운동기능과 정신장애 - 정신분열병을 중심으로 - (Motor Function and Neuropsychiatric Disorders - With a Focus on Schizophrenia -)

  • Many disorders in neuropsychiatric field demonstrate variable motor disturbances as their clinical feature or in their courses of illness and also due to psychopharmacological treatment. Although association of such motor disturbances with the pathophysiological aspect of various neuropsychiatric illness are still lacking, some form of motor disturbance offer a window through which pathophysiologic mechanism of such illnesses can be viewed. Cognitive control of motor functions are briefly reviewed in this article and the importance and method of motor function assessment in major neuropsychiatric disorders are also discussed. Motor dysfunction of major neuropsychiatric illness such as schizophrenia and mood disorders may offer a chance of a deeper understanding on the pathophysiologic aspect of their clinical presentation.

들뢰즈, 가타리의 분열분석에서억압과 무의식의 문제 (The Problem of the Repression and the Unconscious in Delueze and Guattari's schizo-analysis)

  • 들뢰즈, 가타리는 "안티-오이디푸스"에서 프로이트, 라캉의 욕망론을 결핍으로서의 욕망인 관념론적 환상이라고 비판하면서 유물론적 욕망론의 가능성을 모색한다. 이들은 프로이트, 라캉이 욕망을 오이디푸스 가족 모델에 가두고, 욕망의 리비도의 흐름을 차단했다고 비판한다. 이 글은 욕망에 대한 정신분석 계열과 분열분석 계열의 해석이 상이한 이유를 이들의 주요 개념 가운데, '억압'과 '무의식'에 대한 해석의 차이에서 찾는다. 그에 따라 우선 프로이트, 라캉의 억압의 양상을 살피고, 이들이 보지 못한 '심리적 억압'과 '사회적 억압'의 구별의 유의미를 들뢰즈, 가타리의 해석에 따라 추적한다. 그 다음, 프로이트, 라캉은 무의식을 정신 활동의 방어 기제에 의해 축출된 영역이나, 언어처럼 구조화된 무의식의 차원으로 이해한다. 이에 반해 들뢰즈, 가타리는 이들의 무의식에 대한 해석과 전혀 다르게 접근한다. 이 글은 이들의 무의식에 대한 다양하고도 새로운 해석 가운데, 특히 고아인 무의식, 생산적 무의식, 분자적 무의식을 집중해서 분석한다. 결론적으로 무의식에 대한 새로운 해석에 따라 욕망과 억압의 문제도 전혀 다르게 볼 수 있음을 보이고자 한다. 이들의 분열분석이 과연 현실에서 어떻게 적용 가능한지에 대한 실천적 적용 가능성을 탐색해 보고자 한다.

  • 공감적 존재인 Homo Empathicus로서의 인간들이 살아가는 세상임에도 불구하고, 실제로 우리 사회는 집단간의 분열과 갈등이 끊이지 않고 있다. 인간의 공감 반응은 타자의 정의로움에 민감하여, 타인의 정의로움 여부에 따라 타인의 고통에 공감 반응을 보이기도 하고 쾌감을 느끼기도 한다. 그러나, 타자에 대한 우리의 공감 반응은 항상 정의로운 것은 아니며, 내집단 편향성이라는 내재적 한계를 가지고 있다. 즉, 자신과 같은 집단이나 범주에 속하는 사람인지 아닌지에 따라, 우리는 스스로 의식하지도 못한 채 타인의 고통에 편향적으로 공감하기도 하고, 편향적으로 쾌감을 느끼기도 한다. 최근의 정보통신기술 발전은, 내집단 관련 SNS혹은 내집단 매체에 대한 편향적 접근을 용이하게 하며, 이를 통해 집단과 관련한 보다 편향된 의미론적 정보 네트워크의 구축을 심화시킨다. 이러한 현상들은 공감의 내재적 한계 속성들과 상호작용하여 더욱 편향된 내집단 공감 및 내집단 활동을 하게 하는 악순환의 고리를 형성함으로써 분열과 대립을 가속화할 수 있다. 저자는 공감의 신경 기전에 대한 이해를 통해 공감의 속성과 한계를 고찰해보고, 신경과학적 관점에서 공감과 정의의 관련성을 탐색해봄으로써, 분열과 갈등의 현대 사회를 기존의 사회과학적 시각과는 다른 차원에서 조명해보고자 하였다.

정신분열병 환자 자녀의 경험 (The Experience of Children with a Schizophrenic Parent -Case Study-)

  • Purpose: The purpose of this thesis is to investigate the experience of children with a schizophrenic parent. Methods: The research methods included biographical in-depth interviews, structured questionnaires, and child's drawings. Data were collected from September until November, 2005 at one university hospital, and interviews were recorded on audio tapes and thereby transcribed. The subjects were three adolescent children (10~15 years old) of a parent with schizophrenia. Results: This study has five themes: It is often the case that the children with a schizophrenic parent are uninformed about their parent's illness, have difficulties dealing with symptoms are confused about their parent's image and display contradictory feelings of fear and sympathy toward their father; The children tend to harbor expectations on mother's role in the family affairs, and simultaneously show pity for her; Family relations without solidarity caused by the absence of their father make the children feel severance, separation, and isolation in and out of the family; The children feel the burden to assume additional household responsibilities; The children are reluctant to mention their father's disease. Conclusion: the study subjects have met negativeness in both developmental characteristics and needs, which supports the need for intervention programs that meet their actual experience.

정신분열병 환자 가족에 대한 해결중심 집단상담 프로그램의 효과 (The Effects of Solution-focused Group Counseling Program for the Families with Schizophrenic Patients)

  • Purpose: This study was designed to examine the effects of a solution-focused group counseling program on the family burden, active coping, expressed emotion, and family support in schizophrenic patients and their families. Method: The subjects consisted of 48 schizophrenic patients and 56 families. Twenty-four schizophrenic patients and 28 families were assigned to both the experimental and control groups. The solution-focused group counseling program was conducted for the families of the experimental group, but not for the control group or the patients of the experimental group. Result: There was a significant greater decrease in scores of family burden and expressed emotion in the experimental groups than the control groups. There was a larger increase in active coping scores in the experimental groups than the control groups, but it was not significant. There was no significant difference between the two groups in family support scores. Conclusion: This program may be an effective nursing intervention program for families with schizophrenic members.

만성 정신분열병환자의 살아온 경험 (The Daily Experiences of People with Chronic Schizophrenia)

  • Purpose: This research investigated the daily experiences of patients' lives to develop a formal theory that explains the lives of schizophrenic patients. Method: A grounded theory method(Strauss & Corbin, 1998) guided the data collection and analysis. Thirteen patients who were diagnosed with schizophrenia in regional communities participated. Result: The experiences of chronic schizophrenic patients are defined as "escaping from a fallen mine" comparing their suffering to that of entrapped miners trying to free themselves from a collapsed mine tunnel with much difficulty and without hope. In observing participant's time lines of having lived with chronic schizophrenia, it advances with 'surrender', 'collapse', 'reaching out', 'rising', 'preparing to spring up', and 'starting anew'. Conclusion: The results of this study indicate that the experience of a chronic schizophrenic patient is like that of a miner caught under a fallen mine channel, who, without external help cannot escape the depths 'Of the mine, but at the same time must have conviction and hope of rescue and avert fear to cooperate with outside help. The result indicates that family members, doctors and nurses as well as an institute's persistent and active support is most critical for the patient's adjustment to social life.

여성 정신분열병 환자의 항정신성 약물 복용 체험 (The Lived Experience of the Women with Schizophrenia Taking Antipsychotic Medication)

  • Purpose: This study was done to identify some natural meaning through the dosage experience of psychoactive drugs in women patients with schizophrenia. Methods: The Hermeneutic phenomenology written by van Manen was used. The period for data collection was from November 2009 to January 2010. This study took place in mental health hospitals and mental health centers in two cities in North Jeolla Province. Nine patients with schizophrenia participated. Data collection was done through individual in-depth interview. Results: The seven natural subjects demonstrated by participants from this study were 'Pills forcibly taken like veiled threats', 'A terrible side effect, a side effect rooted slowly', 'Shame which cannot be hidden as a woman', 'A bad medicine took away from motherhood', 'The fate of a wife who can't be equal', 'A struggle for the complete recovery without promise', and 'Participants want the future without medicine'. Conclusion: The results of this study indicate that the urgent need to develop a safe and believable psychoactive drug for woman patients considering the time of menstruation, pregnancy, childbirth, and child raising.

정신분열병 환자 가족의 대처 측정도구 개발 (Development of a Coping Scale for Families of Patients with Schizophrenia)

  • Purpose: The purpose of this study was to develop and evaluate a coping scale for families of patients with schizophrenia (CSFPS). Methods: Item construction was derived from literature reviews and interviews with family members and psychiatric nurses. Content validity was tested by experts. Each item was scored on a four-point Likert scale. The preliminary questionnaire was administered to 188 family members of patients with schizophrenia. The data were analyzed using item analysis, factor analysis, Pearson correlation coefficients, and Cronbach's alpha. Results: From the factor analysis, 32 items in five factors were derived. The factors were named active coping strategies, avoidance coping strategies, hospital treatment-oriented coping strategies, emotional coping strategies, and suppressive coping strategies for problematic behaviors. The five factors explained 49.7% of the total variance, Cronbach's alpha of the total items was .83 and the factors ranged from .66 to .86. Conclusion: The results of this study suggest that CSFPS is a reliable and valid instrument to measure coping in families of patients with schizophrenia.

정신분열병 환자의 자기효능과 삶의 질에 관한 연구 (A Study on Self-Efficacy and Quality of Life in Schizophrenic Patients)

  • The purpose of this study is to identify the correlation that exists between quality of life and self-efficacy of schizophrenic patients, focusing on the influence of related factors on these varables. Method: The subjects of this study were 151 schizophrenic patients. The data were collected using questionnaires. The instruments used for this study were the general self-efficacy scale developed by Sherer & Maddex(1982), the specific self-efficacy scale modified and complemented by these co-researchers on the basis of the past studies and modified SIP by Voruganti(1996). The period of data collection was from July. 2000 to Jan. 2001. Data analysis was done by SPSS, t-test, ANOVA and the Pearson Correlation Coefficient. Result: 1. The level of self-efficacy showed a mean score of 60.0 and the level of quality of life, a mean score of 47.0. 2. The general characteristics affecting the self-efficacy of schizophrenic patients were staying with family (p=0.05) and employment (p=0.00). 3. The general characteristics affecting the quality of life of schizophrenic patients were staying with family (p=0.04), employment (p=0.05) and duration of illness (0.03). 4. A positive correlation was identified between self-efficacy and quality of life (r=-0.469, p=0.000). Conclusion: The study suggests that nursing intervention strategy should be worked out to develop a psychiatric rehabilitation program that can promote self-efficacy and thus enhance the quality of life of schizophrenic patients.

  • Purpose: This study was aimed at understanding the nature of the suffering of families with patients in mental health nursing homes and hoped to contribute to the rehabilitation process of those with a chronic mental disorder. Method: Research methodology was based upon Parse's human becoming research methodology. Results: a) Despite the despair the family feels by the violence caused by their now-institutionalized relative, they also realize anew the importance of their role as protectors b) Although they fear social stigmatization they also try to be supportive, out of guilt feelings; c) They regret their severe rearing style and wish to be more sympathetic, d) They find courage and hope through family therapy, which leads to a better understanding of the illness, e) With hopes of rehabilitation, the family members feel happy and go through an emotional release, by sharing the pain with each other. Conclusion: Families of nursing home residents share a focus on the process of human-health-universe. This is a positive,'human-becoming' process with which, based on past feelings of despair, fear, resignation, and pain, one can render meaning into his or her experiences in the present in the pursuit of love, conquest, hope, liberty and success.
