Effects of Gymnasts Shoes on Risk Factors of Anterior Cruciate Ligament Injuries during Drop Landing in Female Gymnasts (여자 체조선수들의 드롭 랜딩 시 체조화 착용유무가 전방십자인대 부상 위험요인에 미치는 영향)

  • The purpose of the study was to investigate the effects of gymnasts shoes on risk factors of anterior cruciate ligament injuries during drop-landing followed by vertical jump in female gymnasts. Thirteen female gymnasts were recruited and performed randomly drop-landing followed by vertical jump in height of her knee with and without shoes. Kinematics and ground reaction data were collected to estimate the anterior cruciate ligament injuries risk factors. Data were analyzed with paired samples t-test with Bonferroni correction. Female gymnasts with shoes showed more reduced thigh maximum adduction angle, and knee maximum extension moment than without shoes. Female gymnasts with shoes showed more increased shank maximum abduction angle than without shoes. In conclusion, Female gymnasts with shoes reduced anterior cruciate ligament injuries risk factors.

A Biomechanical Analysis According to Passage of Rehabilitation Training Program of ACL Patients (전방십자인대 수술자의 재활트레이닝 경과에 따른 운동역학적 분석)

  • The purpose of this study was to analyse scientific according to period of rehabilitation training of ACL patients. ACL patients seven subjects participated in this study. Gait (1.58 m/sec) analysis was performed by using a 3-D Cinematography, a Zebris system and a electromyograph system. The data were analyzed by paired t-test. The joint angles were recorded from the ankle, knee, hip joints. Peak max dorsi-flexion and peak max plantar-flexion identified significant differences (p<0.05). Another angles were no significant difference. Vertical force (Fz) and max pressure variables improved 6 month RTP better than 3 month RTP. EMG were collected from 4 muscles (rectus femoris, biceps femoris, gastrocnemius, tibialis anterior) with surface electrides in gait system. EMG signals were rectified and smoothed data. EMG signas were no significant difference but they also improved 6 month RTP better than 3 month RTP. More research is necessary to determine exactly what constitutes optimal rehabilitation training period for ACL patients.

The Effects of Closed Kinetic Chain Exercises on Thigh Circumference and Lysholm Scale of the Knee Joints of Patients with ACL Reconstruction (전방십자인대 재건술 후 닫힌사슬운동이 슬관절 대퇴둘레, Lysholm 척도에 미치는 영향)

  • Purpose: This study compared the thigh circumference and Lysholm scale of a stable and unstable exercise group of patients who had undergone an anterior cruciate ligament reconstruction (ACL reconstruction). Methods: The subjects were patients more than 4 weeks after their ACL reconstruction and could stand on one leg. The patients were divided into a control group with 9 patients performing closed kinetic chain exercises on a stable floor and an experimental group with 10 patients performing closed kinetic chain exercises en an unstable floor. The degree of muscle atrophy was compared by measuring the circumference of the injured thigh before the exercise program, and 3 weeks and 6 weeks after the exercise program. The Lysholm scale was used to assess the function of the knee joint. Results: There was no significant increase in thigh circumference according to the exercise periods in the two groups. However, there was a statistically significant increase before exercises and 6 weeks after the exercises (p<0.05). There were statistically significant differences in the Lysholm scores between the two groups (p<0.05). Conclusion: There were no statistically significant increase in the two groups, but there was a significant difference between before the exercise program and 6 weeks after the exercise program (p<0.05).

Clinical Analysis of Postoperative Prognostic Factors of Cervical Anterior Decompression and Interbody Fusion for Ossification of Posterior Longitudinal Ligament (경추 후종인대 골화증의 전방경유 감압술 및 골 융합술후 예후인자에 대한 임상분석)

  • Objective : To investigate the prognostic factors associated with outcome in patients with ossification of posterior longitudinal ligament. Method : During the past 4 years, we have operated on 35 patients with cervical OPLL. Anterior cervical decompression(total or subtotal corpectomy, discectomy, and removal of the OPLL) and interbody fusion with iliac bone were performed in all patients. Results : Eight cases(22.9%) were continuous type, 11(31.4%) segmental, 13(37.1%) Mixed, and 3(8.6%) localized type. Thirty-two patients(91.4%) showed an excellent or good results. Conclusion : These results indicate that surgical treatment should be considerated in case of clinical grading higher than II and the surgical outcome is worse when duration of preoperative symptom is longer and when percentage of spinal narrowing is higher. Anterior cervical decompression and interbody fusion seems to be a better method in patients with lesions limited to one or two level. Age at surgery did not significantly affect the outcom.

  • Optimal treatment of the torn anterior cruciate ligament (ACL) remains controversial. The complexity of surgically reproducing the natural biomechanical and anatomical function of the ACL has led to a diversity of reconstructive procedures. Controversy continues to exist regarding the best reconstructive procedure for the ACL deficient knee, but currently, there is no ideal method. Because of the increased frequency of ACL injury and the functional impairment resulting from that, the role of mechanoreceptors in the ACL recently has attracted considerable attention. Proper reconstruction of the ruptured ACL does not always have good results. Success after operation may depend not only on the mechanical stability but also on the quality of recovery of proprioception. It is well known that most ACL are ruptured in proximal half and most mechanoreceptors have been reported to be located in the subsynovial layer and near the tibial insertion of the ACL. Expected roles of tibial remnant is to enhance the revascularization and cellular proliferation of the graft, to preserve proprioceptive function, and to be able to acquire anatomical placement of the graft without roof impingement. The remnant of the ruptured ACL has been removed to clearly visualize the ACL footprint or decrease the risk of impingement and Cyclops lesion in most current techniques for ACL reconstruction. Therefore it seems reasonable to assume that preserving the tibial remnant as much as possible as a source of reinnervation, if technically possible without causing impingement, would be of potential benefit to the patient. In addition, it will facilitate the vascular ingrowth and ligamentization of the grafted ACL.

Before and After Cruciate Ligament Reconstruction Comparison of Isokinetic Muscle Strength for Knee Extensor Muscles (십자인대재건술 전과 후의 무릎신전근육에 대한 등속성 근력 비교)

  • Background: The purpose of this study is to compare and analysis the changes in strength and endurance of the knee extensor muscles after Anterior cruciate ligament reconstruction. Methods: Twelve male subjects with ligament injury were seated on Biodex and the hip joint was flexed at 110°, and torso, lower extremities, and legs were fixed using Velcro. The resistance device was placed at a point 3cm above the ankle, and measurements were taken before and a surgery at 60°/sec and 180°/sec when the knee was maximally extended. The postures ingeniously combine forward-bending poses countered with backward-bending ones. Results: There was no significant difference in the comparison of muscle strength of 60°/sec and 180°/sec before surgery. But there was a significant difference after surgery. There was significant difference in muscle endurance both before and after surgery. There was a significant difference in the pre- and post- operative comparison values of muscle strength and endurance in isokinetic movements at 60°/sec and 180°/sec during knee extension on the injured side. Conclusion: For cruciate ligament tears, Orthopedic reconstruction is recommended. Correct alignment of the knee ligament after surgery can enhance the strength and endurance of the quadriceps femoris during knee extension rejuvenated.

  • Recurrent motor branch entrapment syndrome is a compressive mononeuropathy of recurrent motor branch of median nerve. It is a rare condition as a cause of thenar muscle wasting and may have different pathogenesis. If such an anatomical variation is the cause, there is a possibility that thenar muscle atrophy remains if only the transcarpal ligament release is performed. We report a 25-year-old male patient with carpal tunnel syndrome with thenar muscle wasting 1 month ago.

Indications of Lateral Ankle Ligament Reconstruction with a Free Tendon and Associated Evidence (유리건을 이용한 족관절 외측 인대 재건술의 적응증과 근거)

  • Ankle sprain is one of the most common musculoskeletal injuries. Although most ankle sprains respond well to conservative measures, chronic instability following an acute sprain has been reported to occur in 20% to 40% of patients. Some individuals are eventually indicated for a lateral ankle ligament reconstruction due to persistent ankle instability. More than 80 surgical procedures have been described to address lateral ankle stability. These range from direct repair of the anterior talofibular ligament (ATFL) and of the calcaneofibular ligament (CFL) to reconstructions based on the use of autograft or allograft tissues. However, the best surgical option remains debatable. The modified $Brostr{\ddot{o}}m$ procedure is most widely used for direct ligament repair, but not always possible because of the poor ATFL or CFL quality or deficiency of these ligaments, which prevents effective shortening imbrication. Furthermore, the importance of a CFL reconstruction has been emphasized recently. On the other hand, it is difficult to achieve an efficient CFL reconstruction during the $Brostr{\ddot{o}}m$ procedure. Others have reported that an anatomic reconstruction of injured ligaments restores the normal resistance to anterior translation and inversion without restricting subtalar or ankle motion, and as a result, anatomic reconstructions for lateral ankle instability utilizing an autograft or allograft tendon have gained popularity.

  • Autotransplantation is the surgical repositioning of an autogenous erupted or unerupted tooth from one site to another in the same individual. This treatment is indicated in traumatic tooth loss, teeth with severe caries, congenitally missing teeth, teeth with bad prognosis and in case of developmental anomalies of teeth. The following 2 cases describe the potential to utilize autotransplantation to replace hopeless teeth with sound wisdom teeth.

Forced orthodontic eruption for augmentation of soft tissue prior to implant placement (임플란트 식립 전 연조직 증대를 위한 교정적 정출술)

  • Forced orthodontic eruption(FOE) is a non-surgical treatment approach that allows augmenting both soft- and hard-tissue profiles of potential implant sites, by forced orthodontic extrusion of "hopeless" teeth and their periodontal apparatus. By stretching the gingival and periodontal ligament fibers during extrusion, tension is imparted to the entire alveolar socket, stimulating osseous apposition at the alveolar crest. FOE increases the width of the attached gingiva, and the mucogingival junction remains stable when the gingival margin migrates coronally. Based on these effects, FOE of non-restorable teeth prior to implant placement is a viable alternative to conventional surgical augmentative procedures in implant site development. The aim of this case report is to describes coronal soft-tissue augmentation around fractured teeth, which was achieved by FOE before implant placement.
