A Study on the Characteristics and Consultation Type of Inpatients Referred for Delirium and Depressive Disorder (섬망 및 우울장애로 자문 의뢰된 입원환자의 임상적 특징 및 의뢰 형태에 관한 연구)

  • Objectives : The purpose of this study is to investigate the characteristics of patients referred for delirium and depressive disorder and to find direction of improvement of consultation-liason psychiatry in general hospital. Methods : We performed a retrospective computed chart review of the 4,966 inpatients hospitalized at Konkuk University Hospital who were referred to the Department of Psychiatry from August 1, 2005 to December 31, 2011. Results : Depressive disorder shows the order of frequency of consultation type Parallel-Complementary-Mending. Delirium shows the order of frequency of consultation type Mending-Parallel-Complementary. When comparing 'follow up consultation' and 'without follow up consultation' group within the depressive disorder, the proportion of men in the 'follow up consultation' group was higher. In the analysis of the consultation type, the 'follow up consultation' group showed the order of consultation type Parallel-Mending- Complementary, and type Parallel-Complementary-Mending in the 'without follow up consultation' group. When comparing 'follow up consultation' and 'without follow up consultation' group within the delirium, the proportion of the surgical field in the 'follow up consultation' group was higher. In the analysis of the consultation type, both group showed the order of consultation type Mending-Parallel- Complementary. Conclusions : Doctors in each department and psychiatrists should pay attention to delirium symptoms that may occur in surgical inpatients and preventive measures should be taken. Screening tests should be conducted in medical patients to properly evaluate coexisting psychiatric diseases. Risk factors of Delirium and Depressive disorder should be identified from the time of hospitalization, and actively discussing treatment plans and early interventions could improve the quality of medical services.

Quality of Sleep in Hospitalized Patients with Lung Cancer (입원 폐암환자의 수면의 질)

  • Purpose: The purpose of this study was to identify the quality of sleep in hospitalized patients with lung cancer. Methods: The data were collected from January to April 2001. Study subjects were 50 lung cancer patients hospitalized at K University and G hospital in Busan, Korea. Quality of sleep was measured using the Quality of Sleep Scale (range $15{\sim}60$) developed by Oh et al (1999). The data were analyzed with the SPSS 10.0 program using frequency and percentage, mean, standard deviation, t-test, ANOV4 and Post-hoc test (Scheffe's test). Results: The results of this study were as follow; 1) The mean quality of sleep score was $35.64{\pm}7.59$ (range: $21{\sim}52$). 2) There was 3 Significant difference in 'ongoing pain' (t=-1.943, P=.046), 'major caregiver' (F=B863, P=.047), 'change of weight' (F=5.906, P=.019), according to general characteristics in the quality of sleep in hospitalized patients with lung cancer. Conclusion: As a results, the level of the quality of sleep in hospitalized patients with lung cancer was moderate. And, the study results indicate that ongoing pain, major caregiver, change of weight impact the quality of sleep of lung cancer patients. Therefore, nurses should consider those factors in providing nursing care for hospitalized patient with lung cancer.

Factors Affecting Length of Stay and Death in Tuberculosis Patients(2008-2017): Focus on the Korean National Hospital Discharge In-depth Injury Survey (결핵 환자의 재원기간과 사망에 영향을 미치는 요인(2008-2017): 퇴원손상자료를 중심으로)

  • The purpose of this study is to identify factors affecting length of stay(LOS) and death in tuberculosis(TB) patients by disease type, patient characteristic, admission and disease characteristic, and hospital characteristic from 2008 to 2017. Survey data was using Korean national hospital discharge in-depth survey data produced by Korea Disease Control and Prevention Agency. Study subjects were 10,634 inpatients with TB(A15, A16, A17, A18, A19, U88.0, U88.1, U84.30, U84.31) and analyzed frequency, chi-square test, Fisher's exact test, and logistic regression by using STATA 13.0. As a study result, the type of TB(extrapulmonary TB, multidrug-resistant TB, extensively drug-resistant TB), sex(woman), age(35-49, 50-64, 65-74, 75 years old or older), admission type(outpatient department), CCI(1-2 point, 3 point over), hospital location(metropolitan city) and bed size(300-499, 500-999, over 1000) were significantly influence LOS. Also, the type of TB(extrapulmonary TB, extensively drug-resistant TB), sex(woman), age(50-64, 65-74, 75 years old or older), residence(small town/rural), admission type(outpatient department), CCI(1-2 point, 3 point over), hospital location(provincial) were significantly influence death. In conclusion, the existing tuberculosis management has been patient management with rapid diagnosis and treatment following early detection. But other studies should be carried out for the system that identifies and supports high-risk groups of the long-term length of stay in hospital or high mortality rates as a result of treatment.

  • Background : Cigarette smoking is the single biggest avoidable cause of death and disability in most countries. Effective prevention of cigarette Smoking and help for those wishing to quit can therefore yield enormous health benefits for populations and individuals. Although most of smokers try to stop smoking for themselves, the success rate of quitting smoking is very low. Promoting and supporting smoking cessation should be an important health policy priority for healthcare professionals in all clinical settings. In this study, we tried to evaluate the effect of the educational program for smoking cessation of our hospital, the smoking pattern, and characteristics of adult smokers enrolled in the program. Methods : We enrolled 640 smokers, who has visited the '5 day stop smoking school' for smoking cessation from Jan 1998 to 2001. we evaluated the characteristics of the smokers based on the self report questionnaire at the beginning of the program and we also checked 100 smokers, enrolled in Inpatient Smoking Cessation Educational program, their smoking behavior and the cessation rate with postal questionnaire after finishing this program from Nov 2002 to Jan 2003. Results : The health was most common motivation for smoking cessation. The stress was the most important motivation of smoking. The overall smoking cessation rate of the smokers enrolled in inpatient smoking cessation school was 60%. The age of starting smoking and the kind of occupation have a significant difference between the success group and failure group of smoking cessation. Conclusion : The inpatient smoking cessation program was effective smoking cessation intervention in the adult smokers who enrolled in '5 day stop smoking school'.

Factors for the Prediction of Pain in Terminally Ill Cancer Patients in Hospice Units (호스피스 병동에 입원한 말기 암환자의 통증 예측요인)

  • Purpose : The purpose of this study was to investigate the impact of depression, discomfort, spirituality, physical care, and opioid use on pain with terminally ill cancer patients in the hospice units. Method : The convenient sample of this study consisted of 58 terminally ill cancer patients at three hospice units in university-affiliated hospitals. Patients were interviewed with structured questionnaires. The data was analyzed using ANOVA, Pearson correlation coefficient, and multivariate multiple regression. Result : The results of this study were as follows : 1) The mean age of the participants was approximately 57 years. Regarding diagnosis, stomach cancer showed the highest frequency (24.1%), followed by lung cancer (17.2%) and rectal cancer (13.8%). Regarding motivation for admission to the hospice unit, the majority of the participants indicated pain control (67.2%), followed by spiritual care (39.7%), and symptom relief (27.6%). 2) The mean pain level measured by VAS was 5.13 (${\pm}2.61$). Regarding pain type, the highest pain frequency the participants experienced was deep pain (53.4%), followed by multiple pain (20.7%), intestinal pain (17.3%), and neurogenic (5.2%) and superficial pain (3.4%). 3) Regarding the factors influencing pain, the pain level was significantly affected by the depression level (P<0.01) and the opioid use (P<0.01). Conclusion, In summary, the higher the level of pain the terminally ill cancer patents had the higher the depression level as well as the opioid use. Thus, health care professionals need to continuously provide holistic care for them to die comfortably.

  • 본 연구는 2008년 1월 1일부터 12월 31일 까지 1년 동안 대전에 위치한 일개 대학병원 퇴원요약 정보를 활용하여 환자 개인요인 측면과 의료적 측면에서 전원 환자의 특성과 요인을 파악하여 전원환자 관리의 자료로 이용하고자 한다. 연구결과 퇴원환자 30,793명중 타 병원으로 전원 된 990명 전체가 대상이었으며, 성별로 보면 남자가 여자보다 높은 분포를 보였다. CART기법에 의한 의사결정나무는 치료결과, 입원경로 등이 타 의료 기관으로 전원되는 특성을 보여주고 있음을 알 수 있다. 결과적으로 의료적인 측면보다는 개인적인 측면으로 환자들이 전원을 하는 비율이 높았다. 이와 같이 전원환자 특성 및 요인 분석이 병원의 지속적인 환자 관리와 환자와의 신뢰를 형성하고 꾸준한 병원방문을 유도할 수 있을 것으로 생각된다.

Inpatient Satisfaction and Dissatisfaction in Relation to Socio-demographics and Utilization Characteristics (입원환자의 사회인구학적 요인 및 의료이용 특성과 환자만족$\cdot$불만족간의 관련성)

  • Purpose: This paper reports a study exploring factors related to patient satisfaction and dissatisfaction with inpatient care. Method: A cross-sectional study design was used, employing data from the National Health and Nutrition Survey conducted in 2001. Socio-demographic factors, utilization, self-rated health status, and disease characteristics were assessed by employing univariate comparisons and multivariate logistic regression analyses. Result: Out of 37,769 respondents, 1,043 aged 20 years and over had been admitted to a hospital or clinic at least once during the past year. About a quarter of the respondents were discharged from tertiary hospitals and $21\%$ from clinics. The majority of patients ($58\%$) were satisfied with inpatient care received, whereas $11\%$ were dissatisfied. Greater satisfaction was found in patients aged 45-64 years and those having formal education, discharge from tertiary hospitals, national health insurance as a payer, medical expenses not being burdensome, good self-rated health status, and neoplasm. Living in non-metropolitan urban areas, shorter length of stay, and musculoskeletal diseases were associated with greater dissatisfaction. Conclusion: Different factors were related to patient satisfaction and dissatisfaction with care. Those factors need to be taken into account when evaluating and comparing satisfaction levels between health care institutions.

A Study of Gender Differences in Inpatients' Privacy Concerns in Multi-bed Hospital Room (다인병실 입원환자의 성별에 따른 프라이버시 특성 연구)

  • This study aims to find gender differences in inpatients' privacy and stress concerns in multi-bed hospital room. In the literature review, we consider the characteristics of multi-bed hospital room and patients' privacy as a psychological environment factor. In the survey research, the questionnaires were used to understand the inpatients' general characteristics and privacy requirements. A total of 109 copies were analyzed through a cross-tabulations and T-test using the SPSS 18.0. Results and conclusions are as follows: (1) In the case of multi-bed room patients, there were some gender differences in privacy and stress, but the differences were limited. (2) There is a difference in the patient 's preference of the hospital room according to gender, and this is related to the privacy characteristic. (3) The social communication and strolling are commonly effective stress relief methods for both men and women, so space is needed to do this methods. The meaning of this study is to specify the privacy and stress of the inpatients in multi-bed patients' room.

Children Pain Using Four Pain Assessment Tools - Faces, Glasses, Chips, Colors (4개의 도구를 이용한 아동의 통증)

  • The purpose of this study is to measure and evaluate the degree of the pain using tools such as faces, glasses, chips, and colors for 96 children in three general hospitals. The children preferred faces, glasses, chips and colors in order, and they preferred orange color for pain-free and black for intense pain. The pain score was significantly high with the children who did not have experience in hospital except colors tool in the difference by means of general characteristics(faces, glasses, chips; p<0.05). Faces, glasses, chips, colors tools are pain assessment tools which are easy simple to handle by children including family and medical personnel. We expect these tools will be used in many ways to assess pain according to a child's age in a clinical setting.

Types and Characteristics of Patient Safety Accidents among Hospitalized Children (입원아동의 안전사고 유형과 특성)

  • Purpose: This study was conducted to identify and analyze the types, characteristics, and frequency of patient safety accidents among hospitalized children. Methods: The data were collected from patient safety reports for 0-19-year-old patients from the National Health Insurance Corporation (NHIC) from January 1, 2016 through December 31, 2017. Using Excel software, a pivot table was used to classify and analyze the safety incidents, severity frequency, and characteristics of hospitalized child patients. Results: A total of 254 accident cases were reported involving child patients. The types of reports included medication accidents, falls, test errors, and others. Medication accidents accounted for 47.2% of the total reported cases. Regarding the severity of reported risk, there were no complications nor sequelae in 80.4% of the cases. Conclusion: This study is significant for systematically analyzing and reporting data reported about safety accidents among hospitalized children. These results will contribute further to preventing safety accidents in hospitalized children and to creating a desirable patient safety culture.
