  • This study was conducted to identify the sleep patterns and influencing factors of hospitalized elderly in a long-term care hospital. The sleep patterns of 142 subjects were recorded using Sleep Charts. The average sleep time of subjects was 10.7 hours a day (3.9 hours in daytime and 6.8 hours in nighttime). Sleep regularity among participants were 71.7% in all day (58.1% in day time and 80.5% in night time). The presence of dementia patients in the room (PDPR) has been identified to be a statistically significant predictor of all day sleep, and pain, PDPR, and physical function have been found to be a significant predictors of sleep regularity in all day among subjects. It suggested that elderly patients in a long-term care hospital do not slept well during night, which leads to increase in daytime sleep and decrease the quality of their sleep. Therefore, an intervention program should be developed to promote the quality of sleep among hospitalized elderly.

Results : Self - efficacy and depression according to subject characteristics were different according to length of stay, and quality of life was different in religion. The correlation between depression, self - efficacy, and quality of life of admitted elderly showed statistically significant negative correlation (p <.01) with depression in both the quality of life and self - efficacy sub - variables. The depression and the self - efficacy of the elderly had the greatest effect on the quality of life. The variables were depression (${\beta}=-.328$), social efficacy (${\beta}=.248$), and physical efficacy (${\beta}=.193$). Conclusion : In order to improve the quality of life of the elderly, it will be necessary to provide and develop medical care services that reduce depression and improve self - efficacy (physical and social).

Data collected from 218 elderly inpatients of geriatric hospitals in Seoul and Gyeonggi region from Sept. 4th to 22nd, 2017 were analyzed with SPSS 23.0. As a results, there was a statistically significant difference in the level of stress, depression, self-esteem of elderly inpatients in geriatric hospitals depending on their Socio-demographic and Hospital-utilization characteristics. Stress of elderly inpatients had a positive influence on their depression(${\beta}=.297$, p=.000), the self-esteem had a partial mediating effect on the relationships between stress and depression(Sobel Z=2.034). These results suggest that the development and provision of self-esteem improvement programs as well as stress management of elderly inpatients are necessary to prevent depression of elderly inpatients in geriatric hospital.

  • The purpose of this study is to examine the factors associated with the probability and levels of the health service utilization among the disabled elderly in Korea. The sample includes 2,111 people older than 65 who are extracted from the 2008 National Survey on People with Disabilities. More than half (54.3%) of the sample experienced at least 1 outpatient physician visit within two weeks and 26.7% were hospitalized within a year. The key factors associated with the outpatient visits were health insurance status, the existence of chronic disease, self-rated health, the Activities of Daily Living (ADLs), as well as renal impairment. Similarly, the utilization of inpatient care was related to health insurance status along with the existence of the internal organ disabilities such as cardiac or respiratory disorders. The study implies the need for the health care policies regarding the prevention of chronic diseases, dependency for daily activities of the elderly, and a management system that specifically targets those with internal organ disabilities. Moreover, the study suggests that financial supports for the low-income group would be helpful to increase their access to health service utilization.

The mean score of depression is $6.83{\pm}4.14$(out of 15), urinary incontinence is $6.44{\pm}9.63$(out of 60), sleep hour is $8.09{\pm}1.55$, quality of sleep is $39.37{\pm}6.55$(out of 60), and dependence is $42.68{\pm}8.78$(out of 85). The economic dependence among the subdomain of dependence is the highest dependence score. Dependence was positively correlated with depression(r=.26, p<.001), urinary incontinence(r=.35, p<.001). Factors Influencing dependence were depression (${\beta}.227$, p=.001), urinary incontinence (${\beta}=.259$, p<.001), and explanation of dependence is 11.5%.
