SURGICAL EXTRUSION OF THE CROWN-ROOT FRACTURED INCISORS: CASE REPORTS (외과적 정출술을 이용한 치관-치근 파절된 미성숙 영구치의 치험례)

  • Crown-root fractures occur throughout both crown and root, and are defined as fractures involving enamel, dentin and cementum. The fractures may be grouped according to pulpal involvement into complicated and uncomplicated one. Crown-root fractures often occur on maxillary anterior teeth and comprise 5% of injuries affecting the permanent dentition and 2% in the primary dentition. To restore crown-root fractured tooth, biologic width must be maintained. For maintaining biologic width, such methods as gingivectomy following osteoplasty or orthodontic extrusion or surgical extrusion are available. Surgical extrusion is a method that extracts the tooth and replants the fractured tooth supragingivally. It is indicated when the length of the crown fragment is less than half the length of the clinical root. In these cases, root canal treatment and crown restoration using light-cured composite resin were performed after surgical extrusion. In following periodic examinations, favorable outcome was observed.

Petrology of the Cretaceous Igneous Rocks in the Mt. Baegyang Area, Busan (부산 백양산 지역의 백악기 화산-심성암류에 대한 암석학적 연구)

  • The Mt. Baegyang in Busan, composed of sedimentary basement rocks (Icheonri Formation), andesite (lava), andesitic pyroclastic rocks, fallout tuff and tuffaceous sedimentary rocks, rhyolitic pyroclastic rocks, intrusive rocks (granite-porphyry, felsite, and biotite-granite) of Cretaceous age in ascending order. The volcanic rocks show a section of composite volcano which comprised alternation of andesitic lava and pyroclasitc rocks, rhyolitic pyrocalstic rocks (tuff breccia, lapilli tuff, fine tuff) from the lower to the upper strata. From the major element chemical analysis, the volcanic and intrusive rocks belong to calc-alkaline rock series. The trace element composition and REE patterns of volcanic and plutonic rocks, which are characterized by a high LILE/HFSE ratio and enrichments in LREE, suggest that they are typical of continental margin arc calc-alkaline rocks produced in the subduction environment. Primary basaltic magma might have been derived from partial melting of mantle wedge in the upper mantle under destructive plate margin. Crystallization differentiation of the basaltic magma would have produced the calc-alkaline andesitic magma. And the felsic rhyolitic magma seems to have been evolved from andesitic magma with crystallization differentiation of plagioclase, pyroxene, and hornblende.

Full Mouth Rehabilitation in a Patient with Limited Restorable Space (수복 공간이 부족한 환자에서의 완전구강회복)

  • Loss of posterior support may cause overloading and excessive wear of remaining teeth. Moreover, the extrusion of antagonistic teeth leads to the destruction of the occlusal plane. The loss of vertical dimension of occlusion (VDO) also emerges clinically, which may bring the loss of esthetic appearance and function. These patients who suffer from the loss of posterior support, often require vigorous periodontal treatments (osteotomy, crown lengthening) and extensive oral rehabilitation. Sixty three years old female patient visited for the prosthetic treatment of the posterior edentulous area. She had no other systematic disease and parafuctional habits for prosthetic treatment. Intraoral and radiographic examinations were done. The evaluation of VDO and vertical dimension of rest position were evaluated for proper prosthetic procedures and diagnostic wax up was done. As a result of diagnosis, VDO was increased by 2 mm considering the loss of VDO and space for the prosthetic treatment. After the pretreatments, initial preparation of teeth and provisionalization were carried out. Six weeks later of provisionalizaion, final preparation and impression was performed. Using the duralay resin copings, jaw relation was registered. The master cast was mounted and definitive restoration was fabricated. After the evaluation of esthetic and function, pick up impression for clinical remounting was done. Lucia jig was made for new jaw relation and occlusal adjustment on the articulator. Definitive restoration was delivered and the patient was periodically recalled for additional occlusal adjustment. From this case, the satisfactory functional and esthetic results through full mouth rehabilitation with increase vertical dimension were achieved.

선캠브리아 홍제사 화강암의 진화과정(한국 북동부지역의 원생대의 화성활동과 변성작용)

  • The Precambrian Hongjesa granite is lithologically zoned from biotite granite in central part to biotite-muscovite granite towards the margin. The X_{Fe}$ (=Fe/(Fe+Mg)) value and the aluminum saturation index of biotite systematically vary as a function of mineral assemblage, and are positively related with those of bulk rock. This relationship as well as the lithological zoning are attributed to the fractional crystallization of the Hongjesa granitic magma. The trace element data corroborate that biotite-muscovite granite is more fractionated than biotite granite. The evolution of the Hongjesa granite is elucidated by using the AFM liquidus topology, where A=$Al_2O_3-CaO-Na_2O-K_2O$; F=FeO+MnO; and M=MgO. At an early magmatic stage where biotite is the only ferromagnesian mineral to crystallize, the X_{Fe}$ value and the alumina content of granitic magma continuously increase.. Muscovite subsequently crystallizes with biotite along the biotitemuscovite cotectic curve where biotite-muscovite granite forms. Local enrichments in Mn and B further crystallize garnet and tourmaline, respectively. The unique zonal pattern characterized by the occurrence of the evolved biotite-muscovite granite at the margin may be accounted for by the passive stoping during the emplacement of the Hongjesa granite. This emplacement may have occurred in continental collision environment, according to the tectonic discrimination diagram using major element chemistry.

Phenocryst Composition of Mafic Volcanic Rocks in the Wangtian'e Volcano (망천아 화산 고철질 암석의 반정광물 조성 연구)

  • There are beautiful scenery with columnar jointing at 15 valley of southern slope of the Wangtian'e volcano in Mt. Baekdu volcanic field. The compositions of phenocryst minerals which have porphyritic textures in mafic volcanic rocks of this area were carried out. The Wangtian'e volcano consists of Changbai basalt~trachybasalt (lower part) and Wangtian'e basaltic trachyandesite~trachyte~alkali rhyolite (upper part). This study is focused on the mafic rocks of the Changbai trachybsalt and the Wangtian'e basaltic trachyandesite. Main phenocrysts are feldspar, pyroxene and olivine. The major element compositions of the phenocrysts were analyzed using EPMA. Plagioclase phenocrysts of the Wangtian'e basaltic trachyandesite are located at the border of andesine and oligoclase ($An_{24.1{\sim}36.0}$) in the An-Ab-Or diagram, and those of the Changbai trachybasalt are labradorite ($An_{54.2{\sim}65.2}$). Pyroxene phenocrysts are augite. Olivine phenocrysts of the Changbai trachybsalt are crysolite ($Mg_{0.79-0.77}Fe_{0.21-0.23}$) and microphenocrysts in the groundmass are hyalosiderite ($Mg_{0.58-0.56}Fe_{0.42-0.44}$). Calculated crystallization temperature of olivine phenocrysts is $1196{\sim}1123^{\circ}C$, clinopyroxene is $1122{\sim}1112^{\circ}C$, phenocrysts and laths of plagioclases are $1118{\sim}1107^{\circ}C$ and $1091{\sim}1089^{\circ}C$, respectively. The temperatures suggests that the olivine phenocrysts, clinopyroxene, plagioclase phenocrysts, and plagioclase laths were crystallized in the magma chamber in sequence.

Oral rehabilitation of a patient with collapsed occlusal plane resulting from loss of posterior teeth (구치부 상실로 인해 교합 평면이 기울어진 환자에서 임플란트를 이용한 구강 회복 증례)

  • Restoring lost teeth is very important in terms of both function and aesthetics. If tooth loss occurs in the posterior region and the loss of support is persistent, it may cause a gradual shift in the position of the mandible and a change in occlusion. This clinical case attempted to restore support for the posterior teeth with a fixed prosthesis using implants in a patient whose opposing teeth were erupted and the occlusal plane collapsed due to long-term loss of the maxillary left posterior teeth and mandibular right first molars. To correct the occlusal plane of remaining dentition, wax-up of maxillary left posterior teeth was duplicated with acrylic resin and placed on maxilla. Surgical template for implant placement were fabricated using digital technology. After the support of the posterior teeth was restored with the placement of the implant, stable occlusion with temporary restorations was observed for a sufficient time. Afterwards, monolithic zirconia prosthesis was placed on the patient to ensure functional and aesthetic improvement.

  • In case of implant-tooth connected prosthesis, a natural tooth tends to intrude. There are several mechanisms that explain an intrusion phenomenon. So it is reco mmended not to connect an implant with a natural tooth. A 68-year-old female had upper left $2^{nd}$ premolar and $2^{nd}$ molar extracted and underwent implant surgery on the missing area. We made an implant prosthesis and treated upper left $1^{st}$ molar with a gold crown. 2.5 year later, the patient complained about loose proximal contact and food impaction between upper left $1^{st}$ molar and $2^{nd}$ molar. Mesial side of upper left $2^{nd}$ molar implant prosthesis was soldered so that proximal contact became tight again. But after 7 months, about 2 mm intrusion of upper left $1^{st}$ molar occurred, and the patient felt periodontally originated pain on intruded upper left $1^{st}$ molar. After the gold crown on upper left $1^{st}$ molar was removed, extrusion occurred and pain was relived.

  • Loss of posterior support may cause extrusion of antagonistic teeth and excessive wear of remaining teeth which often leads to the destruction of the occlusal plane. In such cases, it is critical to verify the need to increase vertical dimension of occlusion (VDO). Should you increase VDO, provisionalization is crucial in evaluating function and esthetics. Double scanning technique is a useful method when fabricating definitive restoration that mimic provisional restoration. In this case, a patient with apparently no loss of VDO and insufficient interocclusal space for dental materials due to loss of posterior support and extrusion of antagonistic teeth was rehabilitated using double scanning technique.

  • In the primary dentitions, the majority of dental injuries involve the anterior teeth, especially the maxillary primary central incisors. When injuries affecting primary and permanent teeth are compared, it appears that trauma to the primary dentition is usually confined to the supporting structures, i.e. luxation and exarticulation, while the largest proportion of injuries affecting the permanent dentition is represented by crown fractures. But, cases reporting trauma affecting primary molars are unusual in the literature and several reports describe fractures of posterior teeth. The main goal of this report is to describe the repositioning treatment using removable appliances to an uncommon case of lingual displacement of primary molar that happened to a 4 year 5 month-old female child.

Occurrence and Genesis of Zeolites from the Tertiary Volcanic Sediments in the Guryongpo Area, Korea (浦項 九龍浦 지역 第 3 紀 火山堆積岩 中의 沸石鑛物의 産出狀態와 成因)

  • Clinoptilolite and mordenite are important constituents of the Nuldaeri Trachytic Tuff and Guryongpo Dacitic Tuff of the Tertiary Janggi Group which were deposited in a lacustrine environment. The diagenetic cystallization sequences of zeolites in different tuffaceous sediments and their chemical behaviors have been studied to know the process of their formation. The paragenetic sequence established from textural observations and chemical data : Ca-smectite ${\leftrightarrow}$(Ca, K)-clinoptilolite${\leftrightarrow}$(K, Na)-mordenite, indicates that the chemical activities of alkalic ions and Si/Al activity ratio in pore fluids changed systematically with diagenetic alteration. The chemical trend of zeolite formation is characterized by decreasing Ca and Mg, non-variable Na and increasing K in the Nuldaeri Trachytic Tuff and by decreasing Ca and Mg, non-variable Na and increasing-decreasing K in the Guryongpo Dacitic Tuff. The paragenesis from glass via smectite to alkali zeolites indicates a sequence of incongruent dissolution reactions and subsequent crystallization. Inhomogeneity in chemical composition of each zeolite may be attributed to such processes.
