Medical Quality Improvement Activity in the Medical Crisis (의료계 위기상황에서의 의료질 향상 활동)

  • Quality improvement activities are especially important in middle to small-sized hospitals as well as in large hospitals. Hospital accreditation would play a crucial role in the re-establishment of the healthcare delivery system, which is now nearly collapsed in Korea. To achieve all these goals, it is also important to educate prospective medical personnel at college on the concepts of quality improvement and patient safety.

Quality Improvement Nurses' Experiences of Working for Healthcare Quality in Public Hospitals (공공병원 의료 질 관리 전담자의 의료 질 향상 활동 경험연구)

  • Purpose : To describe and understand the experience of QI (quality improvement) nurses related to the hospital QI activities in public general hospitals. Methods : Purposive sampling was conducted 10 QI nurses and who have QI work experience for more than 1 year. Data were collected through focus group interviews. Interviews were recorded, transcribed and analyzed with qualitative content analysis using Strauss and Corbin's methodology. Results : The core categories of experience with QI activities were 'the success and failure of the medical care and overall health service sector', 'the degree of activation of current QI activities', 'characteristics of public hospital QI activities', 'what is needed to activate future QI activities'. The key themes were derived as follows. 'Success of quality enhancement activities according to the characteristic of public hospitals', 'activation of public hospital QI activities through leadership and QI education', 'reorganizing the role of regional hospitals in public hospitals'. Conclusion : Physician participation is important in the success of QI activities in public hospital practice. To lead these physician participation, Sharing doctor's QI experience and providing the necessary knowledge in QI activities and helping their leadership in QI activities are needed. QI nurses at public hospitals should lead QI activities to improve national hospitals' care quality through cooperating with local hospitals.

A Comparative Study on National and International Hospital Accreditation Systems Focusing on Korea, the U.S., and Australia (의료기관인증제도의 국내외 비교 연구 -한국, 미국 및 호주 중심으로-)

  • The purpose of this study was to ascertain new policies fo the development of the Korean Hospital Accreditation Program (HAP) by comparing and reviewing the domestic HAP of Korea, the U. S. and Australia as well as the International Accreditation Program of the International Society for Health Care Quality. The results of this study are as follows. First, it is necessary to improve the standards of the HAP such as 'patient and community centered standards' and 'staffing related standards'. Second, it is necessary to strengthen patient safety through the HAP. Third, the linkage among the HAP, clinical indicators and patient experience evaluation should be strengthened. Fourth, the competencies of surveyors should be improved. Fifth, the HAP of small and medium hospitals also needs to be vitalized. Sixth, it is necessary to improve the disclosure of the healthcare quality evaluation results. The findings of this study can be used as a basis to improve the Korean HAP.

A Review on Palliative Care Research Outcomes: Focus on the Quality of Life (완화의료연구에서의 결과에 대한 검토: 삶의 질을 중심으로)

  • Over the past several decades, the assessment of quality of life (QoL) has increasingly played a prominent role in both clinical practice and research regardless of the medical field. Palliative care is defined as an approach that improves the QoL of patients and their families and optimizing their QoL is the primary goal of palliative care. However, it is difficult to compare related studies due to several obstacles such as discrepancies in definitions for palliative medicine, lack of consensus on the central domains and diverse instruments. In this paper, we examined the current status of and challenges in QoL studies and discussed possible solutions. We are convinced this review will be helpful for further palliative care studies.

The Effect of Perceived Medical Services Quality by Outpatient on the Hospital Image, Satisfaction and Re-use Intention (외래환자가 지각한 의료 서비스의 질이 병원 이미지, 만족도 및 재이용 의도에 미치는 영향)

  • The purpose of this study is to investigate the effect of perceived medical service quality by outpatient on hospital image, satisfaction, and re-use intention in a general hospital. A total of 176 questionnaires were administered to outpatients who presented to a S general hospital in Daejeon. The results of this study are as follows. First, medical service quality had significant effects on re-use intention, satisfaction, and hospital image. Second, the mediating effect of satisfaction on the relationship between medical service quality and re-use intention was not significant but the mediating effect of hospital image on the relationship was significant. Third, satisfaction and hospital image had significant effects on re-use intention. Consequently, this study verified the influence of perceived medical service quality by outpatient on hospital image, satisfaction, and re-use intention in a general hospital. Further, considering that hospital image, rather than satisfaction, mediates the relationship between perceived medical service quality and re-use intention, it suggests that much more general satisfaction is required in the marketing activities of hospitals.
