Quantitative Micro-CT Evaluation of Microleakage in Composite Resin Restorations (Micro-CT를 이용한 복합 레진 수복물 미세 누출도의 정량 분석)

  • One of the most important and basic test of dental restorative materials is the evaluation of microleakage into the tooth-restorative interface. There are many techniques to test microleakage, but most of them have several disadvantages. Recently developed microtomography(micro-CT) can provide the three dimensional image and information about the internal component in non-destructive way, therefore using micro-CT, it is possible to evaluate microleakage exactly in quantitative manner. The purpose of this study is to find a new method for quantitative and non-destructive evaluation of microleakage in composite resin restorations using micro-CT and to compare the new method with conventional dye penetration method. Thus, microleakages of two kinds of dentin bonding systems were evaluated with above two methods. 40 extracted sound human premolars were randomly divided into two groups consisting of 20 samples and restored accordingly. Group 1 : Class V resin restorations with $Adper^{TM}$ Singe Bond Group, 2 : Class V resin restorations with $Adper^{TM}\;Promp^{TM}$ L-pop. The $Filtek^{TM}$ Supreme was applied to the Class V cavities of all teeth. After that, 10 teeth from each group were applied to evaluation of microleakage using micro-CT, and other 10 teeth from each group were using conventional dye penetration method. The conclusions of this study were as follow : 1 Using micro-CT, Group 1 showed significantly less microleakage than Group 2 and there was statistically significant difference(p<0.01) between two groups. 2. Using conventional dye penetration method, Group 1 leaked less than Group 2 and there was statistically significant difference(p<0.01) between two groups 3. The difference between two groups is more evident in the method using micro-CT. 4. In all two methods, microleakage appeared more into the cavities to dentinal margins than enamel margins.

The effects of physical factors in SPECT (물리적 요소가 SPECT 영상에 미치는 영향)

  • Using the 2-D and 3-D Hoffman brain phantom, 3-D Jaszczak phantom and Single Photon Emission Computed Tomography, the effects of data acquisition parameter, attenuation, noise, scatter and reconstruction algorithm on image quantitation as well as image quality were studied. For the data acquisition parameters, the images were acquired by changing the increment angle of rotation and the radius. The less increment angle of rotation resulted in superior image quality. Smaller radius from the center of rotation gave better image quality, since the resolution degraded as increasing the distance from detector to object increased. Using the flood data in Jaszczak phantom, the optimal attenuation coefficients were derived as 0.12cm$\^$-1/ for all collimators. Consequently, the all images were corrected for attenuation using the derived attenuation coefficients. It showed concave line profile without attenuation correction and flat line profile with attenuation correction in flood data obtained with jaszczak phantom. And the attenuation correction improved both image qulity and image quantitation. To study the effects of noise, the images were acquired for 1min, 2min, 5min, 10min, and 20min. The 20min image showed much better noise characteristics than 1min image indicating that increasing the counting time reduces the noise characteristics which follow the Poisson distribution. The images were also acquired using dual-energy windows, one for main photopeak and another one for scatter peak. The images were then compared with and without scatter correction. Scatter correction improved image quality so that the cold sphere and bar pattern in Jaszczak phantom were clearly visualized. Scatter correction was also applied to 3-D Hoffman brain phantom and resulted in better image quality. In conclusion, the SPECT images were significantly affected by the factors of data acquisition parameter, attenuation, noise, scatter, and reconstruction algorithm and these factors must be optimized or corrected to obtain the useful SPECT data in clinical applications.

The Usefulness of Scintigraphy for the Detection of Gastroesophageal Reflux and Pulmonary Aspiration (위식도 역류와 폐 흡인 진단 방법으로서 위식도 역류 신티그래피의 유용성)

  • Purpose: Chronic pulmonary disease may be caused by aspiration of gastric contents secondary to gastroesophageal reflux. At present, there is no gold standard for documenting pulmonary aspiration. The purpose of this study was to investigate the usefulness of radionuclide scintigraphy in the detection of gastroesophageal reflux and pulmonary aspiration. Methods: Thirty-five patients with suspected aspiration pneumonia, and five normal control subjects, were included in the study. All subjects underwent gastroesophageal reflux scintigraphy after the ingestion of a $^{99m}Tc$-tin colloid mixture. Dynamic images to detect gastroesophageal reflux were obtained for 1 hour. Additional static images of the chest, to detect lung aspiration, were obtained at 6 and 24 hours after oral ingestion of the tin colloid. In addition to visual analysis, pulmonary aspiration was quantitated by counting the number of pixels labeled with radioactive isotope in the region of interest (ROI) of both lung fields. Aspiration index (AI) was obtained by subtracting the pixel counts of the background from the pixel counts of the ROI. Results: Among 35 patients with suspected aspiration pneumonia, 23 proved to have gastroesophageal reflux by scintigraphy. One patient showed definite pulmonary accumulation of activity by visual analysis of the 6-hour image. Thirty of 35 (85.7%) patients showed higher AI beyond the upper limit of AI in the healthy controls. When we compared the reflux group with the non-reflux group, there was a significantly higher AI at 6 hours in the reflux group (p<0.05). Conclusion: The results suggest that radionuclide scintigraphy is useful in detecting small pulmonary aspiration in patients with suspected aspiration pneumonia secondary to reflux.

The Effectiveness of CT and MRI Contrast Agent for SUV in 18F-FDG PET/CT Scanning (18F-FDG PET/CT 검사에서 정량분석에 관한 CT와 MRI 조영제의 효과)

  • In this study, among various factors having influence on SUV, we intended to compare and analyze the change of SUV using CT(4 type) and MRI(3 type) contrast agents which are commercialized now. We used Discovery 690 PET/CT(GE) and NEMA NU2 - 1994 PET phantom as experimental equipment. We have conducted a study as follows; first, we filled distilled water to phantom about two-thirds and injected radioisotope(18F-FDG 37 MBq), contrast agent. Second, we mixed CT contrast agent with distilled water and MRI contrast agent with that water separately. And then, we stirred the fluid and filled distilled water fully not to make air bubble. In emission scan, we had 15minutes scanning time after 40 minutes mixing contrast agent with distilled water. In transmission scan, we used CT scanning and its measurement conditions were tube voltage 120 kVp, tube current 40 mA, rotation time 0.5 sec, slice thickness 3.27 mm, DFOV 30 cm. Analyzing results, we set up some ROIs in 10th, 15th, 20th, 25th, 30th slice and measured SUVmean, SUVmax. Consequently, all images mixed 3 types of MRI contrast agent with distilled water have high SUVmean as compared with pure FDG image but there was no statistical significance. In SUVmax, they have high score and there was statistical significance. And other 4 images mixed 4 types of CT contrast agent with distilled water have significance in both SUVmean and SUVmax. Attenuation correction in PET/CT has been executed through various methods to make high quality image. But we figured out that using CT and MRI contrast agents before PET/CT scanning could make distortion of image and decrease diagnostic value. In that reason, we have to sort out the priority of examination in hospital not to disturb other examination's results. Through this process, we will be able to give superior medical service to our customers.

Comparison of Dose Distribution in Spine Radiosurgery Plans: Simultaneously Integrated Boost and RTOG 0631 Protocol (척추뼈전이암 환자의 체부정위방사선치료계획 비교: 동시통합추가치료법 대 RTOG 0631 프로토콜)

  • In this study, we compared dose distributions from simultaneously integrated boost (SIB) method versus the RTOG 0631 protocol for spine radiosurgery. Spine radiosurgery plans were performed in five patients with localized spinal metastases from hepatocellular carcinoma. The computed tomography (CT) and T1- and T2-weighted magnetic resonance imaging (MRI) were fused for delineating of GTV and spinal cord. In SIB plan, the clinical target volume (CTV1) was included the whole compartments of the involved spine, while RTOG 0631 protocol defines the CTV2 as the involved vertebral body and both left and right pedicles. The CTV2 includes transverse process and posterior element according to the extent of GTV. The doses were prescribed 18 Gy to GTV and 10 Gy to CTV1 in SIB plan, while the prescription of RTOG 0631 protocol was applied 18 Gy to CTV2. The results of dose-volume histogram (DVH) showed that there were competitive in target coverage, while the doses of spinal cord and other normal organs were lower in SIB method than in RTOG 0631 protocol. The 85% irradiated volume of VB in RTOG 0631 protocol was similar to that in the SIB plan. However, the dose to normal organs in RTOG 0631 had a tendency to higher than that in SIB plan. The SIB plan might be an alternative method in case of predictive serious complications of surrounded normal organs. In conclusion, although both approaches of SIB or RTOG 0631 showed competitive planning results, tumor control probability (TCP) and normal tissue complication probability (NTCP) through diverse clinical researches should be analyzed in the future.

A Study on the Necessary Number of Bolus Treatments in Radiotherapy after Modified Radical Mastectomy (변형 근치적 유방절제술 후 방사선치료에서 볼루스 적용횟수에 대한 고찰)

  • Purpose: Post-mastectomy radiotherapy (PMR) is known to decrease loco-regional recurrence. Adequate skin and dermal dose are achieved by adding bolus. The more difficult clinical issue is determining the necessary number of bolus treatment, given the limits of normal skin tolerance. The aim of this study is to evaluate the necessary number of bolus treatment after PMR in patients with breast cancer. Materials and Methods: Four female breast cancer patients were included in the study. The median age was 53 years(range, $38{\sim}74$), tumor were left sided in 2 patients and right sided in 2patients. All patients were treated with postoperative radiotherapy after MRM. Radiotherapy was delivered to the chest wall (C.W) and supraclavicular lymph nodes (SCL) using 4 MV X-ray. The total dose was 50 Gy, in 2 Gy fractions (with 5 times a week). CT was peformed for treatment planning, treatment planning was peformed using $ADAC-Pinnacles^3$ (Phillips, USA) for all patients without and with bolus. Bolus treatment plans were generated using image tool (0.5 cm of thickness and 6 cm of width). Dose distribution was analyzed and the increased skin dose rate in the build-up region was computed and the skin dose using TLD-100 chips (Harshaw, USA) was measured. Results: No significant difference was found in dose distribution without and with bolus; C.W coverage was $95{\sim}100%$ of the prescribed dose in both. But, there was remarkable difference in the skin dose to the scar. The skin dose to the scar without and with bolus were $100{\sim}105%\;and\;50{\sim}75%$. The increased skin dose rates in the build-up region for Pt. 1, Pt. 2. Pt. 3 and Pt. 4 were 23.3%, 35.6%, 34.9%, and 41.7%. The results of measured skin dose using TLD-100 chips in the cases without and with bolus were 209.3 cGy and 161.1 cGy, 200 cGy and 150.2 cGy, 211.4 cGy and 160.5 cGy, 198.6 cGy and 155.5 cGy for Pt. 1, Pt. 2, Pt. 3, and Pt. 4. Conclusion: It was concludes through this analysis that the adequate number of bolus treatments is 50-60% of the treatment program. Further, clinical trial is needed to evaluate the benefit and toxicity associated with the use of bolus in PMR.

  • Studies for diagnostic analysis using three-dimensional (3D) CT images are recently in progress and needs for 3D craniofacial analysis are increasing in the fields of orthodontics. It is especially essential to analyze the facial soft tissue after orthodontic treatment and orthognathic surgery. In this study 3D CT images of adults with normal occlusion were taken to analyze the facial soft tissue. Norms were obtained from CT images of adults with normal occlusion (12 males, 11 females) using a computer program named V works 4.0 program. 3D coordinate planes were established using soft tissue Nasion as the reference point and a total of 20 reproducible landmarks of facial soft tissue were obtained using the multiple reconstructive sectional images (axial, sagittal and coronal images) of the V works 4.0 program: soft tissue Nasion, Pronasale, Subnasale, Upper lip center, Lower lip center, soft tissue B, soft tissue Pogonion, soft tissue Menton, Endocanthion (Rt/Lt), Alare lateralis (Rt/Lt), Cheilion (Rt/Lt), soft tissue Gonion (Rt/Lt), Tragus (Rt/Lt), and Zygomatic point (Rt/Lt). According to the established landmarks and measuring method, the 3D CT images of adults with normal occlusion were measured and the normal positional measurements and their Net (${\delta}=\sqrt{{X^2}+{Y^2}+{Z^2}}$) values were obtained using V surgery program, In the linear measurement between landmarks, there was a significant difference between males and females except Na' -Sn and En(Rt)-En(Lt). The normal ranges of Na'-Zy, Na'-Ch and Na'-Go' (facial depth) were obtained, which was difficult to measure by two-dimensional (2D) cephalometric analysis and facial photographs. These data may be used as references for 3D diagnosis and treatment planning for patients with malocclusion and dentofacial deformity.

Study of the CatcherTM Couch's Usefulness (토모치료기 CatcherTM Couch의 유용성에 대한 고찰)

  • Purpose: Recently, A Catcher was added to prevent sagging in Radixact® X9. In this study, We quantitatively compared general couch of Tomo-HDA® with catcher couch of Radixact® X9 using the human phantom and evaluated usefulness of catcher. Materials and methods: We used rando phantom for phantom study and set the each iso-center of head and neck region and Pelvis region for region parameter. Furthermore, We used hand made low melting point alloys for weight parameter. MVCT(Mega Voltage Computed Tomography) images were acquired for vertical error and rotation(pitch) error measurement increasing weight(A: 15kg, A+B: 30kg, A+B+C: 45kg). We selected 120 patients who has been treated using Tomotherpy machine for patient study. 60 patients has been treated in Tomo-HDA® and the other 60 patients treated in Radixact® X9. In the patient study methods, vertical error and rotation(pitch) error was measured for mean value calculation using MVCT images acquired on first day of radiation therapy. Result: Result of phantom study, Vertical error and rotation(pitch) error was increased proportionally increased as the weight increases in general couch of Tomo-HDA®. each maximum value was 7.52mm, 0.38° in head and neck region and 11.94mm, 0.92° in pelvis region. However, We could confirm that there was stable error range(0.02~0.1mm, 0~0.04°) in Catcher couch of Radixact®. Result of patient study, The head and neck region was measured 4.79mm 0.33° lower, and the pelvis region was measured 7.66mm, 0.22° lower in Catcher couch of Radixact® X9. Conclusion: In this study, Vertical error and rotation(pitch) error was proportionally increased as the weight increases in general couch of Tomo-HDA®. Especially, The pelvis region error was more increased than the head and neck region error. However, Vertical error and rotation(pitch) error was regularly generated regardless of weight or regions in CatcherTM couch of Radixact® X9 that this study's purpose. In conclusion, CatcherTM couch of Radixact® X9 can minimize mechanical error that couch sagging. Furthermore, The pelvis region is more efficiency than head and neck region. In radiation therapy using Tomotherapy machine, it is regarded that may contribute to minimizing unadjusted pitch error due to characters of Tomotherapy.

Airway Responses to Bronchoprovocation Using High-Resolution Computed Tomography in Patients with Bronchial Asthma (기관지천식환자에 있어서 고해상도 전산화단층촬영술을 이용한 기관지유발에 대한 기도의 반응)

  • Background: Bronchial hyperresponsiveness and abnormal response such as a loss of distensibility are pathophysiologic characteristics if bronchial asthma. The only means of direct in vivo measurement of airway size had been a tantalium bronchography, until high-resolution computed tomography(HRCT) enabled to measure noninvasively two dimensional airway area more accurately and reliably. Method: To investigate airway area responses to bronchial provocation with methacholine and evaluate the major sites of bronchial constriction in patients with bronchial asthma. We examined HRCT scans in five patients with bronchial asthma who had significant bronchoconstriction(20% or more decrease in $FEV_1$) using CT scanner(5,000T CT, Shimadzu Co, Japan) before and in 3~5 min. after methacholine inhalation. Airways which were matched by parenchymal anatomic landmarks in each patient before and after methacholine inhalation were measured using film scanner(TZ-3X scanner; Truvel Co. Chatsworth CA, USA) and a semiautomated region growing method. Results: 1) We identified 9 to 12 airways in each patient which were matched by parenchymal anatomic landmarks before and after methacholine inhalation. 2) Airway responses to methacholine are quite different even in a patient. 3) The constriction of small airways(average diameter <2 mm; area < $3.14mm^2$) was 48.7%(8.3; SEM, n=43), being more prominant than that of large airways(average diameter >2 mm; area > $3.14mm^2$), 53.8% (4.4;SEM, n=10), but not significantly different(p>0.05). 4) There was no significant difference in the degree of constriction between upper(44.3% +5.8; mean + SEM, n=30) and lower lung regions(56.7% +4.5, n=23). Conclusions: Thus airway responses to methacholine bronchoprovocation is quite variable in a patient with bronchial asthma and has no typical pattern in patients with bronchial asthma.

Perfusion Impairment in Infantile Autism on Brain SPECT Using Tc-99m ECD : Comparison with MR Findings (유아 자폐증 환아에서의 Tc-99m ECD를 이용한 뇌 단일 광전자 방출 전산화 단층 촬영술상의 관류 저하: 자기 공명 영상과의 비교 분석)

  • Neuroanatomic substrate of autism has been the subjects of continuing investigation. Because previous studies had not demonstrated consistent and specific neuroimaging findings in autism and most studies comprised adults and school-aged children, we performed a retrospective review in search of common functional and anatomical abnormalities with brain SPECT using Tc-99m ECD and correlative MRI The patient population was composed of 18 children aged 28 to 89 months(mean age : 55 months) who met the diagnostic criteria of autism as defined in the DSM-IV and CARS. Brain SPECT was performed after intravenous injection of 185-370MBq of Tc-99m ECD using brain dedicated annular crystal gamma camera. MRI was performed in all patients including T1, T2 axial and T1 sagittal sequences. SPECT data were visually assessed. Thirteen patients had abnormal SPECT scan revealing focal areas of decreased perfusion. Decreased perfusion of cerebellar vermis(12/18), cerebellar hemisphere(11/18), thalami(13/18), basal ganglia(4/18), posterior parietal(7/18), and temporal(4/18) area were noted on brain SPECT. Whereas, only 3 patients had abnormal MR findings which were subtle volume loss of parieto-occipital white matter in 3 and mild thinning of posterior body of corpus callosum in 2 and slightly decreased volume of cerebellar vermis in 1. Comparison of the numbers of abnormal findings revealed that regional cerebral blood flow (rCBF) abnormalities seen on SPECT were more numerous than anatomical abnormalities seen on MRI. In conclusion, extensive perfusion impairment involving cerebellum, thalami and parietal lobe were found in this study. SPECT may be more sensitive in reflecting pathophysiology of autism than MRI. However, further studies are mandatory to determine the significance of thalamic and parietal perfusion impairment in autism.
