Trevor's Disease; Dysplasia Epiphysealis Hemimelica - A Case Report - (Trevor 병; 편측성 골단 이형성증 - 증례 보고 -)

  • Trevor's disease, also known as dysplasia epiphysealis hemimelica, is rare abnormality characterized by aberrant cartilaginous overgrowth forming one half of epiphysis. Typically the patient is 2 to 14 years of age. Until now only six cases, including a case recurred after surgery, have been reported in Korea. A seven year-old boy complained pain of the left knee. However, any deformity of the knee was not seen. The plain radiographic findings showed multiple ossifications asymmetrically on the medial epiphysis of distal femur. The magnetic resonance imaging (MRI) clearly demonstrated the epiphyseal mass which contained low signal spots corresponding to calcified foci and abnormal cartilaginous overgrowth. We report one additional case with a brief review of literature with the MRI findings.

Fibrosarcoma in the Scapula of a Maltese Dog (말티즈 견의 견갑부에서 발생한 섬유육종)

  • A six year-old male Maltese dog was presented to the Konkuk University Veterinary Teaching Hospital (KUVTH) with mass in right scapula region. Physical examination, radiography, magnetic resonance imaging (MRI), complete blood count (CBC), serum biochemistry, cytological analysis of fine needle aspiration and histopathological examination were performed. There was no remarkable finding in CBC. Alkaline phosphatase (ALP) was found to be increased in serum chemistry profile. Osteolysis was seen on radiographs of the scapula. MRI revealed mixed signal intensity in scapula. Cytological examination of fine-needle aspirate (FNA) was suggestive of sarcoma. We confirmed that the neoplastic cells were fibrosarcoma by immunohistochemical and Masson's trichrome staining.

Cervicofacial Actinomycosis with Orbit Involvement (안구 침범을 동반한 두경부 방선균증)

  • Actinomycosis is caused by filamentous Gram positive anaerobic bacteria from the Actinomycetaceae family, and known as a rare cause of the infection at the eyeball. We report magnetic resonance findings of a 60-year-old Korean man with cervicofacial actinomycosis, including cellulitis in the eye and central nervous system actinomycosis. On orbital magnetic resonance imaging, gadolinium-enhanced T1-weight images showed multiple abnormal enhancing lesions in head and neck including right eye, and some include low signal intensities which considered as abscesses. The lesions was diagnosed as actinomycosis by incisional biopsy, and since then was cured by using antibiotics of penicillin family.

  • Amyloidosis is a multisystemic disease characterized by the accumulation of abnormal proteins in extracellular spaces in various organs, with frequent involvement of the myocardium. We report a case of a patient who had cardiac amyloidosis with a trend of reduction in native T1 and T2 values and extracellular volume fraction on serial cardiac magnetic resonance imaging after chemotherapy and stem cell transplantation. The native T1 value and the extracellular volume fraction are closely associated with tissue amyloid burden in amyloidosis patients. This case demonstrated that cardiac magnetic resonance imaging may be used as a non-invasive and quantitative biomarker in the treatment monitoring of amyloidosis.

  • Purpose : The purpose of this study was aimed to evaluate the BOLD(blood oxygen level dependent) contrast fMRI(functional MR imaging) in the occipital lobe and to compare with the metabolic changes based on H MRS (MR spectroscopy) and MRSI (MR spectroscopic imaging) before and after visual stimulation Materials and Methods : Healthy human volunteers (eight males and two females with 24-30 year age) participated in this study. All of the BOLD fMRI were acquired on a 1.5T MR with EPI during supervised visual stimulation in the occipital lobe. The red flicker with 8Hz was used for visual stimulation. After imaging acquisition, the MR images were transferred into unix workstation and processed with acquired from the same location based on the activation map. MRSI (magnetic resonance spectroscopic imaging) was also acquired to analyze the lactate changes before and after stimulation. Results : The activation maps were successfully produced by BOLD effect due to visual stimulation. NAA (N-acetyle aspartate)/Cr (creatine) ratio varied only from $1.79{\pm}0.28{\;}to{\;}1.88{\pm}0.20$ in activation area before and after stimulation. However, the signal intensity of lactate was elevated $9.48{\pm}4.38$ times higher than before activation. Lactate metabolite images were consistent with the activation maps. Conclusion : The BOLD contrast fMRI is enough sensitive to detect the activated area in human brain during the visual stimulation. Lactate metabolite map presents the evidence of lactate elevation on the same area of activation.

Analysis of $^1H$ Magnetic Resonance Spectroscopy Pattern in Invasive Ductal Carcinoma of Breast (유방 침윤성 관상피암에서 수소핵 자기공명분광상의 특성 분석)

  • To evaluate the potential value of $^1H$ Magnetic resonance spectroscopy (MRS) for detecting and characterizing invasive ductal carcinoma of breast. We conducted $^1H$ Magnetic resonance spectroscopy (MRS), using a 3.0T MR scanner, on 40 patients who were histologically diagnosed to have invasive ductal carcinoma (IDC); tumor areas of the patients were designated as experimental samples, and non-tumor areas as control samples. The peak at 3.2 ppm is characteristically intense and observed in 34 cases of the total 40 invasive ductal carcinoma (sensitivity 86.2%; specificity 100%; positive predictive value 100%; negative predictive value 60%). In constrast peak at 1.3 ppm is characteristically intense and observed in normal breast (sensitivity 86.2%; specificity 100%; positive predictive value 100%; negative predictive value 60%). The study shows that $^1H$ MRS can effectively discriminate invasive ductal carcinoma from normal breast in most cases. It also demonstrates the feasibility of localized in vivo $^1H$ MRS technique as a new diagnostic modality in the detection of breast tumor.

  • This study involves an experiment using functional magnetic resonance imaging(fMRI) to delineate brain activation for execution functional performance. Participates to this experiment of the normal adult (man 4, woman 6) of 10 people, is not inserts the metal all closed phobia and 24.5 year-old average ages which the operating surgeon experience which are not they were. With functional MRI resultant execution function inside being relation, the prefrontal lobe, anterior cingulate gyrus, parietal lobe, orbitofrontal gyrus, temporal lobe, parietal lobe was activated. The execution function promotes a recovery major role from occupational therapy, understanding about the damage mechanism is important. When confirms the brain active area which accomplishes an execution function brain plasticity develops the cognitive therapeutic method which is effective increases usefully very, will be used.

Perfusion RRI of the Brain Using Oxygen Inhalation (산소 호흡을 이용한 뇌의 관류 자기공명영상)

  • Purpose : To know the possibility of clinical application of MRI using oxygen inhalation as a perfusion MRI Materials and methods : Two healthy volunteers and three patients of one moyamoya disease, one acute infarction and one meningioma were studied using a 1.5 Tesla MRI unit. Oxygen (15 liters/min) mixed with room air was given using face mask from 8 second to 35 second during the study. Images were acquired 25 times (scan time per study were 1.6 seconds) using susceptibility contrast EPI (echo planar image) sequence. Difference maps were acquired by early (study 12-18), and late (study 19-25) O2 inhalation image groups minus pre-O2 inhalation image group (study 3-9) with a Z-score of 0.7-1.0 using VB31C program of Magneton Vision. The resulting perfusion images were created by superimposition of difference maps on corresponding T1 weighted anatomic images. On moyamoya patient, similar perfusion images were acquired after Gd-DTPA injection, and compared with O2 inhalation perfusion images. Results ; The author can get the perfusion images of the brain by oxygen inhalation with susceptibility contrast EPI sequence at the volunteers, and the patient of moyomoya disease, acute infarction and meningioma. On moyamoya patient, perfusion images with O2 inhalation are similar with perfusion images by Gd-DTPA injection. Conclusion 1 This study has demonstrated that the susceptibility contrast EPI by oxygen inhalation can be used as the clinically useful perfusion MRI technique

The Study of Vascular Dynamics for the Effect of a Compress Pack on Pain Relief using Magnetic Resonance Angiography (자기공명영상을 이용한 찜질 자극에 의한 통증 완화 효과에 대한 혈류 영상학적 고찰)

  • This study was to investigate the effects of the hot compress pack on alleviating local muscular discomfort, stiffness in limbs as well as the chronic pains such as migraine in terms of hemodynamics. In this study, the hot compress band was put on the neck and the local physiological change on the stimulation site and the cranial blood circulation change were examined. We recruited healthy volunteers (n=8, mean age: 32.13 (4.61)), who participated in the magnetic resonance imaging (MRI) study. Local skin color and temperature were measured for the local effect of the hot compress band and the changes of intra-cranial and extra-cranial blood vessels were examined with MR angiography (MRA) images. The skin temperature increased from $36.4^{\circ}C$ at the rest condition to $36.7^{\circ}C$ and $37.1^{\circ}C$ after 15 min and 30 min stimulation, respectively. The change of the extra-cranial blood vessels between pre-stimulation and post-stimulation of 30 min was significantly increased (+38.8%), while the change of the intra-cranial blood vessels was negligible. In this study, we demonstrated that the hot compress band on the neck yielded the increase of local skin temperature on the stimulation site and it made an effect on the extracranial circulation. In conclusion, the stimulation with a hot compress could facilitate the blood circulation, causing to relieve the muscular discomfort, stiffness in limbs as well as the chronic pains such as migraine.

Correlation of the Neuropsychological Screening Battery (NSB) and Neuroanatomy for the Parkinson's Disease with Mild Cognitive Impairment by Using the Analysis of Cerebral Cortex Thickness in the Brain MRI (뇌 자기공명영상에서 대뇌 피질 두께 분석법을 이용한 파킨슨병의 경도인지장애 신경심리검사와 신경해부학적 상관관계)

  • This study is to investigate neuro-anatomical correlation between neuropsychological results and cerebral cortex thickness of cognitive ability in the brain MRI targeting the patients with mild cognitive impairment. It was that 78 people who were diagnosed as first Parkinson's disease followed by neuropsychological screening battery(Parkinson's disease with mild cognitive impairment: 39 people; Parkinson's disease with normal cognition: 39 people) and 32 people of normal group were selected. Correlation between mild cognitive impairment and normal cognitive impairment and correlation between neuropsychological screening battery and cerebral cortex thickness in the brain MRI were performed by independent sample t-test or Pearson correlation coefficient and then level of significance of collected data was verified in p<0.05. As a result, cerebral cortex thickness of the Parkinson's disease with mild cognitive impairment in both side precuneas and right inferiortemporal lobe had statistically significant decrease. In addition, function of visuospatial ability, verbal and visual memory was reduced in neuropsychological screening battery for cognitive assessment. Especially, there was correlation between neuropsychological screening battery of verbal and visual memory anatomical left precuneus.
