Quality of Life in Children and Adolescents with Cancer (소아청소년 암환자의 삶의 질)

  • Purpose: This study was done to provide basic data for developing nursing interventions to enhance quality of life of pediatric patients with cancer (children and adolescents) by examining the quality of life and related factors. Methods: Participants were 134; 67 pediatric patients and 67 parents. The PedsQL$^{TM}$ 3.0 Cancer Module was employed to measure quality of life in the participants. The related factors included general and clinical characteristics of the participants. Results: Mean score for quality of life in the patients was 75.07, and mean score for patient quality of life as perceived by their parents was 64.40. Among the quality of life subscales, treatment anxiety had the highest score whereas nausea had the lowest score. Mean score in adolescent patients (13-18 years of age) was 71.62, lower than the 78.04 for child patients (8-12 years of age). Regarding general and clinical characteristics of the participants, there were no significant differences in the scores. Conclusion: The results indicate that there is difference in perception of quality of life between patients and their parents, and between children and adolescents and these differences should be taken into account when planning and providing nursing care.

Pruritus in Burn Survivors (화상환자의 소양증 특성 조사)

  • This study examined pruritus to provide the data based on burn patient pruritus. A self-report questionnaire survey was conducted among ninety five burn survivors, who were over 18 years of age with admission periods longer than two weeks. The results indicated that the average burn range was $18.95{\pm}17.10%$, severity was $48.83{\pm}23.96$, and distress was $46.42{\pm}25.80$. A significant difference in pruritus consequences was observed according to frequency. The patients consistently experienced pruritus, even if it happened less often, difficulty in falling asleep (F=4.817, p=.004), poor mood (F= 5.955, p=.015)), and loss of concentration (F=6.483, p<.000). A significant difference in the pruritus consequences according to duration was observed, lasting for 30 minutes for all groups, more than the population and less than 30 minutes, difficulty in falling asleep (F=7.452, p<.000), poor mood (F=3.655, p<.015), and loss of concentration (F=12.564, p<.000). The frequency, duration (r=.29, p=.004), severity (r=.45, p<.000), distress (r=.44, p<.000), and consequences of pruritus in burn patients (r=.29, p=.004) showed a positive correlation. The results of this study could be used as a base material for the care of burn patients experiencing pruritus.

Influences of Self-esteem, Rehabilitation Motivation on Quality of Life in Stroke Pateints with Rehabilitation Therapy (재활치료 중인 뇌졸중 환자의 자아존중감, 재활동기가 삶의 질에 미치는 영향)

  • This study was conducted to investigate the levels of self-esteem, rehabilitation motivation and quality of life and to identify the influencing factors of quality of life in stroke patients who were receiving rehabilitation therapy. Data were collected from February 22 to March 20, 2016 using self-administered questionares and the data from 216 participants were analyzed using SPSS WIN 21.0 program(SPSS Inc., Chicago, IL, USA). The results of this study were as follows: the mean self-esteem was 3.09/5, rehabilitation motivation was 3.60/5, and quality of life was 2.95/5. Sex, age, education, caregiver, occupation, duration after attack, the degree of disability(dependence), and cognitive function were significantly associated with quality of life. There was positive correlation between quality of life and self-esteem(r=.45, p<.001) and there was positive correlation between quality of life and rehabilitation motivation(r=.27, p<.001). Variables such as self-esteem, duration after attack, the degree of disability(dependence), and cognitive function explained 43.6%(F=14.87 p=<.001) of the variance in quality of life in model 3. Consequently, it is needed to develop and apply nursing intervention and programs for improving self-esteem during rehabilitation in order to improve quality of life of stroke patients.

Evaluation of Patients서 Satisfaction with Foodservice of Mid-size Hospitals in Busan Area (부산지역 중소병원 급식서비스에 대한 환자만족도 평가)

  • The purpose of this study was to evaluate the patients' satisfaction with the quality of hospital food service in Busan area. For the purpose, questionnaires were distributed to 271 hospitalized patients in 4 hospitals (three hospitals for self-operated foodservice, one for contracted foodservice) within 130 beds. The average scores were -0.99/5.00 for quality satisfaction of meal characteristics, -0.68/5.00 for service characteristics, and -0.37/5.00 for nutrition characteristics. The items of low scores in quality satisfaction were treatment of complaints, nutrition of meals, provision of nutritional information, the seasoning of the meals, selective menu, and sanitation of the meals. At the expectation and perception grid, high expectation and high perception items were cleanliness of employees' clothes, cleanliness of dish, employees' courtesy, exactness of meal time, and sanitation of the meals. High expectation and low perception items were treatment of complaints and nutrition. The patients with little appetite showed significantly (p<0.01) lower average scores in meal, service, and nutrition characteristics than those with much appetite. The self-operated foodservice operation had significantly (p<0.01) higher average scores in meal, service, and nutrition characteristics than those of the contracted foodservice operation. The patients hoped to improve the taste of cooked rice and side dish, quantity of the side dish, variety of menu, and sanitation of the meals.

Two cases of congenital atretic encephalocele misdiagnosed as dermoid cyst (유피낭종으로 오인된 atretic encephalocele 2례)

  • Atretic cephalocele is a degenerative form of encephalocele, which is detected as a cystic mass in the head, primarily in infants. Its presentation and prognosis vary and depend on various factors, including the nature of the tissues within the cyst, other concomitant anomalies, the site of development, and the presence or absence of an embryonic straight sinus. We here report 2 cases of atretic encephalocele, that were transferred to our hospital because round tumors, misdiagnosed as dermoid cysts, were detected in their parietal lobes immediately after birth. On diagnostic and differential MRI, an embryonic straight sinus was detected while histochemical results indicated that the lesions contained cerebral tissues. Despite these structural anomalies, the two patients developed normally neurologically and no other anomalies were detected. We here discuss these two cases and present a review of the relevant literature.

Prescribing Patterns for Treatment of Pediatric Bipolar Disorder in a Korean Inpatient Sample (소아청소년 양극성장애 입원 환자의 약물 사용 특성 : 후향적 차트 분석 연구)

  • Objectives : This study was conducted in order to describe prescribing practices in treatment of pediatric bipolar disorder in a Korean inpatient sample. Methods : We performed a retrospective chart review of 66 youths who had been hospitalized and diagnosed with bipolar disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fourth edition criteria. Demographics, clinical characteristics, medications used, doses, and related adverse events were examined. Results : Mood stabilizers and/or atypical antipsychotic medications were the primary treatment. Risperidone, valproate, and lithium were the most commonly used. Thirty seven patients (58.1%) were treated with combination therapy of an atypical antipsychotic and mood stabilizer for improvement of manic/mixed symptoms. Conclusion : Combination pharmacotherapy was necessary for most patients in this admission sample group. Conduct of further studies will be needed for evaluation of treatment response according to the clinical characteristics, and the safety and efficacy of treatment for child and adolescent bipolar disorder.

A Comparative Study of the Time Use and Quality of life in Local Community and Convalescent Hospital of the Stroke Patient (거주 환경에 따른 뇌졸중 환자의 시간 사용과 삶의 질 비교 연구)

  • Objective : The purpose of this study was to comparatively analyze time use and quality of life in stroke patients who use Local community or convalescent Hospital environment. Methods : The study subjects were 74 stroke patients who were admitted to convalescent Hospital or who were being rehabilitated as outpatients. The Occupational Questionnaire(OQ)was used for measuring time use and the Stroke Specific Quality of Life(SS-QOL) utilized to measure Quality of life. Excluding 8 patients who did not fit selection criteria, 66 patients were selected and analyzed. Results : After comparatively analyzing time use in Local community and convalescent Hospital environment, significant differences were found in daily living(p<.05), resting(p<.05). In addition, in quality of life, significant differences were found in family(p<.05), self-management(p<.05), and social roles(p<.05). Conclusion : Local community patients used their time in daily living, resting more efficiently than convalescent Hospital patients, and also had a higher quality of life.

A Composite Trend Test with Symptom Occurrence and Severity Symptom Scores (증상 발현과 증상 심각성을 병합한 추세검정법)

  • During clinical trials a researcher is frequently able to observe a disease symptom in a subject as well as a severity score for those who experienced a symptom after a fixed length of treatment. The traditional method to evaluate a decreasing trend in proportion, when there is an intrinsic order in the treatment groups (for example control and two or more treatment groups) is a Cochran-Armitage test, while the method to evaluate a decreasing trend in continuous non-normal data is a Jonckheere-Tersptra test. The Cochran-Armitage test emphasizes the dichotomous data of symptom occurrence and the Jonckheere-Tersptra test emphasizes the continuous non-normal data of severity symptom scores. In this paper we propose new test statistics that consider the combined evidence from a symptom occurrence and disease severity score. We illustrate these methods with example data of schizophrenic inpatients that demonstrated antipsychotic-drug induced constipation. A small-scale simulation is conducted to compare the new trend tests with other trend tests.

Determinants of Bypass Healthcare Utilization for Hospitals in Seoul -The Case of KTX passengers- (서울지역으로의 원거리 의료 이용에 대한 영향 요인 -KTX 이용자를 중심으로-)

  • The study analyzed the determinants of bypass healthcare utilization for hospitals in Seoul, of which phenomenon has rapidly risen since the introduction of KTX in 2004. 203 interviewees who had visited hospitals in Seoul using KTX were analyzed. We found that probability of utilization of inpatient service is increased with statistical significance by visiting hospital for the purpose of disease treatment, being aged over 40, being married and so on. This implies that the government needs to consider policies to support higher-level general hospitals in non-Seoul areas to enhance the quality of healthcare. Also hospital accreditation program and P4P program need to be used to help patients choose hospitals based on the accurate information on the quality of health care.

Symptom Checklist-90-Revision(SCL-90-R) in Inpatients on Tension headache and Insomnia (긴장성 두통 및 불면을 주소로 입원한 환자의 간이정신진단검사(SCL-90-R)의 진단별 특성)

  • SCL-90-R is a multidimensional self-report symptom inventory devoloped by Derogatis and his coworkers. Since it was standardized into Korean version in 1978 by Won and Kim et aI., but it has been rarely studies for the clinical groups. Objectives : This study sought to define a diagnotic character of SCL-90-R of tension headache and insomnia inpatients groups. Methods : We determined a diagnotic character of 17 tension headache and 23 insomnia patients by means of SCL-90-R, and compared with normal groups in order to characterize subscale of SCL-90-R in patients with tension headache and insomnia from Neurasthenia. Results : 1. Male tension headache group has significant difference in Somatization and Depression subscales, female group in Somatization, Obsessive-Compulsive, Depression and Hostility subscales. 2. Female insomnia group has significant difference in Anxiety subscale. Although there are a little significiant, many subscales such as Somatization, Obsessive-Compulsive, Depression and Hostility are higher than normal group. 3. There are significant difference in GSI and PDSI of Global index, which means that we use that for objective index of tension headache and insomnia from Neurasthenia. Conclusions : All of these results show that SCL-90-R is useful inventory to characterize and screening for Neurasthenia.
