  • The cause of esophageal cancer is still unknown in the majority of patients, but pre-existing diseases of esophagus related to the development of cancer were reported by many authors: e.g. lye stricture, achalasia, Plummer-Vinson syndrome, dietary and alocholic habbits etc. Recently the authors had experienced one case of esophageal cancer with complaint of dysphagia, developing at the site of lye stricture, which was diagnosed by the esophagogram and esophagoscopic biopsy.

  • 조호환경이란 환자의 지속적인 추적 및 관찰이 필요한 환경으로써, 병원 입원실, 요양원 등을 의미한다. 조호환경 내 환자의 이상 증세가 발생하는 시간 및 이상 증세의 종류는 예측할 수 없기에 인력을 통한 상시 관리는 필수적이다. 또한, 환자의 이상 증세 발견 시간은 발병 시점부터의 소요 시간이 생사와 즉결되기에 빠른 발견이 매우 중요하다. 하지만, 인력을 통한 상시 관리는 많은 경제적 비용을 수반하기에 독거 노인, 빈민층 등 요양 비용을 충당하지 못하는 환자들이 수혜받는 것은 어려우며, 인력을 통해 이루어지기 때문에 이상 증세 발병 즉시 발견에 한계를 가진다. 즉, 기존까지 조호환경 내 환자 관리 방식은 경제적 비용과 이상 증세 발병 즉시 발견에 한계를 가진다는 문제점을 가진다. 따라서 본 논문은 YOLO 모델의 조호환경 내 환자 탐지 성능 비교 및 바운딩 박스 앙상블 기법을 제안한다. 이를 통해, 딥러닝 모델을 통한 환자 상시 관리가 이루어지기에 높은 경제적 비용문제를 해소할 수 있다. 또한, YOLO 모델 바운딩 박스 앙상블 기법 WBF를 통해 폐색이 짙은 조호환경 영상 데이터 내에 객체 탐지 영역 정확도 향상 방법을 연구하였다.

Statistical Analysis on Hospitalized Patients with Thoracolumbar Compression Fracture at Single Korean Medicine Hospital: Retrospective Review (일개 한방병원에서의 흉요추 압박골절 입원 환자에 대한 통계적 분석: 후향적 차트리뷰)

  • Objectives: This study was designed to statistically analyze patients hospitalized for thoracolumbar compression fractures at Daejeon University Daejeon Korean Medicine Hospital from January 1, 2017 to December 31, 2021. Methods: A total of 62 patients were analyzed through electronic medical records in this study. The extracted data was analyzed using IBM SPSS ver.27.0. Results: 1. Traffic accident patients were more likely to reduce pain by more than half compared to falls and other patients. Patients without spinal disease were more likely to reduce pain by more than half compared to those without. The shorter the absolute bed rest and the longer the hospital stay, the higher the probability of pain reduction by more than half. 2. The duration of pain half-reduction was longer in patients with diabetes than in patients without diabetes. 3. The longer the hospitalization period and the shorter the absolute bed rest period, the less pain was reported upon discharge. Males complained of less pain at discharge than females, and patients without spinal disease complained less than those without. Patients who did not receive absolute bed rest complained of less pain upon discharge than those who did not. Conclusions: This study included patients hospitalized for thoracolumbar compression fractures and showed that etiology, absolute bed rest period, hospitalization period, gender, spinal disease, diabetes statistically affected the degree of pain reduction.

  • Recently, Ubiquitous innovation is being promoted so that they can support the uHealthcare provide management to human's health. It is thus necessary to conduct such research on the medical care environment where there is a high demand for utilization of status information. In the current situation, there is a lack of research on measures of security processing and monitoring patient status information produced from rapid growth of infra within Medical environment. This study suggests a solution of using RFID to gather patient information such as inpatient information, location of treatment room, progress of patient, humidity, temperature, and diagnostic status, after which the information are protect and processed using security level method.

Analysis of Treatment Effect According to the Period of Herbal Treatment in Cerebral Infarction Patients Admitted with Hemiplegia: Retrospective Medical Chart Review (편마비를 주소증으로 입원한 뇌경색 환자의 한방 치료 시기에 따른 치료 효과 분석 : 후향적 의무기록 분석)

  • Objective: This study was aimed at evaluating the effect and clarifying the treatment period of Korean medicine treatment for cerebral infarction. Method: This study was carried out on patients with hemiplegia who were hospitalized in the Department of Korean Internal Medicine of Jecheon Korean Medicine Hospital of Semyung University from June 2014 to May 2019. A retrospective study was performed on 253 patients who were diagnosed with cerebral infarction by brain CT or nuclear magnetic resonance imaging. Results and Conclusion: 1. Korean medicine treatment has a significant effect on improving the movement disorder and daily life independence of cerebral infarction. 2. In the group that started Korean medical treatment within one month after the onset of cerebral infarction, with the exception of MMSE-K, the indicators related to the movement disorder and daily life independence showed significant effect. 3. MMSE-K showed no statistically significant change in any of the patient groups. 4. Within three months after the onset date, the longer the period of treatment with Korean medicine, the better the symptom improvement of upper extremity movement disorder. 5. The longer the hospital stays, the better the symptom improvement of the lower extremity movement disorder. 6. The combination of Korean and Western medicine did not affect liver or kidney function.

Clinical and Epidemiological Study of 1,165 Hospitalized Cases of Rotaviral Gastroenteritis Before and After the Introduction of Rotavirus Vaccine, 2006-2013 (로타바이러스 백신 도입 전후 입원한 로타바이러스 위장관염 1,165례의 역학 및 임상적 연구, 2006-2013년)

  • Purpose: This study was performed to assess the clinical and epidemiological changes after the introduction of the rotavirus vaccine in Korea, as well as to determine the efficacy of the rotavirus vaccine among hospitalized rotaviral gastroenteritis patients over the past two years. Methods: We analyzed yearly and seasonal patterns of 1,165 inpatients who were hospitalized for rotaviral gastroenteritis under the age of 5 years between 2006 and 2013. We also conducted a survey among 460 gastroenteritis patients who were hospitalized between 2012 and 2013 regarding the rotavirus vaccination and the symptoms of gastroenteritis. Among those individuals surveyed, clinical indices were analyzed for 124 patients who were tested positive for the rotavirus antigen. Results: The incidence of Rotaviral gastroenteritis have decreased significantly by year 2010. After the introduction and widespread dissemination of the rotavirus vaccine, the onset of the disease and the seasonal peak have been delayed. Overall, the vaccinated group showed a lower rate of positivity than the unvaccinated group. Among the hospitalized rotaviral gastroenteritis patients, the vaccinated group had a shorter hospitalization period, less severe clinical symptoms of gastroenteritis, and better laboratory test results. Conclusions: After introduction of the rotavirus vaccine in Korea, there were two main trends observed: 1) the overall level of disease incidence was reduced; 2) the severity of rotaviral gastroenteritis cases also decreased. Based on this data, more children should receive vaccination in order to prevent the rotavirus infection and decrease the severity of rotaviral gastroenteritis.

The Changes in the Outbreak of Rotavirus Gastroenteritis in Children after Introduction of Rotavirus Vaccines: A Retrospective Study at a Tertiary Hospital (로타바이러스 백신 도입 후 로타바이러스 위장관염의 추이: 일개 3차 병원의 후향적 연구)

  • Purpose: This study aimed to examine the changes in the outbreak of acute gastroenteritis, rotavirus gastroenteritis after the introduction of the rotavirus vaccine in Korea. Methods: The current study investigated the number of inpatients in the pediatric ward of Inje University Sanggye Paik Hospital during the periods of 2005-2006 and 2011-2012. A retrospective analysis was conducted on the medical records of 2,840 patients <5 years of age who were hospitalized at Inje University Sanggye Paik Hospital in these time periods. Results: When we compared 2 separate sets of data from before (2005-2006) and after (2011-2012) vaccine introduction, there were statistically significant decreases in the number of patients who were hospitalized for acute gastroenteritis across all of the groups of patients <5 years of age except those <2 months of age. The number of patients with rotavirus gastroenteritis in all age groups declined except for children <2 months of age and those 2-5 months of age. Conclusion: These results show that after the introduction of a rotavirus vaccine in Korea, the incidence of rotavirus gastroenteritis decreased in 6-59-month-old patients hospitalized for acute gastroenteritis.

The Research about Expected Hospital Management in Gynecology area based on the Medical Information Record of a University Hospital (일개 대학병원 의무기록정보 통계를 활용한 산부인과 병원경영 예측에 관한연구)

  • This research is based on the medical information record of a university hospital on analysis of outpatients, inpatients and delivery related information in gynecology area. The result of application having analysis of patients as basic material for hospital management is as follows. The number of new patients in average was 140.9, and as the year passes by, it seemed to decrease noticeably(p=0.000). In the first year (2001) the number was only 212.6, but increased to 140 by the year of 2007, showing increase of 80.9. The actual number of patients in the hospital was 124.6 in average and it showed noticeable decrease after each year (p=0.000), from 144.6 patients on the first year 2001, to 104.8 patients in 2007, showing approximately 40 patients decreased. Multiple regression analysis was performed having independent variables as characteristics of patients and cause of delivery related factors, and dependent variables as the number of patients in the hospital. According to analysis, the cause of affecting the number of patients in the hospital was selected as the number of new patients, the number of delivery per year. The reliability rate was recorded as 62.8%. Therefore, apart from the services on marketing and patient management which must come first, the effect of inviting them cannot be avoided, which directly links to trust resulting from the consideration to patients.

Mumps outbreak in Incheon, Korea, 2009 (2009년 인천지역의 볼거리 유행 : 단일기관 연구)

  • Purpose : Recently, we have noticed an increase in the number of patients with mumps in Incheon, Korea. The aim of this study is to estimate the regional trend in mumps incidence and to evaluate the factors related to the recent increase. Methods : We reviewed the medical records of 66 patients with mumps who had been admitted to Incheon St. Mary's Hospital from July 1999 to June 2009. We compared the differences in records between "outbreak, 2009" and "nonoutbreak, 2000-2008." Results : Of the 66 patients, 35 (53%) were admitted in 2009, and 31 (47%) were admitted between 2000 and 2008. Most of the patients admitted during the outbreak were over 15 years of age (80.0%), were born before 1993 (77.1%), and had received a single dose of mumps vaccine (62.9%). There were no significant differences in contact history, present address, clinical manifestations, and complications, except for orchitis, between the two groups. Conclusion : There was a sharp increase in the number of inpatients with mumps who were born before 1993 and who were over 15 years of age. We recommend that a booster vaccination be considered for adolescents and young adults born before 1993 who are susceptible to mumps infection due to their reduced opportunities for receiving two doses of mumps vaccine.

  • As expectations for medical service have increased recently, the interest in medical quality improvement and patient safety has increased. In addition, as part of efforts to improve the training environment of the residents, the working hours of the residents are reduced which means that the medical workforce is expected to change significantly in the future. As a result, in hospital care of surgical patients, it is inevitable to change the patient care system by surgeon-residents to the new system. For this, the introduction of a surgical hospitalist system is proposed as an alternative. This article is an introduction to the process of introducing a surgical hospitalist system in a single institution and describes in detail the considerations This is the first case of the introduction of surgical hospitalist in Korea. Through this case, we intend to lay the groundwork for the successful implementation of the surgical hospitalist in Korea.
