management self-efficacy were shown to be positively correlated. The levels of diabetes management self-efficacy and distress as well as levels of self-management knowledge and distress were shown to be negatively correlated. The significant factors influencing diabetes management self-efficacy were distress and self-management knowledge. The results suggest that appropriate diabetes management self-efficacy programs should be provided in order to improve self-management knowledge and decrease distress in type 2 diabetes patients. This study provides basic data to promote the effective education and development of arbitration in order to enhance self-efficacy of diabetes management.

Improvement of Insulin Resistance by Curcumin in High Fat Diet Fed Mice (고지방식이 급여 마우스에서 curcumin의 인슐린 저항성 개선 효능)

  • Rapid increase of diabetic population is a major health concern in Korea. In a trial to develop food components which can prevent and/or cure diabetes, we investigated the anti-diabetic activity of curcumin in high fat diet (HFD)-induced type 2 diabetes mellitus (T2DM) animal model. C57BL/6 mice were divided into three groups: normal diet (ND), high fat diet (HFD), and curcumin (CUR, HFD+0.02% curcumin). Mice were fed each diet for 16 weeks. CUR significantly reduced body weight gain, the levels of plasma glucose, insulin, total-cholesterol (T-C), and LDL-C, whereas increased HDL-C compared to those of HFD group. Notably, insulin signaling pathway was activated by CUR. This suggests that CUR improves obesity-associated T2DM by overcoming insulin resistance in part.

Clinical Characteristics of Type 2 Diabetes in Children and Adolescents (소아 및 청소년 2형 당뇨병의 임상적 특징)

  • Purpose : The incidence of type 2 diabetes in children and adolescents has been reported to increase recently. The aim of this study is to investigate the clinical features of type 2 diabetes developing during childhood and adolescent period. Methods : The medical records of 33 patients with type 2 diabetes were reviewed. We analysed clinical manifestations, demographic data, and modes and responses of treatment. Results : Age at diagnosis was $13.4{\pm}1.8$ years. Seventy percent of patients revealed pubertal signs at diagnosis. Half of the patients had BMI more than $25kg/m^2$. Seventy-three percent of patients had family history of type 2 diabetes. Acanthosis nigricans were found in 18% of patients. Nineteen(57.6%) patients were diagnosed incidentally by random urine or blood glucose test without any typical diabetic symptom or sign. The modes of therapy to control hyperglycemia were insulin alone(75.8%), oral hypoglycemic agents alone(9.1%), insulin and oral hypoglycemia agents(9.1%), and only diet with exercise(6%). At the time of investigation, 45.5% of patients were not using insulin. The typical diabetic symptoms at diagnosis were more prevalent in patients who required insulin for more than two years than patients who did not(P<0.05). Conclusion : The development of type 2 diabetes in children and adolescents is possibly related to puberty, obesity, family history, and defects in insulin secretion rather than insulin resistance. Many children and adolescents with type 2 diabetes required insulin initially and some of them could discontinue. More than half of the patients were diagnosed as diabetes without any typical symptom or sign, which might be one of the predictive factors of the prolonged insulin requirement.
