五臟與神志活動對應關係的考察 (오장(五臟)과 신지활동(神志活動)의 대응관계(對應關係)에 대한 고찰(考察))

  • 개요 및 목적: 고금명의(古今名醫)들의 신(神), 혼(魂), 백(魄), 의(意), 지(志) 등의 이상에 대한 치료를 고찰분석(考察分析)함으로서 오장과 신(神), 혼(魂), 백(魄), 의(意), 지(志)의 대응관계를 연구하였다. 방법: 임상병례(臨床病例)들을 취하고 통계방법을 사용하여 고금명의들의 처방 중에서 조건에 부합되는 처방 589예를 선택하여 약물귀경에 대하여 통계처리를 함으로서 각 종류의 정신중상에 사용된 약들이 어느 한 장부계통과 특정적인 대응규칙이 있는지 여부를 분석해 보았다. 결과: 임상치료(臨床治療)에서 여러 가지 정신증상에 사용된 약들은 오장계통에 모두 영향을 주었는데 여러 가지 정신증상을 치료하는 약들은 어떤 한 가지 증상에 그와 대응되는 한 가지 장부 계통이 연관되는 특정적인 규칙은 없었다. 각 종상에 사용된 약들은 오장계통(五臟系統)중에서도 심계통과 비위계통을 치료하는 약이 나타난 빈도가 가장 높았다.

  • For ethical and economic reasons, clinicla trials must be repeatedly monitored for evidence of treatment benefit or harm. Repeated testing at conventional critical values can substantially inflate the overal type I error rate. To maintain acceptable levels, group sequential procedures have been developed for clinical trials. This article gives a brief overview of the group sequential procedures.

The Effectiveness of Intravenous Immunoglobulin for Clinically Suspected Neonatal Sepsis (임상적으로 의심되는 신생아 패혈증에서 정맥용 면역 글로불린의 치료 효과)

  • Purpose : The purpose of this study is to determine the effectiveness of intravenous immunoglobuin (IVIG) administration in fullterm neonates having clinically suspected neonatal sepsis. Methods : Forty full-term neonates admitted to the neonatal intensive care unit with clinically suspected neonatal sepsis, who had at least two positive diagnostic criteria were enrolled. Twenty neonates were enrolled into the IVIG arm and 20 in the placebo arm. Neonates with a gestational age of less than 36 weeks and those with any major congenital malformation were excluded. The neonates were randomized to receive 1 g/kg of IVIG or equivalent amount of normal saline. The treatments including antibiotics and supportive care were administered. Results : The neonates in the therapy and placebo groups were comparable in terms of birth weight, gestational age, sex distribution, duration of antibiotics therapy and admission, elevation of serum IgG level, mortality rate, change of CBC, and serum level of acute phase reactants etc. Conclusion : Serum IgG values increased significantly 5 days after administration of IVIG in the IVIG-treated group and decreased significantly 5 days after administration of normal saline in the placebo group. However, there was no significant difference in the duration of antibiotics therapy and admission, or of mortality between the IVIG-treated and placebo groups. No adverse reactions to the IVIG infusions were noted during the study. Our preliminary observations suggest that the administration of 1 g/kg IVIG to neonates had some effect on augmentation of humural immune status in neonates with clinically suspected sepsis. But further study is needed to verify the benefit of IVIG infusion to neonatal sepsis.

  • Purpose: This review aims to explain the definition and basic principle of statistical analysis and to clarify statistical issues related to the sample size calculation. Materials and Methods: Many formulas are available that can be applied for different types of data and study design. Results: The sample size is the number of patients or other experimental units that need to be calculated prior to the study. Determining the appropriate sample size is required to answer the research question. Conclusion: Caution is needed when applying formula for the calculation of the sample size, as it is sensitive to error and even small differences in selected parameters can lead to large differences in the sample size.

Maximum Tolerated Dose Estimation with Dose De-Escalation Design in a Phase I Clinical Trials (제 1상 임상시험에서 용량 감량을 허용하는 MTD 추정법)

  • The main purpose of phase I clinical trials is to estimate the Maximum Tolerated Dose (MTD), which minimizes side effect and assures safety of a new drug by evaluating the toxicity at each dose-level. The conventional MTD estimation methods is Standard method (Storer, 1989; Korn et al., 1994), Accelerated Titration Designs (Simon et al., 1997) and DM method (Dixon and Mood, 1948) etc. In this paper, MTD estimation method with de-escalation is suggested phase I clinical trials. The proposed MTD estimation method is compared to Accelerated Titration Designs, SM3 without de-escalation method and SM3 with de-escalation method using a Monte Carlo simulation.

  • The purpose of a Phase I Clinical Trial is to determine the maximum tolerated dose (MTD). MTD is important because it affects subsequent clinical trials; however, the existing method has a problem due to an inadequate dose allocated to patients. In this paper, an MTD estimation method is proposed to complement the problems of the existing MTD estimation method. The suggested method applies the initial acceleration step to the modified continual reassessment method. Monte Carlo Simulation Study is adapted to compare a suggested MTD estimation method with the standard design and the modified continual reassessment method.

  • Objectives : 본 연구를 통하여 한방안이비인후과 임상실습에서 Objective Structured Clinical Examination (OSCE) 수업의 효용성을 알아보고자 한다. Methods : 2014학년도 한의과대학 본과 4학년 학생을 대상으로 안이비인후과 임상실습의 한 방법으로 OSCE를 시행한 후, 학생들은 OSCE 수업 만족도에 관한 설문지를 작성하고 레지던트는 학생들을 평가하였다. OSCE 평가 성적과 기타 임상실습 및 필기시험 성적과의 연관성을 분석하여 OSCE 성적의 학점 부여 타당도를 살펴보았다. Results : 만족도 관련 설문 결과 총 평균점수가 4.34점이 나왔다. OSCE성적과 기타 임상실습 및 필기시험 성적과의 상관성을 pearson correlation을 통하여 살펴본 결과, OSCE성적은 임상실습 성적 및 (r=0.210, p=0.056), 필기시험 성적과 통계적으로 유의한 상관성이 없었다 (r=0.064, p=0.567). Conclusions : 위의 결과로부터 임상실습에서 OSCE 시행은 학생들의 학습 만족도를 높인다는 것을 알 수 있었다. 임상실습에서 효율적으로 OSCE를 활용하는 방안을 모색하기 위해 더 많은 연구가 필요할 것으로 생각된다.

Convergence Study on Nursing Students' Stress and Satisfaction with Clinical Practice (간호학생의 임상실습 스트레스와 임상실습 만족도에 관한 융합적인 연구)

  • The purpose of this study was to examine the relationship between stress and satisfaction with clinical practice of 113 nursing students. The subjects scored 3.17 points of full 5 on stress with clinical practice and 3.24 of full 5 on satisfaction with clinical practice. There were no significant correlations between their stress with clinical practice and satisfaction with clinical practice(r=-.048, p=.615). The findings of this study provide the basis for expanding nursing practice and education for nursing students, and nursing administrators and professors should make efforts to increase nursing students' satisfaction with clinical practice and decrease stress with clinical practice.

The Effects of Simulation-based Nursing Education on Diabetic Patient in Nursing Students (간호대학생의 당뇨환자 시뮬레이션기반 간호교육의 효과)

  • This study aimed to determine the effects of simulation-based education in diabetic patient care to nursing students on their level of diabetic knowledge, clinical nursing competence, practicum satisfaction and communicative competence. The pre-post test were conducted to compare the differences in diabetic knowledge, clinical nursing competence, practicum satisfaction and communicative competence. The research data for the subject of study was collected from October 21 to November 1, 2013 in 3rd grade 75 nursing students. The student who had received the simulation-based education, did show significant increase in diabetic knowledge(t=-9.684, p<.001), clinical nursing competence(t=-4.173, p<.001), and communicative competence(t=-7.685, p<.001) than before. But, there was no significant different in the practicum satisfaction(t=-1.144, p=.256). Based on the above results, simulation-based education can enhance the nursing knowledge, clinical performance and communication ability of the subjects and can be applied to various education courses.

Comparison of Clinical Manifestations of Rotaviral Gastroenteritis between Neonates and Infants (신생아와 영유아 로타바이러스 위장염의 임상 경과에 대한 비교)

  • Purpose: This study was designed to investigate rotavirus infection by comparing the clinical characteristics in neonates and infants. Methods: We enrolled 104 neonates and 250 infants wiht gastroenteritis and a rotazyme test positive reaction at the Soonchunhyang University Bucheon Hospital from February 2001 to January 2003. Results: The seasonal peaks of infection in infants occurred from February to June. However, in neonates, it occurred from October to December due to nursery outbreaks. Diarrhea, vomiting, fever and convulsions were significant symptoms in infants; however, metabolic acidosis with dehydration, jaundice, irritability, apnea, bloody stool, gastric residual, grunting, poor oral intake, lethargy as well as fever and diarrhea were more common in the neonates. Upper respiratory infection, pneumonia and bronchitis were present in the infants; however, necrotizing enterocolitis was more commonly observed in the in neonates. Among the patients with rotaviral infection, formula feeding was more popular than breast milk feeding in both the neonates and infants; however, this finding was not statistically significant. Conclusion: Rotavirus can be a significant pathogen in neonates as well as infants. Neonates suffering from fever, poor oral intake, lethargy and apnea should be investigated for rotaviral infection. A new vaccine, rotaviral specific immunoglobulin and treatment guidelines are needed for eradicating rotavirus infection. Further studies on isolation, infection pathway, immune response and treatment of rotavirus are needed.
