Measuring heart rate variability by deep breathing and using this to evaluate the autonomic nervous system is a well-known method widely used in various clinical fields. In hospitals, expensive equipment is installed and managed by experts for this purpose. This paper discusses an app we developed that can easily test the autonomic nervous system by deep breathing anytime, anywhere using a smartphone, and then reviews experiments performed to verify the results. The experiments were conducted in the clinical room of the Department of Neurology at Soonchunhyang University Cheonan Hospital on eight volunteers. We tested the autonomic nervous systems of the volunteers first with an electromyography device installed in the hospital and then with the app under identical conditions. We performed a correlation analysis on the results of these two methods using the Pearson method, and this yielded a very high correlation of 0.98.

Purpose : Syncope appears to be common. However, the mechanism of syncope is not clear. Increased vagal activity and withdrawal of sympathetic stimulation cause hypotension, bradycardia and finally loss of consciousness. The purpose of this study was to evaluate changes of cerebral blood flow velocity, blood pressure, and heart rate during tilt test in children with vasovagal syncope. Methods : Sixty four children with a past history of syncope were evaluated. The stand up test was performed for 15 minutes after a rest at supine position for 10 minutes, followed by an $80^{\circ}$ tilt test lasting 45 minutes. If presyncope(lightheadedness, nausea, blurred vision, or sweating) or syncope occurred, the study was discontinued. 12-lead electrocardiography, echocardiography, and electroencephalography were performed. Transcranial Doppler study was performed at the middle cerebral artery with 2 MHz continuous Doppler probe in 10 children with positive tilt test. Systolic, diastolic, mean cerebral blood flow velocity, integral, and pulsatility index were measured with blood pressure, heart rate, and $O_2$ saturation. Results : The positive rate of tilt test was 31.3%(20/64). Systolic, diastolic, and mean cerebral blood flow velocity decreased significantly in absence of hypotension or bradycardia during presyncope. Time velocity integral of cerebral artery also decreased significantly. Conclusion : Decreased cerebral blood flow velocity can predict the presyncope manifestation. Impairment of autoregulation of cerebral blood flow might play an important role in the pathophysiology of vasovagal syncope.
Objectives : Alzheimer's disease(AD) and mild cognitive impairment(MCI) affect several nervous structures involved with the autonomic nervous system. Association between neuropsychiatric deficits and heart rate variability has been observed. But cardiac autonomic function in AD has been scarcely studied and the results reported are conflicting. We investigated autonomic function in normal control, MCI, AD using heart rate variability(HRV) technique. Methods : Time and frequency-domain variability of 5-min R-R interval series was comparatively evaluated in 26 normal control subjects, 22 MCI subjects and 34 AD subjects. Analysis of variance(ANOVA) was used to compare the differences across groups. Correlations between MMSE-KC and HRV components were performed using Pearson's correlation coefficient. Results : No significant difference was observed among the groups in time, frequency-domain analysis of HRV(p>0.05). HRV were not found to be significantly correlated with the degree of cognitive impairment. Conclusions : There were no differences in HRV with MCI, AD subjects when compared with normal controls. Further investigation is required to use HRV technique as noninvasive parameters of MCI and AD.

Purpose: We performed this study to evaluate the changes of gallbladder ejection fraction (GBEF) in diabetic patients with or without autonomic neuropathy. Materials and Methods: This study included 37 diabetic patients (25 women, 12 men, mean age 51 years) and 24 normal controls (10 women, 14 men, mean age 38 years). After intravenous injection of 185 MBq of $^{99m}Tc$-DISIDA, serial anterior abdominal images were acquired before and after fatty meal. Regions of interest were applied on gallbladder and right hepatic lobe on 60 and 90 minute images to calculate GBEF. Results: GBEF was significantly reduced in diabetes with autonomic neuropathy ($43{\pm}12.3%$) and without autonomic neuropathy ($57.5{\pm}13.2%$) compared with normal controls ($68{\pm}11.6%$, p<0.05). And also, GBEF was significantly reduced in diabetes with autonomic neuropathy compared with diabetes without autonomic neuropathy (p<0.05). Fasting blood glucose level, age, sex, hemoglobin Alc, body mass index, serum lipid level were not different in these two diabetic patient groups (p>0.05). When 50.2% of GBEF was used as the criteria for diabetic autonomic neuropathy, the sensitivity and specificity were 80%, 76.5%, respectively. The area under receiver operating characteristic curve was 0.846. Conclusion: GBEF of diabetic patients with autonomic neuropathy was significantly reduced than that of diabetic patients without autonomic neuropathy.

The purpose of this study was to investigate the effect of acute listening to music on the cardiovascular reactivity to sympathoexcitation. In this crossover design study, 15 healthy adults(23.1±1.94(yrs) were randomized to either (1)acute listen to the subject's preferred music for 30 minutes and (2)sat as a time control by an experiment coordinator. After completing each trial, the cold pressor test(CPT) was conducted. Heart rate(HR) and blood pressure(BP) were measured for 4 times at baseline, during and after the CPT. Heart rate variability(HRV) were measured for 3 times at baseline, prior and after the CPT. HR and BP increased during the CPT in both trial and returned to baseline after CPT(time effect, p < .001). After CPT, brachial systolic BP reactivity to the CPT was attenuated in listening to music trial compared to control trial(p = . 008). As a result of heart rate variability(HRV), the difference values between the baseline and prior to the CPT showed a significant increase in standard deviation of the NN intervals(SDNN), total power(TP) and high frequency(HF) only in the music trial (p = .001, p = .002, p = .011). The difference value between prior to and after the CPT did not show significance. But compared with the control trial, the music trial was confirmed that SDNN, TP and HF were more activated. Therefore, listening to music alleviated anxiety and tension before the CPT, and it is estimated that it had a favorable effect on stability after the CPT. This findings showed that listening to music may have a positive effect on brachial systolic BP and HRV to sympathoexcitation.
The aim of this study was to investigate the influence of rosemary oil inhalation on memory, attention and autonomic nervous system according to the concentration difference in the aged. The research design was non-equivalent control group non-synchronized design. Participants were 89 individuals aged 65 or older who live in the community. Participants inhaled almond carrier oil(control group), 10%(experimental group A) and 100%(experimental group B) rosemary oil. Memory, attention, and autonomic nervous system responses were measured. Data were analyzed by SPSS win 24.0. The differences of the group and time were analyzed through repeated measure ANOVA. There were no significant differences in immediate recall (F=.42, p =.656), delayed recall (F=.45, p=.639), recognition (F=1.45, p=.242), digit span-forward (F=1.53, p=.223), digit span-backward (F=.46, p=.636), activities of sympathetic nerve system (LF)(F=.19, p=.828), activities of parasympathetic nerve system (HF)(F=.37, p=.694), LH/HF(F=1.39, p=.256), systolic blood pressure (F=.37, p=.694), diastolic blood pressure (F=1.25, p=.291). The inhalation of 10% and 100% rosemary oil for five minutes showed no significant effects on memory, attention and automatic nervous system in the aged.

Objectives : This study aims to evaluate the relationship between HRV indices and scores of emotional questionnaires and to find out the effective way to assess patients emotional and physical condition. Methods : We selected 144 patients who had both HRV data and BDI, STAI and STAXI scores on the chart among outpatients from July 2006 to December 2010. The relationship between the scores rated from the questionnaires and HRV indices are analyzed. And the HRV indices of patients included in the top 30 percent group and the bottom 30% group are compared. Results : 1. There were no significant correlations between HRV indices and scores of BDI, STAI and trait anger of STAXI. 2. SDNN and TP of HRV significantly decreased with higher state anger scores of STAXI. The top 30 percent group of state anger had lower SDNN, TP, LF, HF and HRV-index and higher pNN50 than the bottom 30 percent group. 3. RMSSD of HRV significantly decreased with higher anger-in scores of STAXI. The top 30 percent group of anger-in had lower RMSSD than the bottom 30 percent group. Conclusions : HRV can be used to evaluate emotional and physical changes related to state anger and inappropriate anger expression.
Objectives: Obstructive sleep apnea syndrome(OSAS) is known to be associated with the changes of autonomic nervous system (ANS). Nasal continuous positive airway pressure(nCPAP) treatment was found to correct abnormal ANS changes in OSAS but it remains to be further clarified. We aimed to assess the effects of nCPAP on ANS manifested on electrocardiogram, using spectrum analysis in the subjects with OSAS. Methods: Digital polysomnography was performed in 18 patients with OSAS(mean age $43.7{\pm}16.6$ years ; 17 males, 1 female ; mean respiratory disturbance index (RDI) $48.6{\pm}20.9$) for one baseline and another CPAP nights. From each night, 300 continuous beats of ECGs without artifact were chosen from both stage 2 sleep and REM sleep and they were used for power spectrum analysis. We compared between baseline and CPAP nights the heart rate variability including VLF(very low frequency power), LF (low frequency power), HF(high frequency power), R-R means, R-R variance, and LF/HF ratio, using Wilcoxon signed ranks test. Results: In all patients, nCPAP proved to be effective in relieving apneas and snoring. During nCPAP night compared with baseline night, decreases in VLF(p<0.05), LF(p<0.01), and R-R variance(p<0.05) were found in stage 2 sleep, and decreased LF(p<0.05) was found in REM sleep. No significant differences in each sleep stage were found in other variables between the two nights. Conclusion: Our findings suggest that OSAS increases the activity of sympathetic nervous system and nCPAP application effectively decreases the activity. And nCPAP does not appear to influence the parasympathetic nervous activity in OSAS.
In this study changes in biorhythm are observed by measuring heart rate variabilities in order to verify, compare, and evaluate stresses in guard duties of guards and CCTV monitoring works of staffs serviced in practical guard sites. Guard duties and CCTV monitoring works similar to a practical situation are implemented for nine students in the department of security at K University over 150 minutes. In the results of observing heart rate variabilities and autonomic function tests for six times with an interval of 30 minutes, the heart rate variability (HRV) in CCTV monitoring works represents lower levels than that of guard duties. Also, in a stable condition the guard duties for 30 and 60 minutes exhibit lower levels than that of 90, 120, and 150 minutes. Regarding SDNN, CCTV monitoring works show higher levels that guard duties and the guard duties for 30 and 60 minutes represent lower levels than that of 150 minutes. In autonomic function tests, there are no differences in TP between groups according to guard duties and CCTV monitoring works. Also, the guard duties for 150 minutes represent more differences in TP compared to that of 30 minutes. The interaction between the duty type and the duty duration is presented. In the case of LF, guard duties for 150 minutes show large differences in duty duration compared to that of 60 minutes. In the case of HF, the CCTV monitoring work group shows higher levels than the guard duty group in which the guard duties for 120 and 150 minutes represent higher levels than that of 30 minutes. The interaction between the duty type and the duty duration is presented. In the case of the LF/HF ratio, the guard duty group exhibits higher levels than the CCTV monitoring group. Also, there is an interaction between the duty type and the duty duration including the difference in durations. The CCTV monitoring works represent lower heart function activities than the guard duties according to increases in parasympathetic nervous activities. It shows that the long-term CCTV monitoring duty repeated everyday shows a high possibility of increasing the exposures of VDT syndrome and nervous breakdown.

Congenital insensitivity to pain with anhidrosis (CIPA) is a rare disease that affects the sensory and autonomic nervous system. The patients do not have the ability to sense different sensations, such as pain, which tends to lead to different injuries. In addition, the patients suffer from fluctuations in body temperature due to autonomic involvement. The present case was a five-year-old girl with a neglected distal femur fracture. X-rays taken during the follow-up showed marked callus formation and pseudarthrosis of the distal femur. She had biting injuries of the tongue, auto-amputation of the fingers, some developmental delay and a history of recurrent fever with an unknown origin. The electrodiagnostic study was normal. The quantitative sudomotor axon reflex test revealed markedly reduced postganglionic sudomotor axonal responses at all sites recorded on the left. She was diagnosed with CIPA. As the initial presentation of CIPA involves the musculoskeletal system, orthopedic surgeons should have a high index of suspicion.
