A Study on Current State of Web Content Accessibility on General Hospital Websites in Korea (국내 종합병원의 웹 접근성 실태에 관한 연구)

  • In the study, we introduce the trend in domestic and foreign web accessibility, as well as the legal system that ensures web accessibility. Based on Korean Web Content Accessibility Guidelines (KWCAG)1.0, we investigated the web content accessibility of 80 tertiary health-care hospitals and general hospitals in Korea. We evaluated accessibility by combining accessibility-based criteria (ABC) with usability-based criteria (UBC). ABC was limited to an alternative text for Guideline 1, using a small number of frames and keyboard accessibility for Guideline 2. UBC checked the voice service (TTS), resizing text, providing multi-lingual websites, and disclosing web accessibility policy. KADO-WAH2.0 was used for representing the compliance rate. The evaluation result was a considerable improvement from previous results, even though the rate of compliance with web accessibility was generally insufficient. There was a significant difference between those medical centers which did and did not comply with web accessibility. Incidentally, many hospitals were found to have attempted to confront and come to terms with web accessibility. In future, the following factors are advisable for medical centers with publicity or public interest: they must employ active and aggressive promotion of establishment of independent accessibility guidelines to secure web accessibility, they should effect an improvement of the realization of web accessibility, there can be constant education and promotion, and there can be an institutional supplementation, as well as others.

A Study on Utilization of non-residential areal hospitals in Inpatient (입원의료의 타 지역 이용에 관한 연구)

  • 본 연구의 목적은 환자가 거주지가 아닌 타 지역에서 입원의료를 이용하는 요인을 규명하여 주민들의 의료이용에 대한 지역적 접근성을 향상시킬 수 있는 방안을 제시하는데 있다. 2005년 환자조사 입원자료 523,782건을 연구대상으로 하였으며, 2004년 의료기관평가 자료, 2005년 인구센서스, 2006년 보건의료자원실태조사를 이용하였다. 자료분석은 기술통계, 카이제곱 검정, 로지스틱 회귀분석을 실시하였다. 타 지역 입원의료 이용의 가장 큰 요인은 의료기관 소재지의 의료서비스 수준으로, 인구 10만명당 의료기관평가 점수가 9.5점 이상인 지역이 9.5점 미만인 지역에 비해 타지역 입원의료 이용확률이 8.3배가 높았다. 반면 인구 10만명당 병상수가 910병상 이상인 지역이 910병상 미만인 지역에 비해 타 지역 입원의료 이용확률이 2.0배 높았다. 정부는 지역주민의 의료의 접근성을 높이기 위해 의료공급량의 확충과 분배보다는 지역의료의 질적 수준을 향상시키는 방향으로 정책을 펼쳐야 한다.

The Comparison of Risk-adjusted Mortality Rate between Korea and United States (한국과 미국 의료기관의 중증도 보정 사망률 비교)

  • The purpose of this study was to develop the risk-adjusted mortality model using Korean Hospital Discharge Injury data and US National Hospital Discharge Survey data and to suggest some ways to manage hospital mortality rates through comparison of Korea and United States Hospital Standardized Mortality Ratios(HSMR). This study used data mining techniques, decision tree and logistic regression, for developing Korea and United States risk-adjustment model of in-hospital mortality. By comparing Hospital Standardized Mortality Ratio(HSMR) with standardized variables, analysis shows the concrete differences between the two countries. While Korean Hospital Standardized Mortality Ratio(HSMR) is increasing every year(101.0 in 2006, 101.3 in 2007, 103.3 in 2008), HSMR appeared to be reduced in the United States(102.3 in 2006, 100.7 in 2007, 95.9 in 2008). Korean Hospital Standardized Mortality Ratios(HSMR) by hospital beds were higher than that of the United States. A two-aspect approach to management of hospital mortality rates is suggested; national and hospital levels. The government is to release Hospital Standardized Mortality Ratio(HSMR) of large hospitals and to offer consulting on effective hospital mortality management to small and medium hospitals.

  • 대전을지대학병원은 지난 2004년 의료 지방화 시대를 선도한다는 기치 아래 중부권 최대 규모의 시설을 갖추고 지금의 대전시 둔산동으로 이전, 개원을 했다. 이후 서울의 유명 병원 못지않은 최첨단 장비와 화려한 의료진을 내세워 최고 수준의 의료서비스를 펼쳤고, 그 결과 보건복지부의 의료기관 인증제 평가에서 대전지역 최초로 인증을 획득하기까지 했다. 이는 당연히 최고 수준의 의료체계를 갖추었기에 가능한 일이었지만, 그것만이 전부는 아니다. 그 이면엔 우수한 체계를 갖출 수 있도록 원활한 지원을 펼친 이들이 있었다. 바로 이곳의 시설안전담당자들이다. 25년의 안전 내공으로 대전을지대학병원을 지역 최고의 의료기관으로 이끈 김덕기 과장을 만나 이야기를 나눠봤다.

Assessment of Covid-19 Response of the Medical Institutions Based on ISO Public Service Quality Management Framework (ISO 기반 공공서비스 품질관리 프레임워크를 바탕으로 한 의료기관의 COVID-19 대응 현황 평가)

  • This study is conducted to improve the quality of healthcare services responding to COVID-19 by applying the public service quality management framework that is developed from ISO18091:2019 by the Ministry of the Interior and Safety of South Korea. The COVID-19 pandemic has been impacting the world since early 2020, and now in November 2020, it still has not been eliminated. The Korean medical institutions were able to prevent the spread of COVID-19 by agile test and strict cohort isolation. As a result, the so-called K-medical defense has achieved a positive reputation from around the world. In this study, we check and evaluate the current status of the COVID-19 response targeting some Korean hospitals by applying a quality control checklist based on the public service quality management framework. Status of 7 categories are analyzed based on the interview with 3 medical institutions. We also suggest improvements for better medical service quality in case of COVID-19 being prolonged.

A Simulation Technique for RFID Adoption in Hospital (의료기관 RFID 도입을 위한 시뮬레이션 기법)

  • As a key technology of U-health, RFID can be applied to the hospitals in a variety of cases such as patient tracking, medical instrument management, and so on. However, adoption of RFID in healthcare does not reach expectations because of huge cost. Exact estimation of cost and effectiveness will boost adoption of RFID in healthcare. This study proposes a novel simulation technique to evaluate cost and effectiveness of RFID in hospital environment. To do this, this study proposes a technique for modeling patients' movements in a hospital. Based on the model, this study provides how to obtain tag event dataset by means of simulating identifications of RFID tags that are attached to patients.
