A study on the treatment of anterior open bite with midpalatal miniscrews (정중구개부 미니스크류를 이용한 개방교합치료에 대한 연구)

  • Recently several techniques have been reported for the treatment of anterior open bite in adults characterized by molar intrusion with skeletal anchorage. In this study, five adult patients who had anterior open bite malocclusion were selected to undergo upper molar intrusion with midpalatal miniscrew to close the open bite. The aim of this study is (1) to validate true intrusion of molars in adults (2) to test the usefullness of midpalatal miniscrews as anchorage for intruding upper molars (3) to evaluate the skeletal and dental changes of open bite closure. The results are as follows. (1) All had true intrusion of the maxillary molars. Mean amount of molar intrusion was 3.4mm(range 1.5-5mm). (2) No movement of midpalatal miniscrew occurred during their use. (3) Open bite closure was achieved for all 5 patients. The mandible closed and B-point rotated anteriorly and upward. The mandibular plane angle and the occlusal plane angle decreased.

Miniscrew Installation Area and Condition on Maxillary Palatal Side (상악구개측 미니스크류 식립위치 및 조건)

  • Anchorage control is important in orthodontic treatment. Recently miniscrew is widly used as maximum anchorage in orthodontic treatment, and then it is important to install miniscrew safely without damaging adjacent anatomic structure. In a view of Miniscrew's stability, maxilla is unfavorable than mandible, and moreover maxillary soft buccal bone has disadvantage on stability. so palatal area comes into notice for installation area. We measured distance between palatal roots and bone thickness at midpalatal area using 3D computed tomography, and have found following results. 1. On the comparison of distance between palatal roots, the distance between 2nd premolar and 1st molar was significantly longest and the distance between premolars was significantly shortest. 2. Going toward lateral area from midpalatal suture and posterior area from zero point, bone thickness significantly became shorter and shorter. And 5.0mm palatal sagittal plane has more significance decrease of bone thickness than 2.5mm palatal sagittal plane. According to these results, we can conclude that the palatal installation of miniscrew between 2nd premolar and 1st molar is safest. And it is more safe that comes closer to midpalatal suture and to anterior area in regard to incisive canal.

Comparison of palatal bone thickness between 3D model and lateral cephalometric radiograph (성인 구개부 측모두부계측방사선 사진과 3D model의 골두께 비교)

  • Objective: This study compared the bone thickness of the palate between lateral cephalogram and 3D model measurements. Methods: The subjects consisted of 30 adults (15 men,15 women) with a normal skeletal pattern and occlusion. The CT images were transformed to a 3D model, and were compared with the cephalometric image. Descriptive statistics for each variable were calculated. Results: In the 3D CT model, the mid-palatal area was the thickest part. It became thinner as the palate tapered laterally. In the male group, the thinnest portion was positioned 6 mm away from the mid-palate, while in the female group the thinnest portion was 8mm away from the mid-palate. Correlation analysis between the lateral cephalometric and 3D CT model revealed a significant correlation except in the mid palatal area and the area 2 mm lateral to the mid-palate in men, whereas there was a significant relationship in every area in the women. In both men and women, the highest correlation appeared in the area 8 mm lateral to the mid palate. Conclusions: Using regression analysis, an actual prediction of the bone thickness between the measured bone thickness of the lateral cephalometric radiograph and 3D model was made. This will provide useful information for mini-implant length selection when inserting into the palate.
