Psychosocial Factors Predicting Delayed Diagnosis of Breast Cancer : The Role of Marital Relationship Functioning (지연된 유방암 진단을 예측하는 정신사회적 요인 : 부부관계기능의 역할)

  • Objectives : Breast cancer has been the most prevalent female cancer in South Korea since 2001. Early detection of this disease is the most effective strategy for reducing mortality. The objective of this study was to identify factors which could predict advanced stage at diagnosis of breast cancer. Methods : Participants who were initially diagnosed with breast cancer and referred to the Stress Clinic of the Breast Cancer Center at Kyungpook National University Hospital were included. Through a semi-structured interview, the authors investigated psychosocial variables such as the extent of marital and family functioning and emotional-economic family burden as well as sociodemographic and health behavior-, health characteristic- and cancer-related variables. Results : Data were collected from 219 participants. One hundred and twenty(54.8%) subjects were diagnosed with advanced-stage breast cancer. Variables that were significantly different between the advanced-stage and early-stage groups included : monthly breast self examination(p<0.000), annual mammographic screening(p<0.000), mode of tumor detection(p<0.000), nature of the first symptoms(p<0.000), time to treatment after diagnosis(p<0.000), overloaded economic and family burden(p=0.018), marital functioning(p<0.000) and family functioning(p<0.00). Logistic regression analysis indicated that irregular annual mammography screening(OR=7.431 ; 95% CI 2.407-22.944) or a lack of screening(OR=25.299 ; 95% CI 7.855-81.482) and a dysfunctional marital relationship(OR=4.772 ; 95% CI 2.244-10.145) were significantly associated with advanced stage at diagnosis of breast cancer. Conclusions : We reconfirmed screening behavior to be a risk factor for delayed diagnosis of breast cancer. Our findings also emphasized the importance of psychosocial factors such as marital functioning in early detection of breast cancer. Psychiatric consultation in the area of martial functioning could be beneficial for increasing early detection in breast cancer.

Effects of Work Participation on the Self-esteem of the Elderly (일자리 참여가 노인의 자아존중감에 미치는 영향에 관한 연구)

  • The purpose of this study is to find out how work participation of the elderly and their general characteristics - socio-demographic, economic, and health traits - influence on their self-esteem. The data was collected through one on one interviews of 295 elderly. Data on general attributes and the discrepancies in the level of self-esteem between work participants and non-participants were analyzed by using t-test and ANOVA, while the effects of work participation on their self-esteem were analyzed by multiple regression analysis. Results of this study shows us that work participation is the strongest predictor that determine the level of self-esteem for the elderly. In other words, work participation can make the aged better off and give them a sense of security and satisfaction. Finally, these findings may be used for realizing us the importance of the work participation program and developing and designing new promising approaches of work participation program, which meet psychological needs of the elderly as well as their economic needs.

A Multi-level Study on Volunteering and Giving - Local Public Social Expenditure and Individual Socio-demographic Characteristics - (자원봉사와 기부에 관한 다층적 영향요인 연구 - 지역 공공복지 지출규모와 개인특성 요인을 중심으로-)

  • We examined how public social expenditure and individual socio-demographic factors affect individual voluntary activities(volunteering, giving). Hierarchical linear model(HLM) was employed to a nested data set with 37,648 individual subjects and 16 local governments in Korea. HLM analyses yield an insignificant direct effect of public expenditure to volunteering and giving, while individual factors all have significant effects on them. Finally, this study discussed why public social expenditure factor does not have significant influence in this data, and suggested policy implications for promoting volunteering and giving.

A Study on Parenting Competency of Mothers with Mental Illness (여성정신장애인의 양육능력에 관한 연구)

  • For women living with mental illness, motherhood may be a normalizing life experiences and offers the opportunity to develop competencies in a major life roles. So, the main premise of this study that social contexts, social support of spouse and extended family, is very important for successful parenting of mothers with mental illness. Thus this study analyzes the impact of marital relationship and social support of networks on three dimensions of parenting competency(expression of love, encouragement of independence, rational discipline) when the socioeconomic(age, number of offsprings, subjective economic level) and disability variables(diagnosis, severity of symptoms, chronicity, frequencies of psychiatric admission) are controlled. This study conducted the survey with 145 mothers with schizophrenia and mood disorder. The major finding of the study are : i) Using step-wise multiple regression, marital relationship, social supports and economic level affect significantly on the expression of love. ii) Using step-wise multiple regression, the most powerful influenced factor on encouragement of independence is marital relationship. iii) Using step-wise multiple regression, diagnosis, severity of symptoms, chronicity and economic level affect significantly on the rational discipline. That is two dimensions of parenting competency, expression of love and encouragement of independence, can be explained well by social contexts variables(marital relationship, social support, economic level). But only rational discipline can be explain well by disability factors. In conclusion, this study recommends service providers to use family based comprehensive case management for the mothers with mental illness.

The Determinants of Economic Status and the Role of Intergenerational Support among Elderly Koreans (한국노인의 경제적 지위 결정요인과 세대간 지원의 역할)

  • This study explores the determinants of economic status and well-being among elderly Koreans. Particular emphasis is paid to the role of intergenerational support in these processes because institutional responses to an aging population in Korea are currently inadequate. Path model results indicate that status maintenance variables have significant effects on economic status and well-being as measured by comprehensive income. Further, inequality in comprehensive income - as measured by the Gini and Theil's indices - is substantial and increases in successive age groups. However, market-based and family-based income, two components of comprehensive income, show different patterns. Inequality in market-based income is apparently increasing, while inequality in family-based income is decreasing gradually in successive age groups. With respect to the role of intergenerational support, results indicate that family-income-focused persons possess highly vulnerable sociodemographic characteristics, and that for the socially disadvantaged elderly Koreans, intergenerational support plays a crucial role in the later stages of life. This study also discusses the implications of these findings for the transition from latency to manifestation of intergenerational support in Korea.

Economic Benefit Analysis of Urban Meteorological Information Service Using Contingent Valuation Method (조건부가치측정법을 이용한 도시기상정보서비스의 경제적 편익 분석)

  • As the concentration of people in urban area become severe and abnormal meteorological disasters such as regional torrential rains, heavy snows, sweltering heat, and so on have been increasing, the interest on the urban meteorological information service, which considers the specific characteristics of metropolitan areas in weather forecasting, are also increasing. The urban meteorological information service is one of up-to-date technologies which observes urban weather in a more microscale perspective compared to the present weather forecasting system and provides useful meteorological information which is specialized for metropolises in real time. Therefore, urban meteorological information service is expected to contribute to the increase in quality of life for citizens and to the development of industry in urban areas. In this study, we estimate the economic benefit of the urban meteorological information service using contingent valuation method with survey data of the citizens who are expected to be the direct customers of this new information service. As a result, we conclude that the household is willing to pay 5,963 Korean won per year on average, during a period of five years, for receiving this meteorological information service, and this willingness-to-pay is varied by the socio-economic characteristics of head of the household.

A Meta-regression Analysis of Wetland Valuation Studies in Korea (우리나라 습지의 환경적 가치 : 메타회귀분석)

  • Wetland provides various environmental services as well as marketable goods to human society. This paper presents a meta-analysis of the wetland valuation studies conducted in Korea during the past decade. Meta-analysis is a technique of summarizing research results from the separate but related studies using statistical methods. The objectives of the paper is to identify the main factors affecting the wetland values and to examine the applicability of meta-regression analysis for summarizing the information from valuation studies of natural ecosystem. The total of 64 observations from 24 studies are assembled, and we observe significant differences in wetland values depending on the characteristics of the studies. With the statistical significance, our results indicate that the method employed affects the wetland values while the wetland function evaluated affects little. In addition, we found that the size of wetland is negatively associated with the values, implying that wetland values exhibit the decreasing returns to scale. The evidences from our empirical analysis suggest that the meta-analysis has a potential as a tool to synthesize the research results from valuation studies and to provide useful information to policy makers. However, this study acknowledges that more effort should be dedicated toward the construction of DB on valuation studies as well as the research on the model improvement to enhance the validity and reliability of meta-analysis.

The Convergence relation of tobacco smoking to tooth loss in older adults (65세 이상 한국 노인들의 결손치와 흡연의 융합관계)

  • This study aims to investigate the factors affecting tooth loss and the association between smoking and tooth loss, by using the forth Korea National Health and National Examination Survey(2007-2009) of 1,565 elderly people over 65 years old. In terms of the characteristics, as age increases, tooth loss increases. In particular, according to the results of the analysis that identify the correlation between smoking to gender, socio-economic status and dental visiting patterns after correcting socio-economic factors and behavior, it is clear that male's tooth loss is higher than female's in current smoking and tooth loss is caused highly in the lower socio-economic status. In the dental visiting patterns, tooth loss is high when visiting more than twice a year. Therefore, it seems to need a preventive non-smoking policy at the level of dental services through further studies.

Effects of the Personality Traits of Baby Boomers on the Preparation Behaviors for the Old Age -Focused on the Cheon-An Industrial Complex's Workers- (베이비부머의 성격특성이 노후준비행동에 미치는 영향 -천안지역 공단 근로자를 중심으로-)

  • The purpose of this study is to find out the differences in personality traits and preparation level of the baby boomers for the old age by the demographic factors and to analyze the effect of Big 5 on preparation for the old age. The 331 questionnaires were distributed among workers born in 1955 to 1963 in the Chunan industrial complex. And the frequency analysis, factor analysis, reliability analysis, t-test, ANOVA, hierarchical regression analysis for the collected data were conducted with program SPSS 18.0 The findings are as follows. First, there is a partial difference in personality and preparation for the old age by the demographic factors. Second, extraversion, openness to experience and conscientiousness from personality have a positive effect on physical preparation for the old age while neuroticism has a negative effect on it. And there is no relationship between agreeableness and personality. Third, conscientiousness, openness to experience and extraversion have a positive effect on social preparation for the old age, but neuroticism and agreeableness do no affect it. Fourth, neuroticism,extraversion,openness to experience and conscientiousness have a positive effect on economic preparation for the old age, but agreeableness does not have any relationship with economic preparation.
