Korea is facing the problems of old aging society recently. In the future the ploblems will become more serious. Specially health care services for the aged are very poor in the aspect of both systems of services and supplies of facilities. This paper aims to identify the types as well as present statistical conditions of health care facilities for the aged in Korea.

Recently the facility requirements and regulations of healthcare facilities for the aged were changed, but the justification of requirements have not been identified. The facility requirements are so important to plan and design the facility. So this paper aims to identify the problems, and to compare & analyse the facility requirements.

본 연구는 경제성장과 함께 빠르게 발전하고 있는 중국의료시설의 최근 의료 환경과 의료시설의 변화 동향을 분석한 것으로 병원의 디자인, 병동 유니트 계획, 의료자원의 지역적 불균형과 건축기준 등 다각도의 변화 동향을 사진, 도면과 함께 제시하였으며, 중국 의료시설의 문제점과 과제를 정리하였다. 중국의 의료시설은 현재 급속한 성장기에 놓여 있으며, 그 과정에서 의료자원의 대도시 집중과 병원의 대형화 현상으로 지역적 의료자원의 불균형 문제가 나타나고 있고, 환자부족으로 중소병원의 경영문제가 심각한 반면, 대규모 병원은 환자집중으로 긴 대기시간과 혼잡함 등의 문제가 나타나고 있다. 또한 대규모병원과 중소병원 간 의료자원의 과다한 중복으로 비효율성 문제가 지적되고 있어 합리적인 경제기반에 기초한 효율적인 의료시설과 건물시스템을 어떻게 개발할 것인가 하는 점이 중국 의료시설의 큰 과제중의 하나가 되고 있다.

병원은 다양한 의료서비스를 제공하기 위해 의료기관 별 운영 전략에 따라 각기 다른 방식으로 운영된다. 특히 종합의료시설은 중증도가 높은 환자를 대상으로 수술이나 입원 등의 의료서비스를 효율적으로 제공하고자 그에 따른 운영 전략이 상이하다. 이러한 병원의 운영방식에 의해 결정되는 건축 역시, 건립 시기, 유형, 중증도에 따른 병원의 규모 등에 따라 시설 수준의 차이가 나타나며 이는 최근 정부에서 요구되는 제도 및 정책의 수용 여부를 결정짓는 중요한 요인이 되기도 한다. 최근 의료법은 기존 의료시설뿐만 아니라 신규 의료시설에 대한 설치 및 운영기준이 강화 추세에 있고, 서울시 또한 공공의료 확충을 위해 감염관리시설이나 필수의료시설 설치를 위한 용적률 인센티브 정책을 추진 중이다. 병원 운영 환경이 상이함에 따라 종합의료시설 설치 및 운영에 대한 인센티브제도의 적용 기준을 일괄적으로 적용하기에는 어렵다. 그러나 공익을 위해서 종합의료시설 인센티브제도를 지속 가능하게 운영하기 위한 객관적이고 합리적인 기준은 반드시 필요하다. 공공의료 기능 확충을 위한 서울시 종합의료시설 지구단위계획 수립·운영 기준은 공공과 민간이 모두 만족하는 의료환경 구축을 위해 매우 깊은 고민이 필요했다. 본 논설은 서울시 공공필요의료시설 확충 제도의 주요 내용과 공공필요의료시설 설치 및 운영 기준에 관해 소개하고자 한다. 특히 기준의 주요 내용을 정립하게 된 배경과 이유 등을 설명하여 본 제도의 의의를 정확히 전달하고 향후 보완해야 할 부분들을 검토하고자 한다.

This study aims to establish a research model to analyze the structural relationship between the working environment of the elderly medical welfare facility workers, the trust of the manager, and the quality of service. As a result, first, it was found that there was a significant causal relationship among service quality in the working environment of the elderly medical welfare facility workers. Second, it was found that there was a significant causal relationship between the working environment and the trust of managers of the elderly medical welfare facility workers. Therefore, in order to achieve the goal of the elderly medical welfare facility and improve the quality of service, it is necessary to create a working environment and establish a trust relationship among members. In addition, in order to cultivate competency and provide high-quality services for the elderly medical welfare facility workers, managers need to understand the individual characteristics of members, and to assign duties according to roles, accurate division of duties and personnel management are required.
