Deep inspiration breath-hold (DIBH) 적용한 림프절이 포함된 왼편 유방암의 방사선 치료계획에 따른 주변 장기 선량 평가

  • Purpose: On the left side, breast cancer patients have more side effects than those on the right side because of unnecessary doses in normal organs such as heart and lung. DIBH is performed to reduce this. To evaluate the dose of peripheral organs in the left breast cancer including supraclavicular lymph nodes and internal mammary lymph nodes according to the treatment planning method of Conventional Radiation Therapy, Intensity Modulated Radiation Therapy and Volumetric Modulated Arc Therapy. Materials and Methods: We performed CT-simulation using free breathing and deep inspiration breath-hold technique for 8 patients including left supraclavicular lymph nodes and internal mammary lymph nodes. Based on the acquired CT images, the contour of the body is drawn and the convention is performed so that $95%{\leftarrow}PTV$, $Dmax{\leftarrow}110%$. Conventional Radiation Therapy used a one portal technique on the supraclavicular lymph node and used a field in field technique tangential beam on the breast. Intensity Modulated Radiation Therapy was composed of 7 static fields. Volumetric Modulated Arc Therapy was planned using 2 ARC with a turning radius of $290^{\circ}$ to $179^{\circ}$. The peripheral normal organs dose was analyzed by referring to the dose volume of Eclipse. Results: By applying the deep inspiration breath-hold technique, the mean interval between the heart and chest wall increased $1.6{\pm}0.6cm$. The mean dose of lung was $19.2{\pm}1.0Gy$, which was the smallest value in Intensity Modulated Radiation Therapy. The V30 (%) of the heart was $2.0{\pm}1.9$, which was the smallest value in Intensity Modulated Radiation Therapy. In the left anterior descending coronary artery, the dose was $25.4{\pm}5.4Gy$, which was the smallest in Intensity Modulated Radiation Therapy. The maximum dose value of the Right breast was $29.7{\pm}4.3Gy$ at Intensity Modulated Radiation Therapy. Conclusion: When comparing the values of surrounding normal organs, Intensity Modulated Radiation Therapy and Volumetric Modulated Arc Therapy were applicable values for treatment. Among them, Intensity Modulated Radiation Therapy is considered to be a suitable treatment planning method.

Study of the Optimize Radiotherapy Treatment Planning (RTP) Techniques in Patients with Early Breast Cancer; Inter-comparison of 2D and 3D (3DCRT, IMRT) Delivery Techniques (유방암 방사선치료 시 최적의 방사선치료계획기법에 대한 고찰)

  • Purpose: A various find of radiotherapy treatment plans have been made to determine appropriate doses for breasts, chest walls and loco-regional lymphatics in the radiotherapy of breast cancers. The aim of this study was to evaluate the optimum radiotherapy plan technique method by analyzing dose distributions qualitatively and quantitatively. Materials and Methods: To evaluate the optimum breast cancer radiotherapy plan technique, the traditional method(two dimensional method) and computed tomography image are adopted to get breast volume, and they are compared with the three-dimensional conformal radiography (3DCRT) and the intensity modulated radiotherapy (IMRT). For this, the regions of interest (ROI) such as breasts, chest walls, loco-regional lymphatics and lungs were marked on the humanoid phantom, and the computed tomography(Volume, Siemens, USA) was conducted. Using the computed tomography image obtained, radiotherapy treatment plans (XiO 5.2.1, FOCUS, USA) were made and compared with the traditional methods by applying 3DCRT and IMRT. The comparison and analysis were made by analyzing and conducting radiation dose distribution and dose-volume histogram (DVH) based upon radiotherapy techniques (2D, 3DCRT, IMRT) and point doses for the regions of interest. Again, treatment efficiency was evaluated based upon time-labor. Results: It was found that the case of using 3DCRT plan techniques by getting breast volume is more useful than the traditional methods in terms of tumor delineation, beam direction and confirmation of field boundary. Conclusion: It was possible to present the optimum radiotherapy plan techniques through qualitative and quantitative analyses based upon radiotherapy plan techniques in case of breast cancer radiotherapy. However, further studies are required for the problems with patient setup reproducibility arising from the difficulties of planning target volume (PVT) and breast immobilization in case of three-dimensional radiotherapy planning.

  • 스크린-필름 X선 유방조영술(Screen-film X-ray mammography)은 유방암 진단을 위한 기준이 되는 방법으로 독보적인 위치를 차지하고 있다. 그러나 현재 사용되는 스크린-필름 방식 유방조영술은 여러 가지 한계점들이 인식되어 왔으며, 만약 유방 X선 사진(mammogram)이 디지탈 형식으로 곧 바로 얻어진다면 영상의 질(image quality)과 방사선량의 감소 가능성 관점에서 향상될 수 있다는 증거가 있다. 본 논문에서는, 디지탈 유방조영술에 대한 논리적 근거가 제시되고, 스크린 대용으로 개발되고 있는 X-선 검출기들과 이들을 사용한 여러가지 디지탈 유방조영술 시스템들을 분석 전망한다. 디지털 유방조영술을 실용화하기 위해 해결되어야 할 문제점들이 논의된다.

A Dosimetric Comparision of IMRT and VMAT in Synchronous Bilateral Breast Cancer (양측성 유방암의 세기조절방사선치료(IMRT)와 부피적조절회전방사선치료(VMAT)의 비교연구)

  • A study was performed comparing dosimetric characteristics of volumetric modulated arc and intensity modulated radiatio therapy on patients with bilateral breast cancer. For 5 patients, 3 plans were made for each patient; IMRT beams 8 and 12 of the beam intensity modulated radiation therapy, volumetric modulated arc therapy plan. The average PTVs volumes and $D_{98}$ for 12-IMRT were $51.04{\pm}0.57$ Gy (right), $50.80{\pm}1.07$ Gy (left), $42.94{\pm}16.16$ Gy (right), $42.56{\pm}2.09$ Gy (left). HI ($D_5{\sim}D_{95}$) and $CI_{90,95}$, even 12-IMRT has shown excellent results. In OAR, 3 plans showed excellent results. But the lowest dose of 12-IMRT. 12-IMRT achieved similar PTV coverage and sparing of organs at risk than 8-IMRT and VMAT.

A Change in an Absorbed Dose of the Heart in General and Respiratory Control Radiation Treatment Plans (일반 및 호흡조절 방사선치료계획에서 심장의 흡수선량 변화)

  • In radiation treatment, it is unavoidable to block the influence of scattered ray on a skin and prevent internal normal organs from being exposed to radiation. It is fair to say that radiation therapy aims to reduce an absorbed dose of normal tissues. In particular, in radiation therapy of left-sided breast cancer, the internal neighboring organs are normal breast tissues, the heart, and the lung. The side effects on the heart include cardioplegy and myocardial infarction. This study tried to observe changes in the volume and dose of the heart in general radiation therapy plan and respiratory control based radiation therapy plan for patients with left-sided breast cancer, and to find the heart's volume and dose generated by respiration. According to the 4D computer tomography (CT), a volume of the heart had $12.8{\pm}8.7cc$ on average, and its dose had $17.3{\pm}12.1cGy$ on average. The differences in the volume and dose may cause side effects in radiation treatment. Therefore, it is necessary to apply respiratory control technique to establish the radiation treatment plan based on an accurate position of the heart.

The Study of Dose Variation and Change of Heart Volume Using 4D-CT in Left Breast Radiation Therapy (좌측 유방 방사선치료 시 4D-CT를 이용한 심장의 체적 및 선량변화에 대한 연구)

  • Purpose: We investigate the results of changed heart volume and heart dose in the left breast cancer patients while considering the movements of respiration. Materials and Methods: During the months of March and May in 2012, we designated the 10 patients who had tangential irradiation with left breast cancer in the department of radiation Oncology. With acquired images of free breathing pattern through 3D and 4D CT, we had planed enough treatment filed for covered up the whole left breast. It compares the results of the exposed dose and the volume of heart by DVH (Dose Volume histogram). Although total dose was 50.4 Gy (1.8 Gy/28 fraction), reirradiated 9 Gy (1.8 Gy/5 Fraction) with PTV (Planning Target Volume) if necessary. Results: It compares the results of heart volume and heart dose with the free breathing in 3D CT and 4D CT. It represents the maximum difference volume of heart is 40.5%. In addition, it indicated the difference volume of maximum and minimum, average are 8.8% and 27.9%, 37.4% in total absorbed dose of heart. Conclusion: In case of tangential irradiation (opposite beam) in left breast cancer patients, it is necessary to consider the changed heart volume by the respiration of patient and the heartbeat of patient.

Radiation Dose Reduction in Digital Mammography by Deep-Learning Algorithm Image Reconstruction: A Preliminary Study (딥러닝 알고리즘을 이용한 저선량 디지털 유방 촬영 영상의 복원: 예비 연구)

  • Purpose To develop a denoising convolutional neural network-based image processing technique and investigate its efficacy in diagnosing breast cancer using low-dose mammography imaging. Materials and Methods A total of 6 breast radiologists were included in this prospective study. All radiologists independently evaluated low-dose images for lesion detection and rated them for diagnostic quality using a qualitative scale. After application of the denoising network, the same radiologists evaluated lesion detectability and image quality. For clinical application, a consensus on lesion type and localization on preoperative mammographic examinations of breast cancer patients was reached after discussion. Thereafter, coded low-dose, reconstructed full-dose, and full-dose images were presented and assessed in a random order. Results Lesions on 40% reconstructed full-dose images were better perceived when compared with low-dose images of mastectomy specimens as a reference. In clinical application, as compared to 40% reconstructed images, higher values were given on full-dose images for resolution (p < 0.001); diagnostic quality for calcifications (p < 0.001); and for masses, asymmetry, or architectural distortion (p = 0.037). The 40% reconstructed images showed comparable values to 100% full-dose images for overall quality (p = 0.547), lesion visibility (p = 0.120), and contrast (p = 0.083), without significant differences. Conclusion Effective denoising and image reconstruction processing techniques can enable breast cancer diagnosis with substantial radiation dose reduction.

Analysis on the Dosimetric Characteristics of Tangential Breast Intensity Modulated Radiotherapy (유방암의 접선 세기조절 방사선치료 선량 특성 분석)

  • The tangential breast intensity modulated radiotherapy (T-B IMRT) technique, which uses the same tangential fields as conventional 3-dimensional conformal radiotherapy (3D-CRT) plans with physical wedges, was analyzed in terms of the calculated dose distribution feature and dosimetric accuracy of beam delivery during treatment. T-B IMRT plans were prepared for 15 patients with breast cancer who were already treated with conventional 3D-CRT. The homogeneity of the dose distribution to the target volume was improved, and the dose delivered to the normal tissues and critical organs was reduced compared with that in 3D-CRT plans. Quality assurance (QA) plans with the appropriate phantoms were used to analyze the dosimetric accuracy of T-B IMRT. An ionization chamber placed at the hole of an acrylic cylindrical phantom was used for the point dose measurement, and the mean error from the calculated dose was $0.7{\pm}1.4%$. The accuracy of the dose distribution was verified with a 2D diode detector array, and the mean pass rate calculated from the gamma evaluation was $97.3{\pm}2.9%$. We confirmed the advantages of a T-B IMRT in the dose distribution and verified the dosimetric accuracy from the QA performance which should still be regarded as an important process even in the simple technique as T-B IMRT in order to maintain a good quality.

A Study on the Usefulness of Breast Shielding Apron for Reducing Exposure Dose in Mammography (유방X선촬영 시 피폭선량 감소를 위한 유방촬영용 차폐복의 유용성에 관한 연구)

  • Mammography, conducted every two years, causes cancer due to regular exposure to radiation while reducing rate of death caused by breast cancer. The study evaluates the effect of breast shielding apron made to shield off scattered radiation that occurs to the breast when the opposite side breast is mammogramed. AGD was measured using ACR phantom, composed of 50% mammary glands and 50% fat, and radiation was measured before and after wearing the apron on the breast when the opposite side of the breast is mammogramed. When CC direction mammography was conducted to a breast, the AGD was 1.84 mGy. When CC direction and MLO direction mammography were done to a breast, the average dose detected from the opposite side breast from four directions(top to bottom and medial to lateral) was $140{\mu}Gy$ with maximum dose of $256{\mu}Gy$ at medial side. After putting on the apron, the dose, caused by scattered radiation, was not detected from any of the four directions. Using of breast shielding apron is expected to minimize the radiation exposure by blocking scattered radiation to the breast shielded, when mammography is done to the opposite side breast.

  • 현재 우리나라의 유방촬영장치의 정도관리 항목에는 국제규격에서 원하는 직선성이 빠져있다. 직선성은 방사선량 및 방사선질의 조정을 위한 요구사항이므로 반드시 측정이 되어야한다. 본 연구에서는 유방촬영장비 5대를 대상으로 IEC 60601-2-45 규격에 맞는 직선성을 시험하고 평가함으로써 좀 더 발전된 정도관리방향을 제시하고자 한다.
