Evidence-based Medicine and Systematic Review (근거중심의학과 체계적 고찰)

  • 체계적 고찰은 명확히 정의된 문헌검색에 기반하고, 결정된 기준(criteria)을 이용하여 선택된 문헌의 질을 평가하며, 타당성이 입증된 방법으로 분석을 시행하는 리뷰의 한 형태이다. 체계적 고찰은 (1)답을 얻고자 하는 질문을 공식화 하고 이에 대한 프로토콜을 작성하고, (2)해당 질문에 대한 일차 연구를 찾아서 포함시키고 (3)자료의 질을 평가하고, (4)자료를 추출하여 뽑고, (5)자료를 분석하며, (6)결과를 해석하고 보고서를 작성하는 순서로 이루어진다 하지만 체계적 고찰 혹은 메타분석이 모든 문제에 대한 해결책은 아니다. 메타분석이 수행되었다고 하더라도 질이 나쁜 연구가 포함되거나 질 문제를 무시한 경우, 이질성에 대해 적절한 주의를 기울이지 못할 때, 무차별적인 자료 병합으로 결론이 정확하지 않은 경우, 출판 비뚤림, 시간 지체 비뚤림, 이중 출판 비뚤림, 언어 비뚤림, 결과 보고 비뚤림과 같은 보고 비뚤림이 문제가 되는 경우에는 메타분석 혹은 체계적 고찰이 잘못된 결론으로 도달하도록 할 수 있다. 따라서 올바른 방법론에 기초한 체계적 고찰만이 현존하는 모든 근거를 결합하여 올바른 결론으로 이끌 수 있다.

  • 우리나라의 건강증진사업은 1995년 국민건강증진법을 제정하면서 시작되었다. 동법의 제정과 더불어 보건소법을 지역보건법으로 전면개정하면서 보건소를 지역주민의 평생건강관리 중추기관으로 육성하기 위한 노력을 하였다. 지역보건소는 보건의료전달 체계에서 일차의료를 담당하고 있기 때문에 접근성이 좋고, 국가주도의 사업을 체계적으로 수행할 수 있는 장점이 있다. 지난 1999년도부터 3년간 각 시도별로 1개소씩 건강증진거점보건소를 선정하여 건강증진시범사업을 실시한 후 그 사업성과를 평가하고, 2002년도 하반기부터 전국의 100개 보건소에 150억 규모의 예산을 투입하여 건강증진사업을 추진하도록 기금을 지원하고 있는 것은 건강증진사업을 보건소를 중심으로 활성화하기 위한 실질적인 노력으로 볼 수 있다. The Spatial Characteristics of Residential and Medical Facilities for the Elderly and the Life of Those Using the Facilities of the Capital Region (수도권 노인 주거.의료 서비스 시설과 그 이용자 생활의 공간적 특성)

  • The population of Korean Society is aging rapidly at a pace which is unprecedented in the world. This demographic change results in the various socioeconomic and cultural changes accompanied by spatial changes. The purpose of this study is to examine major characteristics of residential medical facilities for the elderly and the life of those using the facilities of the Capital Region in Korea. Major findings of the study are as follows. The characteristics of the location of residential medical facilities for the elderly and the life of those using the facilities are influenced not only by the mobility of the elderly. but also by the distance between the facilities and Seoul and also by the residential environments of the facilities. The location of the facilities and the mobility of the elderly, followed by the residential location of the family have an effect on everyday life of the elderly. Therefore, the location for residential medical facilities is not recommended to be far from the metropolitan area.

  • The purpose of this study is to promote efficiency of hospital management with customer satisfaction by understanding qualitative system about more systemic hospital service coordinator's job under analyzing the satisfactory degree and coherence of hospital service coordinator's job because of increase of customer need level by medical environment improvement and the variety of offering method of medical service. I allocated a questionnaire per each hospital around dentistry, Oriental medicine clinic, plastic surgery in medical examination department and withdrew questionnaire on total 662 hospitals. According to the study results stated above, generally necessity on employment of a hospital service coordinator appeared high and To the necessity, I considered that a hospital service coordinator's job must strengthen the role for going still more to customer with discriminated mind and service not simple kind service to medical consumer, with establishing more systematic and realistic educational culture in qualification and educational department of a hospital service coordinator. Also a hospital service coordinator must be changed to promote customer satisfaction and efficiency of hospital management by more systematic business assignment and role establishment.

Development of a Community-based Participatory Global Health Project Model for Primary Health Care Capacity Development: A Case Study from a Rural Community in Ecuador (일차보건의료 역량 개발을 위한 지역사회 기반 참여형 국제보건사업 모델 개발: 에콰도르 일개 지역을 중심으로 한 사례연구)

  • Purpose: The aims of this study were to identify successful strategies and propose a community-based participatory global health project model for primary health care capacity development. Methods: The study used case study methodology. A The unit of analysis was an international cooperation health project entitled "Community-based Primary Health Care Improvement in San Lorenzo, Ecuador" using community-based participatory research conducted in 2007~2008. Data were collected through windshield surveys, focus group discussion, key informant interviews, and provider surveys. Results: Identified successful strategies for the international cooperation health project were reciprocal partnership between researchers and community, partners' capacity building, south-to-south cooperation, and continuous monitoring and feedback. Community participation was found to be an essential tenet to guarantee the improvement of primary health care in the underserved rural community. Evidence from the activities of community health practitioners in Korea was applicable to the development of training programs for primary health care providers in Ecuador. Conclusion: Strategies for strengthening primary health capacity may be tailored depending on socio-cultural, political, and economical situations of each country. The model, however, would be applicable to the entire process of community-based global health projects in underserved rural communities of other countries.

  • The purpose of this study was to examine how Customer Value perceived by customers affected Customer Loyalty through Customer Satisfaction in Primary Care and to reveal the structural relationship of selected variables related to CRM. The major findings of the study were as follows: First, Convenience Value, Response Value and Reliability Value exerted an influence on Customer Satisfaction. Second, Customer Satisfaction affected Customer Loyalty that referred to Repurchase and Word-of-Mouth Intention. Third, Convenience Value, Response Value and Reliability Value had an effect on Repurchase through the perfect mediating role of Customer Satisfaction, and Convenience Value and Response Value exercised an influence on Word-of-Mouth Intention through the perfect mediating role of Customer Satisfaction. Reliability Value served to stir up Word-of- Mouth Intention even if there was no Customer Satisfaction.

Job Analysis of Nurse Care Coordinators for Chronic Illness Management in Primary Care Settings: Using Developing a Curriculum Process (데이컴 기법을 적용한 일차의료 만성질환관리 간호사 케어코디네이터 직무분석)

  • Purpose: This study aimed to conduct a job analysis of nurse carecoordinators and to identify the frequency, importance and difficulty of each task of their job. Methods: A committee for developing a curriculum (DACUM) was formed and members of the committee defined nurse care coordinators' jobs and enumerated the duties, tasks and task elements by applying the DACUM technique. Then nurse care coordinators enrolled in the pilot project evaluated the frequency, importance and difficulty of each task. Results: From the job descriptions of nurse care coordinators, we identified 12 duties and 42 tasks. Each task comprised 1~5 task elements. Among tasks, 'assess the patient's general health status' was carried out most frequently. Nurse care coordinators perceived that 'check vital signs' and 'strengthen patient competence to promote health behaviors' were more important than all other tasks. The most difficult task was 'develop professionalism as a nurse care coordinator'. Conclusion: The nurse care coordinators' roles developed in this study will serve as the key guidelines for human resource management of care coordinators. Further, job specifications for nurse care coordinators need to be developed, which is necessary for designing education and training programs. We also need to integrate primary health care as an essential component in nursing education.

Patients' Perception of Quality of Health Service and Utilization considering convergence factors: A Survey of Elderly Patients of Public Health Centers, Seoul, Republic of Korea (융합적 요소를 고려한 의료서비스 질 인식도와 의료이용의 관련성: 서울시 보건소를 이용하는 노인을 대상으로)

  • Health service quality is increasingly emphasized, and primary care service is no exception in this. This study identified patients' perception of quality considering convergence factors associated with increased utilization among patients over the age of 65 using community health centers. A total of 307 respondents were included in the analysis. The results showed that positive perceptions on facility and environment, patient-centered care, technical knowledge and skills of physicians, and emotional support with nurses and non-medical workers were statistically and significantly associated with higher utilization. In order to increase utilization of primary care service at community health centers, they need to manage and improve these factors.
