The Latest Research Trend on Korean Medicine of Rib Fractures: Focused on China National Knowledge Infrastructure (CNKI), PubMed (늑골골절의 한의학적 치료에 대한 최신 연구동향: China National Knowledge Infrastructure (CNKI), PubMed를 중심으로)

  • Objectives The purpose of this study is to identify the latest research trends regarding the nonsurgical treatment of rib fractures and suggest the direction for future research. Methods We searched for papers published from January 1, 2015 to July 1, 2022 in PubMed and the China National Knowledge Infrastructure (CNKI) based on keywords 'rib fracture'. 'Korean medicine', 'oriental medicine', 'TCM', 'moxibustion', 'acupunture', 'cupping', 'electroacupuncture', 'pharmacopuncture', 'fire needling'. Results A total of 79 studies were searched at first research. Then the studies were screening according to criteria and Finally 20 studies were selected. The oriental medical interventions analyzed in this study were fumigation method, herbal medicine, external application, acupuncture, ointment, herb fomentation, Tending Diancibo Pu far-infrared therapy. Conclusions Various oriental medical interventions such as herbal medicine, acupuncture, fumigation, herb fomentation, ointment, and physical therapy are being studied abroad for the treatment of rib fractures. It is considered that additional research related to the nonsurgical treatment for rib fractures is needed in the future in korea.

Therapeutic Effects of Acupuncture for Shoulder Impingement Syndrome: A Systematic Review and Meta-Analysis (어깨충돌증후군에 대한 침치료의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives This review was conducted to evaluate the therapeutic effects of manual acupuncture (MA) for shoulder impingement syndrome (SIS). Methods We searched 12 electronic databases (DBpia, Korean studies Information Service System [KISS], Oriental Medicine Advanced Searching Integrated System [OASIS], Research Information Sharing Service [RISS], China National Knowledge Infrastructure [CNKI], CINAHL, Clinical Key, Cochrane Library, Embase, JAMA, PubMed, Web of Science) to find randomized-controlled clinical trials (RCTs) investigating therapeutic effects of MA for treating SIS. Shoulder Pain and Disability Index scores and numeric pain rating scale or visual analogue scale were analyzed as the main evaluation criteria. Results Among 181 studies, 169 were screened and only 12 RCTs were eligible in our review. Finally, 11 RCTs could be statistically analyzed. MA was more effective than sham treatment and physical therapy in terms of reducing pain (p=0.003, p=0.0007 each). Electroacupuncture (EA) showed more significant effect than physical therapy (PT) for improving shoulder pain (p<0.00001) and shoulder functionality (p<0.00001). Conclusions These results suggest that MA and EA could be superior option for treating SIS than sham treatment or PT. However this review has its limitations due to the small sample size and lack of well-designed RCTs that were included in the study. Further well-designed RCTs are necessary to provide high-level evidence.

  • Transcranial magnetic stimulation (TMS) has been a gold standard for investigating central motor pathways in humans. Diffusion tensor imaging with fiber tractography (DTI FT) is known for its usefulness in detecting white matter lesion in vivo. We investigated the clinical usefulness of elucidating the integrity and continuity of corticospinal tract (CST) by combined use of TMS and DTI FT in this study. We report two cases who have presented with left hemiparesis and evaluated by both TMS and DTI FT; 10-year-old boy with Mitochondrial Encephalomyopathy with Lactic Acidosis and Stroke-like episode syndrome and 20-year-old woman with traumatic brain injury. Combined use of TMS and DTI FT successfully led to localize the brain lesion that might cause motor impairment in patients with abnormal signal intensities in MRI. The results of this study suggest that TMS and DTI FT might provide the detailed information between function and anatomy of the CST, complementarily.

  • Radiation recall is an uncommon phenomenon in which administration of a chemotherapy or another systemic agent induces an acute inflammatory reaction in previously irradiated tissues, often weeks to years after completion of radiotherapy. Gemcitabine can induce an inflammatory reaction within an area of prior radiation. Radiation recall is known to medical oncologists, however only few cases have been reported in Korean journals, therefore physiatrist who diagnose and treat the treatment-related physical impairments of cancer patients must know about it. We emphasize the importance of knowledge of this phenomenon when considering the differential diagnosis of painful limb edema in a patient who has received cancer treatment.

  • Multifidus cervicis plane block has been effectively used to provide analgesia during and after cervical spine surgery, but not for any other purpose. Here, we report three cases of chronic axial neck pain unresponsive to medical treatment. We performed multifidus cervicis plane block bilaterally, which lowered numerical rating scale (NRS) score within 2 weeks. Thus, multifidus cervicis plane block is one of the novel options that may be used for chronic axial neck pain.

Clinical Practice Patterns for Carpal Tunnel Syndrome in Korean Medicine: An Online Survey (손목터널증후군의 한의 임상 진료 현황 조사를 위한 온라인 설문조사)

  • Objectives We conducted an online survey to understand the current status of Korean Medicine clinical practice related to carpal tunnel syndrome (CTS) to develop the CTS Korean Medicine Clinical Practice Guidelines (CPG). Methods A thoroughly reviewed questionnaire was distributed by e-mail to 21,719 Korean Medicine doctors between March 26 and April 30, 2021. All raw data were arranged and analyzed systematically. Results The response rate among the doctors was 4%. Of the respondents, 82.1% responded that it is necessary to develop the CTS Korean Medicine CPG. We also obtained data related to the specific diagnosis, treatment, and prognosis management methods used at various treatment sites for patients with CTS. Conclusions This online survey will be helpful in developing the CTS CPG, which will contribute to the standardization of guidelines and improvements in the reliability of Korean Medicine. Our findings will provide valuable data and resources for future clinical studies on CTS.

The Clinical Effects of Complex Korean Medicine Treatment in Patient with Avulsion Fracture of the Proximal 5th Metatarsal (제5 중족골 근위부 견열골절 환자의 복합 한의치료 효과)

  • The purpose of this study is to report the effect in treating a patient with a case of avulsion fracture of the proximal 5th metatarsal with a combination of Korean medicine therapy. We treated the patient with acupuncture, pharmacopuncture, moxibustion, cupping, and herbal medicine The numeric rating scale, X-ray were applied as outcome measures. After complex Korean medicine treatment, this patient's avulsion fracture of the proximal 5th metatarsal was improved and pain was reduced. This result shows that Korean medical therapy may be an effective option for avulsion fracture of the proximal 5th metatarsal.

Chuna Manual Therapy Alone for Cervicogenic Dizziness: A Systematic Review (추나 단독 치료의 경추성 현훈에 대한 효과: 체계적 문헌 고찰)

  • Objectives The purpose of this study is to provide an evidence base for the effectiveness of Chuna manual therapy (CMT) alone for cervicogenic dizziness. Methods We conducted a search up to October 2018 in 7 electronic databases. The randomized controlled trials (RCTs) that performed CMT for cervicogenic dizziness were included. The risk of bias was assessed using the Cochrane risk of bias tool. Results Ten RCTs met the inclusion criteria. The meta-analysis of 4 RCTs showed favorable results for the use of CMT alone compared to medication. Conclusions In 10 RCTs, we found that CMT was effective in cervicogenic dizziness. However, most of the included RCTs were unclear risk of bias. Therefore, well designed RCTs would be needed to obtain the stronger evidence level of CMT for cervicogenic dizziness.

Korean Medical Treatment Including Daoyin Exercise Therapy in Patient with Lower Back Pain Who Suffered from Idiopathic Scoliosis: A Case Report (특발성 척추측만증의 과거력을 가진 요통 환자에서 도인운동요법을 병행한 한방치료의 효과: 증례 보고)

  • This retrospective study reports the effectiveness of Daoyin exercise therapy in a patient with lower back pain who suffered from idiopathic scoliosis. A patient was treated with Korean medicine containing Daoyin exercise therapy for 4 weeks. The patient was assessed for the numeral rating scale (NRS), Cobb's angle, correctability, and coronal balance. After treatment, the NRS of low back pain decreased from 9 to 1. The Cobb's angle of the thoracic curve decreased from 27.31° to 17.66°. The Cobb's angle of the lumbar curve decreased from 21.86° to 9.05°. Correctabililty was 35.34% in the thoracic curve and 58.60% in the lumbar curve. And coronal balance decreased from positive 32.80 mm to negative 3.20 mm. This study suggests that Daoyin exercise therapy could be effective therapeutic choice for lower back pain with idiopathic scoliosis.

Complex Korean Medical Treatment of Postoperative Ankylosis in Septic Arthritis of the Knee: A Case Report (화농성 무릎관절염의 수술 후 관절강직에 대한 한의복합치료: 증례보고)

  • A 50-year-old Korean male tour guide without any medical or family history complained of left knee pain. After receiving umbilical cord blood injection treatment, the pain gradually worsened. After being diagnosed with septic arthritis in the left knee, arthroscopic lavage, debridement, antibiotic treatment, and routine rehabilitation therapy were performed, but the symptoms persisted. In the hospital, acupuncture, pharmacopuncture, acupotomy, Chuna manual therapy, and cupping therapy were performed in addition to the usual treatment for 59 days. To evaluate the patient's improvement, the numeric rating scale, EuroQol 5-dimension, pain disability index, and Lysholm knee scoring system were used. After treatment, the symptoms improved in all assessment tools, swelling, and range of motion of the joint. Through this study, it was found that complex Korean medical therapies may be effective for postoperative ankylosis in septic arthritis of the knee, and further studies are needed to clarify the therapeutic effect.
