Treatment of Prolonged Air-leak after Bilobectomy by Artificial Pneumoperitoneum (폐엽절제 후 발생한 지속적 공기누출의 인공기복에 의한 치료)

  • Pulmonary resection often leaves a pleural space problem that can cause prolonged air leaks. We describe a patient with lung cancer undergoing a right middle and lower lobectomy complicated by prolonged air leaks. He had a history of pulmonary tuberculosis and COPD. Artificial pneumoperitoneum was made by instillation of air via a small central vein catheter into the left lower quadrant on postoperative day(POD) 21. The air leak ceased on POD 23 and the chest tube was removed on POD 25.

  • Fungal infection of intracardiac pacing wire is very rare. We experienced a case of patient with functioning transvenous pacemaker lead, inserted 3 years previously, which was completely encased in a large thrombus infected with aspergillus. The lung biopsy also confirmed aspergillus infection.

  • We successfully treated a 59-year-old male with adenocarcinoma in the right lower lobe who had developed a dead space with prolonged air-leak, which continued for 21 days after lower and middle lobectomy, by creating artificial pneumoperitoneum. He had a history of subtotal gastrectomy due to stomach cancer 5 years ago. Artificial pneumoperitoneum was made after introducing a peritoneal dialysis catheter into the right upper quardrant. The chest tube was removed 14 days after creating pneumoperitoneum.

  • 미병의 예방과 관리의 중요성이 거론되고 있으나, 미병에 대한 분류나 진단을 위한 확고한 근거가 미약한 상황으로서 미병 진단 인자 분류를 위한 생리시스템 모델 개발이 필요한 시점이다. 본 연구의 목적은 개발한 생리학적 모델이 미병 단계를 구별하는데 효과 및 유용성이 있는지를 임상 검증하기 위하여 생리학적 모델 인공지능 시뮬레이션을 개발하고자 함이다. 인공지능 계산은 3층으로 구성된 네트워크를 이용하였으며 각 층은 30개의 neuron들로 구성하였다. 인공지능망의 입력 값은 나이, 수축기 혈압, 이완기 혈압, 심박수 값 (입력 값 4개)이고 출력 값은 혈관 저항값인 Ra이다. 머신러닝 차수를 높이면서 인공지능을 사용하지 않은 생리적 모델로부터 도출된 결과와 인공지능을 통하여 계산된 결과를 비교하였다. 개발된 인공지능계산을 이용한 생리시스템 모델은 대량의 표본집단에서 임상 검증에 기여할 것이다.

Angiographic Results of Radial Artery Grafts that are Used for Myocardial Revascularization (관상동맥 우회술 후 혈관조영술을 이용한 요골동맥의 개통률 분석)

  • Background: The radial artery is gaining widespread acceptance as complementary arterial conduits for surgical myocardial revascularization, but there have been limited reports about its angiographic patency compared with that of internal thoracic artery or saphenous vein. We tried to evaluate angiographic patency of radial artery graft and to compare that of radial artery and other gratis with retrospective manner. Material and Method: From January 2001 to Jure 2006, totally 132 patients (male 92, female 40) who underwent coronary artery bypass graft using radial artery were re-admitted to our hospital for follow up angiographic examination. Mean age was 58.2+8.87 and mean follow up duration was 32 month ($2{\sim}110$ month). Off pump and on pump bypass surgery were performed 74 and 58 patients respectively. Along with radial artery, left internal thoracic arteries were used in 57 cases, concomitant left internal thoracic artery and saphenous veins were used in 47 cases and bilateral internal thoracic arteries were used in 20 cases. Result: Totally 412 distal anastomosis were performed and 376 anastomosis remained patent (91.2%). Left internal thoracic artery showed the most excellent patency in all of the conduits (98.5%). Radial artery graft patency was 90.8% (169/186). There was no statistical difference of the patency by conduit between on-pump and off-pump group. But radial artery showed more higher patency rate (98/110, 89%) in the severe stenotic lesion that preoperatively revealed more than 90% stenosis than in the lesser severe (<90%) stenotic lesion (60/76, 78%)(p < 0.005). Radial artery conduit represented the worst result when it was grafted in the right coronary system. But when it was positioned in the left heart especially diagonal or obtuse marginal area, patency was comparable with left internal thoracic artery. Conclusion: Radial artery graft showed good midterm patency when it was used in the severe stenotic lesion more than 90% and left coronary system. But great notice should be taken when it is grafted in the right coronary system or less severe stenotic lesion.

  • Recently, we report a surgical experience of one case of acute prosthetic valve failure due to throe- bosis. The patient was 39-year old male who complained of dyspnea and orthopnea for 3 days before admission. The patient had not taken anticoagulant therapy for recent 4 months against medical ad- vice. The limitation of valve motion was revealed on echocardiography. We performed emergency reoperation with thrombectomy and prosthetic valve replacement. The patient had anuncomplicated postoperative course and have been followed up.

  • Acute myocardial infarction due to coronary occlusion by emboli originating from the prosthetic valve thrombosis is very rare but fatal disease which needs immediate diagnosis and urgent treatment. We report a case of acute myocardial infarction after left anterior descending embolic occlusion in whom had previous mitral valve replacement. Redo valve replacement following the interventional catheterization and anti platelet therapy lead to successful results.
