건강한 생활습관실천으로 암을 예방합시다

  • 세계에서 매년 약 1,000만명의 새로운 암환자가 발생하고 있으며, 650만명이 암으로 사망하고 있다. 우리나라에서도 매년 약 85,000명의 새로운 암환자가 등록되고 있으며, 55,000명이 암으로 사망하고 있다. 이는 우리나라 전체 사망자의 20%를 차지하는 수치로서 사망원인으로는 단연 1위에 해당한다. 이처럼 암 발생과 암으로 인한 사망이 급격하게 증가함에 따라 암을 일으키는 원인인자를 밝히고 이러한 원인을 제거하여 암을 예방하려는 노력들이 활발하게 이루어지고 있다. 실제로 몇몇 암의 경우에는 그 발생 원인들이 일부 밝혀지고 있는데, 대표적으로 금연을 통하여 폐암의 발생을 예방하고 간염백신 접종을 통해 간암을 예방하는 경우 등이다. 한편, 세계보건기구에서는 의학적인 관점에서 암 발생인구 가운데 1/3은 예방가능하고, 1/3은 조기진단만 되면 완치가 가능하며, 나머지 1/3의 환자도 적절한 치료를 한다면 완치가 가능한 것으로 보고하고 있다. 특히 우리나라에서 가장 흔한 위암, 유방암, 자궁경부암, 대장암 등은 조기에 발견할 경우 거의 완치가 가능하기 때문에, 이러한 암들에 대해서 정기적으로 검진을 실시할 겨우 암으로 인한 사망을 상당수 줄일 수 있을 것으로 기대된다. 이에 정부는 2002년부터 저소득층을 대상으로 위암, 유방암, 자궁경부암에 대해 조기검진을 무료로 실시하고 있으며, 암 예방 교육 및 홍보를 통하여 국민들에게 암에 대한 정확한 지식과 조기검진의 중요성을 전달하고, 나아가 생활양식의 변화를 유도하여 암을 예방하도록 하는 마스터플랜을 추진하고 있다.

Factors Affecting HPV Vaccination Rates of Daughters Aged 12 years (만 12세 초등학생 딸의 자궁경부암 예방접종 실태 및 영향요인)

  • Purpose: The purpose of this study was to investigate the rate of human papilloma virus (HPV) vaccinations among 12-year-old daughters and to analyze factors influencing the mothers' decisions to vaccinate their daughters. Methods: A cross sectional descriptive study was conducted using a convenience sample of 139 mothers. Data was analyzed using SPSS WIN 23.0. Descriptive statistics and a logistic regression analysis were conducted. Results: The vaccination rate of HPV among children was 43.9%. Mothers showed a higher knowledge of the HPV vaccination than of cervical cancer and of human papilloma virus. In self-efficacy scores, the subscores of self-regulation efficacy were higher than self-confidence and task difficulty preference. The factors influencing the vaccination of daughters included the age of the daughters at the time of the vaccination, the provision of information on the HPV vaccine, knowledge of the HPV vaccination, and self-confidence. Conclusion: In order to increase the HPV vaccination rate, it is necessary to provide accurate scientific knowledge to mothers. A variety of intervention strategies should be developed to enhance the confidence of mothers so that mothers who want to provide the HPV vaccination to their daughters are able to follow through and provide the vaccination.

Knowledge regarding cervical cancer, human papillomavirus and acceptance of vaccination in Korean adult men (자궁경부암, 인유두종 바이러스(HPV)에 대한 성인 남성의 지식 및 백신 접종 의향)

  • Purpose: The purpose of this study was to investigate knowledge of cervical cancer and HPV in adult men and their acceptance of vaccination for HPV. Methods: A convenience sample of 229 men between 19 and 39 years of age, responded to self-report questionnaires on knowledge of cervical cancer and HPV, vaccination intention, and general and sexual characteristics. Data were collected during September and October, 2010 and analyzed using t-test, ${\chi}2$-test, and ANOVA. Results: Mean scores for knowledge of cervical cancer and HPV were 4.88, and 7.87 respectively. Seventy-five percent stated they intended to be vaccinated. There were significant differences in knowledge of cervical cancer depending on education (F=3.04, $p$=.030), sexual frequency (F=4.62, $p$=.011), condom use (t=2.19, $p$=.029), and frequency of condom use (F=3.28, $p$=.040), and in knowledge of HPV depending on sexually activity (t=2.35, $p$=.020), condom use (t=0.03, $p$=.019), and frequency of condom use (F=4.65, $p$=.011). There were significant differences in vaccination intention depending on education (${\chi}2$=10.47, $p$=.015), but sexual characteristics were not significant for vaccination intentions. Conclusion: The results indicate a need to consider levels of education and knowledge of cervical cancer and HPV in target populations of men, when planning a cervical cancer prevention education program for men.

Factors Influencing Mothers' Acceptance of Human Papillomavirus Vaccination to Prevent Cervical Cancer in their Daughters (딸의 자궁경부암 예방 관련 HPV 백신접종에 대한 어머니 수용도와 영향요인)

  • Purpose: This study was done to examine mothers' acceptance and its influencing factors in daughters' human papillomavirus (HPV) vaccination to prevent cervical cancer. Methods: From July 20 to August 31, 2010, 220 mothers of unmarried daughters responded to self-administered questionnaires about their acceptance of HPV vaccination for their daughters and the optimal age for HPV vaccination. Descriptive statistics, univariate logistic and multiple logistic regression were used for data analysis with SPSS/WIN 12.0. Results: Mothers indicating their willingness to pay for their daughters to receive HPV vaccination accounted for 61.8%, and willingness with health insurance coverage, 84.5%. Mean optimal age for HPV vaccination was $19.78{\pm}3.96$) years. With self-payment the factor influencing mothers willingness was necessity of HPV vaccination (OR= 30.45, 95% CI=10.33~89.15). With health insurance coverage, income (OR=3.02, 95% CI: 1.19~7.62), necessity of HPV vaccination (OR=15.05, 95% CI=4.15~54.64), concern about HPV vaccine safety (OR=2.99, 95% CI=1.15~7.76), and experience of Pap test (OR=3.16, 95% CI=1.24~8.05) were factors influencing mothers willingness. Other influencing factors were optimal age for HPV vaccination, knowing about HPV (OR=7.66, 95% CI=2.19~26.82), and age of youngest daughter (OR=3.95, 95% CI=1.34~11.68). Conclusion: Financial support is anticipated for low income families in a primary approach to increase HPV vaccination. And focusing on earlier age and concerns about vaccination are necessary to gain mothers' acceptance.

Evaluation of Human Papillomavirus Genotyping from Formalin-fixed Paraffin-embedded Specimens in Cervical Cancers (자궁경부암 파라핀 조직에서 인유두종바이러스 유전형 검사의 유용성 평가)

  • Cervical carcinoma is the second leading cause of cancer-related deaths in women around the world, and it is associated with the Human Papillomavirus (HPV) infection. HPV genotyping is important for vaccine policy, etiology, natural history, and epidemiology studies. The use of formalin-fixed paraffin-embedded (FFPE) tissues for HPV genotyping by reverse blot hybridization assays (REBA) has not been clearly confirmed in retrospective studies. The aim of this study was to evaluate the usefulness and efficiency of FFPE tissues from cervical cancers for HPV genotyping. HPV genotypes were detected in 52 FFPE tissues from cervical carcinoma specimens by REBA. HPV was detected in 32 (61.5%) of 52 specimens from FFPE, among which 27 (84.4%) harbored single infections and 5(15.6%) contained multiple infections. The HPV single infections (27) were analyzed by high-risk type 18(8), 58(6), 16(5), 33(1), 35(1), 39(1), 56(1) and low risk type 11(2), 6(1), 70(1).   • This research attempted to identify the factors associated with the intention of human papilloma virus vaccination among mothers of elementary school daughters. The subject of study is 132 mothers of elementary school students. Data collection was conducted from July 1 to August 30 in 2017, and analyzed using SPSS 23.0 Program. Results showed that 94.7% children of the participants had not been vaccinated, whose reasons were the low knowledge about HPV vaccination, concerns about its side effects, burden for cost. The attitude toward HPV vaccination appeared to be the most important predictor of vaccination, which was followed by perceived behavior control and subjective norm as meaningful predictors, as well. Based on these results, further researches should be conducted to develop educational programs, which are able to offer exact information about the effect and side effect of HPV vaccination, and verify the effectiveness.

A Text Mining Analysis of HPV Vaccination Research Trends (텍스트마이닝을 활용한 HPV 백신 접종 관련 연구 동향 분석)

  • Purpose: The purpose of this study was to identify human papillomavirus (HPV) vaccination research trends by visualizing a keyword network. Methods: Articles about HPV vaccination were retrieved from the PubMed and Web of Science databases. A total of 1,448 articles published in 2006~2016 were selected. Keywords from the abstracts of these articles were extracted using the text mining program WordStat and standardized for analysis. Sixty-four keywords out of 287 were finally chosen after pruning. Social network analysis using NetMiner was applied to analyze the whole keyword network and the betweenness centrality of the network. Results: According to the results of the social network analysis, the central keywords with high betweenness centrality included "health education", "health personnel", "parents", "uptake", "knowledge", and "health promotion". Conclusion: To increase the uptake of HPV vaccination, health personnel should provide health education and vaccine promotion for parents and adolescents. Using social media, governmental organizations can offer accurate information that is easily accessible. School-based education will also be helpful.

Factors Associated with Intention to Receive HPV Vaccination among Marriage-Immigrant Women (결혼이주여성의 자궁경부암(인유두종 바이러스, HPV) 백신 접종의도 영향 요인)

  • This study investigated knowledge about the human papillomavirus (HPV), attitudes toward HPV vaccination, and intention to receive HPV vaccination among marriage-immigrant women, and identified factors associated with intention to receive HPV vaccination. The subjects of this study were 136 marriage-immigrant women in J Province. Data were collected from September 28 to December 21, 2017, and analyzed using SPSS for Windows version 24.0. The participants' knowledge of HPV was low (mean score, 2.74) and their attitude towards HPV vaccination was positive (mean score, 36.66). The factors affecting the intention to take the HPV vaccination included Vietnamese nationality (odds ratio [OR]=2.899, p=.048), desire for education about the HPV vaccine (OR=5.967, p<.001), and a positive attitude towards HPV vaccination (OR=1.083, p=.034). Therefore, educational programs about HPV vaccination targeting marriage-immigrant women are needed. It is also necessary to provide accurate information about the HPV vaccination using various methods, at health care facilities and through mass media.

  • Cervical cancer is the third most common cancer among women worldwide. Cervical cancer is significantly associated with human papillomavirus (HPV) infection. The prevalence of HPV infection is influenced by geography, immune status, sexual history and genetic factors. For example, geographically, HPV prevalence varies from 1.5% to 39%. However, little is known about the relationship between HPV prevalence and age. An analysis of HPV prevalence by age will help determine when high-risk groups are exposed to HPV. Such an analysis could also demonstrate a correlation between specific HPV genotypes and age. In addition, the analysis might clarify the optimum age for using vaccines. In this study, HPV prevalence and genotype distribution among Korean and Chinese women are analyzed by age. The REBA HPV-ID$^{(R)}$ assay (YD diagnostics, Yong-in, Republic of Korea) was used for detecting HPV genotypes in uterine cervical liquid-based cytology samples from 533 women from Korea and 324 from East China (Western Shandong province. Women with severe dysplasia such as SCC (Squamous cell carcinoma) and HSIL (High-grade squamous intraepithelial lesion) groups were primarily in their 40s and 50s, whereas women with mild and moderate dysplasia (ASCUS and LSIL groups) were primarily in their 30s and 40s. Women with HPV genotype 16 and 18 infections were primarily in their 40s. The results suggest that HPV infection is associated with certain age groups in the Korean population.
