Malocclusion with Congenital Missing of Lower Incisors (하악 전치의 선천적 결손을 동반한 부정 교합의 치험례)

  • Positions, angulation and mesiodistal dimension of lower incisors are important in esthetics, occlusion and post-treatment stability of tower arch. When lower incisor is congenitally missing, problems such as increased overjet and overbite, closing in of adjacent teeth and size/space discrepancies may occur. When creating treatment plans, incisor position and angulation, lip support, anteroposterior skeletal relationship canine-molar relationship, overjet overbite, remaining growth potential, crowding and anterior tooth ratio have to be considered. For an accurate analysis of incisal size discrepancy, diagnostic model set-up may be helpful. The two patients in this presentation both had two lower incisor missing, but the degree of crowding, skeletal relationship, lip support, molar relationship are different and therefore treatment plan was different as well. Long term follow-up may be necessary for stability and retention.

Guided Bone Regeneration using Fibrin Glue in Dehiscence or Fenestration Defects Occurred by Maxillary Anterior Implants: Case Report (상악 전치부 임플란트 식립에 의한 열개 및 천공형 골결손 발생 시 조직 접착제를 이용한 골유도 재생술: 증례보고)

  • Dental implants are universal restorative method on edentulous site in oral cavity and generally recognized by patients as well as clinicians. Rapid bone resorption of labial portion of maxillary anterior area is performed due to dental trauma, chronic periodontitis, and so on. Accordingly, Implants on maxillary anterior alveolar ridge with narrow labiopalatal width would lead to bony defects of dehiscence or fenestration. In this case, guided bone regeneration procedure is used to augment maxillary anterior alveolar ridge. It can have mechanical and biological advantages to mix tissue adhesive with bone graft materials in guided bone regeneration procedure. In these cases, when the dehiscence or fenestration defects was occurred by dental implants on maxillary anterior alveolar ridge with narrow labiopalatal width, guided bone regeneration procedures were performed with various combination of particle bone graft materials(allograft, xenograft, and alloplast) mixed with fibrin glue, excepting autogerous bone. We reported that all of 4 cases showed favorable alveolar ridge augmentations.

  • Proper management of provisional prosthesis is key to success in prosthodontics. Provisional restoration on maxillary anterior missing area frequently come across a incident of falling off especially in patients with long span pontics and oval arch shape. This is because maxillary anterior teeth are more exposed to horizontal force than the posterior teeth and additional anterior cantilever effect will negatively affect to the retention of provisional prosthesis. Beside that maxillary anterior provisional prosthesis should provide proper incisal guidance during the mandibular functional movements. However occlusal contacts on the prosthesis in maximum intercuspal position are located on opposite side of fulcrum line of prosthesis which will cause removing force against the provisional prosthesis. This case report present that provisional implant prevent pre-described harmful effect on maxillary anterior fixed provisional prosthesis and provide comfort and satisfactory result during post-extraction healing period.

  • The prosthodontic treatments in maxillary anterior teeth focus on achieving esthetic appearances. It is possible to improve the esthetic appearance by adjusting the shape, arrangement, and color of the maxillary anterior teeth. For anterior teeth restoration, it is necessary to evaluate the relationship of teeth, lip and gingival architecture with the facial profile of patient. Also, clinician needs to fully understand what the patient wants to. DSD (digital smile design) concept can be applied as a tool to improve communication with the clinician, technician and patient. In addition, DSD can help to meet the needs of the patient. In this case, it was impossible to achieve symmetry due to congenital missing of maxillary central incisor. The definitive treatment goal was to get the harmony of maxillary anterior teeth, lip and the patient's face. This case report describes that the patient and clinician got the satisfying esthetic outcome by using DSD.
