Nurses' Knowledge and Performance for Prevention of Extravasation at Peripheral Intravenous Therapy (간호사 특성에 따른 일혈예방 지식과 간호수행)

  • This study was aimed to identify the level of knowledge and performance on nurses for the prevention of extravasation. Subjects of this study was 293 nurses working at three university hospitals, two general hospitals and one specialized hospital that frequently performed chemotherapy in D city. The highest score for the knowledge of nurses was extravasation drug related that it was $18.50{\pm}2.18$. The knowledge and performance were also higher for nurses with age greater than 50 years and married, with a clinical career of more than 10 years and high regard for extravasation. It is essential to develop other plans that promote the prevention of extravasation and to provide a continuous education for preventing extravasation. Therefore, it is necessary for medical institutes to operate patient safety management teams to develop standardized protocols and educating nurses and related persons.

Incidence of Extravasation in Acute Care Hospitals and Nurses' Knowledge and Attitude toward Extravasation Prevention and Management (급성기 의료기관의 일혈 발생 현황 및 임상간호사의 일혈 예방 및 관리에 대한 지식과 태도에 관한 연구)

  • Purpose: Extravasation of diagnostic and therapeutic materials might occur when the intravascular solution leaks into the surrounding tissues. Injury associated with extravasation depends on various factors. It may range from mild skin reaction to severe necrosis. However, the incidence rate for extravasation is largely unknown because of the limited reporting in Korea. Therefore, this study was conducted to identify the incidence of extravasation and nurses' attitude and knowledge of extravasation for providing high-quality nursing care. Methods: Three acute care hospitals were surveyed to estimate the occurrence of extravasation. Knowledge and attitude toward extravasation were investigated from 793 nurses working in six hospitals. Results: The incidence rate of extravasation was 0.5%. Extravasation commonly occurred in elderly patients aged 66 or older (59.9%) and internal medicine (48.2%), and it happened 13.73±20.68 days after hospitalization on average. It mostly occurred in the forearm site (52.9%) and was mainly caused by parenteral nutrition (33.6%). The mean scores of nurses' knowledge and attitude were 14.63±2.86 and 28.91±36.00, respectively. There was a significant negative correlation between the subjects' knowledge and attitude (r=-.11, p=.002). Conclusion: It is necessary to have a reporting system that can accurately monitor the occurrence of extravasation for patient safety management. In addition, it is necessary to develop a protocol that can be applied to clinical practice and a nurse education program.

  • 심장질환과 뇌일혈은 미국에서의 중요한 사망원이다. 관상동맥질환의 위험인자들 - 가족력, 고혈압, 당뇨병, 지질대사 이상, 흡현 - 은 단지 이 질환의 일부분 만을 설명할 수 있다. 스트레스 또는 작업장이나 대기중에 존재하는 독성물질의 노출과 같은 인자들은, 그것의 위험정도에 대해서는 확실히 알려지지 않지만, 심장질환의 유발인자라고 생각된다. 이 장은 작업장내에 존재하는 여러가지 독성물질로 인한 심장혈관질환에 대하여 서술하였다.

Factors Related to Extravasation of Non-chemotherapy Vesicant Drugs in Peripheral Vein Catheters (말초 정맥주사를 통해 비항암제 발포성 약물을 투약한 성인 환자의 일혈 발생과 위험요인)

  • Purpose : This study aimed to investigate the incidence of intravenous extravasation and the risk factors associated with the use of peripheral intravenous catheters in adults. Method : This prospective observational study included 203 adult patients admitted to the general ward who received non-chemotherapy vesicant drug infusion treatments. Data were analyzed using frequencies, percentage, means, standard deviations, and odds ratios (ORs) from multiple logistic regressions. Results : The incidence of extravasation was 43.3%. Risk factors for intravenous extravasation included continuous injections (OR=5.35, 95% CI [1.38, 20.83]), and parenteral nutrition (OR=3.53, 95% CI [1.43, 8.73]). Conclusion : The present findings revealed that gastrointernal medicine problems, continuous injection, and parenteral nutrition were related to intravenous extravasation. Further research is necessary to reduce the incidence of extravasation related to peripheral intravenous catheterization in adults, and to prevent secondary complications. Finally, patients should be provided appropriate and continuous care based on the type of intravenous infusion.

Clinical Analysis of the Early Result of Coronary Artery bypass Graft (관상동맥 우회로술 154예이 조기 임상 결과)

  • Recently, coronary artery obstructive disease and coronary artery bypass graft surgery have increased, and the operative result has been improved. We reviewed 154 cases of coronary artery bypass graft surgery from Jan. 1985 to Jun. 2004. Material and Method: We reviewed 148 patients, 154 cases of coronary artery bypass surgery from Jan. 1985 to Jun. 2004. This investigation is designed to illustrate the preoperative diagnosis, severity of disease, operative method, the kind of used bypass graft used, number of distal anasomosis, associated surgery, and postoperative morbidity and mortality. Result: There were 84 males, 64 females and the average age was $58.9\pm8.3$ years old. Preoperative clinical diagnosis were unstable angina in 97 cases $(63.0\%)$, stable angina in 31 cases $(20.1\%)$, acute myocardial infarction in 12 cases $(7.8\%)$ and postinfartion angina in 14 cases $(9.1\%)$. Preoperative angiographic diagnosis were three-vessel disease in 68 $(44.2\%)$, two-vessel disease in 39 $(25.3\%)$, one-vessel disease in 35$(22.7\%)$, and left main disease in 12$(7.8\%)$ cases. There were 78 cases of on-pump coronary artery bypass graft surgery and 76 cases of off-pump coronary artery bypass graft surgery. The total distal anastomoses number was 319, mean number of anastomoses was $2.06\pm0.96$. There were 10 concomitant procedures. Postoperative intra-aortic balloon pump was used in 21$(13.6\%)$ cases, but only 4 cases were used at off-pump coronary artery bypass surgery. Total early mortality was $7.8\%$. The mortality was decreased as $4.5\%$ from Jan. 2001 to Jun. 2004. Post operative complication was perioperative yocardial infarction in 9cases$(5.8\%)$, low cardiac output syndrome in 17 cases$(11\%)$, and arrhythmia in 30 cases$(19.5\%)$ cases. Conclusion: Since 1985, The result of coronary artery bypass graft surgery has been improved because of more refined technique, use of off-pump coronary artery bypass surgery, use of internal thoracic artery and radial artery as bypass graft. We should study the long-term follow up more for better operative results.
