구순 구개열 성장기 환자에서 임플란트 치료

  • 성장기 환자에서의 각 부위별 임플란트 식려 시기를 요약해보면 다음과 같다. 1) 상악전방부 : 외상이나 선천적 결손을 해결하는 방법으로 임플란트를 사용하는 술식이 권장하지만 성장이 완료되지 않았기 때문에 야기되는 문제를 해결하기 위해 성장이 완료될 때까지 연기하는 것이 좋다. 2)상악 후방부: 수직적인 성장량이 최고 1cm까지 일어날 수 있으면 상악동의 흡수로 조기에 임플란트 침강과 치근부의 노출을 야기할 수 있다. 그리고 동시에 보철에 의한 구개열의 횡적인 성장방해를 피해야한다. 3) 하악 전방부 : 무치증 아이에서 임플란트의 조기식립이 적응증이 되는 유일한 곳으로 치아가 없을 때는 치조골의 성장이 최소이며 초기에 횡적, 후방 성장이 대부분 완료된다. 그러나 단일 임플란트 식립은 권장되지 않는다. 4)하악후방부: 늦게까지 성장변화가 지속되기 때문에 전후방, 횡적, 수직적 성장의 많은 양이 일어난다. 하악의 회전성장의 영향을 많이 받아 하악 무치악에서 조기 임플란트 사용한 보고도 적고. 자연치아에 인접하여 임플란트를 식립한 기록은 있으나 골격성장이 완료될 때 까지는 식립하지 않는 것이 좋다.

Full mouth rehabilitation in a patient with peri-implantitis: A case report (Peri-implantitis 환자에서의 전악 재수복 증례)

  • Peri-implantitis appears in almost 20% of patients who received implant treatment, and increase in its number is inevitable as time goes by. Although it can be treated by both non-surgical and surgical procedures, in cases which include severe bone loss, explantation and rehabilitation may be necessary. Careful treatment planning and considerations to prevent recurrent peri-implantitis should be taken into account. In the following case presented, a patient with chronic periodontitis and peri-implantitis was successfully rehabilitated after removal of several implants. Extraction and explantation of multiple teeth and implants were followed by full mouth reconstruction with fixed implant prostheses on the mandible and implant retained overdenture on the maxilla. Surgical and prosthetic measures to prevent recurrent peri-implantitis were taken into consideration.

  • For treatment of partially edentulous patients, a treatment using implant is widely used. Treatment method using implant are implant fixed prostheses and removable partial dentures, and for patients with severe bone resorption, removable implant overdenture with the effects of aesthetic and reducing cost can be used as treatment options. Specially, prosthesis with milled-bar and attachment has the effect of being splinted between implant fixtures, higher retention and stability than conventional removable partial denture. And it has the effect of improvement of aesthetic through lip support by denture base. In this case, the patient with severe alveolar bone resorption and partial edentulous maxilla and mandible was treated by implant-assisted removable partial denture using Milled-bar and ADD-TOC attachment. The esthetic was improved by removing the clasp because of effects of additional retention by using the attachment, and reducing palatal coverage of implant-assisted removable partial denture. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.

  • Adult patients who need implant and prosthodontic treatment often need treatment to improve the existing occlusion through orthodontic treatment for long-term stable treatment results. However, due to non-aesthetic and uncomfortable orthodontic treatment with orthodontic brackets and wires, many adult patients give up treatment even though they know the need for it. Recently, as digital dentistry has affected all areas of dentistry, clear aligner orthodontic systems have begun to be widely used, and their use is increasing in adults and old-aged people due to the esthetic advantage and convenient oral care. SERAFIN clear aligner system developed in Korea has been developed with the aim of implementing a functional occlusion harmony and is used not only for partial orthodontic treatment but also comprehensive orthodontic treatment. This patient presentation is shown the treatment using SERAFIN clear aligner system for the treatment of patient with TMD, severe extrusion of maxillary second molars, and extracted teeth.

A retrospective statistical analysis of dental implants (치과 임플란트 환자의 통계 분석에 관한 후향적 연구)

  • Statement of problem: The number of edentulous patients is increasing due to an aging society, which leads to increased demands and interests on implant restorations. Implant restorations are an effective treatment method for both complete and partially edentulous patients, and the success rate has been reported to be high. But because of the increased use of implants in various situations have resulted in frequent reports of failures on implant restorations. Various efforts to overcome these failures have been made. Purpose: The aim of this study was to investigate the statistic survival rate of dental implants on patients who were treated at Dankook University Dental Hospital during the past 10 years based on their charts. Materials and methods :The research was made for two different periods of time; The first investigation was for patients from August, 1998 to August, 2003 and the second was for patients from September, 2003 to April, 2007. Information on the number of implants placed, the location and distribution of implants, the duration between the first and second surgery, and the survival rate of implants were investigated from the patients'charts. Results: 1. According to the first investigation, 1680 implants were placed on 612 patients and the second investigation showed 2438 implants placed on 933 patients. Thus a total of 4118 implants on 1545 patients. 2. Among the 1545 patients, 884 patients were male (57.2 %) and 661 patients were female (42.8%). Out of 4118 implants, 1739 implants (42.2%) were placed on the maxilla, and 2379 implants (57.8%) on the mandible. Implants were placed most frequently in the posterior region of the mandible. A total of 2043 implants (53.2%) were placed in this region. 3. According to the first investigation, 57 out of 1680, implants failed, while from the second investigation, 17 out of 2438 implants were reported as failure. In total, 74 implants failed, which results in a 98.2% survival rate. 4. The average duration between the first and the second surgeries in maxillas decreased from 7.4 months to 6.8 months. The duration also decreased from 5.6 months to 5.0 months in mandibles. Conclusion: As shown in the results, the number of placed implants and the survival rate of implants were higher in the second investigation than that of the first investigation. And the time spent after the first surgery to the second surgery was less in the second investigation. Consequently, it can be presumed that the demand and consumption of dental implants as well as the survival rate will increase in the future.

  • In a edentulous patient, various methods can be employed for prosthetic treatment using implants, such as implant-supported fixed prostheses, overdentures, hybrid prostheses, and implant assisted removable partial denture. In this case, in a patient with moderate to severe chronic periodontitis requiring full arch extractions, implants were strategically placed using computer-guided surgery. In the maxilla, due to inadequate bone quality and quantity leading to insufficient initial stability, delayed loading was implemented, and interim prosthesis was used during the osseointegration period. In the mandible, stable initial stability was achieved, allowing for immediate loading to reduce patient discomfort. Primary stability is considered the most crucial factor for obtaining immediate loading, so a thorough clinical and radiological evaluation of the remaining alveolar bone quantity and quality must be conducted before surgery.

Implant-assisted removable partial denture restoration in small number of residual teeth in mandible: A case report (하악 소수 잔존치 환자에서 임플란트 보조 국소의치 수복 증례)

  • The patient in this case presented with a desire to have new dentures due to discomfort with existing ones. At the initial visit, all of teeth were missing except for the mandibular left second molar. As the patient was 65 years old, treatment with dentures and implant-supported prostheses was possible under the national health insurance system, and the patient opted for the mandibular denture using implant. Temporary prostheses were initially provided for patient adaptation, and following successful adaptation period, the treatment progressed. A maxillary complete denture and a mandibular implant-supported denture using two implants in the canines were fabricated. The mandibular denture is a Kennedy Class II removable partial denture which consists of a six-unit porcelain fused to metal fixed dental prostheses supported by the implant in the canines on both sides and left second molar serving as the abutments. Despite severe bone resorption and insufficient abutment teeth, the patient expressed satisfaction with the treatment results. In cases with economic and anatomical constraints affecting the feasibility of complete denture, implant-supported overdenture, and implant-supported fixed dental prostheses, an implant-assisted removable partial denture using implant surveyed crowns proves to be a viable and effective alternative treatment option. Nevertheless, the current dearth of scientifically rigorous studies underscores the necessity for meticulous regular check-up and occlusal assessment.

  • There are various methods for restoring the dentition of completely edentulous patients. Removable complete dentures have the advantage of being relatively economical, but they can be uncomfortable to wear. With the introduction of implant prosthodontics, various options such as implant-supported overdentures and hybrid prostheses have become available. If there is inadequate remaining ridge or limited financial resources, an overdenture supported by a few implants with additional attachments may be more suitable. In this case, due to severe peri-implantitis and other complications, the implants were removed. Subsequently, four implants (two on each side) were placed in the maxilla and a milled-bar with attachment was fabricated for each side of the maxilla.

  • Edentulous patients with a severely resorbed mandible or maxilla often experience problems with conventional dentures, such as insufficient stability and retention, together with a decrease in chewing ability. Because of the good prognosis of dental implants, these patients can be successfully treated with implant-retained or implant-supported prosthesis. Ideally, a maximum number of implants of maximum length are placed in appropriate surgically prepared sites that are surrounded by a maximum amount of bone of favorable quality. The implants are favorably aligned faciolingually and mesiodistally to enhance optimal prosthodontic design. This article describes the clinical problems and complications encountered when treating a consecutive number of edentulous patients with osseointegrated implant-supported prostheses.

  • Implant supported prostheses have improved the quality of lives of totally edentulous patients. However, there are some limitations on the number of implants related to the residual bone level or the economic concern and so on. In this situation, applying fixed detachable prosthesis with bilateral cantilevers can be considered. This clinical report describes implant supported prosthetic rehabilitation of a patient with severe bone resorption. The patient was satisfied and felt comfortable with upper complete denture and lower fixed detachable prosthesis.
