Effectiveness of Self-efficacy Promoting Vestibular Rehabilitation Program for Patients with Vestibular Hypofunction (전정기능저하 어지럼 환자를 위한 자기효능증진 전정재활운동 교육의 효과)

  • Purpose: In this study an examination was done of the effect of self-efficacy promoting vestibular rehabilitation (S-VR) on dizziness, exercise self-efficacy, adherence to vestibular rehabilitation (VR), subjective and objective vestibular function, vestibular compensation and the recurrence of dizziness in patients with vestibular hypofunction. Methods: This was a randomized controlled study. Data were collected 3 times at baseline, 4 and 8 weeks after beginning the intervention. Outcome measures were level of dizziness, exercise self-efficacy, and level of adherence to VR. Subjective and objective vestibular function, vestibular compensation and the recurrence of dizziness were also obtained. Data were analyzed using Windows SPSS 21.0 program. Results: After 4 weeks of S-VR, there was no difference between the groups for dizziness, subjective and objective vestibular functions. However, exercise self-efficacy and adherence to VR were higher in the experimental group than in the control group. After 8 weeks of S-VR, dizziness (p =.018) exercise self-efficacy (p <.001), adherence to VR (p <.001), total-dizziness handicap inventory (DHI) (p =.012), vision analysis ratio (p =.046) in the experimental group differ significantly from that of the control group. The number of patients with recurring dizziness were higher in the control group than in the experimental group (p <.001). Conclusion: The results indicate that continuous 8 weeks of S-VR is effective in reducing dizziness, and improving exercise self-efficacy, subjective vestibular function and adherence to VR. Objective vestibular function and vestibular compensation were also improved in the experimental group at the end of 8 weeks of S-VR.

  • This is a non-experimental and retrospective study aimed at determining the effects of long-term hospitalization on the body mass index (BMI) and lipid metabolism in long-term hospitalized patients. The study subjects included 120 patients aged 40-65 years who were hospitalized for >3 months in 2 long-term care hospitals in Gyeonggi-do, South Korea. In this study, the BMI and levels of total cholesterol, triglycerides (TG), high-density lipoprotein (HDL), and low density lipoprotein (LDL) at admission and 3 months after hospitalization were compared and analyzed, and the related changes over time were followed up. The general characteristics of the subjects were analyzed by using descriptive statistics and frequency analysis. In addition, logistic regression analysis was performed to determine the effects of the general characteristics on the BMI and Dyslipidemia. The changes in the BMI and blood lipid levels between admission and 3 months after hospitalization were analyzed using the paired t-test. The results showed that with regard to the changes in the blood lipid levels, the triglyceride levels significantly increased 3 months after hospitalization (p<.05). These findings imply that long-term hospitalization for care and rehabilitation after acute-phase treatment should be considered a potential high-risk factor for dyslipidemia, which could be prevented or alleviated by providing the patients with health education, including exercise and dietary education.

Relationship of Knowledge of Cardiac Rehabilitation, Educational Needs, and Self Care in Inpatients with Coronary Artery Disease in China (중국 관상동맥질환자의 심장재활지식, 교육요구도 및 자가간호행위)

  • Purpose: The purpose of this study was to investigate the knowledge of cardiac rehabilitation, educational needs, and self care in inpatients with coronary artery disease in China and to provide basic data for development of cardiac rehabilitation programs. Method: From Jan. 16th to Feb. 26th, 2009, 104 patients with coronary artery disease were hospitalized in general hospitals in K and H city in China. They were interviewed with a structured questionnaire. The data was analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation analysis. Results: In inpatients with coronary artery disease in China, the score of knowledge of cardiac rehabilitation was average ($66{\pm}.18$), and the score of educational needs was high ($4.27{\pm}.49$). The score of self care was average ($3.12{\pm}.58$). The regular exercise group (p= .005) had a high knowledge of cardiac rehabilitation, the nonsmoking group (p= .022) and regular exercise group (p= .037) did well by themselves. Educational needs also affected the overall knowledge of inpatients with coronary artery disease (r= .288, p= .003). Conclusion: This study suggests that knowledge of cardiac rehabilitation and educational needs should be considered in enhancing cardiac rehabilitation programs designed for inpatients with coronary artery disease in China.

Effects of Video-guided Education for Primary Family-caregivers of Stroke Patients (뇌졸중 환자의 주간호제공자를 위한 비디오재활교육의 효과)

  • Purpose: Family-caregivers of stroke patients usually go through hardship and life style changes during the protracted course of a family member's rehabilitation. There is need for programs to educate family-caregivers to better manage the medical crisis. In this study an evaluation was done of the degrees of burden and well-being experienced by primary family-caregivers following video-guided education (VGE) on rehabilitation and family lifestyle changes. Method: Fifty-eight primary family-caregivers of stroke patients on a neurological ward were divided into VGE (29) and control (29) groups. VGE was started within 7 days of patient admission. Interventions included VGE, counseling, and demonstration - re-demonstration. The control group received standard education but not VGE. Data were analyzed using Chi-square test, t-test, ANCOVA, and Pearson correlation coefficients with the SAS program. Results: The VGE group had a significantly lower score for total burden (F=7.19, p=.010) and for sub-scale of time-dependent burden (F=8.44, p=.005) than the control group. There was a negative correlation between primary family-caregiver burden and well-being (r=-.7151, p<.001). Conclusion: Results suggest that the rehabilitation program using VGE was an effective nursing intervention to reduce the burden of primary family-caregivers of stroke patients.

  • 뇌종양 치료성적은 점차 향상되고 있지만 백혈병 등 다른 소아암에 비해 향상 속도가 느리다. 하지만, 근래 MRI, PET scan 등 neuro-imaging 기술의 발달, 뇌종양의 분자유전학적 연구, 외과 수술 방법의 진전, 치료방사선요법의 다양화 등 많은 분야에서 꾸준한 발전을 보이고 있다. 그리고 여러 가지 신약개발에 의한 제 1, 제 2상의 약제시험, antiangiogenesis 약제의 임상시험, gene therapy 등의 연구가 활발하게 진행되고 있으므로, 이에 따른 환자의 치료 성적도 향상될 것으로 기대하고 있다. 외국에서는 여러 대학이 함께 참여하여 작성한 공동의 치료 protocol에 의한 치료가 활발하게 진행되어 생존율을 높이는데 반해, 우리나라에서는 아직까지 각 기관의 협조체제가 구축되지 못한 형편이다. 하지만 금년에 처음으로 여러 대학에서 임상각과가 참석하는 한국소아 뇌종양연구회가 탄생되어 우리나라 뇌종양 환아들의 치료 protocol 개발에 기여할 것을 기대하고 있다. 뇌종양 치료 후 생존하는 아이들을 위한 정밀한 추적관찰이 필수사항인데 여기에 소아과 의사들의 역할은 클 것으로 사료된다. 지능장애, 부적절한 대인관계, 사회적응의 어려움, 치료 후 발생한 내분비계통의 후유증 그리고 2차 암의 발생 등의 정확한 진단 및 치료가 소아과 의사들의 몫으로 남는다. 또한, 이러한 장애가 있는 환아들이 사회에 잘 적응하도록 하기 위해서는 재활교육에 대한 사회적인 관심과 그에 적절한 지원제도의 확립이 절실히 요구된다. 이 점에 있어서도 소아과 의사들의 관심과 지원이 필요하다.

  • Background : This study was identified the effects of a video CD Rehabilitation program on the range of knee angle and activities of daily living in patients with a total knee replacement Method : The data was collected from March 1 to May 31 2009 for the control group and from July 1 to September 31 2009 for the experimental group. The control and experimental groups contained 19 subjects each. A video CD Rehabilitation program configured to 8 minutes in length was shown 8 times every other day. Result : 1. On the day of Hemovac removal, the knee angle and CPM angle in the experimental group increased significantly, but they remained relatively constant on post operation day 21. 2. The activities of daily living were significantly different in the experimental group. Conclusion : 1. An Analysis of Intervention Contents in PNF Therapists for the Patient with Stroke (뇌졸중환자를 대상으로 한 PNF 적용 치료사들의 중재 내용 분석)

  • Purpose : The purpose of this study was to record, analyze and describe in detail, content of PNF intervention for recovery of function after stroke using SPIRIT intervention recording tools. Methods : A convenience sample of 21 physical therapist from a rehabilitation hospital recorded the intervention for 69 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the intervention. Results : A convenience sample of 21 physical therapist from a rehabilitation recorded the interventions for 64 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the interventions accurately and investigate the difference according to time since stroke. Conclusion : Facilitation techniques were the most frequently used intervention(n=1226,35.6%), practice(n=967, 28.0%) and exercise(n-670, 19.4%) in clinical practice of physical therapist. In contrast, Teaching careers and health care professionals to do or assist category is not frequently used intervention(n=5, 0.1%). Therefore, We need to be careful again with category of teaching careers and health care professionals.

Factors Affecting Activity Restriction in the Elderly with Chronic Disease: Using data from the 8th period of the National Health and Nutrition Examination Survey (만성질환 노인의 활동 제한에 영향을 미치는 요인: 국민건강영양조사 제 8기 자료를 활용하여)

  • In this study, a complex sample logistic regression analysis was performed to identify the factors affecting the activity restriction of 2,701 normal elderly and chronically ill elderly aged 65 and over using raw data from the 8th period of the National Health and Nutrition Examination Survey. It was found that the elderly with chronic disease felt more restricted in their activities than the normal elderly. Activity limiting factors in stroke and hypertension patients are subjective health status, economic level, stress perception, and moderate-intensity work and leisure. The factors limiting activity in patients with heart disease were subjective health status and economic level, and factors limiting activity in patients with joint disease were subjective health status and high-intensity work and leisure. Activity limiting factors for lung disease patients are education level, high intensity work and leisure, and endocrine system activity limiting factors include subjective health status, stress perception, high intensity work and leisure, and activity limiting factors for cancer patients. is subjective health status, stress perception, moderate-intensity work and high-intensity leisure. Rehabilitation programs and policy support are needed for the continuous participation of the elderly with chronic diseases.

A Study on the Effect of Virtual Reality Intervention on Cognitive Function in Individuals With Stroke Through Meta-analysis (메타분석을 통한 뇌졸중 환자의 인지기능에 대한 가상현실 중재 효과 연구)

  • Objective : The purpose of this study was to verify the effect of virtual reality interventions (VRIs) on cognitive function in individuals with stroke through a systematic literature review and meta-analysis. Methods : We reviewed randomized controlled trials (RCTs) the last 10 years using academic databases. PubMed, MEDLINE, and CINAHL were used for international studies, and DBpia, KISS, Kyoboscholar, and e-article were used for Korean studies. For the quantitative meta-analysis, subgroups of outcomes were classified into general cognitive function (G-CF), attention and memory (A&M), and executive function (EF). Results : Nine RCTs were analyzed. The total number of participants was 271 (140 in the experimental group). The effect size (Cohen's d) was estimated using a random effects model. The effect sizes of the outcome subgroups of were as follows: small to medium for G-CF (d=0.422; 95% CI: 0.101~0.742; p=0.010), small for A&M (d=0.249; 95% CI: -0.107~0.605; p=0.170), and medium for EF (d=0.666; 95% CI: 0.136~1.195; p=0.014). Conclusion : Considering the various stimuli provided by the virtual environment and the results from available research, virtual reality should be applied to interventions for integrated cognitive functions. In addition, it would be appropriate to be used as an additional intervention to traditional cognitive rehabilitation for stroke.

A Study on the Therapists' Protective Actions of Medical Information Privacy - With a focus on physical and occupational therapists - (환자 프라이버시 보호행동 실천에 관한 연구 - 물리치료사와 작업치료사를 중심으로 -)

  • The present study was carried out with a view to examine the protective actions of physical and occupational therapists for patients's privacy. For this purpose, a survey was conducted with the questionnaires that were distributed to 297 physical and occupational therapists engaged in general hospitals as well as in rehabilitation clinics across the country from January 2 to 13, 2012. From the survey, it was made known that the overall extent of protective actions practiced by them was $3.54{\pm}.733$ in average. Divided by area, the average protective action in communication was $3.62{\pm}.776$, followed by $3.57{\pm}1.013$ in direct contact and $3.53{\pm}.780$ in maintenance and management, respectively. The analysis of general characteristics by each area showed that the older their age gets and the longer their career is, the more the protective action is practiced by them. Although they are well aware of the fact that the medical information of patients should be kept 100% secret and they practice it to almost excellent grade (5), the physical and occupational therapists, as key experts in charge of improving functions of patients, were found to have little concern for and little knowledge about contents of protecting medical information. It is thus required to perform public relation and instruction continuously to intensify their awareness of needs to protect patients' privacy and to implant ethical sense in this regard deep in their mind.
