Parenting Experience of Parents with a Disabled Child (장애자녀를 둔 부모의 양육경험)

  • Purpose: This study used a phenomenological method, which is a qualitative study, in order to understand the vivid experience of parents who have a disabled child. Methods: The number of participants was 10. The subjects of this study were parents who are raising their disabled child who attends a special school or normal middle or high schools in D city. The period for collecting materials was from September 2005 to July 2007. The phenomenological analysis method suggested by Giorgi was used. Results: The meanings of the child parenting experience of parents with a disabled child are (1) feeling of frustration of the delayed treatment, (2) difficult to accept the diagnosis of disability, (3) having eyes centered on her child, (4) accepting the disabled child, (5) giving all his energy to his disabled child, (6) making efforts for his family's unity, (7) recognizing the difficulty of parenting disabled child, (8) recognizing the difficulty of education for disabled child, (9) feeling pitiful for his their normal children, (10) planning for disabled child's future. Conclusions: All the programs for disabled child should be programed for their entire life. It is necessary to provide physical, psychological, emotional, social and economic nursing intervention to relieve or reduce the burden of parents with disabled children.

IMPACT OF PARENTAL PSYCHIATRIC DISORDER ON OFFSPRING'S DEPRESSION, ANXIETY, SELF CONCEPT AND PERCEPTION OF FAMILIAL RELATIONSHIP (정신과 환자 자녀의 우울, 불안, 자기 개념 그리고 가정환경의 특성에 대한 연구)

  • Objectives:This study was to investigate the impact of parental psychiatric disorder on offspring's depression, anxiety, self concept, perception of familial relationship compared with offspring of normal control. In offsprings of parents with psychiatric disorder, this study explored whether their psychopathology, self concept, and perception of familial relationship were influenced by parent’s sex, onset time of parent’s psychiatric disorder and parent’s psychiatric diagnosis. Methods:52 offsprings aged 10-18 years of 39 psychiatric outpatient were surveyed from June, 1997 to April, 1998 and completed several questionnaaire, including Korean from of the Family Environment Scale, Korean form of the State-Trait Anxiety Inventory for Children, Korean form of Kovac’s Children’s Depression Inventory, and Korean form of Piers-Harris Children’s Self-Concept Scale. Their score was compared with offsprings’ of normals. In offsprings of parents with psychiatric disorder, they were compared according to parent’s sex, onset time of parent's psychiatric disorder and parent’s psychiatric diagnosis. Results:The results were as follows:1) Offsprings of parents with psychiatric disorder reported higher level of state anxiety and lower level of the FES expressive subscale than offsprings of normals(p<0.05). But they reported higher level of PHCSCS intellectual & school status subscale and popularity subscale than normals(p<0.05). 2) There were no differences in anxiety, depression, self concept, and perception of familial relationship between patient’s sex. 3) Offsprings less than 3 years old when parent’s psychiatric disorder had developed showed higher level of trait anxiety and lower level of FES control subscale than offsprings more than 3 years old (p<0.05). 4) There were no diferences in anxiety, depression, self concept, and perception of familial relationship between patient’ diagnostic groups(schizophrenia spectrum disorder-mood disoderneurosis). Conclusion:The finding indicated that self reported scale of anxiety and depression showed no significant difference between offsprings of psychiatric patients and offsprings of normals. In offsprings of parents with psychiatric disorder, parent’s sex and psychiatiric diagnosis had no influence on offspring’s psychopathology. But the offspring’s age(before 3 years old) when the parent’s psychiatriric disorder developed had influence on higher level of offspring’s trait anxiety. For further high risk group study, direct interview and evaluation of parent-child agreement or teacher-child agreement will be needed in longitudinal study.

ASSOCIATION BETWEEN THE PERCEIVED PARENTING AND PROBLEM BEHAVIORS IN KOREAN MALE ADOLESCENTS (청소년에서 자녀가 지각한 부모 양육태도와 문제 행동의 연관성 - 서울시내 남자 고등학생을 대상으로 -)

  • Objectives:Dysfunctional parent-child relationships have been associated with various mental and conduct disorders in adolescence and mental problems in adulthood as well. Most studies have done in clinical settings and little is known about the relationship between parenting and adolescent problem behaviors in community settings. This study is done to compare the perceived parenting and problem behaviors between problem behavior group and those of non-problem behavior group in male adolescents. Methods:The subjects of this study were 147 male high school students in Seoul, who do not have any known mental or conduct disorders and live with their real parents. Parental Bonding Instrument and Youth Self Report were administered to evaluate the perceived parenting and problem behaviors of adolescents. Results:There were significantly higher scores of mother overprotection in problem behavior group when comparing to non-problem behavior group. There was a significant association between mother overprotection and problem behavior when adjusting the number of family members, mother's job, parents' education level, grades, and religion. Conclusion:The parenting such as mother overprotection can be helpful to predict the problem behaviors in male adolescents, and appropriate parenting may prevent various problem behaviors in male adolescents.

  • 1970년 이후 우리나라는 산업화로 인하여 사회구조와 가정구조가 크게 변화 되었다. 도시집중화로 인한 각종 공해 발생, 교통난, 인구이동의 문제를 비롯하여 그리고 핵가족화로 인한 가부장적 권위상실 및 가정교육부재, 청소년 비행등 수 많은 문제가 야기되었다. 특히 핵가족화로 발생된 심각한 사회문제는 전통윤리의 붕괴와 가정교육의 부재현상을 발생 시켰다. 이와같은 문제발생에 관한 선행연구를 종합해보면 첫째, 핵가족화로 인해 발생한 가족규모의 축소현상으로 자녀들의 다면적 인격형성에 결손을 초래하여 성장후 독립심과 의지가 약하고 고집이 세며 이기적이고 자기중심적인 성향이 되어 사회생활에 부적응 현상을 초래하게 되었다고 한다. 그 이유는 다면적 인간관계의 결여로 상호 협조하고 사랑을 교환한 경험 부족과 외동자녀에 대한 부모의 관심이 자녀의 욕구에만 집중되어 자녀종속적인 태도가 크게 적용하였기 때문이다. 둘째는 산업화 사회에서의 부부동시 사회참여가 자녀양육에 어머니와의 애착형성이 결여되어 모성실조를 겪게 된 문제를 들 수 있다. 어머니와 자녀간의 애착형성이 결여 됨으로 발생되는 문제는 지적 정체현상과 정서발달의 장애가 주 원인이 되어 감정 통제력이 약해서 공격적 행동이나 반 사회적 행동으로 확대되어 청소년문제 발생의 원이 되고 있음을 시사하고있다. 세째로 산업화사회와 경쟁력 조장을 들 수 있다. 산업사회를 개방사회, 경재력사회 혹은 능력사회라하여 고학력위주의 선발형태가 입시경쟁을 부추기고 있으며 그에 따라 많은 청소년들과 학부모들이 상위 지향적인 심리가 작용하여 자녀들의 입시경쟁에만 집중하게 되었다. 이러한 원인으로 인하여 인간교육 즉 가정교육을 외면하여 반도덕적, 반사회적 문제를 야기하게 하였다. 특히 전통적 가정윤리가 붕괴되어 부모에 대한 효관의 약화, 부부간의 애정약화 및 물질지향적인 사고가 이혼률을 증가시켜 가정붕괴를 가져왔다. 이상에서 논의된 자녀교육의 부재가 자녀들의 지적 정서적 문제로 확대되어 산업화사회의 모순점을 노출시키고 있음을 알 수 있다. 이와같은 문제를 해결하기 위한 대안으로 연구자가 제시하는 방안은 훌륭한 가정교육을 위해서 우선되어야 하는것은 부모자신의 인격적 개인완성을 제안하였다. 인격의 완성은 양심과 행동이 일체하는 사람을 지칭한다. 다음은 서로 위하여 사는 삶을 영위하는 인격체가 되어 양심의 법에 따르는 삶, 타인을 위하여 사는 삶을 자녀들에게 보여줌으로서 반두라의 모델링 교육을 제안하였다. 마지막으로 오늘날과 같은 암기위주의 경쟁교육이 아니라 타고난 잠재성 개발에 주력하는 교육으로, 적성과 소질을 개발하는 창조적 산 교육장을 마련하여 주고 또한 그것을 뒷바침하는 가정윤리교육의 회복이 무엇보다도 이 시대가 절실히 요구하는 바람직한 교육이라 하겠다.

A CASE OF CONDUCT DISORDER CONFINED TO FAMILY CONTEXT (가정에 국한된 품행장애 1례)

  • Conduct disorder has been subclassified by the symptom characteristics of socialization, aggression, or age of onset. In ICD-10, ‘Conduct disorder confined to family context’ has been developed as a distinct subtype. Which delineates the conduct problems which take place only in the family and related situation. These authors experienced a case which presents a child who shows aggressive and violent behavior to his parent and brother, severe tamper tantrum and destructiveness only in family. By through history taking, observation and treatment after admission, we could find that insecure and ambivalent attachment between parent and child due to chronic neglect and abuse, and inconsistent parental behavior contribute to many behavioral and emotional problems of this child. We reviewed the relationship between conduct disorder and family pathology and treatment strategy for conduct disorder with family problems.

The Relationship between Level of Participation in Children's play, Empathy Ability and Coping Behavior in Mothers of Children with Disabilities (장애아동 어머니의 놀이참여 수준과 공감능력, 대처행동과의 관계)

  • The aim of this study was to investigate the relationship between level of participation in children's play, empathy ability and coping behavior in mothers of children with disabilities. The study included 94 mothers of children with disabilities and the study was conducted from May 2018 to August 2018 based on the questionnaire about the level of participation in children's play, empathy ability and coping behavior. According to the study, Mothers of children with disabilities actively participate in physical play with their children and have a higher emotional empathy compared to cognitive empathy, and have acted to actively solve the problems for their children. When children have been treated for more than 1 to 2 years, the level of participation in children's play and the coping behavior were the highest. The longer their children's treatment period, the higher the mothers' empathy. The correlation between level of participation in children's play and coping behavior were significant. And empathy and coping behavior had a strong positive linear relationship. These results suggest that there are differences in level of participation in children's play, empathy and coping behavior depending on mothers' of children with disabilities general characteristics and the higher level of mothers' of children with disabilities participation in children's play, the more empathetic they have. When individualized intervention is provided, we look forward to providing professional service that take into account the characteristics of mothers of children with disabilities.

Social Interaction of Caregivers and Their Children with Down Syndrome or Without Disability (다운증후군 아동과 정상아동의 보호자와의 사회적 상호관계에 대한 비교연구)

  • This study investigated caregivers' communication styles and children's emotional development. Emotion-laden puzzle tasks were used to elicit caregivers' communication styles while interacting with their children. Participants included children with Down syndrome (N=10) and typical children (N=15) and their caregivers. As expected, caregivers of children with Down syndrome (DS) used more behavior and attention directives with their children, and caregivers of typical children used more conversation-eliciting prompts with their children. Parents of children with Down syndrome also used a unique communication style in which they asked a question and immediately answered it themselves. Additionally, caregivers of typical children focused more on emotion concepts in their communications with their children and caregivers of DS used more cognitive concepts such as labeling colors and shapes. The results revealed that caregivers of children with Down syndrome usually tried to educate children by emphasizing cognitive concepts to compensate for their delayed development. Because the children are delayed in their emotional development, parents may need help in intervening on the area of emotional development.

NURSING PROBLEMS OF THE INPATIENTS WITH CONDUCT DISORDER (행동장애 입원환아의 간호문제)

  • Nursing problems of 48 hospitalized patients with Conduct Disorder at a Child-Adolescent psychiatry inpatient were analyzed by reviewing nursing records. The results showed that the problems such as ineffective individual coping, impaired social interaction, disturbance in self-concept, potential for violence, alteration in parenting, altered growth and development were continued from early to later phase of the hospitalization and the other problems such as self-care deficit, anxiety, sleep disturbance, altered nutrition, hyperthermia were temporary. The etiologic factors related to these problems were underdeveloped ego, low self-esteem, dysfunctional parent-child relationship, some situational crises in family and handicap like mental retardation or epilepsy. Therefore nursing approach for the patients with Conduct Disorder should focus on ego growth and improvement of interpersonal relationship through systematic and long-term nursing plans and interventions for these patients and their family.

Adaptation Resilience in Relation to Parenting Stress for Mothers with Children of Developmental Disabilities (학령 전 발달장애아 어머니의 적응유연성과 양육스트레스의 관계)

  • The following research is based on a strengths perspective for analyzing the effects of adaptation resilience on parenting stress. The subject were 170mothers whose children have developmental disabilities and attend pre-school and social welfare centers for early education. The results were following: 1) The order of dimensional scores from highest to lowest were social resources, structuralization of personality, social achievement, personal strengths, perception of future, strength of family cohesion. 2) The parenting stress of the subjects came out to be lower than average scoring 2.71 out of 5. Dimensionally, grief of parents came out to be the highest. 3) Adaptation resilience and parenting stress for subjects exhibited a negative correlation. Within adaptation resilience, the dimensions of perception of future, personal strengths, social resources came out to be factors that have significant effects on parenting stress. 4) factors relating to the mother's adaptation resilience were more important than general characteristics belonging to handicapped children and their mothers for the purpose of explaining and predicting parenting stress.

Heart Rate Variability and Parenting Stress Index in Children with Attention-Deficit/Hyperactivity Disorder (주의력결핍 과잉행동장애 아동에서의 심박 변이도와 양육 스트레스)

  • Objective:The aim of this study was to evaluate the relationship between sustained attention deficits in Attention-Deficit/Hyperactivity Disorder(ADHD) children and short-term Heart Rate Variability(HRV) parameters. In addition, we evaluate the relationship between The ADHD rating scale(ARS), the computerized ADHD diagnostic system(ADS) and Parenting stress index- short form(PSI-SF). Methods:This study was performed in the department of children and Adolescent psychiatry, Korea university Guro hospital from august 2008 to January 2009. We evaluated HRV parameters by short-term recordings of 5 minutes. K-ARS and ADS are used for screening and identifying ADHD children. Intelligence was measured using Korean educational Developmental Institute-wechsler Intelligence Scale for Children. The caregivers Complete Parenting Stress Index scale for evaluation parent stress. Results:The low frequency(LF) was significantly correlated with response variability of ADS. However, the other variables of ARS and ADS were not significantly correlated with LF. Hyperactivity subscale of ARS was significantly correlated with parental distress subscale and difficult child subscale of PSI-SF and inattention subscale of ARS was also significantly correlated with dysfunctional interaction and difficult child subscale of PSI-SF. Conclusion:The LF, 0.10-Hz component of HRV is known to measure effort allocation. This study shows that the LF component of HRV is significantly correlated with the response variability of ADS. This means that more severe symptoms of ADHD were correlated with the increase in the LF that means decreased effort allocation. These results also support the clinical usability of HRV in the assessment of ADHD. Furthermore, PSI-SF is correlated with hyperactivity and inattention variables of ARS.
