Treatment Strategies for Psychotic Depression (정신병적 우울증의 치료 전략)

  • Objectives : Several factors, such as biological markers, clinical correlates, and course of the depressive disorders with psychotic symptoms differ from those without psychotic symptoms. Therefore, specification of a treatment algorithm for depressive disorder with psychotic symptoms is legitimated. This article provides a systematic review of somatic treatments for depressive disorder with psychotic symptoms. Methods : According to the search strategy of the Clinical Research Center for Depression of Korean Health 21 R & D Project, first, PubMed and EMBASE were searched using terms with regard to the treatment of depressive disorders with psychotic symptoms(until July 2006). Reference lists of related reviews and studies were searched. In addition, relevant practice guidelines were searched using PubMed. All identified clinical literatures were reviewed and summarized in a narrative manner. Results : Treatment options, such as a combination of an antidepressant and an antipsychotic versus an antidepressant or an antipsychotic alone are summarized. In addition, issues regarding the electroconvulsive therapy( ECT), combination therapy, and maintenance treatment are discussed. Conclusion : In former times, the combination of an antidepressant and an antipsychotic or ECT were recommended as the first line treatment for depressive disorder with psychotic symptoms. Recently, however, there was a suggestion that there was no conclusive evidence that the combination of an antidepressant and an antipsychotic drug is more effective than an antidepressant alone. More evidence regarding the pharmacological treatment for depressive disorder with psychotic symptoms is needed.

Hypnotherapy in Cases with Psychosomatic Disorders (정신신체장애의 최면치료)

  • Hypnosis as a traditional healing method, in its recent development, has generated a multitude of techniques. These serve as practical tools which can be combined with other therapy techniques for the treatment of a variety of psychiatric and medical conditions. The empirical evidence for the effectiveness of hypnosis is considerable and proves its clinical impact in various areas of application. This case review describes the integration of hypnotherapeutic methods into the continuum of psychiatric encounters in a general practice. Guidelines for the application of hypnosis in approaching and treating each patients with headache, sexual dysfunction and bronchial asthma were illustrated. As hypotheses mechanism of effectiveness in psychosomatic disorders has been formulated. Training in hypnotherapy provides the psychiatrist with skills needed to address psychophysiological disorders. Emphasis is placed on the necessity and opportunity for research on the efficacy and specific technique of hypnosis in the psychosomatic disorders.

  • A large proportion of patients with schizophrenia show a poor response to first-line antipsychotic drugs, which is termed treatment-resistant schizophrenia. Previous studies found that a different neurobiology might underlie treatment-resistant schizophrenia, which necessitates the development of different therapeutic approaches for treating treatment-resistant schizophrenia. This study reviewed previous studies on the pathophysiology of treatment-resistant schizophrenia and the pharmacological intervention, and forthcoming investigations of treatment-resistant schizophrenia are suggested.

  • Attention deficit/hyperactivity disorder is one of the most comminly treated conditions in the child psychiatric units and results in substantial impairment in peer, family and academoc functioning. For 70% to 80% of children with a diagnosis of ADHD, stimulant tratment results in successful improvement of the core ADHD symptoms. However, children with ADHD have high level of comorbidity and may respond differently from children with ADHD without comorbidity. Therefore they may need specific treatment plan. We reviewed efficacy and safety of prescribed medication for treating children with ADHD, the relationship between ADHD and the comorbid conditions and treatment algorithm projects of ADHD with/without comorbid conditions performed Korean and American child psychiatrists. Our main objective is to increase the uniformity of treatment and improve the clinical outcomes of children with comorbid ADHD.

  • This study aims to understand main contents of the National Mental Health Strategy and changes after mental health reform through the National Mental Health Strategy in Australia, and to find some convergence policy implications on mental health policy in South Korea. The direction of reform has changed considerably over the 20 years that the National Mental Health Strategy has been in place including the National Mental Health Policy(1992, 2008), the National Mental Health Plan(1993~2014), COAG National Action Plan on Mental Health(2006~2011), the Roadmap for National Mental Health Reform 2012-2022. The National Mental Health Strategy has advocated fundamental change in the national spending on mental health, mental health service provision, the expansion of community-based mental health services and care system, NGO, consumer and carer participation in mental health care.

Assessment and Treatment of Somatization (신체화의 평가 및 치료)

  • Somatization is regarded as a process rather than a diagnostic entity. It should be emphasized to identify psychopathology rather than to make a choice regarding diagnosis in assessing somatizing patients. Psychiatrists should be aware of the psychosocial cues underlying the patients' physical symptoms. Special skills and strategies are required by nonpsychiatric physicians to facilitate the patients' acceptance of psychiatric treatment. The goal of treatment for somatization is management but not cure. The approach should be flexible, depending on the patients' responses and need. The difficulty in diagnosing and treating somatization is likely to be related to abnormal illness behavior such as the patients' denial of their psychosocial problems and resistance to psychiatric approach. In conclusion, biopsychosocial approach is needed to treat these patients effectively. Psychiatrists should also teach other physicians the interview skill that they could identify these patients as early as possible and facilitate their acceptance of psychiatric treatment.

  • While the therapeutic efficacy of antipsychotic drugs is not in doubt, a variety of undesirable side effects are common. They can be a disincentive to good compliance with treatment, resulting in increased possibilities for relapse and hospitalization. They can be distressing and disabling and thus interfering with patient safety and quality of life. Furthermore, they may be counter-therapeutic by exacerbating the condition that the drug was prescribed for. In this article, we will provide an overview of management of antipsychotic- induced side effects, with a particular emphasis on the most common side effects as well as less common but serious side effects. In addition, some practical issues regarding the management of side effects will be discussed.

  • Objective : This study aimed to establish a consensus on treatment strategies for psychological therapy in generalized anxiety disorder (GAD) as outlined in the 2024 Korean guidelines for treating GAD. Methods : The executive committee developed questionnaires on treatment strategies for patients with GAD based on previous treatment guidelines and academic articles published internationally and in Korea. Sixty-two experts responded to questionnaires developed. The consensus of expert opinions was classified into three categories (first-line, second-line, and third-line choices) using chi-square test and 95% confidence intervals. Results : Findings of this study suggest that individual cognitive behavioral therapy (CBT), group CBT, mindfulness-based cognitive therapy (MBCT), acceptance and commitment therapy (ACT), and applied relaxation are appropriate psychotherapies for GAD. Additionally, it is recommended to initiate CBT early when treating GAD, with eight treatment sessions being the most preferred for both individual and group CBT. Conclusion : This expert consensus survey reflected clinical experiences of Korean practitioners. Results of this study provide valuable insights for Korean clinicians when implementing psychotherapy for GAD.

  • The author introduced 12 session short term dynamic psychotherapy for the intervention of stress related disorders. An examplary case was introduced to suggest the clinal usefulness of time limitted short term dynamic psychotherapy for stress related disorders. It was suggested that the term "stress" should be limitted to certain natural and man-made catastrophies and disasters whose consequences rep quire a total reorganization of the person. Common defensive manuvers and psychological responses in the stress disorders were also reviewed.
