Development of Detailed Clinical Models for Pain Assessment (통증 간호사정을 위한 임상내용모델 개발)

  • Purpose: The aim of this study was to develop and validate Detailed Clinical Models (DCMs) for pain assessment in nursing. Methods: First, we identified the entities of pain assessment from ICNP. We identified the attributes and values of the attributes to describe the entities in more detail by reviewing the literature. Data types and optionalities of the attributes were defined. Second, we modeled the DCMs by linking an entity and its corresponding attributes with values and by specifying the data types and optionalities of the attributes. Finally, the DCMs were validated by a group of domain experts using a content validity index. Results: In total, 19 DCMs with 11 attributes for pain assessment were developed. The experts' evaluations showed that the DCMs were valid enough to represent pain related information of nursing assessment. Conclusion: The DCMs developed in this study can be used in electronic nursing records. The DCMs for pain can ensure the semantic interoperability of pain related information in electronic nursing records.

Clinical presentation with 『PangYakHapPyon(方藥合編)』 in Korean medicine (방약합편과 한의학 임상표현)

  • Objectives: The aim of this study is to introduce the Clinical presentation and announce the importance of developing Clinical presentation of Korean medicine and suggest about development direction of Clinical presentation of Korean medicine. Methods: To Investigate the Clinical presentation used in western medicine. I think that Clinical presentation of Korean medicine is a systematic list of Korean medicine symptoms and a standard syndrome differentiation and treatment(辨證論治). So I would like to offer "PangYakHapPyon(方藥合編)" as a basis for developing Clinical presentation of Korean medicine. Results: The clinical presentation term has become widespread in use at Calgary Medical college. Calgary Medical college created a list of 120 clinical presentations In 1991. In Korea, 101 clinical presentations were made in 2016. "PangYakHapPyon(方藥合編)" has been used effectively for over 130 years and widely used in the public. In addition, "PangYakHapPyon(方藥合編)" is summarized in the symptoms and prescriptions that occur frequently in Korea. Conclusions: For the globalization and standardization of Korean medicine, Clinical presentation of Korean medicine should be developed. The overall form of Clinical presentation of Korean medicine uses the form of Clinical presentation of Canada and a standard syndrome differentiation and treatment(辨證論治) for diagnosis and treatment is based "PangYakHapPyon(方藥合編)".

  • Expectation of the patient for esthetic improvement through the dental treatment is diverse and complicated. Chief complaints of patient should be translated to objective factors reflecting patient's needs and wants. Treatment planning would be based on these demands and verified through the communicating with the patient through the procedure. Esthetic approach in dental treatment include subjective elements as well as objective ones.

  • 이 논문에서는 자연어로 구성된 수학 문장제 문제를 자동으로 풀이하기 위한 Transformer 기반의 생성 모델인 KoEPT를 제안한다. 수학 문장제 문제는 일상 상황을 수학적 형식으로 표현한 자연어 문제로, 문장제 문제 풀이 기술은 실생활에 응용 가능성이 많아 국내외에서 다양하게 연구된 바 있다. 한국어의 경우 지금까지의 연구는 문제를 유형으로 분류하여 풀이하는 기법들이 주로 시도되었으나, 이러한 기법은 다양한 수식을 포괄하여 분류 난도가 높은 데이터셋에 적용하기 어렵다는 한계가 있다. 본 논문은 이를 해결하기 위해 우선 현존하는 한국어 수학 문장제 문제 데이터셋인 CC, IL, ALG514의 분류 난도를 측정한 후 5겹 교차 검증 기법을 사용하여 KoEPT의 성능을 평가하였다. 평가에 사용된 한국어 데이터셋들에 대하여, KoEPT는 CC에서는 기존 최고 성능과 대등한 99.1%, IL과 ALG514에서 각각 89.3%, 80.5%로 새로운 최고 성능을 얻었다. 뿐만 아니라 평가 결과 KoEPT는 분류 난도가 높은 데이터셋에 대해 상대적으로 개선된 성능을 보였다.

  • Acute coronary syndrome involves three types of coronary artery disease associated with sudden rupture of coronary artery plaque, and has a clinical presentation ranging from ST-segment elevation myocardial infarction (STEMI) to non-ST-segment elevation myocardial infarction (NSTEMI) or unstable angina. Cardiac CT can help quantify and characterize atherosclerotic plaques. According to a previous study, low-attenuation plaque, napkin ring sign, positive remodeling, spotty calcification, and increased perivascular fat attenuation are associated with plaque ruptures on cardiac CT. Therefore, coronary artery stenosis, as well as acute coronary artery syndrome, can be diagnosed using cardiac CT.

  • Immunoglobulin G4 (IgG4) related disease (IgG4-RD) is currently considered an immune-mediated condition and is recognized as a disorder group with common pathological, hematological, and clinical characteristics. This disease may involve diverse organs of the head and neck, and include mainly the lacrimal gland, orbit, thyroid gland, pituitary gland, and the meninges. Here, we report a case of IgG4-RD in a 65-year-old female showing head- and neck-limited but synchronously and mainly manifesting as otalgia and facial neuritis.

  • Adrenal gland trauma is uncommon and is diagnosed at an increasing frequency using CT scans. However, owing to the rarity of this injury and its diverse clinical presentations and prognoses, there is no consensus on its management. In this case report, a 73-year-old male patient experienced recurrent bleeding in the right adrenal gland due to an in-car traffic accident, which was treated with repeated transcatheter arterial embolization.

  • MRI is a crucial tool for diagnosing adenomyosis and identifying its related pathologies. To accurately diagnose adenomyosis, it is necessary to recognize both the typical MRI findings and atypical features of the condition. Recently, a standardized classification system has been developed to facilitate precise presurgical diagnosis of adenomyosis and to determine the appropriate treatment method. Differentiating between various subtypes based on MRI-based classification and identifying different MRI phenotypes can aid in categorizing patients with adenomyosis into specific treatment groups and monitoring their response to therapy.

  • Over the last 30 years, the use of chairside computer-aided design (CAD) and computer-aided manufacturing (CAM) systems has evolved and has become increasingly popular in dentistry. Although CAD/CAM restorations have been used in the anterior dentition, satisfying the esthetic requirements of clinicians and patients, where the restorations are limited to the chairside, remains a challenge. To reproduce multi-shades of CAD/CAM restorations in the clinic, a preliminary experiment to express several shades on A2 lithium disilicate (LS2) blocks using a staining kit was performed. After measurement of the CIE L*a*b* value of specimens, it was compared with that of the commercial shade guide. This report presents two cases with individual customization of shade and surface characterization of the CAD/CAM restorations using predictable methods based on the preliminary experimental data. The anatomical shape of restoration was obtained from 'copy and paste technique' and 'mirror image acquisition technique'. All treatment procedures and fabrication of restorations performed in this report were executed in the clinic itself.

Acupuncture Expands the Boundary of Mind-Body Medicine (침술이 심신의학의 지평을 확장하다)

  • Purpose : The acupuncture existed more than 2000 years and has flourished not only in Asia but also in medical field of modern world. In addition, empirical evidences from biomedicine have been expended in many ways in order to objectify the field of acupuncture. Thus, the effect and mechanism of the acupuncture has been revealed in human and other animals. In this article, I attempt to explain what doctors and patients subjectively experience through acupuncture treatment and its clinical significance based on mind-body medicine. Methods : The experience and clinical significance of the acupuncture was examined in the view of mind-body medicine, also reviewed research trends related to meditation understanding the clinical effectiveness of acupuncture. Results and Conclusions : Traditional Korean medical doctor perceive generally "jogi " and "chishin " meaning the regulation of the balance of the various functions in the human body and patient's consciousness and psychology state as mightily important concept concerning the therapeutic effect of acupuncture. Despite scientific approach to objective understanding of the acupuncture was performed in the meantime, practitioners put emphasis on experience based on sensate and interaction between doctor-patient. As it reflects the recent understanding of the psychological effects associated with the clinical efficacy of acupuncture, it has been presented one by one through neuroscience and cognitive science. Therefore it is important that interpreting the clinical effectiveness of acupuncture into Mind-body medicine can form a new awareness and attitudes toward traditional medicine.
