Compartment Syndrome and Deep Vein Thrombosis after Repetitive Posterior Cruciate Ligament Reconstruction (반복된 후방 십자 인대 재건술 후 발생한 구획 증후군 및 심부 정맥 혈전증)

  • Posterior cruciate ligament reconstructions are dangerous procedure in terms of neurovascular complication. But deep vein thrombosis and associated compartment syndrome after posterior cruciate ligament reconstruction has not been reported, yet. We have experienced a patient who developed a deep vein thrombosis and compartment syndrome after revision posterior cruciate ligament reconstructive surgery, and report the importance of diagnosis and management with the discussions.

  • Few cases have been reported in which the femoral interference screw has migrated into the posterior compartment after an ACL reconstruction. It usually requires removal, because it leads usually to mechanical symptom. However, the arthroscopic removal of a screw is a technically demanding procedure, especially in the case of an intact integrated ACL graft or one that is encapsulated around the screw. We present a case in which a displaced femoral interference screw migrated within the posterior compartment 11 years postoperatively, after the graft had been successfully incorporated at the femoral site and showed good continuity on MRI and arthroscopic examination. Although it is often technically challenging, through the use of a posterior trans-septal portal, we can successfully remove a displaced femoral interference screw even in the most difficult locations in the posterior compartment without damage to ACL graft.

Mid-term Results of Arthroscopic Anterior Cruciate Ligament Reconstruction using Autologous Bone-Patellar tendon-Bone versus Hamstring tendon (자가 골-슬개건-골과 슬괵건을 이용한 관절경적 전방 십자 인대 재건술의 중기 추시 결과)

  • 목적: 자가 골-슬개건-골과 슬괵건을 이용한 전방 십자 인대 재건술의 5년 중기 추시 결과를 비교 분석하고자 한다. 대상 및 방법: 전방 십자 인대 재건술을 시행한 후 5년 이상 추시 관찰이 가능한 65예를 대상으로 하였고, 골-슬개건-골이 38예, 슬괵건이 27예였다. 술 후 평가는 Lysholm 점수, 관절 운동 범위, 대퇴부 중간 부위 둘레 길이, Lachman 검사, 축 이동 검사, KT 2000 관절 계측기를 시행하였고, 술 후 합병증을 평가하였다. 결과: Lysholm 점수는 골-슬개건-골군과 슬괵건국에서 각각 평균 91점, 94.2점으로 슬괵건군이 우수하였고, 대퇴부 중간 부위 둘레 길이는 건측에 비해 각각 평균 1.7cm, 1.3cm의 차이를 보였다. 양 군간에 관절 운동 범위, Lachman 검사, 축 이동 검사 및 KT 2000 관절 계측 결과는 통계적으로 유의한 차이가 없었으나, 술 후 합병증은 과도한 보행이나 운동 후 슬관절 동통이 골-슬개건-골군에서 7예, 슬괵건 군에서 4예 관찰되었고, 전방 슬관절 동통이 골-슬개건-골군에서 4예 관찰되어, 골-슬개건-골군에서 더 많이 발생하였다. 결론: 5년 중기 추시에서 자가 슬괵건군이 우수한 슬관절 기능, 적은 합병증 등 임상적으로 우수하였으나 장기 추시가 요할 것으로 사료된다.

Pretibial Cyst after Anterior Cruciate Ligament Reconstruction Using bio-absorbable interference screw fixation - A Case Report - (생체분해성간섭나사를 이용한 전방십자인대 재건술 후 발생한 경골 근위부 낭종-증례 보고-)

  • Pretibial cyst formation is a rare complication after anterior cruciate ligament (ACL) reconstruction and there are only few cases reported. Pretibial cyst can be caused by variable reasons. Foreign body reaction due to breakdown of bio-absorbable screw for fixation, graft necrosis at tibial site, joint fluid leakage to tunnel and incomplete incorporation of graft to bony tunnel. The authors experienced one case of massive pretibial cyst after arthroscopic ACL reconstruction using bio-absorbable interference screw in 38-year-old male patient. Thus, authors report this rare case with literature view.

  • An inappropriate insertion of a interference device to fix the graft in the tibial tuunel can cause potential complications. We describe a case of 27-year-old man who underwent anterior cruciate ligament reconstruction using IntraFix device(DePuy Mitek, Raynham, MA). In this report we present a case of migration of IntraFix sheath by inappropriate insertion of IntraFix screw along with a review of the current literature.

  • Pretibial ganglion cyst after arthroscopic anterior cruciate ligament reconstruction is a very rare complication, and a few are reported in and outside the country. Some authors report that it may break out with a variety of causes at any time. We experienced a case of pretibial ganglion cyst after arthroscopic anterior cruciate ligament reconstruction using Achilles tendon allograft and bioabsorbable interference screw in 34-year-old male. So we report this case including the cause and the treatment with a review of the literature.

A Comparison of Outcomes after Early and Delayed Reconstruction in the Acute Anterior Cruciate Ligament Injuries (급성 전방십자인대 손상 환자에서 조기 재건군과 지연 재건군의 결과 비교)

  • Purpose: This study performed to compare degree of joint stiffness and clinical results between early and delayed reconstruction of acute anterior cruciate ligament (ACL) injuries. Materials and Methods: Thirty-four Patients who underwent ACL reconstruction between March 2008 and October 2010 enrolled this study. We divided the patient into 2 groups, early reconstruction group underwent surgery before a week, delayed reconstruction group underwent surgery after 3 weeks, before 6 weeks. All the patients underwent aggressive joint motion exercise till surgery and enrolled post operative rehabilitation program including self exercise. We checked range of motion, the Lachman test, the pivot shift test, the Lysholm score, the International Knee Documentation Committee (IKDC) score and the Tegner score to evaluate the results. Results: At the final follow up. The Lysholm score was 91.82 in the early group and 94.83 in the delayed group. All the cases were rated above B (near normal) on IKDC score (P=0.217, P=0.845). The Tegner score was 6.7 in the early reconstruction group and 7.1 in the delayed group (P=0.840), there was no difference between the groups for the range of motion (P=0.873, P=0.873), no complication such as deep vein thrombosis or infection, no difference in the Lachman test, pivot shift test (P=0.606, P=0.118). Conclusion: We could obtain satisfactory clinical results in both the early and delayed reconstruction groups of acute ACL injuries. Therefore, the early reconstruction of ACL performed before a week could be one of the treatment options for acute ACL injury.

  • Puropse: To evaluate the outcome of arthroscopic posterior cruciate ligament (PCL) reconstruction using fresh-frozen achilles allograft tendon with preservation of ligament remnant or elongated ligament. Materials and Methods: From October 2004 to March 2010, we performed PCL reconstruction with Achilles tendon allografts in 22 complete rupture patients. Mean age was 31.5 years. 14 cases were male and 8 cases were female. Average follow-up period was 3 years and 7 months, range from 1 year to 6 years and 4 months. Subjective and objective parameters were utilized in analyses, such as the mean range of motion, post. drawer test, Lysholm knee score, Tegner activity score, IKDC score, and second look arthroscopic examination. Results: Postoperative Lysholm knee score, IKDC scores, Tegner activity scale, and posterior displacement by the Telos stress test demonstrated statistically significant improvement compared to the preoperative state. Conclusion: Arthroscopic PCL reconstruction using fresh-frozen achilles allograft tendon with preservation of ligament remnant showed good clinical results and posterior stability.

Arthroscopic Posterior Cruciate Ligament Reconstruction without Tourniquet (지혈대 없이 시행한 관절경하 후방십자인대 재건술)

  • Purpose: To report the results of arthroscope assisted posterior cruciate ligament reconstruction without tourniquet, which has not yet been reported in documents within the country. Materials and Methods: Out of the 75 cases of arthroscope assisted posterior cruciate ligament reconstruction on patients with posterior cruciate ligament damage from January, 1998 to May,2003 in this hospital, study was done on 49 cases of patients with isolated posterior cruciate ligament damage, excluding 16 cases of patients who were applied tourniquet in extraarticular operation due to combined damage such as in lateral collateral ligament and posterolateral collateral ligament, and 10 cases where the postoperative observation period was less than 12 months. There were 13 cases where reconstruction was done using autogenous bone-patella tendon-bone, 19 cases using allogenous bone-patella tendon-bone, and 17 cases using allogenous achilles tendon. For assessment, the 49 cases were compared using Lysholm knee scoring scale. Results: The average duration of operation was 105 minutes. There were no severe complications such as swelling, bleeding, infection etc . Assessment results attained by Lysholm knee scoring scale was 74(fair) for cases where autogenous bone-patella ten-don-bone was used,75(fair) for cases where allogenous achilles tendon was used, and 76(fair) for cases where allogenous bone-patella tendon-bone was used. Conclusion: There are no difficulties to perform the arthroscopic assisted posterior cruciate ligament reconstruction. Reconstruction without tourniquet can be thought to prevent complications that could follow when using it.

Gait Analysis After Posterior Cruciate Ligament Reconstruction Using Fresh-Frozen Achilles Allograft (신선 동결 아킬레스 동종건을 이용한 후방십자인대 재건술 후 보행분석)

  • Purpose: The purpose of this study was to analyze the clinical result and the gait patterns for patients who had undergone the arthroscopic posterior cruciate ligament (PCL) reconstruction using Achilles allograft. Materials and Methods: Among the patient who had undergone the arthroscopic PCL reconstruction using fresh-frozen Achilles allograft between March 2004 to March 2005, we selected 12 patients who compliance to our rehabilitation program. Clinical result and gait analysis were carried out at 36 months postoperatively. There were measured by using range of motion (ROM), Lysholm knee score, Tegner activity score, IKDC score, posterior stress test, and posterior translation measured by using Telos stress arthrometer. Kinematic gait analysis was carried out using 3-dimensional gait analysis system. Results: The PCL reconstruction surgery yielded statistacally significant results in all of clinical evaluation. In gait analysis, the average knee flexion angle increased during stance phase and decreased during the swing phase, but was not statistically significant and there were no significant difference between both sides of their knees overall. Conclusion: Since there was no statistically significant difference between the injured and uninjured sides as a result of the gait evaluation of the patients who had arthroscopic reconstruction surgery on their PCL injured knee and conformed to the rehabilitation program, systemic and strict rehabilitation ought to be important in the reconstruction surgery.
