A Study on the Medical Records and Qian Yi(錢乙)'s Medical Thought in $\ll$Xiao Er Yao Zheng Zhi Jue$\gg$ (소아약증직결(小兒藥證直訣)의 의안(醫案)에 나타난 전을(錢乙)의 의학사상에 관한 연구)

  • Objectives The Medical record consist of symptoms, the cause of a disease, diagnostic studies, medical treatment, prescription, and so on. Doctors can use medical records as data for diagnosis, treatment, and education. Therefore, medical records are beneficial. Methods Translating $\ll$Xiao Er Yao Zheng Zhi Jue$\gg$ into modern Korean, and studying the medical records of Qian Yi(錢乙)'s Medical concept in $\ll$Xiao Er Yao Zheng Zhi Jue$\gg$ were examined. Results Regarding the diagnosis of the pediatric diseases, Si Zhen He Can(四該合參), consider in the eye exam closely. In addition, he closely combined the concept of Wu Zang Bian Zheng(五藏辨證), and diagnosis the pediatric diseases. Wu Zang Bian Zheng(五藏辨證). what Qian-Yi(錢乙) established method was based on Zheng Ti Guan(整體觀) in Huang Di Nei Jing(黃帝內經). Qian-Yi(錢乙) took a close look on Chuang Zhen(瘡疹) which had been an inadequate field in those days. He criticized on the habitual misuse of medications. He treated Ji Jing(急驚) and Man Jing(慢驚) separately; Ji Jing(急驚) as Liang Xie(凉瀉) and Man Jing(慢驚) as Wen Bu(溫補). He proposed Zong Gan Zhu Feng and Xin Zhu Jing(縱肝主風, 心主驚) theory and formulated Jing Feng(驚風) theory as well. He emphasized the treatment to Gong Bu Shang Zheng, Bu Bu Zhi Xie, Xiao Bu Jian Shi(攻不傷正, 補不滯邪, 消補兼施) because he had a clear demonstration to Han Re Xu Shi(寒熱虛實) of the five viscera in the field of Fang yao(方藥). Conclusions Understanding $\ll$Xiao Er Yao Zheng Zhi Jue$\gg$ and its medical records of Qian Yi(錢乙)'s medical concept had quintessential meaning to both modern pediatrician and oriental medical doctors of all medical fields in treating children.

  • Smallpox and measles might be the typical epidemics through Choson Danasty. Especially Sookjong was the first king who suffered from smallpox during the period of his reign. This study was examined critically focused on the Clinical Records about Smallpox which occurred in the 9th year of Sookjong's reign. The clinical records in Seungjeongwon Ilgi("承政院日記") was the main source and the Annals of the Joseon Dynasty was reffered. It tells us that Sookjong caught infectious disease, smallpox spread in town and the whole process of smallpox. Seungmagalgeuntang(升麻葛根湯) and hwadoktang(火毒湯) were given at the early term for treating Sookjong's smallpox, but later on, Bowontang(補元湯) was given for the treatment of smallpox. It showed us the medical view which placed emphasis on the vital force of human body. A dermatologist, Yoo Sang had joined since the beginning of the treatment and Siyakchung(侍藥廳) was established for Sookjong. Sasunghoichuntang(四聖回天湯) which was Chunyoohyung(全有亨)'s prescription showed us enough clinical experience and the ways of treatments. At the same time, the medical culture at that time was well known through the fact that smallpox was taboo among the royal family.

Study on the Medical Records in Naekyung-pyun of Dongeuibogam ("동의보감"(東醫寶鑑) 내경편(內景篇)에 수록된 의안에 대한 연구)

  • The Medical records (醫案) are very important materials in studying the developmental process of traditional Korean medicine. In this study, we investigated the medical records which are described in Naekyung-pyun(內景篇) of Dongeuibogam(東醫寶鑑), the most famous medical literature in Korean. There are 50 medical records in Naekyung-pyun and all of these records are cited from 12 Chinese medical literatures. The medical records were mainly cited from medical literatures written in Jin-Yuan(金元) and Ming(明) dynasty such as Yixuegangmu(醫學綱目) and Danxizanyao(丹溪纂要), although most records were written by medical scholars in Jin-Yuan(金元) dynasty, including Zhudanxi(朱丹溪) and Zhangzihe(張子和). Most of records were omitted and/or modified reflecting the Purpose of editors of Dongeuibogam. In addition, medical essay(醫論) was changed into medical record in some cases. The authors of the records used oral medication, acupuncture-moxa, psychological treatment and surgical manners. And in some cases of only having principal of treatment, the editors newly added the herbal prescription. The further study on medical records in Korean traditional medicinal literatures, would reveal the developmental progress of Korean medicine and inform more actual proof on medical condition.

  • "HanGaekChiHeom" is a book that arranged the medical records left by a Japanese doctor Junso Higuchi who treated a group of diplomats from Chosun and questions and answers exchanged between him and Chosun's doctor ChoSoongSoo. There are 14 kinds of medical records here with treatments and prescriptions as well as the detailed descriptions about the name, age and symptoms of patients. Various diseases and symptoms are included among them such as bums, common cold, skin diseases, edemas, etc. The characteristics of Junso Higuchi seem to be affected by medicine in Ming Dynasty in China.

  • ${\ulcorner}Volume\;1{\lrcorner}$ of ${\ulcorner}$Medical Records as a Guide to Clinical Works${\lrcorner}$, written by Ye Tian Shi, showed some clinical cases of dizziness. In this study, his diagnosis and treatment was studied with 16 clinical cases of dizziness with pathogenic factor, pathogenesis and symptoms of dizziness. Ye Tian Shi thought that phlegm, fire, wind and insufficiency were the causes of dizziness and phlegm-fire, phlegm-fire-wind, wind-phlegm and insufficiencyfire-wind were the causes of dizziness, clinically. Dizziness is caused when the body is in condition of excess in the upper and deciency in the lower. The acompanying clinical symptoms of dizziness are endogenous wind, fire of deficiency type, phlegm wind and phelegm fire. For the treatment of dizziness, Ye Tian Shi used the combination of medicines with some modifications by the cases for phlegm, fire, wind and insufficiency. He also encouraged the mental therapy for the treatment of dizziness. He emphasized the early treatment of dizziness to prevent hemiplegia after apoplexy. It can be postulated from Volume 1 of ${\ulcorner}$Medical Records as a Guide to Clinical Works${\lrcorner}$, diagnosis and treatment of symptoms and illness of Ye Tian Shi was strictly based on actual clinical cases.

A Study on the Medical Records of Heebin Jang in Seungjeongwon Ilgi (『승정원일기』에 기록된 장희빈 의안 관련 연구)

  • The objective of this article is to investigate the medical records written in Seungjeongwon Ilgi ("承政院日記") about Heebin Jang (張禧嬪), who was the mother of King Kyeongjong. She was one of the royal concubines of King Sukjong and later became the queen and then was demoted back to the concubine. The method to do this study was to search the records of Seungjeongwon Ilgi ("承政院日記") from Sukjong 15th year (A.D. 1689) to 20th year (A.D. 1694) on the website databased and serviced by National Institute of Korean History. The results were as follows. According to the website search, Heebin Jang (張禧嬪) suffered from three kinds of diseases. The first was a mastitis right after her delivery. The second was an abscess on the back of her head. The third was a relapse of phlegmturbidity and heat, which was her chronic disease. Also, three features could be found regarding characteristics of Royal medicine. The first was how the queens were taken care of before and after their delivery. The second was who lanced the queen's abscess. The third was how the Royal medicine and people's medicine interchanged.

  • "Kenjurok" is a set of medical records of Todo Yoshimasu, a Japanese docotr in the eighteenth century, who suggested that all diseases have originated from a poison, which is his own pathological term describing abnormal states of the body, The 54 records in "Kenjurok" were analyzed in statistical respects, including gender ratio, demographic distribution of patients, types of diseases, and herbal prescriptions used. Among 54 cases, male patients outnumbered female, as much as four times. The patients were quite evenly distributed according to ages. In 23 cases out of 54, abdomen palpation data were mentioned, Majority of the prescriptions used were originated from Sanghanron(傷寒論:Treaties on Febrile Diseases)/Geumgeyoryak(金匱要略: Synopsis of Golden Chamber). In frequency of use of prescriptions, however, showed somewhat different result, that is although Sanghan/Geumge prescriptions were used most often, esoteric prescriptions handed down in his family also composed significant part. The speculations derived from these statistical results are: Although Todo favored abdommen palpation to locate the poison and to decide a prescription, the proportion of abdomen palpation was not as high as expectation, He did use prescriptions not only in Sanghan/Geumge, but also other diverse prescriptions, rather often than not, which are regarded unique Japanese traditional prescriptions including poisonous minerals such as mercury and arsenic.

A Study of the Case Records of a Herbalist En Su-ryong in the late Chosun dynasty (19세기 고창지방 의원 은수룡이 남긴 경험의안)

  • This paper analyzes the case records of a herbalist En Su-ryong who lived at the Kochang area of Chollapukdo province in the 19th century. The records, which were included in his collection of works, Tantojip(呑吐集), were consisted of 11 clinical diagnosis and prescriptions. The result of the analysis is as follows. First, En Su-ryong's records are estimated to be valuable enough to contribute to the development of the Korean clinical medicine, in light of the fact that the present established prescriptions or medical theory came from the repetition of trial and error by many herb doctors. Second, his case records are unique in the style of writing, because they were consisted of only his own clinical diagnosis and prescriptions case by case, while those of ordinary herbalists were classified by the types of the symptoms of a disease, with their prescriptions modified from the past established. Third, in the records he minutely wrote not only the names and the addresses of the patients under his care, but also the names of the diseases, the progress and the contents of his treatment, and even the perfect cure or not. Therefore, his case records are appreciated to be very important from the standpoint of the history of the society.

  • This paper suggests a vibration control method long span structures with trussed roof. Basic concept of this method is based on the energy absorption through hysteresis loop of an elasto-plastic element. This element is attached on the top of the column supporting the roof. Two different types of roofs and three of earthquake waves are used in the investigation. It shows that this is very efficient method to reduce the seismic energy of roof member transferred from the column.
