The Correlation between Benign Laryngeal Mucosal Disease and Metabolic Syndrome (양성후두점막질환과 대사증후군의 상관관계)

  • Background and Objectives : The aim of this study is verify the correlation between benign laryngeal mucosal disease and metabolic syndrome. Materials and Method : Data for patients diagnosed with benign laryngeal mucosal disease and metabolic syndrome from 2006 to 2015 were selected for analysis from the National Health Insurance Service database. Results : The prevalence of Metabolic syndrome was 2,179,785 out of 6,437,051 patients (33.86%). The prevalence of benign laryngeal mucosal disease was 516,594 out of 6,437,051 patients (8.03%). Metabolic syndrome was a risk factor for benign laryngeal mucosal disease [hazard ratio: 0.99, 95% confidence interval: 0.984-0.997] after adjusting for age and other variables including age, gender, smoking status, alcohol intake, exercise, body mass index, and diabetes. The number of metabolic syndrome components was also risk factor. Conclusion : Metabolic syndrome was related to the incidence of benign laryngeal mucosal disease. However this correlation did not seem to be high.

  • 동물과 사람의 연구에서 위장관 영양 인자는 위장관 점막이 점막손상으로부터의 회복에 중요하고 출생 후 경구 영양에 적응할 수 있게 하는 데에 중요하다. 경구적으로든 전신적으로 투여된 성장 인자들은 위장관의 성장과 발달을 촉진시킨다. 신생아 혈중의 영양인자들이 장관 세포의 수용체를 통해 작용하여 위장관의 성장을 조율한다. 위장관 영양인자들은 체내에서 합성될 수도 있고 모유를 통해 공급된다. 사람에서 출생 후 위장관이 장관영양에 신속히 적응할 수 있도록 위장관 영양 인자들이 중요한 작용을 한다. 모유 내의 성장 인자들이 신생아 생존에 필수적인 것들은 않아도 모유를 먹은 영아들이 조제분유를 먹은 영아들에 비하여 급성 설사, 괴사성 장염, 크론씨 병과 같은 위장관 질환의 위험율이 낮다. 위장관 영양 인자들의 대부분이 시판 조제분유에는 존재하지 않고 주로 모유에 존재함을 앎으로써 모유의 장점을 설명하는 데에 적용할 수 있을 것이다. 그리고 위장관 영양인자는 위장관 점막 손상된 경우 치료 목적으로 사용될 수 있는 여지가 높다. 이러한 임상적 이용은 특히 미숙아, 수술 후의 영아 등에서 적용될 수 있다. 그러나 향후 더욱 연구되어야 할 항목들로는 작용기전, 경구 및 정맥 투여 방법에 의한 효과의 차이, 체내 성장 인자들과의 상호 작용, 외부적 투여가 체내 인자에 대한 영향, 위장관 이외의 타 기관에 대한 영향, 그리고 안전성과 약물 역동학적인 특성 등이다.

  • Hemangiomas in the esophagus comprise less than 3% of all benign esophageal neoplasms. They are frequently small and easily treated with resection via either endoscopy or thoracotomy. We report a cavernous hemangioma occurred in the distal esophagus successfully treated with circumferential myomectomy.

  • Recurrent aphthous stomatitis is common oral disease in the world. It is characterized by multiple, recurrent, painful ulcer with circumscribed margins, erythematous haloes and yellow or grey floors. Patients with recurrent aphthous stomatitis suffer from its painful ulcer. But unfortunately, its etiology and pathogenesis is not clear and still unknown. So we review etiology and pathogenesis of recurrent aphthous ulcer and wish to propose direction of the future study.

  • Melanosis coli is described as a black or brown discoloration of the mucosa of the colon. Such a discoloration is largely due to pigment granule deposition in macrophages in the colonic mucosa, which arises from anthraquinone-containing laxative abuse. Melanosis coli has usually been reported in the elderly. We present the first case of melanosis coli associated with aloe consumption in a Korean child.

  • 기관협착증은 이비인후과 영역에 있어서 치료가 어려운 질환중의 하나로 이의 재건에는 여러가지 자가조직이 각각의 장점에 따라 사용되어 왔지만 완전하고 만족할만한 결과를 얻지 못하는 실정이다. 저자들은 가토의 자가 늑연골 (1군) 및 늑연골막 (2군)을 이용한 기관 재건술을 시행하여 다음과 같은 결과를 얻었다. 1) 이식편은 술후 10주 (1군) 및 12주 (2군)에 흡수나 섬유조직으로의 대치는 없었으며, 기관강의 크기는 직상 하부의 정상기관과 큰 차이가 없었다. 2) 늑연골을 이식한 부위는 연골막의 내세포층에서 연골세포가 분화, 이동되어 절단된 연골사이로 이동하여 새로운 연골을 형성하였다. 3) 늑연골막을 이식한 부위는 연골막에서 형성된 연골세포 및 연골기질이 절단된 연골사이로 이동하여 새로운 연골을 형성하였다. 4)) 이식한 부위의 점막층은 점막재생이 이루어져 원주상피로 피복되었다. 5) 기관 전벽의 결손부위는 늑연골 및 늑연골막을 이용하여 성공적으로 재건되었다.

  • Stevens-Johnson syndrome (SJS) presents with widespread blisters, erythematous or purpuric macules, and one or more mucous membrane erosions. Various etiologic factors, including infection, vaccination, drug administration, systemic diseases, physical agents, and food have been implicated as causes of SJS. Mycoplasma pneumoniae is the most common infectious agent to cause SJS in children. In recent literature, M. pneumoniae-induced SJS with mucositis that lacks the typical target lesions has been described. We report a case of a 6-year-old boy with swelling, peeling of the lips, and red eyes with photosensitivity. On physical examination, he showed severe oral mucositis and conjunctivitis with no evidence of skin lesions. Mycoplasma antibody, which was positive with titers of more than 1:2,560. For patients presenting with fever and mucositis of unknown origin, M. pneumoniae should be considered.

  • Hereditary hemorrhagic telangiectasia is a rare autosomal dorminant disease that features abnormal and fragile vascular dilations of terminal vessels in skin and mucous membranes, as well as arteriovenous malformations of internal organs, particularly lungs, brain, and liver. Often patients have not been diagnosed with HHT for a long time, and undiagnosed HHT patients unnecessarily develop serious complications such as severe life-threatening hemorrhage, stroke or brain abscess. Therefore, early detection and appropriate screening is very important. Early detection of HHT allows the appropriate screening for the presence of silent disease such as AVMs in the lungs, liver, or brain, and preventive treatment in the patient and their affected family members. Dentists should be familiar with HHT because the telangiectases on skin and oral mucosa are often the most dramatic and most easily identified component of HHT. Recently, we experienced a case of HHT. We present the case with a review of the literature.

  • Hematopoietic stem-cell transplantation (HSCT) is a treatment for immune deficiency, autoimmune diseases, and hematopoietic malignancies. The main complication of allogenic HSCT is graft-versus-host disease (GVHD). Oral mucosal biopsy is needed for a definitive diagnosis and treatment planning of GVHD, but this procedure causes bleeding and bacteremia in a poor general condition. We evaluated the efficacy of laser-assisted biopsy as a minimally invasive treatment. Three cases were described in this article. All patients' medical records, clinical photographs, and histopathologic findings were reviewed. All patients felt comfortable and no severe complications occurred. The quality of the obtained biopsy material was adequate for a definitive diagnosis of GVHD. Laser-assisted, minimally invasive biopsy of the oral mucosa does not cause bleeding, and it reduces the chances of infection, bacteremia, and postoperative scarring compared to the usual histopathologic biopsy procedure. It would thus be advantageous to use this procedure to biopsy GVHD patients.

Intermuscular Hematoma in Esophagus : without Tearing (식도 혈종)

  • Spontanous hematoma of the esophagus is a rare condition affecting middle aged or elderly women. We experienced one case of esophageal hematoma which attracted our attention due to its confusing presentation clinically. The pathogenesis has been in dispute so far. The diagnosis has traditionally been made by barium esophagogram. We proved the diagnosis of spontaneous hematoma of the esophagus by utilizing CT scan and MRI. This condition led to conservative treatment and full recovery ultimately, but we performed the surgical correction because the filling defect persisted and the dysphagia got worse on the 20th day of hospital stay. Hematoma was located between the inner layer of circular muscle and the outer layer of longitudinal muscle which we considered as intermuscular hematoma of the esophagus.
