Assessment of Effect of Pulmonary Rehabilitation on Skeletal Muscle Metabolism by $^{31}P$ Magnetic Resonance Spectroscopy (호흡재활치료 전후 $^{31}P$ 자기공명분석법을 이용한 골격근대사의 변화에 관한 연구)

  • Pulmonary rehabilitation has been known to improve dyspnea and exercise tolerance in patients with chronic lung disease, although it does not improve pulmonary function. The mechanism of this improvement is not clearly explained till now; however some authors suggested that the improvement in the skeletal muscle metabolism after the rehabilitation could be a possible mechanism. The metabolc changes in skeletal muscle in patients with COPD are characterized by impaired oxidative phosphorylation which causes early activation of anaerobic glycolysis and excess lactate production with exercise. In order to evaluate the change in the skeletal muscle metabolism as a possible cause of the improvement in the exercise tolerance after the rehabilitation, noninvasive $^{31}P$ magnetic resonance spectroscopy(MRS) of the forearm flexor muscle was performed before and after the exercise training in nine patients with chronic lung disease who have undertaken intensive pulmonary rehabilitation for 6 weeks. 31p MRS was studied during the sustained isometric contraction of the dominant forearm flexor muscles up to the exhaustion state and the recovery period. Maximal voluntary contraction(MVC) force of the muscle was measured before the isometric exercise, and then 30% of MVC force was constantly loaded to each patient during the isometric exercise. After the exercise training, exercise endurance of upper and lower extremities and 6 minute walking distance were significantly increased(p<0.05). There were no differences of baseline intracellular pH (pHi) and inorganic phosphate/phosphocreatine(Pi/PCr). After rehabilitation pHi at the exercise and the exhaustion state showed a significant increase($6.91{\pm}0.1$ to $6.99{\pm}0.1$ and $6.76{\pm}0.2$ to $6.84{\pm}0.2$ respectively, p<0.05). Pi/PCr at the exercise and the recovery rate of pHi and Pi/PCr did not show significant differences. These results suggest that the delayed intracellular acidosis of skeletal muscle may contribute to the improvement of exercise endurance after pulmonary rehabilitation.

Simple Camera-based Evaluation System for Lower Limb Alignment during Pedalling (자전거 페달링 시 하지 정렬 평가를 위한 영상 시스템 개발)

  • Simple camera-based system for evaluation of lower limb alignment as a part of an automated cycling fitting system was developed and verified in this study. Developed imaging system can evaluate lower limb alignment quantitatively during pedaling using a general camcorder and single marker attached on the knee. Threshold-based marker detection algorithm was proposed in this study. Experiment was carried out to compare the trajectory data from marker detection algorithm of the developed imaging system with the trajectory data from 3-D motion capture system. Results showed that average error between trajectories was 2.33 mm (0.92 %) in the vertical direction and 0.62 mm (1.86 %) in the medio-lateral direction. There existed significant correlation between two measured values (r=0.9996 in the vertical direction and r=0.9975 in the medio-lateral direction). It can be concluded that developed imaging system be applied to evaluate lower limb alignment which is an important factor for dynamic bicycle fitting.

Effects of Virtual Reality Based Video game and Rehabilitation Exercise on the Balance and Activities of Daily Living of Chronic Stroke Patients (가상현실기반 비디오게임과 재활운동이 만성기 뇌졸중 환자의 균형 및 일상생활동작에 미치는 영향)

  • 연구목적: 닌텐도 위를 이용한 가상현실기반 비디오게임 프로그램을 만성기 뇌졸중 환자에게 적용하여 균형능력 및 일상생활동작에 어떠한 영향을 미치는지 알아보고, 뇌졸중 환자의 재활프로그램으로서 적용 가능성이 있는지 알아보고자 한다. 연구방법: 만성기 뇌졸중 환자를 무작위로 가상현실기반 비디오게임 프로그램을 적용한 실험군(n=7)과 적용하지 않은 대조군(n=10)으로 구분하여 연구를 진행하였다. 연구에 참여한 모든 대상자에게 30분의 Bobath therapy와 15분간의 FES 치료를 기본적으로 실시하였다. 이에 더하여 실험군은 가상현실기반 비디오게임 프로그램을 1일 30분이내, 주 5회, 3주간 실시하였다. 대조군은 자전거 운동과 보행훈련으로 30분간 시행하였다. 실험 전 후 눈뜨고 외발서기(OLST; open leg standing test), Timed Up and Go(TUG) 검사, 10m 걷기 검사, Functional Independence Measure(FIM)를 측정하였다. 실험 전과 실험 후 측정값의 차이를 비교하기 위해 Wilcoxon Signed Ranks Test를 실시하였다. 그리고 각 측정값의 변화량에 대한 실험군과 대조군 사이의 차이를 알아보기 위해 Mann-Whitney U Test를 실시하였다. 연구결과: 실험결과는 다음과 같다. 1) 실험군에서는 FIM의 유의한 증가와 TUG, 10m 걷기 검사의 유의한 감소를(p<.05) 보였다. 대조군에서는 OLST의 증가와 TUG, 10m walking test의 감소가 나타났지만 통계적으로 유의하지 않았다. 오직 FIM에서만 유의한 증가가 나타났다(p<.05). 2) 실험 전 후의 실험군과 대조군의 각 측정값들의 평균차를 비교한 결과 실험군은 대조군보다 실험 전 후 OLST, TUG, 10m walking test 차이의 평균은 컸지만 통계적으로 유의하지 않았다. 결론: 이상의 결과로부터 가상현실기반 비디오게임이 만성기 뇌졸중 환자의 동적균형능력 및 일상생활동작 향상에 효과가 있음을 알 수 있었다.

A Study on the Park Using Pattern Focusing on user Behavior in River-eco-park (하천생태공원 이용자의 이용행태에 따른 시설이용패턴에 관한 연구)

  • This study is about facilities using pattern for 'River-eco-park' that is Hwa-myeong Eco-park, Sam-rak Eco-park, Maek-do Eco-park. Through doing surveys, most patterns could be divided 5 patterns; 'Eco observation facility pattern', 'Promenade pattern', 'Bike path pattern', 'Sports facility pattern', 'Fitness center pattern'. Then we find channel of movement of user patterns by follow-up surveys. On the basis of this surveys, we are going to suggest some directions for River-eco-park design.

Analysis of Respiratory Gas by Training on Healthcare Indoor Bicycle (헬스케어용 실내 자전거 운동에 의한 호흡가스 분석)

  • This study was conducted to observe the change of limbs stroke and respiration gas parameters in our new bicycle fitness system. We hypothesized that the variable force of left and right limbs might be effective for sensing stimulation in modified new unequal pedal bicycle system. It has been developed, which can provide visual information and different length of pedal with left and right limbs. Experimental results showed different activities between the left and right limbs where the activity of the left limb increased than that of right limb. Especially, the soleous muscle activity increased both in control and experimental groups by this training method. But oxygen and carbon dioxide partial pressures in respiratory gas increased during training method. These results suggest that acidosis of blood was led by this process. Consequently, this bicycle training is concluded that aerobic training could affect different limb activities. Finally, we expect that our new bicycle system will be effective for healthcare with proper balance between the left and right limbs.

Continuous Blood Pressure Prediction Using PTT During Exercise (PTT를 이용한 자전거 운동 중 지속적인 혈압의 예측)

  • The purpose of this work is to predict the systolic blood pressure (BP) during exercise from pulse transit time (PTT) for warning of possible danger. PTT was calculated as the time between R-peak of ECG and the peak of differential photoplethysmograph (PPG). For the PTT-BP model, we used regress equations from previous studies and 3 kinds of new models combining linear and nonlinear regress equation. The model parameters were estimated with the data measured under low to middle intensity exercise, and then was tested with the data measured under high intensity exercise. Predicted BP values after high intensity exercise were compared with those measured by cuff-type sphygmomanometer. The results showed that the error between measured and predicted values were acceptable for the monitoring BP. We tested PTT-BP models 1 month after the identification without further calibration. Models could predict the BP and the errors between measured and predicted BP were about 5mmHg. The suggested system is expected to be helpful in recognizing any danger during exercise.

  • Modern people, who are fully enjoying convenience with improved technology, are pursuing health next to convenience. Accordingly, the smart healthcare industry is rapidly increasing, and various companies are launching healthcare system products with applied VR. However, existing products on the market are expensive or professional products. Therefore, there are parts that are difficult for the general public to use. In this paper, we propose smart health care contents that can experience virtual reality anytime and anywhere at an affordable price and take care of health through exercise using Arduino and general bicycles sold in the market.

Long-term Combined Exercise has Effect on Regional Bone Mineral Density and Cardiovascular Disease Risk Factors of the Elderly with Osteoporosis (장기간의 복합운동이 골다공증 노인의 신체부위별 골밀도와 심혈관질환 위험요인에 미치는 영향)

  • The purpose of this study was to find the effects of long-term combined exercise on regional bone mineral density(BMD) and cardiovascular disease(CVD) risk factors in the elderly with osteoporosis(OP). For the purpose, the subjects of this study were separated by two groups with thirty-one elderly women, who the first group was combined exercise group(CEG, n=16) and second group was non exercise group(CON, n=15). The combined exercise program was made up of warm-up (10min), work-out (aerobic; 30~45min/HRR 40~60%, resistance; 1RM * 50-70%, 8-10 * 2set ~ 10-15 * 1set), and cool-down (10min). Exercise group of the inspection have been trained 5 times a week for 1years. The results : At first, the variables of regional BMD were significantly different to pelvis, spine, trunk and T-score in two groups. At second, the variables of CVD risk factors were significantly different to SBP and DBP as well as TC, TG, LDL-C and HDL-C in two groups. As results of these conclusion, this study have positively effect shown that CEG was superior to CON in regional BMD(pelvis, spine, trunk and T-score), blood pressure(SBP, DBP) and plasma lipids(TC, TG, and LDL-C). Especially, the long-term combined exercise was provides a striking overall health quality of life with improving BMD and reduced CVD risk factors in the elderly with OP. In the future, other researches should deal with specific measures that reduction in mortality due to chronic disease and improvement quality of life for the development of programs in multiple researches of osteoporosis and chronic diseases.

An Assessment on the Urban Riverfront in Shincheon, Daegu - Focused on a Universal Design Concept - (대구광역시 신천의 친수공간 평가 연구 - 유니버설디자인 개념을 중심으로 -)

  • The purpose of this study is to derive a desirable riverfront construction plan to me for the activities of citizens through the evaluation and analysis of the urban riverfront space from the perspective of universal design. Therefore, previous studies were examined in order to induce evaluation tools that bhve been grafted from the universal design concept; in addition, a field survey was conducted in Shincheon, Daegu, which was selected as the study target, in order to induce problems and improvement directions from the perspective of universal design. The major results can be summarized as follows. (1) In the 'fairness' aspect, all the items such as installation of integrated functional signage, showing pictures, symbols, foreign language signs, and restroom entrances signage were determined to be 'All Unsuitable' for all sections; and therefore, it is necessary to improve the fairness of usage for everybody. (2) In the 'Functionality(Usability)' aspect, all items such as installation of bicycle paths beside access roads, installation of integrated functional signs, and night light signs were determined to be 'All Unsuitable' for all sections; therefore, it is necessary to improve the functionalities of these facilities. (3) In the 'Convenience' aspect, all items such as the installation of bicycle parking areas, continuous rest facilities, and back and ann support(handles) at resting facilities were determined to be 'All Unsuitable' for many sections; therefore, it is necessary to improve these facilities for the convenience of usage. (4) In the 'Information(Recognizability)' aspect, all items such as showing pictures, symbols, foreign languages and installation of night light signs, and restroom entrances signage were determined to be 'All Unsuitable' for all sections; therefore, it is necessary to improve the recognizability to minimize misunderstandings and confusion. (5) In the 'Safety' aspect, all items such as the installation of safe pedestrian paths in parking areas, using anti-slip and shock absorption materials on restroom floors, and the continuous installation of pedestrian paths that are separate from bicycle paths were determined to be 'All Unsuitable' for all sections; therefore, it is necessary to improve the safety to prevent accidents. (6) In the 'Amenity' aspect, access roads, parking areas, hygiene facilities, convenience facilities, and waterside facilities for many sections were determined to be 'All Unsuitable'; therefore, it is necessary to conduct more concentrated hygiene management. (7) In the 'Accessibility(Mobility)' aspect, all items such as the installation of safe pedestrian paths in parking areas, and continuous pedestrian paths that are separate from bicycle paths were determined to be 'All Unsuitable' for all sections; therefore, it is necessary to improve the accessibility to provide safety and convenience. (8) In the 'Durability' aspect, access roads, parking areas, rest facilities, convenience facilities, fitness facilities, and waterside facilities were determined to be 'All Unsuitable' for many sections, therefore, it is necessary to improve sunken or damaged areas by inspecting facilities by section.

Walking test for assessing lung function and exercise performance in patients with cardiopulmonary disease (심폐질환 환자에서 걷기검사를 이용한 폐기능 및 운동기능의 평가)

  • BACKGROUND : Dyspnea is common among patients with cardiopulmonary disease, and "daily disability" is defined as a functional impairment resulting from exercise intolerance. The maximal oxygen uptake(VO2max) during exhausting work is not only the best single physical indicator of the capacity of a man for sustaining hard muscular work, but also the most objective method by which one can determine the physical fitness of an individual as reflected by his cardiovascular system. However, the expense, time and personnel requirements make this procedure prohibitive for testing large group. The walking test is well-known type of exercise and it cost nothing to perform and have good reproducibility. Thus we performed the walking test and investigated correlations with spirometry, ABG and exercise test. METHOD: We observed the walking test and exercise test by cycle ergometer in 37 patients who visited our hospital because of dyspnea. Arterial blood gas analysis and spiromety, dyspnea index were performed, too. RESULT : (1) The VO2max was significantly lower in patients with COPD and cardiovascular disease than asthma and dyspnea on exertion group(p<0.05). The walking test distance was also lower in former. (2) The 12 minute walking test was significantly correlated with VO2max, PaCO2, FVC(%), FEV1(%) in all patients(p<0.05), and the walking test was only conelated with VO2max in patients with COPD(p<0.05). (3) In COPD patients, the VO2max was best correlated with FEV1(%) and FVC(%) and significantly correlated with walking test. But there was no correlation between walking test and FEV1(%) & FVC(%). (4) The 6 minute walking test was well correlated with 12 minute walking test(r=0.92. p<0.01). CONCLUSION : The walking test is the simple method for assessing exercise performance in patient with cardiopulmonary disease and a reliable indicator for VO2max. And the walking test is practical method for assessing on everyday disability rather than maximal exercise capacity. The 6 minute walking test is highly correlated with 12 minute walking test and a less exhausting for the patients and a time-saving for the investigator.
