Convergent effects of oral health behaviors on number of remaining teeth of the elderly welfare recipients (기초 수급 노인들의 구강건강 행태가 잔존 치아 수에 미치는 융복합적 영향)

  • This study was to investigate their relevance between oral health behaviors and number of remaining teeth of the elderly welfare recipients through the fluoride and scaling of the business. Researcher collected 660 subjects over 60 elderly welfare recipients investigated self-administered survey and use denture and number of remaining teeth though oral examination. Increasingly age number of teeth remaining reduced, there is a difference on the number of remaining teeth according to gender, age, the use of oral hygiene products, self diagnosis oral condition, brushing after lunch and showed significant difference(p<0.05). The remaining number of teeth can be prevented by attention and efforts of subjects. Therefore, we investigated convergent effects of oral health behaviors on number of remaining teeth of the elderly, I hope that oral health care program continually provides expansion of preventive treatment and to maintain the provision of health care about vulnerable group.

The relationship between dementia and the number of remaining tooth of the elderly women on senior center (경로당 여성 노인들의 치매와 잔존 치아 수와의 관련성)

  • Recently, with the population growth of elderly people, concerns about oral health related to the quality of life of the elderly are increasing. The purpose of this study is to assess the association between the elderly women divided into dementia, suspected dementia, healthy groups and the remaining teeth of them. Total 177 elderly women of over 60 years old, visiting on senior center in some community dwelling, were assessed for oral condition and their cognitive function with MMSE score. All the collected data were analyzed by chi-square test, t-test, and multiful logistic regression using SPSS. Multiful logistic regression was used to analyze the relationship of dementia according to MMSE score and remaining teeth, and 95% confidence intervals were computed. Odds ratio(OR) of the number of remaining teeth 0-10 was 3.43 (95% confidence interval: 1.382-8.997). This study showed significant difference and the relationship between dementia according to MMSE score and the number of remaining teeth of the elderly women.

Influence of the number of remaining teeth of on sleep time and stress of adults older than 40 years (40세 이상 성인의 수면시간과 스트레스가 잔존 치아 수에 미치는 영향)

  • This paper aimed to study on the relevance of mental health and oral health. The subjects were recruited total 3,261 elderly of over 40 years old, using the Sixth Korea National Health and Nutrition Examination Survey(KNHANES). All the collected data were analyzed by chi-square test, t-test, and logistic regression using SPSS 23.0 version. Logistic regression was used to analyze the relationship of sleeping time or stress and the number of remaining teeth, and 95% confidence intervals were computed. Sleeping time odds ratio(OR) of the number of remaining teeth was 1.52 (95% confidence interval: 1.074-2.135) and Stress odds ratio(OR) was 2.39 (95% confidence interval: 1.855-3.090), but there is no statistically significant difference after adjusted age, gender and other variables.

한국 노인의 잔존치수와 저작능력과의 관계

  • 본 연구는 한국인 노년층이 대표적으로 섭취하는 강도별 음식을 조사하고 각 음식별로 씹어 먹을 수 있는 음식들이 무엇인지를 조사하여 자신이 보유하고 있는 자연치아수와의 상관관계를 알아보아 자연치아의 건강도를 유지, 관리, 예방하는데 하나의 지표로 사용하고 더 나아가 국민구강건강향상에 이바지 하고자 실시되었다. 전국의 60세 남 녀 586명을 대상으로 치과의사와 치과위생사가 구강검사와 면접법으로 설문내용을 기록하는 방식으로 진행하였다. 수집된 자료는 SPSS WIN 14.0 프로그램을 이용하여 분석하였다. 분석기법으로는 잔존치아수와 저작능력간의 관계를 상관계수와 회귀분석으로 분석 검증을 실시하여 다음과 같은 결과를 얻었다. 1. 저작능력과 구강상태와는 치석, 동요도, 충전점수, 우식경험점수, 발거치아수, 임플란트 수, 자연치아수와 나이, 고혈압, 당뇨병, 기타질환, 흡연이 유의한 것으로 나타났다(p<0.05). 2. 회귀분석 결과 나이가 높을수록 저작능력이 감소하며, 자연치아수가 많을수록 저작능력이 높은 것으로 나타났으며 회귀방정식은 저작능력=79.417-(연령$\times$0.281)+3.813$\times$치아수와 같았다(p<0.05). 3. 회귀분석결과 저작능력점수와 자연치아수와의 관계가 가장 높은 것으로 나타났고 회귀방정식은 저작능력=3.817$\times$자연치아+58.694와 같았다(p<0.05). 4. 자연치아로 식사를 하기 위해서는 한국 노인의 주식인 밥과 김치정도를 잘 씹어 먹으려면 잔존 자연치아수가 18개 정도는 있어야 아주 잘 씹어 먹을 수 있고, 잘 먹으려면 12개의 자연치아가 있어야 하며, 육류를 잘 먹기 위해서는 최소한 18개는 있어야 잘 먹을 수 있고, 25개가 있으면 아주 잘 씹어 먹을 수 있다는 연구결과가 나왔다.

Incidence of Wolf Teeth in Jeju Ponies and Jeju Pony Crossbreds (제주마와 제주산마에서 이리치아 잔존율)

  • Wolf teeth have been reported with incidence rate of 13% to 31.9% in horses of both sexes. However, there is no report about the incidence of wolf teeth in pony breeds. In order to determine the incidence of wolf teeth in Jeju Ponies and Jeju Pony crossbreds, oral examination and palpation of the interdental space of ponies at the Jeju Race Park were performed from 2006 to 2010. Results of this study showed that a total of 90 (3.4%) of the 2,675 examined Jeju Ponies and Jeju Pony crossbreds had wolf teeth. Among 30 Jeju Ponies that had wolf teeth, 22 were male (73.3%) and 8 were female (26.7%). Among 60 Jeju Pony crossbreds that had wolf teeth, 25 were male (41.7%) and 35 were female (58.3%). In Jeju Ponies, wolf teeth were most common in 3-years-old, followed by ${\geq}4$-years-old, then 2-years-old. For Jeju Pony crossbreds, wolf teeth were most common in 3-years-old, followed by 2-yearsold, then ${\geq}4$-years-old. In conclusion, the incidence of wolf teeth in Jeju Ponies and Jeju Pony crossbreds was considerably lower than what has been reported for horses in other countries.

Full Mouth Rehabilitation in a Patient with Limited Restorable Space (수복 공간이 부족한 환자에서의 완전구강회복)

  • Loss of posterior support may cause overloading and excessive wear of remaining teeth. Moreover, the extrusion of antagonistic teeth leads to the destruction of the occlusal plane. The loss of vertical dimension of occlusion (VDO) also emerges clinically, which may bring the loss of esthetic appearance and function. These patients who suffer from the loss of posterior support, often require vigorous periodontal treatments (osteotomy, crown lengthening) and extensive oral rehabilitation. Sixty three years old female patient visited for the prosthetic treatment of the posterior edentulous area. She had no other systematic disease and parafuctional habits for prosthetic treatment. Intraoral and radiographic examinations were done. The evaluation of VDO and vertical dimension of rest position were evaluated for proper prosthetic procedures and diagnostic wax up was done. As a result of diagnosis, VDO was increased by 2 mm considering the loss of VDO and space for the prosthetic treatment. After the pretreatments, initial preparation of teeth and provisionalization were carried out. Six weeks later of provisionalizaion, final preparation and impression was performed. Using the duralay resin copings, jaw relation was registered. The master cast was mounted and definitive restoration was fabricated. After the evaluation of esthetic and function, pick up impression for clinical remounting was done. Lucia jig was made for new jaw relation and occlusal adjustment on the articulator. Definitive restoration was delivered and the patient was periodically recalled for additional occlusal adjustment. From this case, the satisfactory functional and esthetic results through full mouth rehabilitation with increase vertical dimension were achieved.

Oral Health-Related Quality of Life of the Elderly under Visiting Health Care (방문건강관리사업 대상 노인의 구강건강 관련 삶의 질과 관련 요인)

  • The aim of this study was to investigate the relationship between the oral health status of elderly under visiting health care and their oral health-related quality of life. The subjects were 300 elderly under visiting health care program over 65 years old in Asan city and they were interviewed. The result showed that average Geriatric Oral Health Assessment Index (GOHAI) score was 37.36 and the lowest scored question was 'never satisfied with the oral condition'. In GOHAI related to oral health behaviors, unmet dental treatment experience was statistically significant (p<0.001). In overall GOHAI, number of remaining teeth (p<0.001) and the age had statistical significance and men had higher score in pain and discomfort (p<0.05), under age of 74 limited function factors score was high (p<0.01). The number of remaining teeth had statistical significances in most factors except pain and discomfort (p<0.001). The factors affecting GOHAI were age (p<0.05), number of remaining teeth (p<0.001) and perceived health state (p<0.001). In order to improve oral health-related quality of life of elderly receiving visiting oral health care services, it is necessary to increase denture support by securing budgets and developing oral health promotion programs at the local government level.

AUTOTRANSPLANTATION OF A MALPOSITIONED MANDIBULAR SECOND PREMOLAR : A CASE REPORT (이소매복된 하악 제2소구치의 자가치아이식을 이용한 치험례)

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. Treatment options for the management of impacted teeth are separated into four categories: observation, intervention, orthodontic or surgical relocation and extraction. Autotransplantation may be defined as the transplantation of embedded, impacted or erupted teeth, from one site to another in the same individual into extraction site or surgically prepared sockets. Autotransplantation ensures preservation of natural tooth, induction of alveolar bone growth and root development, offers one of the fastest and most economically feasible means in the replacement of young patients' missing teeth. This case presents a malpositioned impacted mandibular premolar of an 11-year-old girl. It was thought that orthodontic traction was difficult because of its unfavorable impacted position. Therefore the tooth was treated by autotransplantation, we can observe good healing pattern during 12 months.

Eruption Guidance of Horizontally Impacted Permanent First Molar with Primary Retention of Primary Second Molars: Case Reports (제2유구치의 일차성 만기잔존이 동반된 제1대구치 수평매복의 맹출유도 : 증례보고)

  • Tooth eruption involves a complex developmental process of tooth migration from the dental follicular origin to the final occlusion position in the oral cavity via the alveolar process. Disturbance of tooth eruption can occur at any point in a series of eruption stages; however, horizontal impaction of the mandibular first molar and primary retention of the primary mandibular second molar are rarely observed simultaneously. This study describes the treatment for two cases of horizontally impacted first molar with primary retention of primary molar. The primary retention of the primary mandibular second molar was extracted, and orthodontic traction was applied to the horizontally impacted primary mandibular first molar. Subsequently, displacement of the premolar tooth bud was improved and space regaining for eruption was achieved, guiding to normal eruption of the first molar.
