Measurement of Radiation Using Tissue Equivalent Phantom in ICR Treatment (자궁강내 근접방사선조사시 인체조직등가 팬톰을 이용한 방사선량 측정)

  • This study is to compare A point doses in human pelvic phantom by film dosimetry, computer planning and manual calculation by using of along-away table. We developed tissue equivalent human pelvic phantom composed of four pieces of cylindrical acryl tubes with water, to simulate intracavitary radiation (ICR) in patients with cervix cancer. When the phantom assembled from 4 pieces, it has a small space for inserting Fletcher-Suit-Delclos applicator like a human vagina. Fletcher-Suit-Delclos applicator inserted into the space was packed tightly with furacin gauzes, and three $^{137}Cs$ sources with radioactivity of $15.7mg\;Ra-eq$ were inserted into the tandem. For the film dosimetry, two pieces of X-OMAT V film (Kodak Co.) of which planes include point A, were arranged orthogonally in the slits between phantoms. A point dose and iso-dose curves were measured by means of optical densitometer. A point doses by film dosimetry, RTP system and manual calculation by using of along-away table were compared, and iso-dose curves by film dosimetry and computer planning were also compared. The dose of A point was 51.2cGy/hr by film dosimetry, 46.7cGy/hr by RTP system and 47.9 cGy/hr by along-away table. A point dose by computer planning was similar to the dose by calculation using of along-away table with acceptable accuracy $({\pm}3%)$, however, the dose by film dosimetry was different from two others with about 10% error. Since most clinical beams contains a scatter component of low energy photons, the correlation between optical density and dose becomes tenuous. In addition, film suffers from several potential errors such as changes in processing conditions, interfilm emulsion differences, and artifacts caused by air pockets adjacent to the film. For these reasons, absolute dosimetry with film is impractical, however, it is very useful for checking qualitative patterns of a radiation distribution. In future, solid state dosimeter such as TLD must be used for the dosimetry of ionizing radiation. When considerable care is used, precision of approximately 3% may be obtained using TLD.

  • A nationwide survey on necrotizing enterocolitis (NEC) was undertaken among members of the Korean Association of Pediatric Surgeons. The members were required to complete a questionnaire and the case registration form for each patient during the three-year period from July 2001 to June 2004. Eighty one patients were included in the questionnaire, but only 71 patients were registered from 22 members in 16 hospitals. At the same time survey on focal intestinal perforation (FIP) was undertaken and 17 patients were registered from 11 members in 10 hospitals. Total 19,041 newborns were admitted to neonatal intensive care unit during the study period. Eighty one patients (17.27 %) were underwent surgery among 469 babies who were managed under the impression of NEC. The male to female ratio was 2.1:1. The premature were 60.6 %. The most common site of involvement was ileum. Overall and operation survival of NEC were 72 % and 79 %, respectively. The survival was lower in smaller babies, multiple segments involvement and involvement of both the small and large intestine. But there was no difference in survival according to sex or prematurity. The FIP showed very similarity with NEC in terms of incidence, and the age of diagnosis and operation. But the survival was much better and 100 %. The results showed the clinical characteristics of NEC and the trend of management in NEC in Korea. In the future we hope we can discuss about this topic in prospective manner.

A Survey on Intake of Protein Supplement of University Students Majoring in Physical Education (체육교육전공 대학생들의 단백질 보충제 섭취실태)

  • The purpose of this study was to investigate intake of protein supplements by university students majoring in physical education. Intake experience rate, reasons for intake, purchasing place, effectiveness, satisfaction level, and side effects were analyzed using a questionnaire. Of 476 students, those who consumed protein supplements were 198 (41.6%). Male's intake experience rate was significantly higher than that of females, and members of health-related clubs also consumed more protein than non-members. The main purchasing place was internet shopping malls, and users obtained information from their friends or upperclassmen. The most frequently consumed protein supplement was 'WPH', and the most frequent reason for intake was 'building muscle or maintaining body shape'. For effectiveness, 'normal' was 49.0% and 'effectiveness' was 33.3%. For satisfaction, 'satisfaction' was 45.5% and 'normal' was 43.4%. The rate of side effects was 44.9%, and digestive issues such as diarrhea and indigestion were observed with high frequency. The results of this study show that education is needed for nutritional knowledge, adequate intake, and side effects of protein supplements.

Study of Scatter Influence of kV-Conebeam CT Based Calculation for Pelvic Radiotherapy (골반 방사선 치료에서 산란이 kV-Conebeam CT 영상 기반의 선량계산에 미치는 영향에 대한 연구)

  • The accuracy and uniformity of CT numbers are the main causes of radiation dose calculation error. Especially, for the dose calculation based on kV-Cone Beam Computed Tomography (CBCT) image, the scatter affecting the CT number is known to be quite different by the object sizes, densities, exposure conditions, and so on. In this study, the scatter impact on the CBCT based dose calculation was evaluated to provide the optimal condition minimizing the error. The CBCT images was acquired under three scatter conditions ("Under-scatter", "Over-scatter", and "Full-scatter") by adjusting amount of scatter materials around a electron density phantom (CIRS062, Tissue Simulation Technology, Norfolk, VA, USA). The CT number uniformities of CBCT images for water-equivalent materials of the phantom were assessed, and the location dependency, either "inner" or "outer" parts of the phantom, was also evaluated. The electron density correction curves were derived from CBCT images of the electron density phantom in each scatter condition. The electron density correction curves were applied to calculate the CBCT based doses, which were compared with the dose based on Fan Beam Computed Tomography (FBCT). Also, 5 prostate IMRT cases were enrolled to assess the accuracy of dose based on CBCT images using gamma index analysis and relative dose differences. As the CT number histogram of phantom CBCT images for water equivalent materials was fitted with a gaussian function, the FHWM (146 HU) for "Full-scatter" condition was the smallest among the FHWM for the three conditions (685 HU for "under scatter" and 264 HU for "over scatter"). Also, the variance of CT numbers was the smallest for the same ingredients located in the center and periphery of the phantom in the "Full-scatter" condition. The dose distributions calculated with FBCT and CBCT images compared in a gamma index evaluation of 1%/3 mm criteria and in the dose difference. With the electron density correction acquired in the same scatter condition, the CBCT based dose calculations tended to be the most accurate. In 5 prostate cases in which the mean equivalent diameter was 27.2 cm, the averaged gamma pass rate was 98% and the dose difference confirmed to be less than 2% (average 0.2%, ranged from -1.3% to 1.6%) with the electron density correction of the "Full-scatter" condition. The accuracy of CBCT based dose calculation could be confirmed that closely related to the CT number uniformity and to the similarity of the scatter conditions for the electron density correction curve and CBCT image. In pelvic cases, the most accurate dose calculation was achievable in the application of the electron density curves of the "Full-scatter" condition.

Reading Deviations of Glass Rod Dosimeters Using Different Pre-processing Methods for Radiotherapeutic in-vivo Dosimetry (유리선량계의 전처리 방법이 방사선 치료 선량 측정에 미치는 영향)

  • The experimental verification of treatment planning on the treatment spot is the ultimate method to assure quality of radiotherapy, so in-vivo skin dose measurement is the essential procedure to confirm treatment dose. In this study, glass rod dosimeter (GRD), which is a kind of photo-luminescent based dosimeters, was studied to produce a guideline to use GRDs in vivo dosimetry for quality assurance of radiotherapy. The pre-processing procedure is essential to use GRDs. This is a heating operation for stabilization. Two kinds of pre-processing methods are recommended by manufacturer: a heating method (70 degree, 30 minutes) and a waiting method (room temperature, 24 hours). We equally irradiated 1.0 Gy to 20 GRD elements, and then different preprocessing were performed to 10 GRDs each. In heating method, reading deviation of GRDs at same time were relatively high, but the deviation was very low as time went on. In waiting method, the deviation among GRDs was low, but the deviation was relatively high as time went on. The meaningful difference was found between mean reading values of two pre-processing methods. Both methods present mean dose deviation under 5%, but the relatively high effect by reading time was observed in waiting method. Finally, GRD is best to perform in-vivo dosimetry in the viewpoint of accuracy and efficiency, and the understanding of how pre-processing affect the accuracy is asked to perform most accurate in-vivo dosimetry. The further study is asked to acquire more stable accuracy in spite of different irradiation conditions for GRD usage.

Validation Study for Image Performance of I-131 Using GATE Simulation Program (GATE 시뮬레이션 프로그램을 이용한 I-131의 영상 특성의 타당성에 관한 연구)

  • The purpose of this study was to validate for GATE (Geant4 Application for Tomographic Emission) simulation by comparing the results of GATE simulation and experiment in real SPECT system. Futhermore, we want to prove that it is possible that the quantitative research of gamma camera/SPECT imaging for therapeutic radio isotope by using GATE simulation. In this study, the SPECT system on simulation referred to the parameters of Stream-R Forte version 1.2 (Philips Medical System, Best and Heerlen, Netherlands). To understand the I-131 image of gamma camera/SPECT system, we acquired the energy spectrum and measured the full width at half maximum (FWHM) which comes from line spread function (LSF) with and without scatter material in real SPECT system. And to compare with experiment, we also measured the FWHM and acquired the energy spectrum without scatter material in GATE simulation. As a result, without scatter material, the energy peak was almost same location, which are located nearby 364 keV, and other spectrum factors are same tendency in both cases. The FWHM was increased by increasing the distance of source to detector, and the error rate was approximately 3.8%. When we used the line source with scatter material, energy spectrum also indicated similar tendency in both cases. As you confirmed earlier, GATE simulation included real instrument and radioisotope characters for therapeutic radioisotope. Therefore this result that it was possible that various quantitative study for therapeutic radioisotope imaging in gamma camera/SPECT using GATE simulation.

Modeling and Validation of Population Dynamics of the American Serpentine Leafminer (Liriomyza trifolii) Using Leaf Surface Temperatures of Greenhouses Cherry Tomatoes (방울토마토에서 잎 표면온도를 적용한 아메리카잎굴파리(Liriomyza trifolii) 개체군 밀도변동 모형작성 및 평가)

  • Population dynamics of the American serpentine leafminer, Liriomyza trifolii (Burgess), were observed and modeled in order to compare the effects of air and tomato leaf temperatures inside a greenhouse using DYMEX model builder and simulator (pre-programed module based simulation programs developed by CSIRO, Australia). The DYMEX model simulator consisted of a series of modules with the parameters of temperature dependent development and oviposition models of L. trifolii were incorporated from pre-published data. Leaf surface temperatures of cherry tomato leaves (cv. 'Koko') were monitored according to three tomato plant positions (top, > 1.8 m above the ground level; middle, 0.9 - 1.2 m; bottom, 0.3 - 0.5 m) using an infrared temperature gun. Air temperature was monitored at the same three positions using a self-contained temperature logger. Data sets for the observed air temperature and average leaf surface temperatures were collected (top and bottom surfaces), and incorporated into the DYMEX simulator in order to compare the effects of air and leaf surface temperature on the population dynamics of L. trifolii. The initial population consisted of 50 eggs, which were laid by five female L. trifolii in early June. The number of L. trifolii larvae was counted by visual inspection of the tomato plants in order to verify the performance of DYMEX simulation. The egg, pupa, and adult stage of L. trifolii could not be counted due to its infeasible of visual inspection. A significant positive correlation between the observed and the predicted numbers of larvae was found when the leaf surface temperatures were incorporated into the DYMEX simulation (r = 0.97, p < 0.01), but no significant positive correlation was observed with air temperatures(r = 0.40, p = 0.18). This study demonstrated that the population dynamics of L. trifolii was affected greatly by the leaf temperatures, though to little discernible degree by the air temperatures, and thus the leaf surface temperature should be for a consideration in the management of L. trifolii within cherry tomato greenhouses.

A Follow-up Study of DMSA Renal Scan in Children with . Acute Pyelonephritis and Vesicoureteral Reflux (급성신우신염과 방광요관역류 환아에서 DMSA신주사의 추적관찰)

  • Purpose : DMSA renal scan is known as a valuable method for evaluating acute pyelonephritis and renal scar in patients with febrile urinary tract infection or vesicoureteral reflux. We made this study, to determine the most effective use of DMSA renal scan. Methods : 155 children were admitted to the Pediatric Department of Yonsei University, Severance hospital with the symptom of urinary tract infection from January, 1992 to June, 1995. DMSA scan, renal ultrasound and VCUG were performed. One consisted of 29 patients with the diagnosis of acute pyelonephritis without vesicoureteral reflux and the otherconsistedofllpatientswithvesicoureteralreflux. Results : The follow-up DMSA scans at mean $0.99{\pm}0.46$ months after the initial DMSA scans were performed in the 29 children with acute pyetonephritis. 21($72.4\%$) of 29 children showed normal DMSA scans. 8 children with abnormal DMSA scans had follow-up DMSA scans at 2.5 months after the initial scans and 6 of 8 children showed normal DMSA scans. Only 12($41.4\%$) of 29 children showed abnormal renal ultrasound. The data about DMSA scans in 32 children with vesicoureteral reflux showed that there were abnormal DMSA scans in Grade $I;25.0\%$, Grade $II;44.5\%$, Grade $III;64.3\%$, Grade $IV;92.9\%$ and Grade $V;100.0\%$. There was a significant difference in that 36 renal unit ($68.0\%$) on DMSA renal scan and 26 renal units ($49.1\%$) on renal ultrasound showed abnormal finding(P<0.05). Conclusion : DMSA renal scan is more useful than renal ultrasound for the diagnosis of acute pyelonephritis. It is considered that if the initial DMSA scan is abnormal, a follow-up DMSA scan must be performed after 10weeks (8-12weeks) and the change in DMSA scan evaluated.

Estimation of Groundwater Recharge by Considering Runoff Process and Groundwater Level Variation in Watershed (유역 유출과정과 지하수위 변동을 고려한 분포형 지하수 함양량 산정방안)

  • In Korea, there have been various methods of estimating groundwater recharge which generally can be subdivided into three types: baseflow separation method by means of groundwater recession curve, water budget analysis based on lumped conceptual model in watershed, and water table fluctuation method (WTF) by using the data from groundwater monitoring wells. However, groundwater recharge rate shows the spatial-temporal variability due to climatic condition, land use and hydrogeological heterogeneity, so these methods have various limits to deal with these characteristics. To overcome these limitations, we present a new method of estimating recharge based on water balance components from the SWAT-MODFLOW which is an integrated surface-ground water model. Groundwater levels in the interest area close to the stream have dynamics similar to stream flow, whereas levels further upslope respond to precipitation with a delay. As these behaviours are related to the physical process of recharge, it is needed to account for the time delay in aquifer recharge once the water exits the soil profile to represent these features. In SWAT, a single linear reservoir storage module with an exponential decay weighting function is used to compute the recharge from soil to aquifer on a given day. However, this module has some limitations expressing recharge variation when the delay time is too long and transient recharge trend does not match to the groundwater table time series, the multi-reservoir storage routing module which represents more realistic time delay through vadose zone is newly suggested in this study. In this module, the parameter related to the delay time should be optimized by checking the correlation between simulated recharge and observed groundwater levels. The final step of this procedure is to compare simulated groundwater table with observed one as well as to compare simulated watershed runoff with observed one. This method is applied to Mihocheon watershed in Korea for the purpose of testing the procedure of proper estimation of spatio-temporal groundwater recharge distribution. As the newly suggested method of estimating recharge has the advantages of effectiveness of watershed model as well as the accuracy of WTF method, the estimated daily recharge rate would be an advanced quantity reflecting the heterogeneity of hydrogeology, climatic condition, land use as well as physical behaviour of water in soil layers and aquifers.

A Survey on the Perception and Usage Status of Dietitians in Food Service Business for Meat Products (단체급식소 영양사의 육가공품에 대한 인식 및 이용실태 조사)

  • This study attempted to investigate the perception and usage status for meat products of dietitians in the food service business in Seoul and Gangwon province. 32.8% of dietitians decided the menu reflecting the taste of students. Most of the dietitians (89.2%) checked the manufacture date of the processed meat products when they were supplied. To address the question as to whether they could distinguish the difference among ham, pressed ham, and sausage, most of them answered; 'can distinguish a little' (47.5%) or 'can't distinguish' (36.7%). The most frequently provided processed meat products were ham (33.9%), followed by sausage (21.7%). However, it is assumed that the dietitians might have mistaken the pressed ham for the classic ham. The most common frequency of the serving processed meat product in the food services was once a month (41.6%). Most desired aspect of quality improvement for the processed meat products was answered as 'hygiene/safety' (31.5%), and followed by 'higher quality' (26%). In conclusion, the improvement of the product quality and the strengthening of public information would be prerequisites for expanding the use of processed meat products in the food services and business-to-business (B2B) market in the future.
