Review of the Reasons in Cases Requiring Varus/Valgus Constrained Prosthesis in Primary Total Knee Arthroplasty (일차 슬관절 전치환술 시내·외반 구속형 치환물이 필요했던 사례들의 원인 분석)

  • Purpose: The least constrained prosthesis is generally recommended in primary total knee arthroplasty (TKA). Nevertheless, a varus/valgus constrained (VVC) prosthesis should be implanted when a semi-constrained prosthesis is not good for adequate stability, especially in the coronal plane. In domestic situations, however, the VVC prosthesis could not always be prepared for every primary TKA case. Therefore, it is sometimes impractical to use a VVC prosthesis for unsual unstable situations. This study provides information for preparing VVC prostheses in the preoperative planning of primary TKA through an analysis of primary VVC TKA cases. Materials and Methods: This study reviewed 1,797 primary TKAs, performed between May 2003 and February 2016. The reasons for requiring VVC prosthesis and the preoperative conditions in 29 TKAs that underwent primary TKA with a VVC prosthesis were analyzed retrospectively. Results: In primary TKA, 29 cases (1.6%) in 27 patients (6 male and 21 female) used VVC prosthesis. Two patients underwent a VVC prosthesis on both knees. The mean age of the patients was 63.4 years old (34-79 years). The mean flexion contracture was 16.2° (-20°-90°), and the mean angle of great flexion was 111.7° (35°-145°). The situations requiring a VVC prosthesis were severe valgus deformity in 10 knees, knee stiffness requiring extensive soft tissue release in 10 knees, previously injured collateral ligaments in five knees, and distal femoral bone defect due to avascular necrosis in four knees. The mean tibiofemoral angle was 25.7° (21°-43°) in 10 cases with a valgus deformity. The mean flexion contracture was 37.5° (20°-90°), and the mean range of motion was 48.5° (10°-70°) in 10 cases with knee stiffness. Conclusion: The preparation of VVC prosthesis is recommended, even for primary TKA in cases of severe valgus deformity (tibiofemoral angle>20°), stiff knee (the range of motion: less than 70° with more than 20° flexion contracture), and the cases with a previous collateral ligament injury. This information will help in the preparation of adequate TKA prostheses for unusual unstable situations.

Ligament Injuries of Knee in the Recreational Skiers (스키에 의한 슬관절 인대 손상)

  • Purpose: This study is to analyse the injury patterns of knee ligament and the factors influencing ligament injuries of knee, and to evaluate the changes of knee function and activity after ski injury. Materials and Methods: Thirty cases of ligament injuries of knee were studied with a questionaire, stress radiographs, magnetic resolution imaging, and physical examination. Mean age was 28.6 years old and mean follow up was 29.6 months. Clinical evaluation of Lysholm knee scoring scale was used for knee function and Tegner activity score was used for activity change after ski injury. Results: The anterior cruciate ligament (ACL) injury accompanied with medial collateral ligament(MCL) injury was most in 11 cases and isolated MCL injury was the next in 9 cases. The common types of injury mechanism were Phantom foot phenomenon (13 cases, 43$\%$) and valgus external rotation injury (12 cases, 40$\%$), which constitute 83$\%$ of all case. At the last follow up, the mean Lysholm score was 93.4 and mean Tegner activity score was 4.2. The reduced Tegner activity score after injury was 1.9. Among several injury groups, the evaluation of knee function and activity was best in the isolated MCL injury group and worst in the ACL injury accompanied with MCL injury group. The factors to influence ski injury were participation to ski class, release of binding, and skiing long time more than 2 hours. Conclusions: Lysholm score at last follow up revealed good grading, but sports activity after ski injury was reduced when compared with pre-injury state. It seems to need a active, systemic sports rehabilitation program after sports injury.

Knee Injury In Soccer Play (축구경기시의 무릎손상)

  • Purpose: This study was performed to analyze the mechanism and type of knee injury during soccer game and to evaluate the functional recovery after treatment. Materials and Methods: From October 2002 to January 2005, 49 patients were treated surgically. Mean age was 28.8 (8-36) years old, mean follow-up duration was 14.9 (8-36) months. Injury was evaluated by physical examination, radiography, and arthroscopic finding. Clinical results were evaluated with Lysholm score and Tegner activity score. Results: According to classification of the ligament injury, the incidence of single ACL injury was the most frequent and 27 cases. Injuries took place, in 2nd half of the games, and in the winter. Among the mechanism of injury, turn & twisting was 20 cases and the most frequent. The preoperative Lysholm score, Tegner activity score were 45, 2 and those of last follow-up were 87.6, 6, respectively. Conclusion: The recovery of Lysholm and Tegner activity score were good, but the sports activity was clearly decreased alter injury. It was suggested that appropriate physical & psychological rehabilitation and education were inevitable.

Changes in Temperature during Arthroscopic Knee Surgery (관절경 수술시 슬관절내 온도변화)

  • Purpose : To document the change in the temperature of the knee joint at the initiation and conclusion of an arthroscopic procedure and correlate this temperature change with other intra-operative variables. Material and Methods : Temperature measurements were performed in 40 consecutive patients(42 cases) that underwent arthroscopic surgery. Temperature measurements were taken at the initiation of the procedure, before and after inflation of the tourniquet. The last measurement was recorded at the end of the surgical procedure. Results : The mean knee joint temperature evaluated before inflation of the torniquet was $35.1{\pm}1.0$, at the end of surgery, $24.6{\pm}1.5^{\circ}C$. The mean temperature change observed from the beginning to the end of the procedure was $10.5^{\circ}C$. The student t test showed a statistically significant difference of the initial joint temperature(p<0.01) between the patient with no and+1 effusion / between the patient with no and +2 effusion. The temperature at the end of the procedure was found to be statistically low correlated (p<0.01) with the lower temperature of the irrigant and the lengthening of the arthroscopic procedure. Conclusion : Consideration should be given to maintaining the saline irrigant to more physiologic temperature to protect the articular cartilage from any possible temperature induced damages.

Comparison of Primary Stability of Different Femoral Fixation Techniques in Anterior Cruciate Ligament Reconstruction (전 십자 인대 재건술에서 대퇴골측 고정 방법의 초기 안정성의 비교)

  • Various methods for fixation of graft have been widely used for reconstruction of anterior cruciate ligament. However, the biomechanical strength of each fixation techniques are not fully understood. The purpose of this study is to compare the pull out strength of different fixation techniques which is probably the most important factor for the success at the initial stage of healing. Biomechanical test was carried out to measure and compare the pull out tensile strength of five different fixation techniques in 35 pig(Yorkshire) knees. ANOVA and Duncan multiple comparison test was applied for statistical analysis. In the two fixation techniques with bone patellar tendon bone graft, the mean maximum tensile strength was $1333.4{\pm}148.5N$ with titanium interference screw, while it was $1310.1{\pm}168.9N$ with biodegradable interference screw. The failure mode were pulled out of bone plugs from the femoral tunnel in majority cases. In the fixations with hamstring tendon, the mean maximum tensile strength were $1405.9{\pm}135.1N$ with SemiFix screw, $820.3{\pm}104.5N$ with biodegradable interference screw, and $682.1{\pm}54.2N$ with Endobutton. The mode of failure was variable in each technique. The tendon was pulled out from the tunnel in biodegradable interference screw fixation, the screw was bent in the SemiFix system, and the polyester tape were ruptured or the buttons were pulled into tunnel in Endobutton fixation. The mean maximum tensile strength of two interference screws with bone patellar tendon bone was statistically comparable to that of SemiFix with hamstring tendon. However biodegradable interference screw and Endobutton with hamstring tendon showed weaker maximum tensile strength than above three fixation techniques (P<0.05).

Comparison of ACL Reconstruction using Patellar tendon Autografts and Allografts (자가 슬개건과 동종 슬개건을 이용한 전십자인대의 재건의 비교)

  • We have studied the results of reconstruction by freeze-dried patellar allografts or patellar autografts in ACL-deficient patients prospectively. From January 1995 to December 1995, we performed ACL reconstruction using an arthroscopic-assisted technique with patellar autografts in 21 patients and patellar allografts in 13 patients. Minimum followup time was 1 year(average 26 months). All patients were evaluated by using KT-2000 arthrometer and MRI as well as by physical examination. Final results were rated as satisfactory or unsatisfactory by using a modified Feagin knee scoring scale. Good or excellent were considered to have satisfactory results and fair or poor were considered to have unsatisfactory results. As measured by the KT-2000, 19 cases$(90.5\%)$ had a 5-mm or Jess side-to-side differential, a satisfactory results in autograft group, 2 cases of unsatisfactory results had joint instability. In allograft group, 10 cases$(76.9\%)$ had a 5-mm or less side-to-side differential, a satisfactory results, 3 cases of unsatisfactory results had joint instability including postoperative infection(1 case). In conclusion, the results of ACL reconstruction with autografts were better than those with allografts. The problem of allograft reconstruction were rehydration, aseptic control and improper mechanical tensioning. So, we thought that success of allograft reconstruction was depended on careful implant preparation including pretensioning technique.

Management of Multiple Ligament Injured Knee (슬관절 다발성 인대 손상의 치료)

  • Multiple ligament knee injury is defined as rupture to at least two of the four major knee ligament structures. Three or four knee ligament injury results in knee dislocation as complete disruption of the integrity of the tibiofemoral articulation. In multiple ligament knee injury, vascular and neurologic assessment should be performed meticulously and systematically. Emergency surgery should be needed if arterial injury is suspected. Surgical treatment rather than conservative management should be done and early surgery might be better than delayed surgery. Reconstruction of ACL and PCL, repair or reconstruction of MCL, and reconstruction of posterolateral corner are recommended, although many debates have occurred. Multiple ligament knee injury requires more aggressive management than single ligament knee injury.

  • Extraarticular synovial chondromatosis is a very rare disease which affects the hands, feet, and wrists most commonly. In cases of involvement around large joints, the tissues around knee are usually involved. It arises from tendon sheath, capsular tissue and bursae. It is an idiopathic process in which the synovial cells undergo the metaplasia into cartilage cells. Chondroid matrix of the cartilaginous nodules calcify and ossify to be the osteochondroma. On simple radiograms before calcification and/or ossification of the chondroid tissues the cartilaginous nodules look normal on radiograms. Therefore MRI is needed to establish the diagnosis. We report a case of extraarticular pan-peri-meniscal synovial chondroma around right knee, initially suspected as a ganglion cyst which clinically mimicked a large rounded lesion or a cystic lesion on MRI.

Heavy Mineral Analysis of the Cretaceous Hayang Group Sandstones, Northeastern Gyeongsang Basin (경상분지 북동부 백악기 하양층군 사암의 중광물분석)

  • The northeastern part of the Gyeongsang Basin is widely covered by the Cretaceous Hayang Group (Aptian to Albian). The Hayang Group consists of the IIjig. Hupyeongdong, Jeomgog, and Sagog formations. Heavy mineral analysis was carried out to define the possible source rocks of the Haynag Group snadstones. Heavy minerals separated from IIjig, Hupyeongdong, and Jeomgog sandstones are hematite, ilmenite, leucoxene, magnetite, pyrite, actinolite, andalusite, apatite, biotite, chlorite, epidote, garnet, hornblende, kyanite, monazite, muscovite, rutile, sphene, spinel, staurolite, tourmaline, and zircon. Based on their close association and sensitiveness, the heavy mineral assemblages can be classified into 6 syutes: 1)apatite-green tourmaline-sphene-colorless/yellowish zircon; 2) colorless garnet-epidote-rutile-brown tourmaline; 3) rounded purple zircon-rounded tourmaline-rounded rutile; 4) augite-hornblende-color- less zircon; 5) epidote-garnet-sphene; and 6) blue tourmaline. The possible source rocks corresponding to each assemblage are 1) granitic rocks; 2) metamorphic rocks (schist and gneiss) ; 3) older sedimentary rocks; 4) andesitic rocks; 5) metamorphosed impure limestone; and 6) pegmatite, respectively. Previous paleocurrent data suggest that the sediments of the study area were mainly derived from the northeastern to southeastern directions. Thus, the most possible source areas would be the east extension part of the sobaegsan metamorphic complex to the northeast and the Cheongsong Ridge to the southeast.

The Relationship between Bone Bruise in MRI and Associated Injuries after the Knee Joint Trauma (슬관절 외상 후 자기 공명 영상에서 관찰되는 잠재성 골 병변과 동반 손상과의 관계)

  • Purpose : The purpose is to analyze the relationship between bone bruise in MRI and associated injuries after the knee joint trauma. Materials and Methods : Total 50 cases were reviewed, divided into two groups according to initial trauma energy. High-energy group (group I), such as traffic accident was 13 cases, low-energy group (group II), such as sports trauma was 37 cases. The type of the lesion was used the classification by Costa-Paz. The site of lesion was analyzed according to femoral/tibial, medial/lateral and anterior/middle/posterior site respectively. Associated injuries were confirmed by physical examination, radiograph, MRI and arthroscopy. Results : In group I there was a various distribution of the bone bruise in the knee joint according to mechanism of injury. The bone bruise with ACL injury was 38$\%$ only and the most frequent type was Costa-Paz type I (52.6 $\%$). In group II more frequent locations were the middle portion of the lateral femoral condyle and the posterior portion of the lateral tibial condyle. Bone bruise associated with ACL injury, was upto 56.8 $\%$ and the frequent type were Costa-Paz type II (48 $\%$), in order type I (42 $\%$). As a result, in the high-energy injury the bone bruise had a various location in both condyle and less frequently associated injury, but in the low-energy injury there was particularly frequent location of bone bruise, associated injury and type. Conclusions : We could assess the associated injury by analysis of the location and type of bone bruise, especially in the low-energy injury, e.g. sports injury. But further study will be necessary with more case analysis.
