건강관리 요인들에 따른 혈중지질 농도 및 영양소 섭취 상태 비교

  • 심혈관계질환의 예방과 영양교육의 기초자료를 제공하기 위하여 성인 남녀를 대상으로 규칙적인 운동 습관과 음주, 흡연, 폐경, 비만 여부에 따라 신체계측치와 콜레스테롤, 중성지방, LDL-cholesterol, LDL-cholesterol 등의 혈청지질 및 영양소 섭취량을 비교 조사한 결과는 다음과 같다. 1)체중이나 신장, 비만 정도 및 WHR 등 신체계측지수는 음주나 흡연 여부에 따른 차이가 없었으나, 혈청 중성지방 농도를 유의적으로 상승시켰다. 2)음주군은 비음주군에 비하여 단백질의 열량비율(% RDA)이 유의적으로 높았으며(p<0.05), 흡연군은 비타민 A와 나이아신, 비타민 B6, 엽산, 비타민 C, 비타민 E의 섭취량에 있어서 비흡연자들보다 낮은 경향을 보였다. 3)운동에 의한 혈청지질 농도에는 뚜렷한 차이가 없었으나, 남성들의 경우 현재의 체중과 PIBW, Quetlet index, 엉덩이둘레가 비운동자들보다 많은 것으로 나타났다. 영양소 섭취량에 있어서 남성의 경우 운동군이 비운동군에 비해 콜레스테롤과 나이아신 및 인을 더 섭취하고 있었으며, 당질의 섭취비율은 유의적으로 더 낮았다(p<0.05). 여성의 경우에는 불포화지방에 유의적 차이를 보여 비운동군의 불포화지방 섭취량이 더 많았다(p<0.05). 4)폐경한 여성의 경우 폐경 전 여성에 비하여 신장은 더 작고 WHR은 더 높았다. 폐경 이후 estrogen 분비의 변화로 인하여 혈중 지질 성분 중에서는 cholesterol이 198.4$\pm$36.3mg/dl, LDL이 119.5$\pm$34.9mg/dl로 폐경 전의 수치보다 유의적으로 높았다(p<0.01). 5)비만 여부는 남자의 경우 콜레스테롤 농도에(p<0.01)), 여자의 경우 중성지방 농도(p<0.01)에 대해 의미있는 설명력을 보이는 변수로 작용하고 있었다. 그러나 식이 섭취상태와의 관련성은 나타나지 않았다. 이상에서 생활습관이 신체계측치 및 혈중 지질 농도와 일부 상관을 보이는 것을 알 수 있었으며, 건강위험 요인도 관찰할 수 있었다. 고지혈증은 심혈관계질환의 주요한 위험 인자로 알려져 있으며, 그 유발 요인에는 여러 환경적인 요인과 식이 요인이 있다. 또한 생활습관은 영양소의 섭취량에도 영향을준다. 이처럼 식이 요인과 생활습관은 단독 혹은 상호 영향을 주게 되므로 고지혈증 및 심혈관계질환을 개선하려면 식사요법과 생활습관 교정을 병행하여야 한다. 고지혈증의 원인 중의 하나로 지적되고 있는 음주와 흡연은 상호 유도작용이 있으므로 금연이나 절주를 하고자 할 경우 두가지를 동시에 자제해야 효과적일 것으로 생각된다. 폐경후 여성들에게 발생 빈도가 높은 심혈관질환의 위험요인인 고지혈증을 예방하고자 하는 노력은 폐경후 여성들의 삶의 질적 향상을 위해서도 매우 중요하다. 또한 균형식 섭취와 운동을 통해 혈중 지질 수준을 건강하게 유지하고, 비만으로의 이행을 방지함으로써 심혈관계질환을 예방하도록 하여야겠다.

Sex Differences in the Association of Habitual Snoring with Metabolic Syndrome (성별에 따른 습관성 코골이와 대사증후군 요소의 관련성)

  • The purpose of this study was to assess the association between habitual snoring and metabolic syndrome in the Korean population and to evaluate it's possible gender variations. The study subjects were 2,007 volunteers aged 40-69 years who participated in regular health check-ups(male:434, female:1,573) from May 2012 to December 2012. Standard interviews, anthropometrics and biochemical studies were conducted. Habitual snoring was defined as snoring more than 4 days per week. The prevalence of metabolic syndrome in our sampling pool was 17.4% (male: 20.7%, female: 16.5%), and this increased with age. After adjusting for age, smoke and alcohol consumption, sleep duration, regular exercise, the odds ratio of metabolic syndrome and it's components were increased significantly by habitual snoring. HDL-cholesterol, blood sugar, waist size were significantly higher in women, but not significantly higher in men.

Alcohol Problems and Related Service Needs in Urban Korean Community (경기도 일지역의 문제음주실태와 알코올 미충족욕구 조사연구)

  • Objectives : The purpose of this study is to investigate the characteristics of drinking patterns and the prevalence of hazardous drinking and alcohol use disorder with the assessment of alcohol policy needs in community population of urban city in Korea. Methods : Data was collected from 1,000 residents in a single urban city in Korea, from June 1 to July 31, 2011 by a questionnaire. As research tools, socio-demographic characteristics composed of variables based on study purpose, alcohol problems and alcohol use disorder state scales composed with Alcohol Use Disorder Identification Test(AUDIT) and Cut down, Annoyed, Guilty, Eye-opener(CAGE), and the assessment of alcohol policy needs in community population questions were used. Results : The lifetime experience of alcohol drinking was 97.4%. For the frequency of drinking, 21.9% of population responded 2 to 4 times per weekly. 24.4% of male had one time or more high risk drinking, defined as having four or more drinks in a sitting, per week. 51.6% experienced the first time drinking of alcohol in adolescence. The lifetime prevalence of hazardous drinking and alcohol use disorder were 14.9% and 3.3%, respectively using AUDIT. Alcohol use disorder were 9.6% with the CAGE scale. Problem drinking were 8.4% in this study. They requested the program such as prevention education(57.2%), construction of liaison system to treatment facilities(50.1%), alcohol counseling and intervention(41.7%), and rehabilitation program(39.8%). Conclusions : These results demonstrate that alcohol problem and alcohol use disorder were much prevalent, and these problem started in the earlier age. It is necessary to develop an effective alcohol prevention program or policy with considering the needs in community population.

Factors affecting suicidal ideation, suicidal plan and suicidal attempt in Korean adolescents (청소년의 자살생각, 자살계획, 자살시도에 영향을 미치는 요인에 관한 연구)

  • This study aimed to identify the influencing factors of suicidal ideation, suicidal plan and suicidal attempt. The data of 2012 Youth Health Risk Behavior web-based Survey Collected by the Korea Center Disease Control was analyzed by using the SPSS program. The influencing factors of suicidal ideation, suicidal plan and suicidal attempt by general characteristics were gender, grade, school record, socioeconomic status, living with both parents, and subjective health status. The influencing factors of suicidal ideation, suicidal plan and suicidal attempt by health behaviors and emotional characteristics were smoking, drinking, effort for weight, sleep satisfaction, perceived stress and depression. This study suggested that suicidal prevention programs should be developed in preventing and reducing health risk behaviors and depression.

Comparison of Risk Factors for Men and Women According to Severity Classification in Patients with Coronary Artery Disease (관상동맥질환 중증도 분류에 따른 성별 위험요인 비교)

  • The aim of this retrospective study was to compare risk factors between men and women through secondary data of nursing information and medical records according to the severity classification in patients of 340 men and 221 women with coronary artery disease(CAD) who were admitted for the first time at a general hospital and underwent first coronary artery angiography. Consequently, men presented with risk factors such as age(p=.004), total cholesterol(p=.040), triglycerides(p=.049), HbA1c(p<.001), smoking(p<.001), alcohol consumption(p=.002) and comorbidities(p=.036) that showed statistically significant differences. Among women, age(p=.002) and comorbidities (p=.018) were the only factors that showed significant differences. Significant risk factors influencing the classification of CAD severity in men were total cholesterol (OR 0.97, 95% CI 0.96-1.00, p=.014) in 1VD, alcohol consumption (OR 52.47, 95% CI 2.99-91.95, p=.007)in 2VD, and total cholesterol in the 3VD(OR 0.98, 95% CI 0.95-0.98, p=.026). A significant risk factor affecting the classification of CAD severity in women was comorbidity (OR 0.30, 95% CI 0.11-0.82, p=.020) in the 3VD. This study identified the importance of nursing care for male CAD patients, such as smoking cessation and quitting drinking, blood sugar control, cholesterol, and accompanying disease management, and provided evidence of individually tailored nursing care.

Anxiety, Impulsiveness, and Drinking Problems in Employed Women (직장 여성의 불안, 충동성과 음주문제)

  • Objectives : This study examined drinking habits, anxiety, and impulsiveness in employed women. Methods : We sampled 925 residents aged over 20 years in Gwanak-gu of Seoul. In the process, we divided subjects into employed women and unemployed women groups and then used the Korean Version of Alcohol Use Disorder Identification Test (AUDIT-K), State-Trait Anxiety Inventory (STAI), and Barratt Impulsiveness Scale (BIS) to identify patterns in subjects' alcohol drinking, anxiety and impulsiveness. Results : Among the 925 respondents, 95.7% drank alcohol, and after the analysis, female problematic drinking ($12{\leq}AUDIT-K{\leq}25$) and alcohol dependence ($AUDIT-K{\geq}26$) was apparent in 61 (9.5%) and 2 respondents (0.3%), respectively. Female respondents who had jobs tended to drink more and exhibit higher ratios of problematic drinking, with a higher exposure to alcohol than those that were unemployed. The psychological attributes related to female respondents' alcohol problems related to state of anxiety and impulsiveness. Employed women respondents also showed a higher state of anxiety and impulsiveness than unemployed women. Conclusion : Controlling the frequency of exposure to alcohol, anxiety, and impulsiveness in employed women should be considered to be crucial controlling factors related to the hazards of excessive drinking.

The Relationship between Smoking, Alcohol Drinking and Vitamin D Level among Korean Adults : From the Korean National Health and Nutrition Examination Survey 2013-2014 (한국 성인에서 흡연 및 음주와 비타민 D 수준과의 관련성)

  • The aim of this study was the relationship between smoking, alcohol drinking and vitamin D level among Korean adults using data from the '6th(2013-2014) Korean National Health and Nutrition Examination Survey'. The data used in this study were analyzed for 3,565 people who were over the age of 19 ages and checked vitamin D level and consumption frequencies of vitamin D rich foods among 15,568 respondents to the '6th(2013-2014) Korean National Health and Nutrition Examination Survey'. The subjects were divided into four groups: None group(non-smoking and non-alcohol drinking), smoking only group, alcohol drinking only group and both group (smoking and alcohol drinking). The mean serum vitamin D level of the subjects was $16.5{\pm}0.20ng/mL$ and 73.5% of vitamin D levels were insufficient and deficiency. The results from hierarchical multiple logistic regression showed that the Odds ratio for insufficient vitamin D due to smoking and alcohol drinking were significantly higher in the smoking only group 2.281(95% CI 1.080-4.817)times and both group 1.356(95% CI 1.010-1.922)times than none group. Our results showed the relationship between smoking, alcohol drinking and vitamin D level was significant in Korean adults. Future studies, such as a more systematic cohort study investigating the relationship between smoking, alcohol drinking and vitamin D level may be helpful in confirming the causal relationship between smoking, alcohol drinking and vitamin D level.

The Structure of Driving Behavior Determinants and Its Relationship between Reckless Driving Behavior (운전행동 결정요인의 구성과 위험운전행동과의 관계)

  • This study aimed to expand and reconstruct the Driving Behavior Determinants' factors in order to confirm the relationship between Driving Behavior Determinants(DBD) and drivers' reckless driving behavior level. To expand the structure of DBD, drivers anger, introversion and type A characteristics were added, which were never considered as related factors in existing DBD studies before. The correlations between the new factors of DBD and reckless driving behavior(includes driver's personal records of driving experiences for the last three years) were verified. A factor analysis result showed us that new DBD questionnaire consists of five factors such as, 'Problem Evading', 'Benefits/Sensation Seeking', 'Anti-personal Anxiety', 'Anti-personal Anger', and 'Aggression'. Also, reckless driving behavior consists of 'Speeding', 'Inexperienced Coping', 'Wild Driving', 'Drunken Driving', and 'Distraction'. The result of correlation between the DBD and reckless driving behavior indicates that inappropriate level of DBD is highly correlated with dangerous driving behavior and strong possibilities of traffic accidents. Based on these results, we might be able to discriminate drivers according to DBD level and predict their reckless driving behavior through a standardization procedure. Futhermore, this will make us to provide drivers differentiated safety education service.

Factors Related to High Risk Drinking in Adult Drinkers by Age Group (연령군별 성인 음주자의 고위험음주 관련 요인)

  • Purpose: The purpose of this study was to identify factors associated with high risk drinking in adults. Methods: Multi-variate logistic regression was used to analyze the data of 15,949 adults age 19 years or older from the sixth (2013-2015) Korea National Health and Nutrition Examination Survey. Results: Factors associated with high risk drinking included education level, employment and smoking status among the age group of 19-39. In the 40-59 year age group, the associated factors were gender, employment, smoking, obesity, and depressive mood. In the over 60 year age group, related factors included gender, employment, smoking, abdominal obesity, and subjective health status. Conclusion: The result of this study suggests that early risk factor screening may be helpful to avoid the progression to high-risk drinking. An individualized approach for each age group can be used as a preventive measure.

Factors associated with high-risk drinking among men in Korea: a secondary analysis (한국 성인 음주 남성의 고위험 음주 관련 요인: 2차 분석 연구)

  • Purpose: This study was conducted to identify factors associated with high-risk drinking in Korean men. Methods: This study was a secondary analysis using data from the Korea National Health and Nutrition Examination Survey (KNHANES VIII-2), 2020. Data were downloaded from the KNHANES website. The subjects of this study were 1,653 alcohol-drinking men between 19 and 64 years of age. Data were analyzed using the Rao-Scott chi-square test and complex sample logistic regression statistics. Results: The high-risk drinking rate among alcohol-drinking men was 27.1%. High-risk drinking was more common in men who smoked (odds ratio [OR] = 2.11, p < .001), men with a middle school education or lower (OR = 1.91, p = .016), men who lived with a spouse (OR = 1.61, p = .025), men who slept less than 6 hours on weekends (OR = 1.51, p = .016), and men who perceived stress (OR = 1.30, p = .044), while it was lower in men who were underweight (OR = 0.19, p = .006). Conclusion: To reduce the rate of high-risk drinking, it is necessary to provide an intervention that considers factors associated with high-risk drinking among men. In particular, smoking was the most important factor associated with high-risk drinking, implying that the integrated management of drinking and smoking is necessary to reduce high-risk drinking.
