Perfusion MR Imaging of Cerebral Gliomas: Comparison with Histologic Tumor Grade (대뇌 교종의 관류 자기 공명 영상: 조직학적 종양등급과의 비교)

  • Purpose : Our purpose was to compare maximum relative cerebral blood volume (rCBV) with histologic grade of cerebral gliomas. Materials and methods : First-pass perfusion MR imaging was performed preoperatively in 16 patients with pathologically proven cerebral gliomas (7 glioblastoma, 2 anaplastic astrocytoma, 1 anaplastic oligodendroglioma, 5 low-grade astrocytoma, and 1 low-grade oligodendroglioma). Maximum rCBV was compared with histologic diagnosis and grade of the tumor. Results : Maximum rCBVs of glioblastomas were in the range of 433% to-1330% (average, 790 %), as compared with those of contra-lateral normal white matters. Maximum rCBVs of two non-enhancing anaplastic astrocytomas were 66% and 284%, respectively. Maximum rCBV of one well-enhancing anaplastic oligodendroglioma was 702%. Maximum rCBVs of low-grade astrocytomas were in the range of 80%-369% (average, 202%). Maximum rCBV of one low-grade oligodendroglioma was 1450%, even higher than those of glioblastomas. Conclusion : Maximum rCBV was higher in glioblastoma than in low-grade astrocytoma without overlapping. However, there was no difference of maximum rCBV between non-enhancing anaplastic astrocytoma and low-grade astrocrtoma.

Canine MR Images from 3T Active-Shield MRI System (3T 능동차폐형 자기공명영상 장비로부터 얻어진 개의 자기공명영상)

  • For veterinary imaging diagnosis, we obtained MR images of the canine brain, spine, kidney and pelvis from 3T MRI system which was equipped with the world first 3T active shield magnet. Spin echo (SE) and fast Spin Echo (FSE) images were obtained from the canine brain, spine, kidney and pelvis of normal and sick dogs using a homemade birdcage and transverse electromagnetic (TEM) resonators operating in quadrature and tuned to 128 MHz. In addition, we employed a homemade saddle shaped RF coil. Typical common acquisition parameters were as follows: matrix=512$\times$512, field of view (FOV)=20cm, slice thickness=3 w, number of excitations (NEX)=1. For T1-weighted MR images, we used TR=500 ms, TE=10 or 17.4 ms. For T2-weighted MR images, we used TR=4000 ms, TE=108 ms. Signal to noise ratio (SNR) of 3T system was measured 2.7 times greater than that of prevalent 1.57 system. The high resolution images acquired in this study represent more than a 4-fold increase in in-plane resolution relative to conventional images obtained with a 20 cm field of view and a 5 mm slice thickness. MR images obtained from 3T system revealed numerous small venous structures throughout the image plane and provided reasonable delineation between gray and white matter The present results demonstrate that the MR images from 3T system could provide better diagnostic quality of resolution and sensitivity than those of 1.5T system. The elevated SNR observed in the 3T high field magnetic resonance imaging can be utilized to acquire images with a level of resolution approaching the microscopic structural level under in vivo conditions. These images represent a significant advance in our ability to examine small anatomical features with noninvasive imaging methods. Moreover, MRI technique could begin to apply for veterinary medicine in Korea.

Symptomatic Abnormal Insertion of the Anterior Horn of Medial Meniscus - A Case Report - (증상을 유발하는 내측 반월상 연골 전각부의 비정상적 삽입 - 1예 보고 -)

  • We report a case of a girl with a symptomatic anomaly of medial meniscus. The complaint of the patient was pain and snapping of the knee. The anterior horn of medial meniscus has anomalous insertion which was extended to the intercondylar notch of the femur on the surface of the anterior cruciate ligament, it attached to lateral wall of medial femoral condyle. This anomalous band was not detected in MRI of knee but found during the operation. After resection of anomalous band, the symptoms completely disappeared.

Evaluation of Renal Oxygenation in Normal Korean Volunteers Using 3.0 T Blood Oxygen Level-Dependent MRI (3.0 T 혈중산소치의존 자기공명영상을 이용한 정상한국인에서의 신장 산소공급의 평가)

  • Purpose : Renal blood oxygen level-dependent (BOLD) MRI has been used in the evaluation of renal oxygenation. We tried to provide the normal $R2^*$ value of the human kidney with 3.0 T, and evaluated the differences in $R2^*$ values according to gender and location. Materials and Methods: Twenty-four healthy volunteers underwent BOLD MRI at 3.0 T. Multi gradient echo-echo planar imaging sequence with seventeen echoes was used. After generation of the $T2^*$ map, the $R2^*$ was calculated. The statistical differences in $R2^*$ values between the cortex and medulla, males and females, and the right and left kidney were analyzed. The regional differences of $R2^*$ within the both kidneys were evaluated respectively. Results: BOLD MRI was successful in all participants. No gross artifact interfered with $R2^*$ measurement. The mean $R2^*$ at 3.0 T was $17.1{\pm}2.60s^{-1}$ in the cortex and $27.7{\pm}4.83s^{-1}$ in the medulla (p < 0.001). The $R2^*$ value in the medulla was significantly higher in the male than female volunteers (p = 0.025). There were no statistical differences of $R2^*$ according to the side and location in the kidney (p = 0.197). Conclusion: Renal BOLD MRI can be efficiently performed with 3.0 T MRI. Renal medullary hypoxia is present in normal volunteers. Our results may be used as reference values in the evaluation of pathologic conditions using BOLD MRI.

Brain Activation During False-Belief Task Performance in Korean Healthy Adults: An fMRI Study (한국 정상 성인의 틀린 믿음 과제 수행 시의 뇌 활성화: fMRI 연구)

  • We applied fMRI to examine brain activation during false-belief task in Korean healthy adults. In the first experiment, brain areas including bilateral precuneus, temoporo-parietal junction, left inferior parietal lobule, posterior cingulate, middle frontal gyrus were found during first -order false-belief task. In the second experiment, the left middle frontal gyrus, medial frontal gyrus and right precuneus, middle frontal gyrus, temoporo-parietal junction were activated during second-order false-belief task. These results are compatible with the suggestions that the ways in which adults understand theory of mind stories are universal.

  • Because of the inherent complex anatomy and functional arrangement of the cranial nerves (CNs), neuroimaging of cranial neuropathy is challenging. With recent advances in magnetic resonance imaging (MRI) techniques, the cause of cranial neuropathy can now be detected in many cases. As an active multidisciplinary team member of cranial neuropathy, it is essential for the neuroradiologist to be familiar with the detailed anatomy of the CNs on MRI. This review contains the basic MRI anatomy of CNs III-XII according to a segmental classification from the brain stem to the extracranial region. The optimal imaging options to best evaluate the specific segment of the CNs will also be discussed briefly.

THE PRELIMINARY STUDY OF THE QUANTITATIVE MORPHOLOGIC ANALYSIS USING MRI OF THE LATERAL VENTRICLE IN EARLY-ONSET PSYCHOTIC DISORDER (조기 발병한 정신증적 장애에서 자기공명영상을 이용한 측뇌실에 대한 구조적인 예비연구)

  • Objective:The enlargement of the lateral ventricle is well established finding in adult-onset schizophrenia, but in the patients with early-onset psychotic disorder(EOPD) is still controversal. To identify structural abnormalities of the lateral ventricle in patients with EOPD and difference of the brain developmental course in EOPD group, planimetrical assessment of the lateral ventricle was conducted by means of retrospective quantitative neuroimaging. Methods:The brain MR images of 14 patients with EOPD(mean age $14.0{\pm}2.1$ years) who met the DSM-III-R diagnostic criteria for schizophrenia and bipolar disorder and 24 control subjects with simple headache(mean age $10.0{\pm}2.1$ years) were collected. Single best view for the lateral ventricle, one axial slice was selected for analysis. MR images were redigitalized with flatbed scanner and data were analyzed with NIH IMAGE software. Results:In the patients with EOPD, there were significant correlations between aging and lateral ventricular sizes and between aging and ventricular brain ratio(VBR). The statistical significance of normal asymmetry of the lateral ventricle was not observed for the EOPD group. The EOPD group had larger lateral ventricular sizes and VBR than control group, but these results were not statistically significant. Conclusion:These findings suggest that the brain abnormalities in patients with EOPD progress from the earlier course of the disorders and the course of brain development in EOPD group is different from control group.

The Findings and Significances of Brain SPECT in Acute Mealses Encephalitis (급성 홍역 뇌증 환아들의 뇌 SPECT 소견과 유용성)

  • Purpose : Acute measles encephalitis(ME) is characterized by an abrupt onset of fever and obtundation, frequently accompanied by seizures and multifocal neurological signs. The aim of this study was to clarify the clinical manifestation, progression and the brain SPECT patterns in patients with acute ME. Methods : This study included 11 children with acute ME admitted to Chonbuk National University Hospital. Ten patients received a first dose of measles vaccine, one patient did not receive a first dose, and no patients received a second dose. ME was diagnosed based on characteristic clinical pictures, measles antibodies by ELISA and abnormal CSF findings. Brain MRI and brain SPECT were performed in 11 patients with acute ME. Results : There were four males and seven females whose ages at onset ranged between 18 months and 14 years(mean : 10.5 years). The main clinical neurologic pictures were loss of consciousness( 10) and seizure(five). The titer of IgG and IgM antimeasles antibodies in serum were positive in 10 patients. In CSF, nine patients had IgG antibodies and one patient had IgM antibodies. The concentration of protein(mean : $124{\pm}60mg/dL$) and WBC counts(mean : $158{\pm}157/{\mu}L$) in CSF were elevated in all patients. In electroencephalographic examination, nine patients showed increased slow waves. Seven of 11 patients(63.6%) revealed high signal intensity on the brain MRI. In contrast, all patients showed hypoperfusion in brain SPECT examination. According to brain SPECT, the perfusion deficits were frequently observed in the frontal lobe(nine), temporal (nine), parietal(eight) and thalamus(eight). Conclusion : Brain SPECT is more sensitive than MRI for the evaluation of brain damage in early stages of acute ME.

Discoid Medial Meniscus - A Case Report - (내측 원판형 연골 - 증례 보고 -)

  • The most of discoid meniscus is located in lateral side. Medial discoid meniscus is rare. The medial discoid meniscus was reported by Cave and Staples at first in 1941 but rarely reported since then. A 8 year's old boy was diagnosed as having a complete medial discoid meniscus by means of MRI and arthroscopy. Authors performed the arthroscopic partial menisectomy for medial discoid meniscus. For confirmation of the bilaterality, arthroscopic examination on contralateral knee was performed but we did not find a medial discoid meniscus. We report 1 case of a medial discoid meniscus with a brief review of literatures.

The Effect of Chemical Shift on MR Imaging (화학적이동이 자기공명영상에 미치는 영향)

  • The chemical bond differences between a normal tissue and a fat tissue make a chemical shift artifact which is caused by a primary inacuracy of resonance signal location. The chemical shift also makes a variation of the transverse time T$_2$. An attempt is made to compare the values of SNR(Signal-to-Noise Ratio), the signal response, and the imaging time computed by applying T$\sub$2/$\^$*/ for a fat-proton with ones of those computed by applying T$_2$ for a water-proton under the conditions of T$_1$/T$_2$=3 and T$\sub$2/$\^$*/T$_2$=0.9. The results of the attempt show that the first two reduce to 5% and 8% out of 100%, respectively, and the last rather increases up to 10%. This shows that the chemical shift contributes to the deterioration of an MR imaging efficiency in addtion to the image distortion.
