  • 인터넷의 생활화와 컴퓨터의 대중화로 인해 의료 분야에서도 웹 환경에서의 지식기반 시스템의 필요성이 증대되고 있으며, 의료정보 자원과 지식기반의 응용시스템이 점차 늘어나고 있다. 또한, 지식기반 시스템에서 의료정보를 의미적 처리하기 위한 의료분야의 온톨로지 개발도 많이 시도되고 있다. 한방 온톨로지는 질병, 증상 등 정보를 체계적이고 논리적으로 표현함으로써 의료정보의 효과적 제공 및 의료지원이 가능한 지식 자원이다. 한방 온톨로지 기반의 지능화된 의료 서비스는 한방분야의 질병 및 증상 정보에 정확성을 부여하고, 체계적이고 질적으로 향상된 데이터를 제공할 수 있다. 또한, 환자의 증상에 따른 연관 증상을 제공하여 지능적인 문진이 이루어질 수 있도록 문진시스템에 적용이 가능하다. 본 논문에서는 한방 의료 분야의 질 높은 의료지원 서비스 제공을 위한 한방 온톨로지 설계 및 구현 방법을 제시하고, 구축한 한방 온톨로지를 소개한다.

Structure Model for TQM and Internal Customer Satisfaction of Medical Institutions (의료기관의 종합적 품질경영(TQM)과 내부고객만족에 대한 구조모형)

  • The study was conducted to find out way that improves service quality for external customers by identifying the impact of Total Quality Management(TQM) on the internal customer satisfaction and then raising the personnel satisfaction measurement in medical institutions. To this end, we carried out a survey of 500 employees from five university hospitals in three metropolitan cities of Busan, Deagu and Ulsan and Gyeongsangnam-do province from February 2 to April 30, 2015, and utilized 476 cases for this study finally. First, in the result of analysis, the leadership of management hierarchy affects education and training and employee participation, but not human resource management nor quality of the work among quality management activities. Secondly, the culture of medical service organization has an effect on every variable of quality management activities. And finally, internal customer satisfaction is influenced by quality management activities, the leadership of management hierarchy and the culture of medical service organization.

  • Hospital Accreditation Programme(HAP) has been introduced in many countries in the world for these recent years. This article reviews the HAP in the aspects of the organization, survey and evaluation process, evaluation criteria, and its impact to the hospital quality improvement. The nations included in this study are USA, UK, Canada, Australia, and Korea. To carry out this comparative study, the authors have reviewed articles and accreditation manuals having been issued in many countries. An expert panel of medical doctor, nurse, pharmacist, administrator, and specialist in health facilities formulated a study framework. The results of this study enhance understanding about hospital accreditation activities according to each nation's health care system. In recent years, the Korean government has launched the plan to improve the quality of health services by strengthening the hospital accreditation programme. This study results can provide useful information in development and implementation of the national hospital accreditation programme in Korea.

  • Objectives : Suicide is a serious sentinel event in healthcare organization. Suicide and suicidal attempt are fatal and long lasting mental and physical damage to themselves, family, and medical staffs. To develope the system to prevent suicidal accident in hospital, we reviewed and analysed one case of suicidal sentinel event. Methods: The risks of suicidal sentinel event were evaluated and analysed through the root cause analysis and failure mode effects analysis. Result: We found several root causes such as initial assessment of oldest patient and security issues. Couple of action plans to fix the problems were done immediately. According to failure mode, we evaluate the risk priority number to modify the action plans. Conclusion: To reduce the risk of sentinel events, we reviewed the suicidal event and established the new system and action plan to prevent sentinel events.

Challenges in Korea Hospital Accreditation: Focused on Post-Accreditation Management System (국내 의료기관 인증제의 과제: 사후관리 중심으로)

  • Objectives: The post-accreditation management system should be systematic in order to ensure that the accredited hospital continues to strive for patient safety and quality improvement during the accreditation period. In this study, we compare the post-accreditation management system in four countries (the U.S., Australia, Japan, and Korea) and provide suggestions for improving the post-accreditation management system in Korea. Results: All four countries had the post-accreditation management system, and the basic structure of the system in Korea was similar to that of others. However, there were differences in the practical operation processes and the use of the results. In the operation process, Korea's monitoring relies on voluntary reporting by accredited hospitals. In terms of results utilization, analytical feedback to data submitted by the acrredited hospital is not provided in Korea. Conclusions: It is necessary to establish a continuous monitoring system for post-accreditation changes and provide feedback to accredited hospitals. It is also necessary to perform a survey without advance notice and establish a firm legal basis for monitoring.

Analysis of Factors Affecting Medication Compliance of Outpatients (의료기관 외래 이용자의 복약순응 관련 요인분석)

  • Background : The purpose of this study was to investigate the current status of medication compliance of outpatients and to analyze the factors contributing to medication non-compliance Methods : Telephone survey was conducted to the 1,000 outpatients who visited medical institutions during the period from January 2002 to April 2002. Subjects were randomly selected from the telephone directories of the nation, and the socio-demographic characteristics of the respondents such as age, gender and region were matched based on those of outpatients in 2001. Results : The results of survey revealed that those who complied with doctors' regimen in the right way accounted for 82.4%. The compliance increased with the strong belief in the medication, less unwanted side effects and inconvenience, more severity of disease, and lower perceived health status. Compliance rate was also higher in the patients group who experienced the drug education by the pharmacists than those who did not. Conclusion : In order to improve drug compliance, drug information on efficacy, adverse reaction, drug interactions, and basic disease information are to be provided to the patients. Drug education needs to be focused not only on providing knowledge of drugs and diseases but also changing attitude on drug use of the patients.

A survey on Healthcare workers' perception of Patient Safety culture and medical error reporting (환자안전문화와 의료과오 보고에 대한 병원종사자들의 인식조사)

  • Background : The purpose of this study was to understand healthcare workers' perception of patient safety culture and medical error reporting to provide basic resources for the settlement of patient safety culture in medical institutions in Korea. Methods : For this purpose, convenience sampling by self-selection was applied to healthcare workers at a university hospital in Gyeonggi-do and a total of 482 people responded. The survey used the translated version of AHRQ in Korean and distributed through the Intranet system of the hospital. Result : The ratio of positive response was low overall. Among the responses, the response for 'Nonpunitive Response to Error' was the lowest at 17.7%, followed by the responses for 'Staffing' at 21.3%, 'Handoffs & Transitions' at 32.9%, and 'Communication Openness' at 44.3%. In result of surveying whether the responders have reported patient safety incidents during the past 12 months, 68.3% responded 'not once.' Conclusion : The perception of healthcare workers' patient safety culture and medical error reporting, when compared to AHRQ, was lower overall. It is important for healthcare workers to pay greater attention to patient safety to create a safe hospital culture where they do not punish or criticize related individuals or departments.

Current Status of Repeated Hospitalization in South Korea: Focused on Ambulatory Care Sensitive Conditions (국내 반복입원의 현황과 환자 특성: 외래진료 민감질환을 중심으로)

  • Purpose: Repeated hospitalization could be a proxy of unnecessary or preventive admission in South Korea where barriers to hospitalization are relatively low. This study aimed to estimate the current status of repeated hospitalization due to ambulatory care sensitive conditions (ACSC) in South Korea. Methods: Using the National Health Information Database, repeated hospitalization databases were constructed in units of episodes for patients who had been admitted more than twice between January 2017 and December 2018. The number of hospitalizations, total in-hospital days, and total medical expenditure were calculated and compared by patient characteristics in both of the entire patient group and the ACSC patient group. Results: Of total hospitalization episodes, 26.6% reported repeated admission, and 6.7% of repeated hospitalization was due to ACSC. A total of 183,110 patients with ACSC had been admitted an average of 2.9 times and spent an average of KRW5,630,118. In other words, KRW1,309 billion had been spent for repeated hospitalization due to ACSC. The scale of medical expenditure was relatively large in the highest and lowest socioeconomic status. Conclusion: Repeated hospitalization for ACSC can be considered a simple and intuitive indicator when assessing unnecessary hospitalizations or evaluating healthcare policy.

The Effect of Occupational Stress among Occupational Groups Related to Healthcare Accreditation on Turnover Intention (의료기관인증평가 관련 직무 스트레스가 직군간 이직의도에 미치는 영향)

  • Purpose: This study aims to investigate the factors influencing health personnel's occupational stress on turnover intention regarding healthcare accreditation. Methods: A survey was conducted from May 17 to May 31, 2021, among participants to examine occupational stress and turnover intention among health personnel working at a 188-bed specialized hospital in Seoul that is preparing for healthcare accreditation. Results: Occupational stress regarding healthcare accreditation was found to have a positive correlation with turnover intention (r=.698, p <.001), and influenced turnover intention the most, which explains the variance of 55.8% (F=29.015, p <.001). There were significant differences between occupational groups in job stress (F=13.292, p<.001) and turnover intention (F=10.930, p <.001) in the healthcare accreditation. Conclusion: Occupational stress regarding healthcare accreditation is higher in nursing than in other occupations, indicating the need to lower the turnover intention of nurses by preparing a national institutional standard for nursing manpower and also put in place an appropriate compensation system for each hospital seeking accreditation.

Fire Response Education for Hospital Healthcare Providers: A Scoping Review (병원 의료종사자 대상 화재 대응 교육 현황: 주제범위 문헌고찰)

  • Purpose: Fire response education is critical for healthcare providers working in hospitals to ensure a safe environment for patients and staff. However, a comprehensive review that thoroughly examines the contents, methodologies, and outcomes of fire response education in hospitals is currently lacking. Methods: We conducted a scoping review by adhering to the framework proposed by Arksey and O'Malley. We searched five electronic databases for literature published after 1990, using the key categories of "hospitals," "fires," and "education." As a result, we identified 15 relevant articles that met our inclusion criteria for the review. Results: Of the 15 articles, 12 had adopted a quasi-experimental design and the remaining 3 had employed a true experimental design. The majority of these studies (11 out of 15) were conducted in the United States, with 4 studies forming committees or teams dedicated to education. Simulation methods were used in 13 studies, while 2 studies had employed a combination of methods. All studies focused on first-response procedures based on RACE (Rescue, Alarm, Contain, Extinguish/Evacuation). Outcome measures included the learners' overall experience, performance in the educational settings, and performance in the field, with all studies reporting positive results following the educational interventions. Conclusion: Our review highlights the importance of multi-professional and multi-departmental educational strategies based on institutional-level initiatives for healthcare providers to create a safe hospital environment.
