The Role and Focus Areas of Medical Technologists in the Field of Diagnostic Tests in the COVID-19 Era (COVID-19 시대 임상병리사의 역할 및 영역)

  • This study attempted to provide the basic data for developing a system to identify the role of medical technologists and ensure an efficient response for quick and accurate diagnostic tests in the COVID-19 era. The research method involved using focus group interviews for a survey and analysis of 15 medical institutions. Eleven sample collection institutions, 10.4 medical technologists, 2.1 minutes of collection time, 5.4 hours of test time, 9,670 tests, 6.2 member test workforce size, and 7 screening center operating institutions were surveyed. The results of the focus group interview analysis revealed that there were no standardized guidelines covering working hours, area, and environment to protect sample collectors and testers in relation to the COVID-19 tests. Also, legal protection measures were insufficient in the event of accidental infections and there were no personnel regulations related to COVID-19. In addition, the professional training of sample collectors and molecular diagnostic testers was required for reliable COVID-19 testing. In conclusion, it is necessary to provide professional education through special test short-term training institutions to cope with emergency infectious diseases such as COVID-19. Legal systems should be put in place to protect the workforce and ensure stability.

Blood Collection Workload and Safety of Medical Technologists at Korean Hospitals (한국 의료기관에서 임상병리사의 채혈 업무 현황과 안전 실태 조사)

  • This study investigates the current situation of medical technologists for blood collection, which is considered the most important step for diagnosis. The survey enrolled 650 medical technologists working in hospitals and medical check centers in Busan, Ulsan, and Gyeongnam. We found that each medical technologist performed blood collection for about 100 patients. There was more than one blood collection failure per day, with more than one case of pain and filing of civil complaints per year. Hence, there was a high work burden on the medical technologists. Cases where a medical technologist was stabbed with a used needle occurred more than once a year, and about 15% of them received infection control and treatment because of stab wounds. Additionally, more than half of the participants suffered from musculoskeletal disorders and mental stress due to blood collection work. Unlike administering intravenous and intramuscular injections using the same needle, no fee is charged for blood collection. Based on the results of this study, it will be possible to improve the safety and rights of medical technologists by calculating the actual fee for blood collection work and assigning a relative value score.

Study on the Professionalism of Medical Technologists' Work in Health Examination Centers: Focusing on the General Health Examination (건강검진센터에서 임상병리사의 업무 전문성 연구: 일반건강검진을 중심으로)

  • "Health examinations" means medical examinations conducted by health examination institutions, including a physical examination, consultation, physical measurement, diagnostic laboratory test (also known as clinical laboratory test, clinical pathology test), pathology test, and Imaging test for checking the health condition and the prevention and early detection of diseases. The types of health examinations include general health examinations, comprehensive health examinations, special health examinations, and other health examinations. The proportion of the general health examination work of medical technologists working at health examination centers has been estimated to comprise more than 50% of the total work. The cost aspect of suspicious diseases can be estimated to be more than 25%. The cost of diagnostic laboratory test has been estimated to be approximately 5%, and the proportion of medical diagnosis use is 70%. The results show that it is time to revisit whether medical technologists are receiving appropriate social, economic, and reasonable policy treatment in health examination work. The future of medical technologists will further expand their professionalism in diagnostic laboratory test and quality control in precision medicine and telemedicine in the future. Therefore, medical technologists will need to participate proactively in government policy decisions related to national health examinations and make efforts to improve treatment.

Survey on the Relationship between the Number of Medical Technologists and the Medical Test Count (검사건수에 따른 임상병리사의 인원편성에 관한 설문조사 분석)

  • This study examined the relationship between the number of medical technologists and the medical test count. Data was obtained from 441 medical technologists in a hospital through a self-reported questionnaire. The Pearson correlation test, student's t-test, ANOVA or descriptive statistics were performed for data analysis. The distribution of medical technologist according to the size of hospitals was small 5.8, medium 14.9, large 25.8, and super 45.4. The analysis demonstrated a relationship between the number of medical technologists and the number of medical tests in the field, number of clinical tests per MT, and number of optimal medical test per MT according to the hospital size (P<0.001). The average time for quality control by the department at a higher hospital was less than two hours. In terms of the satisfaction of salary, work environment, test accomplishment, and welfare service, the dissatisfaction of medical technologists in small and medium hospitals was higher than those in large and super hospitals. Overall, a focus on intensifying systemic supplementation and improving the condition of medical technologists is needed to provide reliable data for medical examinations in medical areas.

Proposals on Basic Data Based on Comparison of Changes in Clinical Laboratory Technologists' National Examination and Job Definition: Focused on Korea, Japan, and Taiwan (임상병리사국가시험 및 직무의 변천 비교를 중심으로 한 기초자료 제안: 한국, 일본, 대만을 중심으로)

  • This study examined the transition process of clinical laboratory technologists' national examination and job definition in Korea and compared the differences in the national examination between Korea and neighboring countries, such as Japan and Taiwan. In Korea, the number of questions made for it was 200 (1965), 200 (1977), 300 (1982), 250 (1992), 330 (2006), and 280 (2015). The practice of clinical physiology is important for real-time monitoring, given the characteristics of physiological testing. On the other hand, there are conflicts between other occupations in the working area. Clinical molecular biology needs to be established as a new major subject considering the diagnostic importance of molecular biological tests and the speed of science and technology development. Clinical laboratory operations provide policy and guidance recommendations to technologist staff. The proposed clinical laboratory technologists' national examination comprises major subjects: clinical biochemistry, clinical hematology, clinical transfusionology, clinical immunology, clinical microbiology, clinical molecular biology, clinical histology, clinical cytology, clinical physiology, and clinical laboratory operations. In addition, this study proposes the job definition of clinical laboratory technologists, revising various chemical or physiological testing to biomedical or physiological testing required for medical practice.

  • When it comes to 'Team Medical Care' in Japan, clinical laboratory technologists have actively engaged in various kinds of teams, such as a diabetes team, a nutrition support team, an infection control team and a medical practice support team. Overall, with continual changes in the medical environment, clinical laboratory technologists are recently being required to not only conduct phlebotomy, specimen collection, laboratory testing, and electrocardiography, but also get actively involved in 'medical practice assistance or support' in collaboration with medical doctors and nurses. Therefore, it is anticipated that resident clinical laboratory technologists in wards or emergency rooms in Korea will be better able to contribute to improving medical quality and securing medical safety by functioning as a link to the clinical laboratory, while medical doctors and nurses will have a reduced burden of work and can dedicate themselves to better patient care.

  • The purpose of this study is to find terms that can give identity to the major and occupation of clinical laboratory technologist (also known as medical technologist). The term clinical pathology includes all branches of pathology, namely anatomical pathology, chemical pathology, hematology, microbiology, and all respective subspecialties. Unfortunately, several countries exclude anatomical pathology from the term clinical pathology, a problem that gets compounded when the title is translated into languages other than English. Clinical pathology (US, UK) is a medical specialty. Similar terms are laboratory medicine (Germany, Poland), medical/clinical biology (France, Netherlands) or clinical analysis (Spain). Depending on the person questioned, medical technology is defined slightly differently by individuals, companies, and institutions. The definition also depends on the language in which the question is asked. Medical technology can be translated to define clinical laboratory technology, allied health sciences, medical equipment, biomedical engineering, and health technology. The terms 'clinical pathology technology and pathological technology' are not used in allied health sciences. The names of 'medical technology·medical technologist' can be replaced by 'biomedical laboratory science·biomedical laboratory technologist' or 'clinical laboratory analysis·clinical laboratory analyst'. In this study, it is proposed to change the name of academic and occupation to 'medical biology·medical biology technologist' that combines the term biomedical.

  • 양의 스크래피(scrapie)는 우리나라에서의 발생보고가 없는 해외가축전염병의 일종이며, 동물의 전염성 해면형 뇌증(Transmissible Spomgifrom Encephalopathies; TSEs)중 역사가 가장 오랜 질병인데, 현재 영국에서 문제되고 있는 새로운 전염병인 소 해면형 뇌증(Bovine Spongiform Encephalopathy; BSE) 즉 일명 광우병(mad cow disease)과의 어떤 연관 가능성 때문에 수의학계의 관심의 대상이 되고 있는 질병이다. 일단 감염되어 발병되면 치료대책 없이 100$\%$ 폐사되는 세기의 불치병으로 알려진, 소 해면형 뇌증(BSE)은 영국에서 현재 사람의 크로이츠휄트-야콥병(Creutzfeldt-Jakob disease; CJD)과의 관련 가능성 여부를 놓고 독특한 문제가 되고 있는 세계적인 희귀질병이다. 이들 질병에 대하여는 아직까지 확실한 병인체가 밝혀져 있지도 않으며, 그렇기 때문에 면역 혈청학적 진단방법도 확립되어 있지 못할 뿐만아니라 예방백신의 개발 또한 불가능하다. 다만, 임상적인 병력과 임상소견, 뇌조직 표본에 대한 현미경 검사 또는 전자현미경 검사에 의한 특이소견 관찰 등 조직병리학적 진단만이 가능할 뿐이다. 본편에서는 소 해면형 뇌증(BSE)의 병리학적 감별진단과 관련, 지금까지 보고된 임상증상을 검토해보고, 우리나라에서 경험한 소의 광견병에 대한 조직병리학적 진단 재료를 근거로하여 감별진단을 위한 참고자료로 설명하고, 국제수역사무국(Office de International Epizooties; O.I.E.)에서 발생한 Manual of Standards for Diagnostic Tests and Vaccines for List A and B Diseases of Mammals, Birds and Bees(1992)(포유류, 조류, 꿀벌에 있어서의 A급 및 B급 질병에 대한 진단시약 및 예방백신에 대한 표준지침) 중에서 소 해면형 뇌증(B 83; p 742-747)과 스크래피(B 32; p 424-427)에 관한 내용 (Chapter 22, 205-215)을 기본자료로 제공하고자 한다.
