  • Following the last issue, we summarized the major medical judgments in 2018. Especially, efforts have been made to introduce as many judgments as they relate to the obligation to explain. This is because the limits of the court's judgments were so diverse that it was unknown. Regarding the extent of damages, attention should also be paid to cases where the cost of care is recognized as a large amount, and cases where the memorandum is effective for the increased cost of treatment. The rulings related to the payment and deduction of medical expenses were the most discussed, although the description was small. The case of multi-institutional operation of medical institutions is an interim decision, but it is a case of interest in the medical community, and regarding uninsured medical expenses, cases of discretionary abuse have been reduced compared to the past, but are still significant.

  • The objective of this study is to compare hospital performance between Local Government Hospitals and General Hospitals. To compare the result of the financial performance between two groups, there were significant differences in current ratio, Fixed ratio, total assets turnover, personnel expenses to gross revenues, management expenses to gross revenues, return on assets, operating margin. The significant impact relations of ROA(Return on Assets) were formed the total assets turnover, salaries, material costs, administrative expenses. Although two groups are the similar beds, most of LGH are in the red so the managers and local government must consider the financial efficiency of LGH.

  • 1895년 독일 과학자 Wilhelm Roentgen 박사가 X-ray를 발견하고 미국의 Herman Carr가 MRI(Magnetic Resonance Imaging)의 기본 원리를 개발하여 발표한 이후 전자기장을 이용한 비침습적 의료영상 이미지 시스템은 놀라운 발전을 계속하여 왔다. 하지만, X-ray, CT(Computed Tomography), PET(Positron Emission Tomography)의 경우는 방사능의 사용과 조영제의 독성이라는 문제로 인하여 안전성에 대한 계속적인 문제를 야기해왔고 상대적으로 안전성이 입증된 MRI의 경우는 장비 자체 및 운용비가 일반화되기에는 고가여서 우리나라를 포함한 선진국에서도 아주 제한적으로 운영되고 있는 실정이다. 따라서, 세계적으로도 이러한 문제를 해결하기 위하여 안전하면서도 상대적으로 저렴한 비용으로 운용이 가능한 의료영상장비를 개발하기 위하여 많은 노력들이 경주되고 있어 관련 연구동향 및 산업화 동향을 소개하고자 한다.

A Study on the Influential Factors Affecting Korean Medical Tourism - Focusing on Chinese K-Medical Tourists (한국의료관광에 영향을 미치는 요인에 대한 연구 - 중국인 의료관광객을 중심으로)

  • Despite of advanced plans from Korean government to encourage Korean medical tourism, the medical tourism in Korea is still under competitive compared to the countries including Thailand, Singapore, and India, in which the government has been driving the development in industries of medical tourism, Moreover, the studies identifying the factors affecting tourists' satisfaction has not been actively performed up to date. Therefore, this study attempts to investigate the influences of quality of medical service, accessibility, cost adequacy, and quality of tourism of Korean medical tourism on Chinese tourists' satisfaction and revisit intention. For the purpose of empirical study, a survey has been conducted to Chinese medical tourists. Findings are as follows. First, the factors of medical tourism attributes including quality of medical service, cost adequacy, and quality of tourism have turned out to have positive effects on medical tourists' satisfaction. Second, tourists' satisfaction has a positive effect on revisit intention.

The Effects of the Revised Elderly Fixed Outpatient Copayment on the Health Utilization of the Elderly (노인외래정액제 개선이 고령층의 의료이용에 미친 영향)

  • Background: In January 2018, revised elderly fixed outpatient copayment for the elderly were implemented. When people ages 65 years and older receive outpatient treatment at clinic-level medical institutions (clinic, dental clinic, Korean medicine clinic), with medical expenses exceeding 15,000 won but not exceeding 25,000 won, their copayment rates have decreased differentially from 30%. This study aimed to examine the changes of health utilization of elderly after revised elderly fixed outpatient copayment. Methods: We used Korea health panel data from 2016 to 2018. The time period is divided into before and after the revised elderly fixed outpatient copayment. We conducted Poisson segmented regression to estimate the changes in outpatient utilization and inpatient utilization and conducted segmented regression to estimate the changes in medical expenses. Results: Immediately after the revised policy, the number of clinic and Korean medicine outpatient visits of medical expenses under 15,000 won decreased. But the number of clinic outpatient visits in the range of 15,000 to 20,000 won and Korean medicine clinic in the range of 20,000 to 25,000 won increased. Copayment in outpatient temporarily decreased. The inpatient admission rates and total medical expenses temporarily decreased but increased again. Conclusion: We confirmed the temporary increase in outpatient utilization in the medical expense segment with reduced copayment rates. And a temporary decrease in medical expenses followed by an increase again. To reduce the burden of medical expense among elderly in the long run, efforts to establish chronic disease management policies aimed at preventing disease occurrence and deterioration in advance need to continue.

  • 의료기기는 사람의 생명과 직접적으로 연관되어 있기 때문에 다른 분야의 기기보다 안전성에 대한 검증이 필수적이다. 의료 분야에서는 안전성 검증을 위해 기기의 허가 심사 조건으로서 소수의 피험자를 대상으로 수행되는 임상 시험이 존재한다. 그러나 임상 시험의 경우 의료기기를 직접 사람에게 적용하여 검증을 진행하기 때문에, 인체에 미칠 위해성을 고려하여 전임상 시험을 수행하고 있다. 하지만 전임상 시험은 동물이나 가상의 물체를 대상으로 수행하여 실제 사람에 대한 적용이 아니기 때문에, 임상 시험에 비해 검증에 대한 효력을 갖지 못한다. 따라서 본 연구에서는 피험자의 안전을 보장할 수 있고, 임상 빅데이터에 축적된 실제 환자의 사례를 활용한 신뢰성 있는 검증 방안을 제안하고자 한다. 그러나 현재 식품의약품안전처에서 제공되고 있는 의료기기 품목군은 개발하고자 하는 의료기기의 임상 빅데이터 기반 검증 가능성을 식별하기 어렵다. 그러므로 본 논문에서는 의료기기에 대한 다양한 특성 분석을 통해 임상 빅데이터 기반 검증 가능성을 식별하기 위한 프로세스를 제안한다. 제안하는 프로세스에서는 의료기기의 검증에 요구되는 데이터의 식별을 통해 임상 빅데이터를 이용한 테스트 데이터 수집 및 이를 활용한 신뢰성 높은 검증을 가능케 한다.

Comparison of Inpatient Medical Use between Non-specialty and Specialty Hospitals: A Study Focused on Knee Replacement Arthroplasty (전문병원과 비전문병원 입원환자의 의료이용 비교 분석: 인공관절치환술(슬관절)을 대상으로)

  • Background: The purpose of this study was to determine the effectiveness of the specialty hospital system by comparing the medical use of inpatients who had artificial joint replacement surgery in specialty hospitals and non-specialty hospitals. Methods: This study utilized 2021-2022 healthcare benefit claims data provided by the Health Insurance Review and Assessment Service. The dependent variable is inpatient medical use which is measured in terms of charges per case and length of stay. The independent variable was whether the hospital was designated as a specialty hospital, and the control variables were patient-level variables (age, gender, insurer type, surgery type, and Charlson comorbidity index) and medical institution-level variables (establishment type, classification, location, number of orthopedic surgeons, and number of nurses). Results: The results of the multiple regression analysis between charges per case and whether a hospital is designated as a specialty hospital showed a statistically significant negative relationship between charges per case and whether a hospital is designated as a specialty hospital. This suggests a significant low in charges per case when a hospital is designated as a specialty hospital compared to a non-specialty hospital, indicating that there is a difference in medical use outcomes between specialty hospitals and non-specialty hospitals inpatients. Conclusion: The practical implications of this study are as follows. First, the criteria for designating specialty hospitals should be alleviated. In our study, the results show that specialty hospitals have significantly lower per-case costs than non-specialty hospitals. Despite the cost-effectiveness of specialty hospitals, the high barriers to be designated for specialty hospitals have gathered the specialty hospitals in metropolitan and major cities. To address the regional imbalance of specialty hospitals, it is believed that ease the criteria for designating specialty hospitals in non-metropolitan areas, such as introducing "semi-specialty hospitals (tentative name)," will lead to a reduction in health disparities between regions and reduce medical costs. Second, it is necessary to determine the appropriateness of the size of hospitals' medical staff. The study found that the number of orthopedic surgeons and nurses varied in charges per case. Therefore, it is believed that appropriately allocating hospital medical staff can maximize the cost-effectiveness of medical services and ultimately reduce medical costs.

The Effect of Farming Labor in Later Life on the Social Welfare Expenditure (노년기 농업노동의 사회복지비용 절감 효과 분석)

  • Recently it has been encouraged in developed countries that labor force participation of the elderly is one of the means to cut down the cost of social welfare for them. However, empirical studies have rarely explored how work in later life contributes to national economy. Especially, even though 56.6 percent of elderly workers aged 65 and older engaged in agriculture and forestry in Korea, their contribution has been socio-economically overlooked. This study aims at examining the effect of farming labor in later life on social welfare expenditure. For this purpose, social welfare expenditure was defined as social benefits provided for the elderly by the social security in public sector and measured as transportation allowance, pension, livelihood aid, medical aid, and health insurance in 2003. Data were obtained from National Health Insurance Cooperation and 37 town/village offices and analyzed by 3 age groups; 65~74, 75~84, and 85 and over using SPSS/PC windows program. Results showed that both livelihood and medical support in all age groups were expended more to non-farm than to farm workers. The amount differences per person between them were 113,959~361,132 won in livelihood support and 15,644~51,418 won in medical support. Also, participation in farming influence reduction of livelihood expenditure for the group 65~74 and 75~84 and that of medical expenditure only for the group 65~74. Based on these results, it was estimated the amount of social welfare expenditure reduced by farming labor in later life. The limit of this study and the policy implications of the results are discussed.

Trend of Medical Care Utilization and Medical Expenditure of the Elderly Cohort (노인 코호트의 의료이용 및 입원진료비 변화 추이 -공.교 의료보험 대상자를 대상으로-)

  • Because of a significant improvement in the economic situation and development of scientific techniques in Korea during the last 30 years, the life expectancy of the Korean people has lengthened considerably and as a result, the number of the elderly has markedly increased. Such an increase of the number of aged population brought about many social, economic, and medical problems which were never seriously considered before. This study was conducted to assess the trend of medical care utilization and medical expenditure of the elderly. The data of each patient in the study were taken from computer database maintained for administrative purpose by the Korea Medical Insurance Corporation. The study population was 132,670 who were 60 years old or more and registered in Korean Medical Insurance Corporation from 1989 to 1993. The study subjects were predominantly female(56.3%) and 10,000-20,000 Won premium group(50.6%). The following are summaries of findings : The total increase of the number of inpatient cases was 40.5% from 1989 through 1993. The average annual increase was 3.7% in inpatient medical expenditures per case, 4.4% in inpatient medical expenditures per day and 0.08% in length of stay per case from 1989 through 1993. Cataract was the most prevalent disease of 10 leading frequent diseases in all ages from 1989 through 1993. The case mix in 1993 compared to 1989 revealed that cataract and ischemic cerebral disease were increased whereas essential hypertension and pulmonary tuberculosis were decreased . The average annual increase of medical expenditures was 3.8% in general hospitals, 6.3% in hospitals and 2.4% in clinics. From 1989 through 1993, medical expenditures used by high-cost patients accounted for about 14% to 20% of all expenditures for inpatient care, while they represented less than 2.5% of the elderly population. Time series analysis revealed that total medical expenditures and doctor's fee for inpatient will be progressively increased whereas drug expenditures for inpatient will be decreased. And there will be no change in length of stay. Based on the above results, the factors increasing medical cost and utilization should be identified and the method of cost containment for the elderly health care should be developed systematically.

Analysis of the positive word-of-mouth, Quality of medical service and customer satisfaction of patients in dental clinics (치과의원 내원환자들의 긍정적 구전과 치과 의료서비스 품질 및 고객만족도 분석)

  • Dental health care in modern society has gradually been developing with patients'expectation, desire, and medical knowledge according to the changes in the medical industry. Dental marketing strategy is necessary to construct a better dental health environment in order to cope with customers' changes and desire. The purpose of this study was to evaluate the positive comments by word-of-mouth experience, quality of dental clinic service and customer satisfaction among dental patients. The study surveyed 5oo subjects being treated at 6 dental clinics in Seoul and Gyeonggi province. It was conducted from March 22 to April 9, 2010. The subject groups with a positive experience through word - of - mouth who had a perception about the quality of dental clinic service (p<.001) and customer satisfaction (p<.001) were higher than other groups who had no comments on their dental treatment and clinics. The results showed that there was a correlation(r=. 852, p<.001) between the positive comments by word-of- mouthabout the quality of dental clinics service and customer satisfaction. The perception of dental clinic service and patients' satisfaction with positive comments by word of mouth was highly recognized by older and more experienced patients of dental treatment. Therefore, the results suggest that empirical information on important factors about strategic marketing lead to positive comments by word of mouth experience for patients in dental clinics.
