Yongia sonchifolia Max. has been used as raw materials of traditional Kimchi and medicinal herb in Korea. This study was performed to investigate biologically active components in the plant. First, the writer carried out the experiment of antitumor screening test against Sarcoma-180A and the cytotoxic activity against Chinese hamster V-79 cells with methanol extract of the plant. And the aqueous solution of the extract from roots of Youngia sonchifolia Max. was partitioned into n-hexane. The concentrated extract of n-hexane layer was chromatographed on silica gel column and developed with n-hexane and ethylacetate. Two yellow elutes, on concentration, were recrystallized from ethylacetate, and the $R_f$ value of TLC of the crystal was 0.43. After analysis by $^{1}H-NMR$, $^{13}C-NMR$ and MS to confirm the structure, the author could identify the compound as bauerenyl acetate, a naturally occurring pentacyclic triterpene. The crystal was colorless plate and m.p. was $280{\sim}282^{\circ}C$.
Objectives : To investigate the clinical features of drug-induced liver injury (DILI) and traditional Korean medicine (TKM) -based management. Methods : A female patient diagnosed with DILI caused by Western drugs had been treated with Oriental therapies, then the subjective clinical outcome and biochemical parameters were monitored. Results : A 73-year-old female had taken Western drugs (nonsteroidal anti-inflammatory and skeletal muscle relaxants) for about 3 months, and complained of severe abdominal discomfort and tiredness. Her RUCAM score was 9, which met the criteria for DILI (AST 90 IU/L, ALT 100 IU/L, ALP 191 IU/L, and GGT 614 IU/L). She was treated with herbal drugs, moxibustion, and acupuncture, and her symptoms completely resolved, with normalized hepatic enzymes within two weeks. Conclusions : This case report provides a clinical characteristic for a typical DILI caused by Western medicine, and shows an example of a TKM-based application.
Newborn gastrointestinal tract emergency diseases include various disorders that occur anywhere along the digestive tract. Most of them are congenital malformations, not acquired diseases. Many of them cannot be confirmed on prenatal examination, and consequently, postnatal evaluation is required. Unlike adults, in newborn babies, detailed evaluations with computed tomography (CT) and magnetic resonance imaging (MRI) are often limited, so making the diagnosis based on early abdominal radiography is most important. Therefore, it is necessary to be familiar with the radiologic findings that may be seen on imaging studies. In addition, it is important to understand abdominal ultrasound and fluoroscopy findings, which can be used in addition to simple radiography and indications that can be used for further diagnosis and appropriate treatment. Therefore, this study describes and organizes the various imaging findings that may occur in neonatal gastrointestinal emergencies.
Mongolian dandelion (Taraxacum mongolicum H.) extracted with solvents having different polarities were prepared to investigate protection activity of gastric mucosa. Ethanol extracts were successively reextracted with methylene chloride, and n-butanol. Concentrations of total flavonoids and luteolin in n-butanol extracts were 27.75 and 1.14%, respectively, much higher than those of other solvent extracts. Results of anti-microbial acitivity test against Helicobacter pylori and urease inhibition test revealed n-butanol extract exerted higher inhibition (13.16%) than other solvent extracts. Based on 1,1-diphenyl-2-picrylhydrazyl (DPPH) assay, n-butanol extract showed the highest efficiency of free radical-scavenger activity among the extracts ($SC_{50}\;47{\mu}g/mL$). Erythrocytic photohemolysis tests, for the protection of cell membrane showed that half-life of human erythrocytes was increased by the addition of n-butanol extract $({\tau}_{50}=172min;C=5.0{\mu}g/mL)$. These results indicate n-butanol extract of Mongolian dandelion may be useful as a adjuvant for gastric diseases.

The disaster from functional gastrointestinal disorders (FGID) has detrimental impact on the quality of life of the affected population. There are, however, rare diagnostic methods for FGID. Our research group identified recently that the gastrointestinal tract well of the patients with FGID became more rigid than that of healthy people when palpating the abdominal regions overlaying the gastrointestinal tract. The objective of the current study is, therefore, to identify feasibility of a diagnostic system for FGID based on ultrasound technique, which can quantify the characteristics above. Two-dimensional finite difference (FD) models (one normal and two rigid models) were developed to analyze the reflective characteristic (displacement) on each soft-tissue layer responded after application of ultrasound signals. Based on the results from FD analysis, the ultrasound system for diagnosis of the FGID was developed and clinically tested via application of it to 40 human subjects with/without FGID who were assigned to Normal and Patient Groups. The results from FD analysis showed that the maximum displacement amplitude in the rigid models (0.12 and 0.16) at the interface between the fat and muscle layers was explicitly less than that in the normal model (0.29). The results from actual specimens showed that the maximum amplitude of the ultrasound reflective signal in the rigid models $(0.2{\pm}0.1Vp-p)$ at the interface between the fat and muscle layers was explicitly higher than that in the normal model $(0.1{\pm}0.0Vp-p)$. Clinical tests using our customized ultrasound system showed that the maximum amplitudes of the ultrasound reflective signals near to the gastrointestinal tract well for the patient group $(2.6{\pm}0.3Vp-p)$ were generally higher than those in normal group $(0.1{\pm}0.2Vp-p)$. These findings suggest that our customized ultrasound system using the ultrasound reflective signal may be helpful to the diagnosis of the FGID.

Purpose : The purpose of this study was to determine the clinical value of bicarbonate gap (BG) in acute gastroenteritis by comparison of clinical manifestations and laboratory findings. Methods : We retrospectively analyzed 135 patients who had acute gastroenteritis. We classified them into two groups: $BG{\leq}-5$ (group A), BG>-5 (group B). We made a comparative study for clinical manifestations, serum electrolyte, albumin, protein, blood urea nitrogen (BUN), creatinine, anion gap (AG), delta anion gap (${\Delta}AG$) and delta bicarbonate (${\Delta}HCO_3{^-}$) between the two groups. Results : The duration of hospitalization and diarrhea was significantly longer in group A than group B. In laboratory findings, serum sodium, serum total $CO_2$, total protein, $AG_{corrected}$ and ${\Delta}AG$ were lower in group A than group B. ${\Delta}HCO_3{^-}$ and chloride were higher in group A than group B. Conclusion : BG may be a sensitive predictor enough to access the severity of acute gastroenteritis.
Functional dyspepsia is one of the most common bowel disorders as prevalent of 7.7% Korean population. The cardinal manifestations include bothersome postprandial fullness, early satiation, epigastric burning or pain. These features are chronic and should be presented recurrently with no other compatible organic disease to explain the symptoms. Even though it is not life-shortening, functional dyspepsia usually make the health-related quality of life worse especially if other functional bowel disorder coexist. The coexistence of functional bowel disorders is called as 'overlap syndrome'. Anxiety, somatization and insomnia is more prevalent in overlap syndrome compared with sole functional bowel disorder. Therefore, it is worthwhile that physician interviews and elucidates whether the dyspeptic patient had other kinds of functional bowel disorders, and manages the underlying psychotic pathology. Placebo effect is large in functional dyspepsia, and there is only four kinds of prokinetics that is proven to be superior to placebo. Adverse events relating prolonged administration of prokinetics sometimes fatal or irreversible, physician willing to describe prokinetics should be familiar to the possible adverse effects and the relating risk factors. Pathologic acid reflux is not uncommon in functional dyspepsia, and acid-suppressant is equivalent to the prokientics in most of dyspeptic patients.

Objective:This study was designed to examine the validity of the Ego Impairment Index(EII) in adolescent psychiatric patients(depression, depressive-conduct disorder, schizophrenia), and to explore the validity of the depressive-conduct disorder as a diagnostic entity. Method:19 depressives, 13 depressive-conduct disordered, and 10 schizophrenics with the age from 12 to 18 were selected based on DSM-IV and ICD-10 criteria, and their responses of the Rorschach and MMPI were analyzed. The EII was empirically developed by Perry and Viglione(1991) in order to assess the degree of ego impairment, and was derived from the Rorschach test. Results:It was founded that a single factor, ego impairment, was derived from the principal component analysis and explained 57.18% of total variances. The degree of ego impairment was grater in schizophrenics than in the other two groups, but there were no difference between depressives and depressive-conducts. Conclusions:These results offered support for the use EII as an empirical means of assessing the degree of ego impairment in adolescent patients. And these results suggest that depressive-conduct disorder group is more similar to the depressive group, implying that depressive-conduct disorder might correspond to so called masked depression. The clinical implication and limitation of present study were discussed.
GRAS probiotics can be used to modulate intestinal microbiota and to alleviate various gastrointestinal disorders. In several recent studies, researchers have explored the potential expansion and usability of probiotics to reduce the risk factors associated with diseases, including obesity, hypercholesterolemia, arterial hypertension, hyperhomocysteinemia, and oxidative stress. In this review, our aim was to clarify the mechanism underlying interactions between hosts (animal or human) and probiotics and the beneficial effects of probiotics on human health.
Objectives: These cases report the benefits of administration of Banhasasim-tang extracts or decoctions to patients with functional gastrointestinal disorders after motor vehicle accidents. Methods: The patients were treated with Banhasasim-tang extract or decoction three times daily. History-taking and imaging tests were used to differentiate other diseases. We evaluated the patients using the Gastrointestinal Symptom Rating Scale (GSRS), which is used to assess overall gastrointestinal symptoms. Results: Treatment with herbal medicine resulted in a decrease in the patients' complaints of symptoms. The GSRS scale showed improvement in all four cases. Conclusions: Banhasasim-tang, regardless of its form, has beneficial effects in patients with functional gastrointestinal disorders after motor vehicle accidents.
