A Study on measurement of scattery ray of Computed Tomography (전산화 단층촬영실의 산란선 측정에 대한 연구)

  • Purpose : Computed tomographic equipment is essential for diagnosis by means of radiation. With passage of time and development of science computed tomographic was developed time and again and in future examination by means of this equipment is expected to increase. In this connection these authors measured rate of scatter ray generation at front of lead glass for patients within control room of computed tomographic equipment room and outside of entrance door for exit and entrance of patients and attempted to ind out method for minimizing exposure to scatter ray. Material and Method : From November 2001 twenty five units of computed tomographic equipments which were already installed and operation by 13 general hospitals and university hospitals in Seoul were subjected to this study. As condition of photographing those recommended by manufacturer for measuring exposure to sauter ray was use. At the time objects used DALI CT Radiation Dose Test Phantom fot Head (${\oint}16\;cm$ Plexglas) and Phantom for Stomache(${\oint}32\;cm$ Plexglas) were used. For measurement of scatter ray Reader (Radiation Monitor Controller Model 2026) and G-M Survey were used to Survey Meter of Radical Corporation, model $20{\times}5-1800$, Electrometer/Ion Chamber, S/N 21740. Spots for measurement of scatter ray included front of lead glass for patients within control room of computed tomographic equipment room which is place where most of work by gradiographic personnel are carried out and is outside of entrance door for exit and entrance of patients and their guardians and at spot 100 cm off from isocenter at the time of scanning the object. The results : Work environment within computed tomography room which was installed and under operation by each hospital showed considerable difference depending on circumstances of pertinent hospitals and status of scatter ray was as follows. 1) From isocenter of computed tomographic equipment to lead glass for patients within control room average distance was 377 cm. At that time scatter ray showed diverse distribution from spot where no presence was detected to spot where about 100 mR/week was detected. But it met requirement of weekly tolerance $2.58{\times}10^{-5}\;C/kg$(100 mR/week). 2) From isocenter of computed tomographic equipment to outside of entrance door where patients and their guardians exit and enter was 439 cm in average, At that time scatter ray showed diverse distribution from spot where almost no presence was detected to spot with different level but in most of cases it satisfied requirement of weekly tolerance of $2.58{\times}10^{-6}\;C/kg$(100 mR/week). 3) At the time of scanning object amount of scatter ray at spot with 100 cm distance from isocenter showed considerable difference depending on equipments. Conclusion : Use of computed tomographic equipment as one for generation of radiation for diagnosis is increasing daily. Compared to other general X-ray photographing field of diagnosis is very high but there is a high possibility of exposure to radiation and scatter ray. To be free from scatter ray at computed tomographic equipment room even by slight degree it is essential to secure sufficient space and more effort should be exerted for development of variety of skills to enable maximum photographic image at minimum cost.

  • Diagnostic imaging systems play a critical role in obtaining anatomical images, which increases the frequency of inspection all over the world. However the likelihood that patients are exposed to relatively high radiation dose increases, which may lead to an increase of patient dose due to unnecessary radiation exposure unless appropriate management is accompanied. Thus the revised edition of IEC 60601-2-44 which is constancy tests for CT equipment which is designated as special medical equipment and is subject to safety management was studied. The results suggested the 3rd has been revised rationally in order to overcome the limitations in the 2nd by adopting clear and enhanced references, which implies the replacement of IEC 60601-2-44 2nd edition with IEC 60601-2-44 3rd will prevent the patients from the harm from improper medical equipment.

  • In study suggested clinical availability to shoulder forced traction method in term of quality of image, the patient's convenience and stability, according to whether to use of shoulder forced traction bend using computed tomography(CT) that X-ray calibration and various mathematic calibration algorithm application can be applied by AEC. To achieve this, 79 patients is complaining of cervical pain oriented that shoulder forced traction bend use the before and after acquires lateral projection scout image and transverse image. transverse image of a fixed size in concern field of pixel and figure the average HU value compare that quantitative analysis. Artifact and pixel and resolution to qualitative clinical estimation image analysis. the patient feel inconvenience degree that self-diagnosis survey that estimate. As a result, lateral projection scout image if you used shoulder forced traction bend for the depicted has been an increase in the number of a cervical vertebrae. transverse image concern field shoulder forced traction bend use the before and after for pixel and the average HU-value changes was judged to be almost irrelevant. Artifact and resolution and contrast, in qualitative analysis of the results relating the observer to the unusual result. So, the patients of 82.27% complained discomfort that use of shoulder forced traction bend in self-diagnosis survey. No merit of medical image by using of bend from result was analyzed quality of image to quantitative and qualitative method judged. Nowadays, CT is supplied possible revision of quality of radiation by reduction of slice and automatic exposure controller, etc and application of preconditioning filter process due to various mathematic revision algorithm. So, image noise by beam hardening artifact should not be a problem. shoulder forced traction bend of use no longer judged clinically availability because have not influence of image quality and give discomfort, have extra dangerousness.

  • Non-invasive technique CT, called automated computed tomography, is used to detect lesion of a patient when diagnosing human body. Information obtained from CT plays an important role in assembling 3 dimensional images. Recently, new equipment, operated by CT, is required which can be appliable to physical and biological research. In accordance to this quest, micro-CT is invented that produce more detail and concrete information. Images supplied by CT are even more detailed and concrete, so it contributes much to the development of biology and polymer material engineering field. However, there has been little reliable reports regarding measuring information of space dose distribution about exposure dose limit of users operating micro-CT. In addition, little reports regarding space dose distribution of exposure has been known about unwanted diffraction light produced by usage of micro-CT. The exterior of micro-CT is covered by lead, which is for removing exposure of diffraction light. Thus, even if it is good enough to prevent exposure of diffraction light, consistent management of equipment will be required as time goes by and equipment are getting old as well. We measured space dose distribution regarding exposure of diffraction light of users operating micro-CT directly. Therefore, we suggest that proper management should be necessary for users operating micro-CT not to be exposed by unwanted diffraction light.

Development of Self-Diagnosis Linearity Quality Assurance Technique in Computed Tomography by Using Iodic Contrast Media (요오드 조영제를 이용한 전산화단층촬영장치의 자가진단 직선성 정도관리 기술 개발)

  • The purpose of this study was to develop a self-diagnostic linearity quality control techniques of computed tomography (CT) by using measured CT number values from the various concentrations of iodine contrast media (CM) is diluted with distilled water under each condition of the tube voltage. The equipment was used for four-channel MDCT, the iodine concentration were using 300 mgI/ml, 350 mgI/ml, 370 mgI/ml and 400mgI/ml. Dilution of CM in distilled water was increased by each 5% until the maximum CT number values were measured. We applied the tube voltages for 90 kVp, 120 kVp, 140 kVp. As a result, we was obtained to the nearest linearity as 0.993 of correlation coefficient between the iodinated CM from 5% to 25% in 400 mgI/ml and the CT number values by 90 kVp. In conclusion, the proposed self-diagnostic linearity quality assurance technique by using iodine CM can be utilized to replace the AAPM CT performance phantom.

  • In foreign countries, the impotance of quality assurance (QA) is increasing, whereas QA evaluation standards are not established properly in Korea. Thus this study was conducted to suggest improved QA evaluation standards and methods by studying/comparing/analyzing the literature in both Korea and foreign countries and prove its suitability through simulated evaluation. result of research in south korea, much check list using the evaluation method subjective, the evaluation of accuracy and reproducibility is meausred in the number of missing. Therefore, attempts to present a clear reference evaluation and objective way to solve the problem, examined the accuracy and reproducibility and 10times of measurement. The results of the simulated evaluation satisfied both proposed evaluation standards and methods. However the QA evaluation standards and methods for the items that are implemented only in Korea and high-contrast spatial resolution could have been established. In this regard, there is a need of further studies on the establishment of QA evaluation standards and methods that suits Korean situation.

  • AC 부하를 갖는 동기전동기에 있어서, 동기전동기의 속도리플을 저감시키기 위한 문제를 풀기위해 3가지 제어기법에 대해 비교를 한 후 가장 강인한 제어기법에 대해 분석하였다. AC 부하를 갖는 특별한 제어 대상으로 엑스선 전산화 단층촬영 장치(CT)용 겐트리를 선정하였으며 시스템이 갖는 특별한 구조에 의해 이러한 시스템 특성을 갖는다. 동기전동기의 출력 축에 링(Fing) 모양의 원판 프레임을 갖으며, 이 원판 표면에 무게가 서로 다른 여러 장치(X_선 튜브, X-선 검출기, 고압발생장치, DAS 장치, 온도조절장치 등)를 부착하여 영상 획득 시스템의 회전부를 구축하기 때문이다. 이러한 시스템에서는 무게 평형을 갖지 못하는 편심 무게가 존재하게 되며 이로써 전동기 관점에서는 AC 부하처럼 인식되는 제어 조건으로 인식 될 수 있다. AC 부하를 갖는 동기전동기에 대해 일반적인 벡터제어 알고리즘으로 제어를 수행하면 정상상태에서도 속도 오차가 "0" 으로 줄어들지 않고 AC 형태의 오차 성분이 존재하며 편심 무게의 크기에 비례하여 진폭이 커지는 특성을 갖는다. 이러한 문제점을 해결하기 위하여 Sine파 보상전류를 갖는 속도제어기법, 펀심부하토크 관측기를 이용한 속도제어기법, 그리고 기준모델제어기법을 소개하였다. 각 방법에 대한 실험 결과로부터 편심무게의 변동과 편심 위치의 변동 조건에서 기준모델제어기법이 강인한 제어 특성과 리플저감 측면에서 가장 우수함을 검증하였다. 이로써 AC 부하 조건에서 고정도 속도 제어기가 요구되는 경우 좋은 선택의 지침이 될 수 있다고 본다.

특수의료장비의 설치 및 운영에 관한 규칙에 의한 정도관리 시행

  • 국민건강보험재정건전화특별법 (제정 2002.1.19 법률 제 6620호)은 건강보험의 재정적자를 조기에 해소하고 재정수지의 균형을 이루도록 함으로써 건강보험제도의 발전과 국민건강 증진 도모를 그 목적으로 하며, 제14조(특수의료장비의 설치ㆍ운영)에서 의료기관은 보건복지부장관이 고시한 특수의료장비를 설치ㆍ운영하고자 하는 때에는 보건복지부령이 정하는 바에 따라 이를 등록하여야 하며 설치 인정 기준에 적합하게 설치ㆍ운영하여야 하고 정기적인 품질관리를 받아야한다고 명시하였다. 특수 의료장비의 설치 및 운영에 관한 규칙 (제정 2003.1.14 보건복지부령 제235호)은 국민건강보험재정건전화특별법이 제정됨에 따라 특수의료장비의 적정 한 설치와 활용을 위하여 의료기관에서 설치ㆍ운영하는 특수의료장비의 등록절차 설치인정기준 및 품질관리 절차 등을 정하고 특수의료장비에 대한 관리체계를 확립하려는 것이다. 특수의료장비의 설치 및 운영에 관한 규칙의 주요 골자는 가. 의료기관에서 특수장비를 설치ㆍ운영하고자 하는 경우 보건복지부장관 또는 시ㆍ도 지사에게 등록하도록 하였는바, 이 등록에 대한 절차와 특수의료장비의 설치인정기준을 정함, 나. 특수의료 장비에 대한 정기적인 품질관리검사를 서류검사와 정밀감사로 구분하여 서류검사는 1 년마다, 정밀검사는 3 년마다 받도록 함, 다. 품질관리검사기관의 장은 특수의료장비품질관리검사성적서를 신청인에게 교부하고, 보건복지부 또는 시ㆍ도지사 및 건강보험심사평가위원장에게 검사결과를 통보하도록 함, 라. 특수의료정비를 설치ㆍ운영하는 의료기관의 개설자 또는 관리자 및 품질관리검사기관의 장이 작성ㆍ비치ㆍ보존하여야할 서류를 정함이다. 보건복지부장관에게 등록하여야할 특수의료장비는 자기공명영상촬영장치와 전산화단층촬영 장치이며, 시ㆍ도지사에게 등록하여야할 특수의료장비는 유방촬영용장치이다. 본 발표에서는 특수의료장비의 설치 및 운영에 관한 규칙에 대한 개요와 연세의료원 세브란스병원에서 시행하고 있는 특수의료장비의 정도관리 검사, 팬텀영상검사, 그리고 임상영상검사를 소개하고자한다.

Comparison of personal computer with CT workstation in the evaluation of 3-dimensional CT image of the skull (전산화단층촬영 단말장치와 개인용 컴퓨터에서 재구성한 두부 3차원 전산화단층영상의 비교)

  • Purpose : To evaluate the usefulness of the reconstructed 3-dimensional image on the personal computer in comparison with that of the CT workstation by quantitative comparison and analysis. Materials and Methods : The spiral CT data obtained from 27 persons were transferred from the CT workstation to a personal computer, and they were reconstructed as 3-dimensional image on the personal computer using V-works 2.0/sup TM/. One observer obtained the 14 measurements on the reconstructed 3-dimensional image on both the CT workstation and the personal computer. Paired Nest was used to evaluate the intraobserver difference and the mean value of the each measurement on the CT workstation and the personal computer. Pearson correlation analysis and % incongruence were also performed. Results: I-Gn, N-Gn, N-A, N-Ns, B-A, and G-Op did not show any statistically significant difference (p>0.05), B-O, B-N, Eu-Eu, Zy-Zy, Biw, D-D, Orbrd R, and L had statistically significant difference (p<0.05), but the mean values of the differences of all measurements were below 2 mm, except for D-D. The value of correlation coefficient y was greater than 0.95 at I-Gn, N-Gn, N-A, N-Ns, B-A, B-N, G-Op, Eu-Eu, Zy-Zy, and Biw, and it was 0.75 at B-O, 0.78 at D-D, and 0.82 at both Orbrd Rand L. The % incongruence was below 4% at I-Gn, N-Gn, N-A, N-Ns, B-A, B-N, G-Op, Eu-Eu, Zy-Zy, and Biw, and 7.18%, 10.78%, 4.97%, 5.89% at B-O, D-D, Orbrd Rand L respectively. Conclusion : It can be considered that the utilization of the personal computer has great usefulness in reconstruction of the 3-dimensional image when it comes to the economics, accessibility and convenience, except for thin bones and the landmarks which are difficult to be located.

The Effects of the CT Voltages on the Dose Calculated by a Commercial RTP System (CT 관전압이 상용 전산화치료계획장치의 선량계산에 미치는 영향)

  • The relationship between the dose calculated with a radiotherapy treatment planning system (RTPS) and CT number verses the relative electron density curve was investigated for various CT voltages and beam qualifies. We obtained the relationship between the CT numbers and electron densities of the tissue equivalent materials for various CT voltages and beam qualifies. At lower CT voltages, the higher density materials, like cortical bone, showed larger CT numbers and the soft tissues showed no variations. We peformed a phantom study in a RTPS, where a phantom consisted of lung and bone legions in water. We calculated the dose received behind the lung and bone regions for 6 MV photon beams, in which the regions below the lung, water and bone received higher doses in this listed order. The result was the same for 10 MV photon beams. For the clinical application, the doses were calculated for the lung and pelvis. No difference was observed when using different electron density conversion tables with various CT voltages from a same CT. A relative dose difference of 1.5% was obtained when the CT machine for the density conversion table was different from that for the CT image for planning.
