Retrospective Study of Survival Rates According to the Type of Dental Restoration of Proximal Caries in Primary Molars (치과용 수복재료에 따른 유구치부 인접면 수복물의 생존율연구 : 후향적연구)

  • Restorative dental materials have advanced rapidly, with improved physical properties that improve survival rates. Accordingly, various materials can be selected. Amalgam, composite resin, glass-ionomer cement, and preformed stainless steel crowns have all been used widely for the restoration of dental caries in primary molars. The various dental materials used to treat proximal caries in the primary molars have distinct advantages and disadvantages. However, few studies have examined their survival rates. This retrospective study examined the 2-year survival rates of more than 700 class II restorations of proximal caries in primary molars clinically and radiologically according to the type of restoration. The study results should help in the selection of class II restorations for molars, one of the biggest concerns of pediatric dentists.

구치부 복합레진 수복 시 고려사항

  • 1960년대 중반부터 도입된 구치부 복합레진 수복은 복합레진과 접착시스템의 향상된 물성 때문에 구치부 사용에 대한 주목할 만한 결과를 보고하고 있다. 현재는 다수의 1급, 2급 수복물에 복합레진이 적응증으로 기술되며, 사실상 미국치과의사 협회(ADA)도 초기와 중등과 크기의 병소를 보존적 I, II급 와동으로 형성된 뒤 복합레진으로 수복하는 시술의 적절성을 인정하고 있다. ADA는 "복합레진을 유치나 영구치의 I급, II급, V급 수복에 올바르게 적용하면 아발감의 수명에 뒤지지 않는다."라고 기술하고 있다. 이에 본 강연에서는 I급과 II급 복진레진 수복을 위한 관련된 재료들의 특성과 단점들을 극복하기 위한 노력들을 설명하고자 한다. 특히 중합수축과 관련된 문제점들을 해결하기 위해 제시되고 있는 방법들의 임상적 적용 가능성의 한계와 술후 과민증을 줄이기 위한 방법, 그리고 구치부에 적용된 레진들의 마모를 줄일 수 있는 방법들을 모색하고 한다. 더불어 구치부 복합레진 수복을 위한 임상 시술 시 가장 흔하게 발생되는 단조로운 인접면 외형 형성 및 접촉점 개방의 문제점을 인식하고 이를 막기 위한 노력 및 기구들의 사용 조작에 대해 토의해 보고자 한다.

5 Year Cumulative Survival Rate of Composite Resin Restorations in Permanent First Molars (제1대구치 복합레진 수복의 5년 누적 생존율)

  • The purpose of this retrospective study was to evaluate the survival rate of composite resin restorations in permanent first molars in pediatric patients focusing on the influence of risk factors related to patients and teeth. 172 patients (6 - 12 years old) who had their permanent first molars restored with composite resin from July 2010 to July 2012 were investigated. From the dental records, the influence of the risk factors on the survival of restorations was assessed. Location of teeth, classification of cavities, patients' age and caries risk were included as risk factors in this study. The caries risks of patients were evaluated by the value of the decayed-missing-filled teeth index with the records of patients taken at 5 years old. Among 354 restorations, 272 restorations retained and 82 restorations were replaced. The overall cumulative survival rate at 5 years was 73.9%. The main reason for replacement of restorations was secondary caries (81.7%). Patients with older age group and with lower caries risk group showed higher survival rate of restorations. No statistically significant influence was detected between the survival rates and the possible risk factors : location of teeth, patients' age and caries risk. The survival rate of restorations was significantly affected by the classification of the cavities (p = 0.002).

A STUDY ON THE MICROLEAKAGE OF COMPOMER RESTORATIONS IN CERVICAL CAVITIES OF PRIMARY MOLARS ACCORDING TO THE LENGTH OF ETCHING TIME (유치 치경부 와동 Compomer 수복에 있어서의 산부식시간에 따른 미세누출에 관한 연구)

  • For the purpose of evaluating the microleakage of compomer restorations in cervical cavities of primary molars according to the length of etching time, an experiment was performed through next procedures. 48 extracted human primary molars were divided into 6 groups according to applied compomers and the etching time of 0, 15, 30 seconds. All the restorations were filled under the manufacturer's direction, and the degree of dye penetration was evaluated with stereomicroscope. The microleakage of etch group was measured and statistically analyzed and obtained the results as follows: 1. Compared with groups without etching, the degree of microleakage was reduced by acid etching the cavities (p<0.05). 2. There revealed similar preventive effects of microleakage when the teeth were etched for 15 seconds and 30 seconds respectively(p>0.05). 3. No significant difference was observed between two compomers examined in this study(p>0.05). It was, therefore, thought acid etching is effective to minimize the microleakage of compomer restorations in primary teeth, and in cases of performing, 15 seconds is adequate for etching.

THE EFFECT OF LIGHT CURING METHODS AND RESIN ADHESIVES ON THE MICROLEAKAGE OF RESTORATIONS IN PRIMARY TEETH (광중합 방법 및 레진 접착제의 종류에 따른 유치 수복물의 미세누출)

  • Objective : The purpose of this study was to evaluate the effects of several light curing methods and several adhesives on the microleakage of composite resin restorations in primary teeth. Materials and methods : In 150 extracted human primary anterior teeth, O-shaped cavities were prepared in the labial surface ; the cavity diameter and depth were 1.6mm. The cavities were filled with light-activated composite resin, Compoglass. Four kinds of adhesives were used. Each filling materials were polymerized with three light cure methods. The restorations were polished using Sof-Lex discs(3M Co., USA). The samples were thermocycled 1,000 times between $5^{\circ}C$ and $55^{\circ}C$ with a 1-minute dwell time. Then, they were immersed in a 2% methylene blue solution(pH 7) for 12hours. Subsequently they were sectioned labio-lingually through the center of the restoration with a diamond saw at low speed with a water coolant, and evaluated by stereomicroscopy. Microleakage analyses were done, using scores from 0 to 4. Results : Results showed the least microleakage in Compoglass group(P<0.05). There were less microleakage in SBMP group among the adhesive groups, but no significant difference was observed(P>0.05). And there were no significant differences among the groups depending on curing methods(P>0.05).

Pulp Treatment of Triple Tooth in Primary Dentition: Two Case Reports (유치열에 나타난 삼중치에서 치수 치료 : 증례 보고)

  • Triple tooth is rare in primary dentition; it is the abnormal fusion of three teeth. There are a few reports of double teeth, but triple teeth are rare. These multiple teeth create several clinical problems, not only esthetic problems but also a high caries susceptibility, congenital missing permanent tooth germ(s), orthodontic problems, and periodontal problems. They also make it difficult to perform pulp treatment because of the complex tooth structure. A 1-year 7-month-old male underwent a partial pulpotomy with mineral trioxide aggregate (MTA) when pulp exposure caused by deep caries occurred at a maxillary anterior triple tooth in the first case. The second case was a 1-year 9-month-old male presenting with a fracture line at a mandibular anterior triple tooth. After removing the fractured fragment, a pulpectomy was performed at the remaining primary lateral incisors. Specific complications were not observed during 24 and 6 months of follow-up, respectively.

RESISTANCE TO DEMINERALIZATION OF ENAMEL OF PRIMARY TEETH ACCORDING TO RESTORATIONS: IN VITRO STUDY USING QLF (유치 수복물에 따른 주변 법랑질의 탈회 저항성: QLF를 이용한 연구)

  • The objective of this in vitro study was to detect and monitor demineralization and remineralization of primary teeth according to restorative materials using quantitative light-induced fluorescence (QLF). A single bur hole was drilled on the each sound forty eight primary anterior teeth, and the specimens were divided into three groups. The cavity was restored with $Filtek^{TM}$ Z250(Group 1), F2000(Group 2), $Ketac^{TM}$ N100(Group 3) following the manufacturer's instructions. The teeth were subjected to the demineralizing buffer for 3 days, and then subjected to a remineralizing buffer for 14 days. The change of mineral loss(${\Delta}Q$) according to the stages was evaluated by QLF and the following results were obtained: 1. When demineralization was done, ${\Delta}Q$ was increased as follows. : Group 1 ($-110.79\;{\pm}\;27.77$) < Group 2 ($-104.84\;{\pm}\;28.95$) < Group 3 ($-90.16\;{\pm}\;21.87$) : Resistance to demineralization was statistically significant in Group 3. 2. There was a statistically significant increase in ${\Delta}Q$ of all groups since 1st day of remineralization 3. The rate of remineralization, ${\Delta}$(${\Delta}Q$)/day, showed significant high value in each group on the 1st day then decreased rapidly. 4. There was no statistically significant difference in the degree of remineralization among restorative materials.

Prosthetic rehabilitation of an oligodontia patient with atrophic maxilla (위축된 상악골을 가진 부분무치증 환자의 보철수복)

  • Congenital tooth agenesis is the most common developmental dental anomaly, of which oligodontia is defined as the absence of six or more permanent teeth, except the third molars. Tooth agenesis causes malocclusion, alveolar atrophy, aesthetic and psychosocial problems. This clinical report describes a multidisciplinary treatment for a patient diagnosed as oligodontia, who exhibited absence of 14 permanent teeth, atrophy of maxillary alveolar bone, and mandibular protrusion. Restoration space was secured and tooth axis was improved by the extraction of deciduous teeth and orthodontic treatment. However, edge-to-edge bite of posterior teeth and arch dimension discrepancy due to atrophic maxilla was remained. To restore the aesthetics and functionality, implant retained prosthesis was planned. Considering minimal bone grafts, location and number of dental implants and prostheses design were determined. Through the gradual adjustment of provisional restoration, the appropriate centric and eccentric occlusion was reflected into a definitive prosthesis. Currently, stable functional results were attained, however, regular follow up and maintenance care over lifetimes should be performed.

CHEMICAL DEGRADATION OF VARIOUS COMPOMERS IN NaOH (콤포머의 NaOH 용액 내에서의 화학적 분해)

  • The aim of this study was to evaluate the resistance to degradation of three commercial compomers in an alkaline solution. Dyract(Dentsply), Elan(Kerr) and F-2000(3M) polyacid modified resin composites(compomers) were used in this study. The resistance to degradation was evaluated on the basis of mass loss(%), degradation $depth({\mu}m)$ and Si, Al, Ba loss(ppm). The results were as follows : 1. The mass loss of each brand was $1.42%{\sim}2.14%$ and there was no statistically significant difference of mass loss among Dyract, F2000 and Elan. 2. The degradation layer depth of each brand was $182.92{\sim}227.7{\mu}m$ and there was no statistically significant difference of degradation layer depth among Dyract, F2000 and Elan. 3. There was statistically significant differences in Si-loss and Al-loss among three compomers (p<0.05). Si loss was the highest value in Dyract and Al loss was the highest value in F2000. 4. There was statistically significant correlation between mass loss and degradation layer depth (r=0.60, p<0.05). 5. In SEM finding, there was some destruction of compomer matrix-filler interface in post-exposure specimen to NaOH solution. As the matrix decreased, the filler particles distinguished and the periphery of the filler particles appeared whitish color due to degradation.

Case report: Full mouth rehabilitation for patient with heavy occlusal force and excessive abrasion (강한 교합력과 과도한 마모를 보이는 환자의 전악수복 증례)

  • Many of the patients with extensive abrasion need comprehensive restorative treatment. The abrasion is usually caused by attrition, besides of it, there are many reasons for it. The plan of treatment should be started on assessment of the type of attrition and the etiologic analysis. Patient with well-developed masticatory muscle, alveolar process, and high occlusal force and also with little muscle length difference between the stable and the contracted state should be carefully assessed for the vertical dimensional loss and the restoration should be carefully designed. Decrease of tooth length can be compensated by the growth of the alveolar bone height; therefore, consistency of the occlusal vertical dimension is maintained. Accordingly, a careless increase of the vertical dimension can produce muscle fatigue, depressed tooth and pain, and fracture of the restoration. In this case, the patient with multiple tooth abrasion and clenching habit, the edentulous maxillary area is restored with amalgam inserted RPD, and the dentulous area of the maxilla and mandible are treated with fixed restoration accompanying with the increase of vertical dimension. Consequently, we are going to report about the satisfying result in both functional and esthetic aspects.
