Iliacus Muscle Rupture with Associated Partial Femoral Nerve Palsy during Soccer Game - Case Report - (축구 경기 중 발생한 장골근 파열과 부분 대퇴 신경 마비 - 증례보고 -)

  • Iliacus muscle tears are a rare injury seen after the high-energy trauma or as a result of low-energy injuries in patients with a bleeding diathesis as coagulopathy, receiving anticoagulation therapy and hemophiliac. Femoral nerve palsy due to compression from a hematoma by iliacus muscle rupture are rarely reported. Routine evaluation includes MRI to confirm and define the pathologic abnormality supplemented by EMG and nerve conduction studies to evaluate patterns and extent of femoral nerve injury. Hematologic evaluation for bleeding diathesis may preceded, if suspicion of coagulopathy is present. We report the case of a healthy 32-year-old male with iliacus rupture and concomitant femoral nerve palsy sustained by kicking motion during soccer game. After 6 months of observation with non-operative treatment regimen, satisfactory results were obtained, so we report it with a review of the literatures.

  • 한국재래산양(韓國在來山羊) 12마리의 후지근(後肢筋)을 절개하여 관찰하였던 바 다음과 같은 결과를 얻었다. 1. 한국재래산양(韓國在來山羊)의 후지근(後肢筋)에서는 다음과 같은 근(筋)들을 관찰할 수 있었다 : 소요근(小腰筋) M. psoas minor, 대요근(大腰筋) M. psoas major, 장골근(腸骨筋) M. iliacus, 요방형근(腰方形筋) M. quadratus lumborum, 대퇴근막장근(大腿筋膜張筋) M. tensor fasciae lata, 중둔근(中臀筋) M. gluteus medius, 심둔근(深臀筋) M. gluteus profundus, 둔이두근(臀二頭筋) M. gluteobiceps, 반건양근(半腱樣筋) M. semitendinosus, 반막양근(半膜樣筋) M. semimbranosus, 봉공근(縫工筋) M. sartorius, 박근(薄筋) M. gracilis, 치골근(恥骨筋) M. pectineus, 내전근(內轉筋) M. adductor, 대퇴방형근(大腿方形筋) M. quadratus femoris, 외폐쇄근(外閉鎖筋) M. obturatorius externus, 내폐쇄근(內閉鎖筋) M. obturatorius internus, 쌍자근(雙子筋) M. gemelli, 대퇴사두근(大腿四頭筋) M. quadriceps femoris, 제삼비골근 M. fibularis tertius, 내측지신근(內側趾伸筋) M. extensor digitorum medialis, 장지신근(長趾伸筋) M. extensor digitorum longus, 전경골근(前脛骨筋) M. tibialis cranialis, 장비골근 M. fibularis longes, 외측지신근(外側趾伸筋) M. extensor digitorum lateralis, 비복근 M. gastrocnemius, 가제미근(筋) M. soleus, 천지굴근(淺趾屈筋) M. flexor digitorum superficialis, 심지굴근(深趾屈筋) M. flexor digitorum profundus, 슬와근(膝窩筋) M. popliteus, 골간근(骨間筋) M. interosseus medius. 2. 천둔근(淺臀筋)의 전부(前部)는 대퇴근막장근(大腿筋膜張筋)과 융합된 것 같고, 후부(後部)는 대퇴이두근(大腿二頭筋)과 융합된 것 같다. 그러나 천둔근(淺臀筋)의 후부(後部)와 대퇴이두근(大腿二頭筋)이 결합된 것으로 생각되는 부분에는 완전융합이 일어나지 않고 천둔근(淺臀筋)을 구분(區分)할 수 있을 정도로 표면으로 2근(筋)을 분리(分離)할 수 있었다. 3. 외측지신근(外側趾伸筋)과 내측지신근(內側趾伸筋)의 건(腱)은 부전골의 원위(遠位) 1/3부(部)에서 서로 건막성(腱膜性)띠에 의하여 서로 연결 되었는데, 이 건막성(腱膜性) 띠는 건섬유(腱纖維)의 방향(方向)으로 보아 외측지신근(外側趾伸筋)의 건(腱)에서 분리(分離)되어 나온 한 가지 (branch)가 내측지신근(內側趾伸筋)의 건(腱)으로 이행되고 있었다. 4, 양(羊)에서 볼 수 있는 이상근(梨狀筋) M. piriformis과 장모지신근(長母趾伸筋) M. extensor hallucis longus은 나타나지 않았다.

  • Gastrointestinal stromal tumor (GIST) occurring outside the gastrointestinal tract is uncommon. When GIST is found outside the gastrointestinal tract, it has greater possibility to be diagnosed with metastatic lesion from other primary focus. But it rarely occurs in extragastrointestinal tract primarily, and then even outside abdomen. We experienced one case of GIST occurred from right buttock area of 25 year-old man. We could not find out the primary focus in abdomen. The tumor was developed inside gluteus medius and minimus and extended to pelvic inner area destroying iliac bone and adjacent sacrum. Here, we report the case of GIST with terminology of extragastrointestinal stromal tumor (EGIST).

A Comparison between Ilioinguinal and Iliohypogastric Nerve Block and Infiltration of Local Anesthetics for Postoperative Pain after Inguinal Herniorrhaphy in Children (소아에서 서혜부탈장 수술 후 장골서혜신경 및 장골하복신경 차단술과 창상국소주사의 진통효과 비교)

  • The present study compared the postoperative analgesic effects of ilioinguinal and iliohypogastric nerve block with infiltration of local anesthetics (bupivacaine) into the wound in children after inguinal hernia repair. Ninety children below 7 years old who were scheduled elective inguinal hernia repair were randomly allocated into one of three groups. The patients in nerve block (NB) group, ilioinguinal and iliohypogastric nerve block was done with 0.5 mL/kg of 0.25% bupivacaine. The patients in infiltration of local anesthetics (LI) group, 0.5 mL/kg of 0.25% bupivacaine was infiltrated into the wound after surgery. The patients in control group were allocated as a Control group. Postoperative pain was assessed at 1, 3, 5, and 24 hours after operation with FLACC scale and additional analgesic consumption were counted. The three groups were not significantly different in age, sex, body weight, and duration of operation. Pain scores at 1 hour and 3 hours after operation were significantly higher in Control group than in NB group and LI group (p<0.01), whereas there were no difference between NB group and LI group. The rescue analgesics administration was significantly higher in Control group (n=11) than in NB group (n=6) and LI group (n=7) (p<0.05). There were 2 cases of transient femoral nerve palsy in NB group. Both of ilioinguinal and iliohypogastric nerve block and infiltration of local anesthetics into the wound provided effective postoperative analgesia in early postoperative period following inguinal hernia repair in children. But no difference between the two methods. Technically, infiltration of local anesthetics into the wound was easier and safer than ilioinguinal and iliohypogastric nerve block.

Clear Cell Chondrosarcoma of the Tibia Diaphysis: A Case Report (경골 간부에 발생한 투명세포연골육종: 증례 보고)

  • Clear cell chondrosarcoma is a very rare malignant bone tumor that shows a strong predilection for the epiphysis or metaphysis of long bones. Many studies have reported that the proximal end of the femur is the most commonly affected site, followed by the proximal end of the humerus. Histopathologically, tumor cells of this type have centrally located round nucleoli with clear cytoplasm and a distinct cytoplasmic membrane. Generally, clear cell chondrosarcomas is not confused with conventional chondrosarcomas. However, when it involves the diaphysis in long bones, diagnosis can be hindered, as only three reports of this exist in the literature. We report herein an unusual case of clear cell chondrosarcoma of the tibial diaphysis in a 42-year-old male.

  • Fracture site osteomyelitis begins rarely with an acute event but rather has a subacute onset. It develops almost exclusively in the mandibular region. The treatment principles of mandibular fracture site osteomyelitis are complete removal of inflammatory tissue and drainage, rigid fixation with or without autogenous bone bone graft and specific antibiotic therapy. But hyperbaric oxygen used as a modality in the treatment of intractable osteomyelitis. In this paper, we reported that the three patients who have osteomyelitis accompying secondary in fection after mandibular fracture.

Bizzare Parosteal Osteochondromatous Proliferation (Nora's lesion) which affects Humeral Shaft -A Case Report- (상완골 간부에 발생한 기괴 방골성 골연골성 증식증 - 1례 보고-)

  • Bizzare parosteal osteochondromatous proliferation is a rare lesion, tends to frequently recur. It was first described in 1983, when Nora and his colleagues reported 35 examples of a proliferative lesion involving small bones of the hand and the feet. It was regarded as occurring only in the small bones of the hand and the feet initially, but from then on, other authors have been reported cases including those involving long bones, skull and maxilla. We experienced a case of bizzare parosteal osteochondromatous proliferation which affects the humeral shaft in a 14-year-old male patient. Radiographs showed calcified mass measuring $4{\times}3$ cm in size and attached to the underlying cortex, which had a broad stalk base. Histologically, it showed hypercellular cartilage maturation to the trabecular bone and plump amount of fibrous stroma in the spaces around the bony trabeculae. Bizzare parosteal osteochondromatous proliferation is a relatively rare lesion, but has a recurrent behavior and histologically tends to be mistaken for malignancy. It can be treated by excision.

[Retraction] Effects of Cynanchum wilfordii and Phlomis umbrosa Extracts on Bone Growth and Serum Insulin Like Growth Factor-I ([논문철회] 백하수오와 속단 추출물의 뼈 골격 성장과 IGF-I 생성 촉진 효과)

  • This study examined the efficacies of Cynanchum wilfordii and Phlomis umbrosa extracts on serum insulin like growth factor-I (IGF-I) and bone growth by raising rats in vivo. C. wilfordii and P. umbrosa extracts significantly increased serum IGF-I by 42% and 22% than the control, respectively. Treatment with ${\alpha}$-amylase when manufacturing these extracts remarkably increased the concentration of IGF-I by 63% and 36% above the control, respectively. This meant that these extracts, especially ${\alpha}$-amylase treated extracts, maintained a higher level of IGF-I secretion in the treated groups. In addition, increases of 6% in femur length were found after 8 weeks of oral administration with these extracts. These results indicate that C. wilfordii and P. umbrosa extracts have beneficial effects on bone growth via IGF-I.

  • An indirect reduction through the bicipital groove and allogenic iliac tricortical bone graft was performed as a treatment for a large Hill-Sachs lesion with a rotator cuff tear in an acute traumatic shoulder joint dislocation in an elderly patient. Six months after surgery, the rotator cuff healing and bone union were confirmed. The patient also showed satisfactory clinical results with 95 points of the American Shoulder and Elbow Surgeons (ASES) shoulder score and active range of motion with 155° flexion, 120° abduction, and 70° external rotation and 30° internal rotation at 90° abduction. The surgical method can be considered for a large Hill-Sachs lesion with rotator cuff tear in an acute traumatic shoulder joint dislocation in elderly patients.
