Study of the Introduction on the Aviation Safety Data Protection System (항공안전데이터 보호제도 도입 방안 연구)

  • To promote the aviation safety reporting system that is operated to enhance aviation safety and to utilize related information, it should first be preceded by standards for non-punishment and data protection. It is because the purpose of collection and analysis of aviation safety related data through the aviation safety reporting system is to prevent recurrence of accidents by investigating their causes through collection and analysis of diverse types of information related to aviation safety. Both mandatory and voluntary reporting systems are in operation for aviation safety under the current Aviation Safety Act. It is said that they were introduced to survey causes for accidents and to prevent recurrences. In fact, however, it is hard to expect active implementation of the reporting system for aviation safety unless the reporters are firstly exempted from punishment. Therefore, the system should be improved so that it can satisfy its purpose and the purposes of data collection concerning aviation safety through examination of the purposes of the reporting system. One of the matters that needs to be considered to promote the reporting system should be the scope of aviation safety hindrances presupposed under the current institution. The voluntary aviation safety reporting system differs from the systems of ICAO or the key advanced countries, including the USA and the UK as it limits the target accidents subject to reporting to minor aviation safety hindrances only. That being said, improvements should be made by requiring mandatory reporting of aviation safety hindrances based on their severity while recognizing a greater variety of aviation safety concerns like international standards. Safety actions and sharing of information based on collection and analysis of diverse data related to aviation safety will greatly contribute to enhance aviation safety as the purposes of the reporting system are to explore causes for accidents and to prevent their recurrences. What is most important in this regard is strict data protection and non-punishment principles; compliance with them should be secured. We can hardly expect the successful operation of the system unless the reporter is exempted from punishment and the relevant data is protected as promotion of voluntary reporting is an essential factor for enhancing the safety culture. Otherwise, the current system may induce hiding of relevant facts or data to evade punishment. It is true that the regulation for enhancing safety tends to have limitations or blind spots; nevertheless, it should still be enforced strictly and completely. Technological progresses and mistakes of operators appear in different forms based on individual cases. The consequential damages may amount to a truly severe level. Therefore, we have studied and suggested to the methods of activiation and amendments on the aviation safety reporting system, which is referred for one of the proactive safety management systems. The proposed improvement of the reporting system and introduction of non-punishment for collection of aviation safety data for deploying a preemptive prevention system would serve as the backbone for enhancing aviation safety in Korea.

Long-term Outcomes of Conjunctivo-limbal Autograft Alone and Additional Widening of Limbal Incision in Recurrent Pterygia (군날개 재발에 있어서 자가윤부결막이식술 단독 및 추가 윤부절개술의 장기 결과)

  • Purpose: We report the clinical outcomes of patients undergoing additional widening of the limbal incision to prevent disease recurrence after conjunctivo-limbal autograft combined with pterygial excision. Methods: We retrospectively compared 95 eyes with primary and 12 eyes with recurrent pterygia randomized to treatment via two surgical methods: conjunctivo-limbal autograft alone and combined with widening of the limbal incision to allow for pterygial removal. We widened the limbal incisions by 1 mm on both the superior and inferior limbal margins and removed the pterygia. The outcomes were compared between 39 eyes undergoing conjunctivo-limbal autograft alone and 68 eyes undergoing conjunctivo-limbal autograft with widening of the limbal incision. Results: The mean overall postoperative follow-up period was $29.6{\pm}10.5months$ and conjunctivo-limbal autograft alone group was $27.4{\pm}11.5months$, combined with widening of the limbal incision group was $30.7{\pm}9.7months$. Six pterygia (15.4%; four primary and two recurrent) developed in those undergoing conjunctivo-limbal autograft alone and two (2.9%; one primary and one recurrent) in those undergoing additional widening of the limbal incision; the recurrence rate differed significantly between the two groups (p < 0.05). In the group treated with conjunctivo-limbal autograft alone, the mean time to development of a new primary pterygium was $6.3{\pm}3.4months$ and that to development of a recurrent pterygium $4.3{\pm}2.5months$; the respective values for the group undergoing additional widening of the limbal incision were $12.1{\pm}2.6$ and $8.4{\pm}4.6months$; the recurrence rates differed significantly (p < 0.05). Conclusions: Conjunctivo-limbal autograft with additional widening of the limbal incision used to treat both new primary and recurrent pterygia was more effective in terms of reducing pterygial recurrence than conjunctivo-limbal aAutograft alone.

해양사고 원인규명 통합 분석 시뮬레이션 시스템

  • 해양사고 원인규명 통합 분석 시뮬레이션 시스템은 해양사고가 발생하는 과정(선회)을 포함하여 충돌, 좌초, 접촉, 전복, 침수 및 침몰 등의 해양사고를 유체-구조 연성 해석기법의 고도 정밀 M&S 시스템을 사용하여 과학적으로 해양사고의 원인을 분석하고 사고의 손상과정을 체계적으로 재현할 수 있는 시스템이다. 해양사고는 육상과 공중에서 발생하는 자동차와 비행기 등의 충돌이나 추락사고와는 달리 공기의 밀도보다 천배의 물에서 발생하므로 물에서 부양되고, 운동하고, 선내에 물이 침수되고, 운항 중일 때 파도도 생성시키고, 두 물체가 근접할 경우에는 압력이 압착되고, 두 물체가 스쳐 지나거나 안벽이나 해저를 근접하여 운항할 경우에는 압력이 저하되는 등 물에서의 연성효과(interface effect)를 충분히 고려하여 재현할 수 있어야 정확하게 해양사고의 원인을 규명 및 분석할 수 있을 것이다. 또한 황천에서 발생하는 해양사고일 경우에는 강한 조루, 강풍 및 해일성 파도 등을 불규칙 스펙트럼을 사용하여 정확히 구현하여야 황천에서 발생하는 해양사고의 원인을 충분히 분석할 수 있을 것이다. 이러한 해양사고 통합 분석 시뮬레이션 시스템을 이용하여 과학적이고 정확한 해양사고의 원인규명 및 분석으로 심판의 획기적인 신뢰 구축과 심판 지연에 따른 사회적 비용을 최소화하고, 해양사고의 원인과 과실 책임, 나아가서 사고 재발방지 대책수립 등에도 활용하는데도 크게 기여할 것으로 사료된다.

  • 한국전력공사에서는 2015년 4월말 기준 808개의 변전소를 운전 중에 있다. 변압기 용량으로 보면 299,734MVA에 달하고 있으며, 매년 증가하는 전력수요에 맞추어 신규 변전소 건설과 더불어 변전설비가 계속하여 확충되고 있다. 또한, 계절별 및 시간대별 부하변동에 따른 변전소 모선의 전압변동에 대응하기 위하여 Sh.C(Shunt Condenser), Sh.R(Shunt Reactor), SVC(Static Var Compensator) 등 다양한 무효전력보상 및 조절 설비들이 지속적으로 설치되고 있다. 이들 무효전력보상 설비들은 주로 변전소 모선에 전용 차단기를 통하여 연결되어 운전되고 있으며, 전용 차단기는 매일 시간대별 부하변동에 대응하여 개폐빈도가 많은 다빈도 차단기로서 잦은 개폐조작에 따른 내구성이 필요하며 변전소 모선전압을 기준전압 범위 이내로 안정적으로 유지하기 위한 신뢰성이 요구되고 있다. 본 논문에서는 345kV Sh.R 개폐용 동일유형의 345kV 50kA 1점절 다빈도 차단기에서 차단조작시 발생한 차단부 절연파괴 고장의 원인을 분석하고 재발방지를 위한 대책에 대하여 논하였다.

  • 자살시도는 결코 개인만의 문제라고 치부할 수 없는 사회적인 중요한 문제임에도 불구하고 국가차원에서의 관심과 예방계획은 아직도 미비한 수준이다. 본 연구에서는 NEDIS 등록자료를 이용하여 응급의료기관에 내원하는 자살시도자의 규모 및 특성에 대한 현황을 파악함으로써 응급실에서의 자살시도자에 대한 효과적인 관리 방안과 정책 마련 등을 위한 기초자료를 제공하고자 한다. 자살시도자는 여성 비율이 상대적으로 높았고, 연령은 20대~40대에서 많이 발생하였다. 서울특별시와 경기도에서 발생률이 가장 높았고, 자살시도 환자 수가 여름, 일요일에 많은 것으로 나타났다. 자살 시도 후 직접 내원하는 비율이 높았고, 약물 및 농약, 열상으로 인한 내원이 점차 증가하는 추세인 것으로 나타났다. 자살시도에 실패한 사람은 재발 가능성이 높은 것을 고려할 때, 응급실을 내원한 자살시도자들에 대한 지속적이고 전문적인 맞춤형 사후관리 프로그램이 절실히 요구되며, 자살 재발 위험을 방지할 수 있는 포괄적인 국가적 관리프로그램을 적극적으로 모색하는 등 다각적인 정신보건정책이 마련되어야 한다.

  • A 2 year-old, 3.8 kg, intact female Maltese was presented with unilateral pain and reddish discharge from the left ear caused by a polyp in the horizontal ear canal. The polyp was diagnosed as osteoma histopathologically and removed through total ear canal ablation. Three months after surgery, osteoma was recurred and broken out of edematous skin lesion. It was considered that an inappropriate surgical removal of the tissue which attached base boarder of the polyp would be cause of the recurrence of the osteoma.

  • In the consideration of the working conditions, which have several kinds of works operating at the same time, at construction sites, it is difficult to prevent industrial accidents. There are a number of works to employ flammable materials and hot works simultaneously operated without fire protection systems. It causes a huge fire and casualties. In this research to analyze an accident case, the reasonable prevention methods are suggested throughout the property tests for the organic solvents and the analysis of the behavior for vapour cloud in the underground area of the construction site.

  • Thoracic sympathectomy is the radical and definite treatment of palmar hyperhidrosis. From January 1992 to March 1997, 4 patients with recurrent hyperhidrosis underwent resympathectomy via VATS at the Department of General Thoracic and Cardiovacular Surgery, Young Dong Severance Hospital. There were 2 men and 2 women and mean age was 20.0 years. There were moderate to severe adhesions at previous resection site but no thoracotomies were performed. There was no sweating on palms in all cases and all patients were greatly 5,Btisfied with those results postoperatively. In conclusion, recurrent hyperhidrosis was successfully treated with resympathectomy via VATS. In order to prevent recurrence and minimize the postoperative complication, the proper localization of the 2nd sympathetic ganglion and the radical excision of anatomical variation including Kuntz fiber are needed.

Full mouth rehabilitation in a patient with peri-implantitis: A case report (Peri-implantitis 환자에서의 전악 재수복 증례)

  • Peri-implantitis appears in almost 20% of patients who received implant treatment, and increase in its number is inevitable as time goes by. Although it can be treated by both non-surgical and surgical procedures, in cases which include severe bone loss, explantation and rehabilitation may be necessary. Careful treatment planning and considerations to prevent recurrent peri-implantitis should be taken into account. In the following case presented, a patient with chronic periodontitis and peri-implantitis was successfully rehabilitated after removal of several implants. Extraction and explantation of multiple teeth and implants were followed by full mouth reconstruction with fixed implant prostheses on the mandible and implant retained overdenture on the maxilla. Surgical and prosthetic measures to prevent recurrent peri-implantitis were taken into consideration.
