Treatment and posttreatment changes following intrusion of maxillary posterior teeth with miniscrew implants for open bite correction (Miniscrew implant를 이용한 상악 구치부 압하와 개방교합 치료의 효과 및 안정성)

  • Objective: This study investigated the skeletal and dentoalveolar changes after intrusion of maxillary posterior teeth using miniscrew implants during the treatment and posttreatment period. Method: The subjects consisted of 11 adults (1 male, 10 females) whose open bites were treated by posterior teeth intrusion with miniscrew. The lateral cephalometric x-rays of pre-treatment, post-treatment, and the retention period were evaluated. Results: The average intrusion of maxillary posterior teeth at the end of the treatment period was 2.22 mm (p < 0.001) and the average extrusion of maxillary posterior teeth at the end of the 17.4 month retention period was 0.23 mm (p = 0.359). The relapse rate was calculated at 10.36%. A decrease in overbite after the 17.4 month retention period was 0.99 mm (p < 0.05). The relapse rate was calculated at 18.10%. The skeletal changes and vertical height change of maxillary posterior teeth during the retention period was statistically insignificant, but there was a significant decrease in overbite (mean 0.99 mm, p < 0.05). The amount of intrusion and the overbite improvement from treatment significantly correlates to the degree of relapse. Conclusions: The results indicate that intrusion of the maxillary posterior teeth using miniscrews is an effective, non-surgical treatment modality which can reasonably be used to address adult open bite.

Acoustic scene classification using recurrence quantification analysis (재발량 분석을 이용한 음향 상황 인지)

  • Since a variety of sound occur in same place and similar sound occurs in other places, the performance of acoustic scene classification is not guaranteed in case of insufficient training data. A Bag of Words (BOW) based histogram feature is foreseen as a method to overcome the problem. However, since the histogram features is made by using a feature distribution, the ordering of sequence of features is ignored. A temporal information such as periodicity and stationarity are also important for acoustic scene classification. In this paper, temporal features about a periodicity and a stationarity are extracted by using a recurrent quantification analysis. In the experiment, performance of the proposed method is shown better than other baseline methods.

A meta analysis of maxillary expansion : comparisons of intercanine/intermolar expansion and rapid/slow expansion (상악확장술 치료효과의 메타분석 : 견치간 폭경과 대구치간 폭경 확장의 비교 및 급속과 완속 확장의 비교)

  • As a research method that weighs and combines evidence, meta analysis produces evidence that is more powerful than the original studies. The purpose of this study was to compare the jntercanine/intermolar expansion and rapid/slow expansion in the maxillary expansion treatment using meta analysis. Medline was searched from 1979 to 2000 for all studies examining the stability of transverse expansion of the human maxilla, and 388 articles were found. Then these articles were reduced to 7 based on the defined inclusion and exclusion criteria, and a cumulative Meta evaluation score was computed for each study. The results were as follows; 1. The mean expansion in intermolar width was 6.0mm. of the 6.0mm, 4.8mm was retained and $20.0\%(1.2mm)$ was relapsed while wearing retainers. 2. In intercanines width, the mean expansion was 3.7mm. of the 3.7mm, 2.6mm was retained and $29.7\%(1.1mm)$ was relapsed while wearing retainers. 3. The differences In the amount of expansion and relapse between rapid expansion group and slow expansion group were less than $6\%(0.1-0.3mm)$. But, there might be differences in the skeletal/dental exapnsion ratios according to the expansion method.

A Study on Relapse Predictors in Korean Alcohol-Dependent Patients - A 24 Weeks Follow up Study - (24주 추적 조사를 통한 한국인 알코올 의존 환자의 재발 예측 인자 규명 연구)

  • Objectives : The aim of this prospective study is to investigate predictors estimating relapse in Korean alcohol-dependent patients using variables like alcohol history, drinking craving, treatment motivation and insight. Methods : Alcohol dependent patients(N=48) who completed questionnaires about sociodemographic variables and drinking history, Timeline Follow-Back(TLFB), Obsessive-Compulsive Drinking Scale(OCSD), Alcohol Urge Questionnaire(AUQ), Pennsylvania Alcohol Craving Scale(PACS), University of Rhode Island Change Assessment(URICA), Hanil Alcohol Insight Scale(HAIS) were followed-up for 24weeks. Subjects who drank heavily(5 standard drinking or more/day) or were not followed up anymore were classified as the relapse group. We used logistic regression analysis with backward elimination of SPSS PC+11.5 to investigate relapse estimate predictors. Results : Average drinking amount per drinking day for last 1 year and HAIS score were predictors of relapse in alcohol-dependent patients. Conclusions : Our findings suggest that therapist should give more attention to alcohol-dependent patients who had more drinks per drinking day for last 1 year and had lower insight level.

SKELETAL RELAPSE AFTER ORTHOGNATHIC SURGERY OF CLASS III SKELETAL OPEN-BITE (개구교합을 가진 3급 부정교합환자의 악교정수술후 재발에 관한 연구)

  • This study was intended to evaluate a post-operative relapse tendency in mandibular prognathism patients with open-bite. 18 patients with or without open-bite have undergone sagittal split ramus osteotomy and were investigated radiologically with cephalogram. The results were as following 1. The preoperative anterior facial height, mandibular plane angle and mandibular gonial angle were larger in open-bite patients than in nonopen-bite patients.(p>0.05) 2. There was significant correlation between surgical change of SNA, mandibular plane length and long-term relapse.(p<0.01). 3. There was relatively stable postoperative anterior facial height in open bite patients. 4. There was no significant correlation between the surgical change of mandibular plane angle, amount of mandibular setback and long-term relapse.(p>0.01) 5. This study showed that horizontal relapse was more significant than vertical relapse.

Prognosis of Recurred TMD Patients According to Conservative Therapy (측두하악장애 재발환자의 보존적 처치에 따른 예후)

  • We examined 104 patients(primary group, controlled group) who had visited PNUH from 1994 to 2002, having been diagnosed as temporomandibular disorders(TMDs) and treated in conservative ways such as Behavior modification, medications, physical therapies and splint therapies. We also examined 54 patients(recurred group, experimental group) who had visited PNUH from 1991 to 2001, having been diagnosed as TMDs and experienced recurrence after conservatively treated. To find out the symptoms of Recurred TMD patients and their results of conservative treatments, we compared these two groups mentioned above. The obtained results were as follows: 1. Both primary and recurred groups have showed great improvements with conservative treatments. 2. Both primary and recurred groups have showed no differences in pain, LOM, MCO in their first visits but the noise were louder in primary group. 3. Both primary and recurred groups have showed no differences in pain, LOM, MCO when the treatments were over but the noise were louder in recurred group. 4. Treatments modalities, diagnosis, sex, kind of disease had not affected the results of treatment in either of groups. 5. It has come out that much better results were achieved when the patients in primary group had treated for over 6 months and for more than 10 times.

  • The purpose of this study is to compare the differences between first visits and the recurred time of TMD patients about the number of visits, the treatment plans, the symptoms and the results after the conservative treatments. This could lead us to assess the prognoses of TMDs treated conservatively. We investigated 54 patients who have visited the Department of Oral Medicine in PNUH from 1991 to 2001, diagnosed as TMDs and treated conservatively with medications, physical therapies and splints. The treatments were terminated since the subjects have shown much improvements and resumed when TMDs recurred from 1992 to 2002. We researched the diagnoses, symptoms, the number of visits, the treatment plans and the results comparing the time before and after the treatments were carried out and following are the results. 1. Both the primary and the recurred groups improved after having been treated conservatively. 2. Both the primary and the recurred groups showed no differences in pains and MCOs when they first visited though the noises and LOMs turned out to be more serious in primary group. 3. Both the primary and the recurred groups had no differences in pains, noises, LOMs, MCOs when the treatments were over. 4. The results of treatments were not affected by treatment plans, sex, diagnoses in both primary and recurred groups. 5. Most of the patents tended to visit less than 10 times in recurred patients.

Radiotherapy of Locally Recurrent Rectal Carcinoma (수술 후 국소재발된 직장암의 방사선치료 결과)

  • Purpose : We reviewed the treatment results for the patients with locally recurrent rectal carcinoma. The object was to evaluate the treatment outcome and to identify the prognostic factors influencing the survival. Methods and Materials: Twenty-eight patients with locally recurrent rectal carcinoma treated principally with external-beam radiation therapy between 1982 to 1996 in the Department of Radiation Oncology at Paik and Hanyang Hospital were reviewed retrospectively Of these, 17 patients had initially abdominoperineal resection, 9 had low anterior resection, and 2 had local excision. No patients had received adjuvant radiation therapy for the primary disease. There were 14 men and 14 women whose ages ranged from 31 to 72 years (median age:54.5). Median time from initial surgery to the start of radiation therapy for local recurrence was 11 months (4~47 months). Radiation therapy was given with total doses ranging from 27 to 64.8 Gy (median=51.2 Gy). Results : The median survival was 16.7 months. The 2-year and 5-year survival rates were 20.1%, 4.1% respectively. Upon multivariate analysis, overall survival was positively correlated with duration of intervals from initial surgery to local recurrence (P=0.039). Relief of pelvic symptoms was achieved in 17 of 28 patients (60.7%). Pain and bleeding responded in 40% and 100% of patients, respectively Conclusions : Patients with locally recurrent rectal carcinoma treated with radiotherapy have benefited symptomatically, and might have increased survivals with a small chance of cure. But, patient were rarely cured (median survival : 10 months, 5-year survival : less than 5%). Overall survival was positively correlated with long intervals from initial surgery to local recurrence. Future efforts should be directed to the use of effective therapy for patients with locally recurrent rectal carcinoma and adjuvant therapy for patients with rectal cancer to reduce the incidence of pelvic recurrence.

Risk Factor for Recurrence in Completely Resected Stage IB Non-small Cell Lung Cancer (완전 절제된 IB기 비소세포폐암에서 수술 후 재발의 위험 인자)

  • Background: Complete surgical resection is the most effective treatment for stage IB non-small cell lung cancer (NSCLC). Recurrence accounts for the disappointing survival rates after resection. There has been renewed interest in adjuvant therapy after complete resection. Appropriate selection of effective adjuvant therapy will depend on the prognostic factors for recurrence. Material and Method: The study included 114 patients with completely resected stage IB NSCLC. The variables selected for the study were gender, age, the type of resection, cell type, the degree of differentiation, the tumor size and the presence of visceral pleura invasion. The Kaplan-Meier method was used to estimate the survival and disease-free survival rate. The results were compared using the log rank test. Multivariate analysis was performed by Cox's proportional hazard model. Two-sided p-valves < 0.05 were considered to be statistically significant. Result: The 3-year overall survival and the disease-free survival rates were 87.0% and 79.4%, respectively. The degree of differentiation showed a significant influence on disease-free survival according to the univariate analysis. According to the multivariate analysis, a poor grade of differentiation was a significant poor prognostic factor. Conclusion: These results demonstrate that poor differentiation may be a poor prognostic factor for patients with completely resected IB NSCLC. Therefore, the patients with a poor grade of differentiation may require adjuvant therapies.

Effect of Prophylactic Cranial Irradiation in Acute Lymphoblastic Leukemia in Children (소아 급성 임파구성 백혈병에서 예방적 전뇌 방사선조사의 효과)

  • CNS prophylaxis with 18 or 24 Gy cranial irradiation plus intrathecal methotrexate was given to 134 childhood acute lymphoblastic leukemia patients who had got bone marrow remission (M1) after remission induction chemotherapy from August 1979 to December 1986. The rate of initial total CNS relapse was 14.2% (19/134), the rate of isolated CNS relapse was 5.2% (7/134), and the rate of CNS relapse concomittantly combined with bone marrow relapse or testicular relapse was 9% (12/134). Male sex or older age was accociated with higher CNS relapes and the initial peripheral leukocyte count over 50,000/ul had higher relapse rate. Relapse with radiation dose of 18Gy was somewhat lower than that with 24Gy. Within 4 years after CNS prophylaxis occurred 89% of the total CNS relapses, 100% of the isolated CNS relapses, and 83% of the combined CNS relapses. Adjusted to exposed cases to risk of CNS relapse, the total CNS relapse rate was 11.9% during maintenance chemotherapy and 4.9% after maintenance chemotherapy.
