HYPOPHOSPHATEMIC RICKETS : CASE REPORT (저인산혈증성 구루병 환아의 증례 보고)

  • Hypophosphatemic rickets is lack of reponse to physiologic doses of vitamin D, different from the vitamin D-dependent rickets. It is inherited in an X-linked dominant fashion. The prime features of this disorder are lowered serum phosphate levels, elevated serum alkaline phosphatase and normal serum calcium levels. The dental manifestation often include apical radiolucencies, abscess and fistulas of clinically sound teeth. Dental radiographs show ricketic bone trabeculations, abscent or abnormal lamina dura and abnormal cementum. This case which was diagnosed to hypophosphatemic rickets, showed multiple spontaneous periapical abscess and gingival fistula enlarged pulp chambers, extension of the pulp horns into the cusp tips and delayed eruption.

  • Skeletal anchorage is recommended as an orthodontic treatment for an impacted immature permanent tooth. Among these methods, C-tube is relatively safe because it is fixed to the cortical bone of interdental and the lower part of the root with several short miniscrews, which causes less damage to the root in patients of early permanent dentition. As it can be easily bent, the traction direction can be adjusted to favorable bone density sites. However, patient cooperation is important and traction based on physiological force in order to gain root and dentoalveolar tissue development in immature permanent teeth is required. Periodic follow-ups should be mandatory.

  • Intrusive luxation that takes approximately three percent of permanent teeth commonly occures at anterior teeth. This intrusion frequently leads to pulp necrosis, root resorption, marginal bone loss and these complications are influenced by depth of intrusion and stage of root development. Various treatment approaches have been suggested to manage of intrusive luxation. Techniques aiming to reposition the intruded tooth include an observation for spontaneous re-eruption, surgical or orthodontic repositioning. We report two cases with clinically satisfactory results for traumatically intruded maxillary central incisor. In one case which has a large open apex and mild intrusion depth, we observed for spontaneous eruption and then repositioning by forced eruption method. In other case, which has been completely intruded, was repositioned by surgical extrusion and followed by apexification.

Examining the Experiences and Significance of a Mothers' Learning Community (어머니 학습공동체의 경험이 영유아기 자녀, 가정, 지역사회에 주는 의미 탐색)

  • This study aims to examine the experiences and significance of a mother's learning community, which is a voluntary small get together of mothers who have infants and young children. For this, a participatory observation and in-depth interview were carried out from June 2012 to January 2013 targeting eight mothers who participated in the mothers' learning community. The mother's learning community is organized by mothers who mainly live and raise young children in P city. First, the mother's learning community gave three kinds of experience to mothers: finding ways of communication through the new meetings, a feeling of fulfillment through voluntary participation, and enjoying the pleasure of being recognized from other people. Second, the significance in this experience could be observed to allow children to have the pleasure of rapport with their mothers, to make more conversation at home, and to allow the community to have significance as network of life. The results of this study will be used as basic data in the mother's learning community in the near future, and for casting a lime light for follow-up research.

  • 돼지 난포란의 체외성숙, 수정 및 배양에 관한 연구는 생명공학 기술을 도입하기 위한 기반 기술로 최근까지 계속 진행되고 있으나, 돼지 난포란을 이용한 효율적인 수정란 생산은 불규칙적인 웅성전핵 형성율과 높은 다정자 침입율 그리고 체내에서 발달된 배반포에 비하여 적은 세포수는 돼지 체외수정 체계에 있어서 끊임없는 문제점으로 제기되고 있다. 따라서 본 연구에서는 난포란을 이용한 보다 효율적인 수정란 생산을 위하여 돼지난자의 체외수정시 정자의 adenylate cyclase 활성을 조절하여 수정능획득 및 자발적인 첨체반응을 조절하여 정상적인 수정을 돕는 것으로 알려진 ADP를 체외수정 배양액에 첨가했을 때 수정율, 다정자 침입율, 배발달율 및 배반포의 세포수에 미치는 영향을 조사하였다.   • During the last few years, there have been many efforts on the fabrication of electronic and optical devices based on semiconductor nanowires. Room-temperature ultraviolet lasing in GaN nanowire, ultraviolet light sensing in ZnO nanowire, and dramatically improved hall mobility in Si nanowire have been demonstrated in this article. The studies on semiconductor nanowire based electronic and optical device is reviewed.

  • Non-Annex I parties announced the voluntary emission reduction targets including the U.S. in the conference of party. NAMAs would be focused to solve the negotiation clue for the post-kyoto regime. Since the country would not be involved in Annex I parties, the voluntary carbon market would be created for the greenhouse gas reduction targets. According to Bali Roadmap, voluntary carbon market should be constructed by the MRV manners since this country does not belong to Annex I parties. Carbon point system would be proposed by the ways of the international voluntary emission reduction credit. The voluntary carbon market should involve the potential GHG reduction credit and link with the ETS in the country. This study proposed the way of linkage between ETS and voluntary carbon market including the carbon-point system.

MANAGEMENT OF THE IMPACTED TOOTH ASSOCIATED WITH DENTIGEROUS CYST IN AUTISTIC YOUNG PATIENTS (자폐증 환자에서 함치성 낭종과 연관된 매복치의 처치)

  • Autism is a developmental, neuropsychiatric disorder that begins in early childhood. A patient with autism seen in the dental office frequently may have many complications. Therefore, it may be needed to consider modified or alternative therapy for dental care of autistic patients. This is the case of a 16-year old boy who have autism. He came to the department of the pediatric dentistry, Yonsei University Dental Hospital, for evaluation and treatment of dentigerous cyst associated with impacted teeth(#33,34,35). Under daily hospitalization and general anesthesia, the cyst was enucleated with surgical extraction of #34 and autotransplantation of #33,35. And during the periodic dental followup, apexification of #33,35 was performed for periapical lesion and root maturity. At 2 year 6 months follow- up, now, bony healing was completed and there are some complications like external resorption of #33 and space loss of #34 area. Generally, the marsupialization has been widely recommended for treatment of dentigerous cyst. However, in this case, there is a little possibility of spontaneous eruption after marsupialization considering of patient's age, location and angulation of the impacted tooth, root maturity. And there is necessity to choose the treatment that has low recurrence risk and needs short-term follow-up for autism. Above all, poor oral hygiene and lack of cooperation for decompression treatment is a matter of primary consideration. Consequently, enucleation of the cyst was chosen for the final treatment plan in this case. It is important to consider the conditions that affect the eruption of a dentigerous cyst-associated tooth to predict the successful eruption and special health care needs of the patient when the treatment plan is settled.
