Background: Ischemia-reperfusion injury related to unsuccessful myocardial protection affects postoperative ventricular function and mortality during open-heart surgery. We prospectively compared the effects of administration of histidine-tryptophan-ketoglutarate (HTK) solution and cold blood cardioplegia (CBC) on myocardial protection and clinical outcome in patients undergoing mitral valve surgery. Material and Method: Seventy patients with mitral regurgitation (MR) undergoing mitral valve surgery were randomly divided into the HTK group (n=31) and the CBC group (n=31 ): eight patients were excluded. Perioperative hemodynamics, cardiac medications, pacing, postoperative outcomes and complications were recorded during the hospital stay. All patients received follow-up for at least 6 months postoperatively for morbidity and mortality. Resuか: There were no significant differences in the hemodynamics between the groups during the study period, except for the mean pulmonary artery pressure (MPAP), PCWP and CVP that were lower in the HTK group at 15 min after weaning of CBP. There were no differences for inotropic support and pacing during the 12 hrs postoperatively between the groups. CK-MB values on day 1 and day 2 were $77{\pm}54$ and $41{\pm}23$ for the HTK group and $70{\pm}69$ and $44{\pm}34$ for the CBC group, respectively (p=NS). Postoperative clinical outcomes were similar in both groups for at least 6 months during the follow-up period. Conclusion: These results suggest that the use of HTK solution is as safe as cold blood cardioplegia in terms of myocardial protection.
Puroose: To present preliminary results of intensity-modulated radiotherapy (IMRT) using the simultaneous modulated accelerated radiation therapy (SMART) boost technique in patients with nasopharyngeal carcinoma (NPC). Materials and Methods: Twenty patients who underwent IMRT for non-metastatic NPC at the Asan Medical Center between September 2001 and December 2003 were prospectively evaluated. IMRT was delivered using the 'step and shoot' SMART technique at prescribed doses of 72 Gy (2.4 Gy/day) to the gross tumor volume (GTV), 60 Gy (2 Gy/day) to the clinical target volume (CTV) and metastatic nodal station, and 46 Gy (2 Gy/day) to the clinically negative neck region. Eighteen patients also received concurrent chemotherapy using cisplatin once per week. Results: The median follow-up period was 27 months. Nineteen patients completed the treatment without interruption; the remaining patient interrupted treatment for 2 weeks owing to severe pharyngitis and malnutrition. Five patients (25%) had RTOG grade 3 mucositis, whereas nine (45%) had grade 3 pharyngitis. Seven patients (35%) lost more than 10% of their pretreatment weight, whereas 11 (55%) required intravenous fluids and/or tube feeding. There was no grade 3 or 4 chronic xerostomia. All patients showed complete response. Two patients had distant metastases and loco-regional recurrence, respectively. Conclusion: IMRT using the SMART boost technique allows parotid sparing, as shown clinically and by dosimetry, and may also be more effective biologically. A larger population of patients and a longer follow-up period are needed to evaluate ultimate tumor control and late toxicity.
We investigated the effect of constant rate infusion (CRI) with doxapram on cardiopulmonary function during total intravenous anesthesia (TIVA) with remifentanil and propofol CRI in dogs. Fifteen male Beagle dogs were randomly divided into 3 groups. All groups were premedicated with medetomidine ($20{\mu}g/kg$, IV) and anesthetized by remifentanil/propofol CRI for one and half hour. At the initiating of the anesthesia, different doses of doxapram for each group were administrated as the followings; D1 group - doxapram 0.25 mg/kg bolus followed by doxapram $8.33{\mu}g/kg/min$, D2 group - doxapram 2 mg/kg bolus followed by doxapram $66.66{\mu}g/kg/min$, control group - normal saline. The anesthetic depth for surgery was well maintained in all groups throughout the anesthetic periods. The respiratory rate was significantly higher in D2 group than that of control group (p < 0.05). The values of $PaO_2$ and $SaO_2$ were significantly increased in both D1 and D2 groups compared with control group (p < 0.05). High dose of doxapram (D2 group) significantly decreased the level of $PaCO_2$ compared with control group (p < 0.05). The values of systolic, mean and diastolic arterial pressure were significantly increased in doxapram 2 group (p < 0.05). There were no significant differences in the values of heart rate and pH of arterial blood. Therefore, doxapram CRI may be useful to alleviate the suppression of cardiopulmonary function including hypoxia and hypotension during TIVA with remifentanil and propofol in dogs.
Not only reducing the carry-over effects of quinclorac [3, 7-dichloro-8-quinoline carboxylic acid] used in paddy field to some following vegetable crops but also rationalizing agro-ecology conservation and farm economy, the reducing feasibility of application rates by various cropping patterns and application timing after rice seeding and transplanting. Four cropping patterns namely dry direct seeding(DDS), flooded direct seed(FDS), transplanting of 8 days old early seedlings(EST) and 25 days old machinery seedling(MST) were experimented with 7 application timings as 0, 5, 10, 15, 20, 25, 30 days after seeding/transplanting and 9 levels of application rates as 0, 75, 150, 225, 300, 375, 450, 525, and 600g ai/ha of the chemical, respectively. Within the maximum permitted limit of rice phytotoxicity, the minimum application rate of quinclorac to complete control of Echinochloa crus-galli as influenced by various cropping patterns with application timing could be evaluated as follows : A. Dry direct seeding : The minimized application rate at application timing upto 10 days after seeding (DAS) was counted 150g ai/ha, and delaying upto 15-30 DAS, the rates were increased upto 225-525g ai/ha. B. Flooded direct seeding and transplanting : The application rates were minimized 75g ai/ha at application timing upto 10 days after seeding/transplanting(DAS/T), 150g ai/haupto 15 DAS/T, and 225g ai/ha at later than 20 DAS/T, respectively.

We investigated the mineralogical and chemical characteristics of oyster shell as the possible substitute for the limestone used as an absorbent of $SO_2$ gas. The oyster shells from Taean and Tongyeong were used for the comparison with limestone and those from Boyreong and Yeosu were additionally investigated. XRD results show that all shells are composed of calcite with the exception of the myostracum layer attached to adductor muscle and ligament, which is composed of aragonite. The marine sediments as impurities exist on the surface of shells or as inclusions in the shells. Calcite is the main mineral composition of the shell of barnacle which is also one of the impurities. The oyster shell is composed of three main layers; prismatic, foliated, and chalk. The oyster shell from Tongyeong with the largest shell size, has the smallest thickness of prismatic and foliated layers which contain protein called conchiolin, whereas that from Taean with the smallest shell size has the largest prismatic and foliated layers. The sizes of those two layers of the shells from Boryeong and Yeosu are larger than that from Tongyeong but smaller than Taean. Those differences are supposed to be due to the different growth environments because the oysters from Tongyeong are cultured under the sea while those from Taean are in the tidal zone. The oyster shells generally show higher amount of sulfur and phosphorus than limestone, mainly due to the composition of protein. Some elements such as Mg show significant variations in different layers. As for trace elements, Li shows much higher amount in oyster shells than limestone, suggesting the influence of the composition of the sea water on the formation of the oyster shells.
Carpophores of ten Korean strains of Lentinus edodes (Berk.) Singer, an antitumor polysaccharide producing fungus, were extracted with 0.1N NaOH solution. The extracts were dialized for seven days in distilled water and lyophilized to produce crude polysaccharide powders. Thus obtained crude polysaccharide samples were assayed for sugar contents by colorimetric method with anthrone reagent. Among ten strains examined Lentinus edododes-DMC7 was found to be the richest strain in polysaccharide content of carpophores. By shake culture experiment for biomass production, L. edodes-DMC7 was found to be the second most productive strain among seven strains examined. Cultural characteristics of L. edodes-DMC7 were investigated by shake culture method. The best result was obtained when L. edodes-DMC7 was cultured in the medium containing glucose 8g, starch 80g, yeast extract 12g, $KH_2PO_4\;0.87g,\;MgSO_4{\cdot}7H_2O\;O.5g,\;CaCl_2\;0.3g,\;FeSO_4{\cdot}7H_2O\;10mg\;ZnSO_4{\cdot}7H_2O\;4mg,\;CuSO_4{\cdot}5H_2O\;lmg,\;MnCl_2{\cdot}4H_2O\;7mg\;per\;11\;at\;28^{\circ}C$, 180 rpm, for 12 days. Thus thirty-three grams of dry mycelia was obtained per one liter of medium.

Purpose : This study tried to evaluate the effectiveness of combined treatment using radiation therapy and concurrent cisplatin as a radiosensitizer in the management of locally advanced head and neck cancer. Materials and methods : From January 1995 to August 1998, 29 evaluable patients with locally advanced head & neck cancels (AJCC stage $II\~IV$) were received curative radiation therapy $(total\;70\~75.6\;Gy/35\~42\;fractions,\;1.8\~2\;Gy/fraction)$ and concurrent cisplatin chemotherapy ($100\;mg/m^2$, D1, D22, D43). The neck dissections were peformed for residual lymphadenopathy. Follow-up ranged from 5 to 55 months (median 24 months). Results : Twenty-one $(72.4\%)$ patients achieved clinical complete responses. The partial response and minimal response rates were $17.2\%\;and\;10.4\%$, respectively. Locoregional failure rate was $27.6\%$, and included 6 patients with local failures, 4 patients with regional failures, and 2 patients with combined local and regional failures. Four of 29 patients $(13.8\%)$ developed distant metastasis. The disease free survival rate at 3 years was $60\%$. Nasopharyngeal primary tumors or complete responders showed significantly higher disease free survival rate. The grade 3 mucositis and nausea/vomiting was noted in $34.5\%$, respectively. Major prolongation of radiation therapy duration was inevitable in three patients. Twenty-one patients $(72.4\%)$ completed 3 courses of cisplatin and 5 patients received 2 courses of cisplatin. Three patients received only one course of cisplatin due to nephrotoxicity and neurotoxicity, and then changed to 5-FU regimen. Conclusions : Concurrent cisplatin-radiation therapy in locally advanced head and neck cancer showed high response rate, reasonable locoregional control, and survival rate. As expected, acute toxicities were increased, but compliance to treatment was acceptable. Assessment of the effect of the combination in this setting requires further accrual and follow-up.
Sink (grain)-filling rates of IR667 line (Suwon 213 and 214) and local leading varieties (Jinheung and Paldal) were investigated in relation to sink-source ratio, sink-source distance, forms of photosynthates, and weather factors. The results are as follows. 1. IR667 line have higher maximum filling rate (g. grain/day. ear) and shorter duration to reach maximum filling rate than local varieties. The curve pattern of sink-filling rate was high and steep type in IR667 line and low and broad type in local varieties. 2. Weather factors seem to give little effect on the curve pattern of sink-filling rate. 3. The functional sink-source distance (actual sink-source distance multiplied by the contribution rate of leaf for grain production) was shorter in upper leaves (flag and 2nd leaf) for IR667 line, the upper leaf-dependent type in grain production, and was longer in lower leaves (3rd and 4th) for local varieties, the lower leaf-dependent type. Specially short first internode from top may contribute to the upper leaf depencency of IR667 line. 4. According to free sugar-starch ratio (sugar/starch) in the culm and leaf sheath IR667 line could be classified as the high sugar type and the local varieties as the high starch type. The ratio of transportable form (sugar) to non transportable photosynthate (starch) seems to relate with sink-filling rate. And high sugar type is expected to have higher efficiency for grain production in view of energy balance. 5. A hypothesis that the higher in the uniformity within the series of productive structure the more efficient in grain production is proposed and discussed in relation to productivity. 6. According to the pattern of percent nutrient retention of each leaf blade IR667 showed the central retention type and Jinheung showed the apical retention type and each retention type appears to be a cause of each canopy conservation pattern. 7. From the content and percent distribution of nutrient in various organ IR667 could be classified as the leaf sheath dominant type and Jinheung as the leaf blade dominant type. 8. The fact that the greater the percent translocation of nutrient into grain the greater the percent nutrient retention in leaf blade was held between nitrogen and phosphorus within a variety and between varieties within a nutrient (N, P or K).
Using commercial radish varieties for processing, about 30% of radish was discarded due to the root shape and low purity. To raise the processing ability, we tried to develop a new variety producing H-shaped root. As another characteristic required in variety for processing is high purity, we tried to raise purity using simple sequence repeats (SSR) markers for testing seed purity in every segregating generation. To develop Male-sterile (MS) seeding parent, we crossed commercial variety of 'Gwan dong spring' and 'Gyeo ryong spring'. One elite inbred was selected as recurrent parent for the MS plant. The major horticultural traits of selected inbred line were disease resistance, late bolting, heat resistance and bright green root top color. To develop pollen parent, we crossed commercial variety of 'Tae sang king' and 'Seoul spring'. We used individual selection method to develop H-shaped hard root and disease resistant inbred. In each segregating generation, we selected one plant based on phenotype and the uniformity of selected plant was tested by SSR markers using self-pollinated seeds. In the first segregating generation, 64.6% of sib plants shared the same band in PCR amplification using ACMP-490 primer and 66.7% using cnu-316 primer. The uniformity of segregating generations using ACMP-490 and cnu-316 raised in second generation to 68.8%, 70.8%, respectively; in third generation to 93.8%, 100%; in fourth generation to 93.8%, 100%; in fifth generation to 95.8%, 100%; in sixth generation to 100%, 100%. A novel cross was made using selected MS parent and pollen parent. When we checker the horticultural traits using autumn cultivation, the novel cross variety produced H-shaped root comparing other commercial varieties and produced highly uniform radish. Thus we registered this novel cross variety as 'YR ORE' at 2013 (Registration No. 4550).
To evaluate the follow-up management state and related factor of lead battery workers in periodic health examination as part of program of group occupational health service, author studied 293 workers with questionnaire on knowledge of results and follow-up management state and related factors, and compared the responses to their periodic health examination result charts. The results were as follows: 1. 252(86%) workers responsed that they had received the health examination result chart, but only 116(39.6%) workers responsed that they had been educated or explained about the results of health examination, and 11(57.9%) workers among 19 workers with non-occupational disease D, 101(44.3%) workers among 228 workers with non-occupational disease C, and 19(28.4%) workers among 67 workers with occupational disease C knew accurately their health examination results. 2. 78(24.8%) of the workers responsed that they had follow-up management, and contents of follow-up management were follow-up(36.6%), out-patient treatment (31%), change worksite(8.5%), temporary retirement(7.0%) and others(16.9%). 3. Most of the workers responsed that the health examination were necessary, but three-fourths of the workers responsed that the health examination had been superficial or that they didn't know. 4. In this study, follow-up management show significant association with only explanation or education about health examination result chart.
